When AI Code Review Is Accurate but Still Wrong for the PR

An AI code-review comment can correctly identify a real issue and still be the wrong change for the pull request. The practical test is not only “Is this observation true?” but also “Would acting on it change the intended user behavior, fix a merge-blocking risk, or merely expand the scope?” John Lee’s 2026 account of a 209-comment review shows how treating every accurate suggestion as mandatory can create an endless fix-and-review loop. A short feature contract, explicit severity triage, and human product judgment turn the review back into a decision process instead of a queue of chores.

What does “accurate is not necessary” mean in an AI code review?

“Accurate” describes the observation: a reviewer has found a behavior, edge case, inconsistency, or possible improvement that really exists. “Necessary” describes the decision to change the code in this pull request. Those are separate judgments. A comment can be technically correct yet irrelevant to the feature’s promised behavior, too low-impact to delay the merge, or risky because the proposed fix would pull unrelated changes into the branch.

John Lee summarizes the distinction in his essay with the sentence, “Accurate and necessary are different things.” The distinction matters more with AI reviewers because a model can produce many individually plausible findings. If a team accepts each one without checking the product goal, the pull request can drift away from its original purpose while the review appears increasingly thorough.

The right response to an accurate comment may be one of four actions: fix it now, investigate it before merging, record it as a follow-up issue, or deliberately ignore it. Choosing among those actions is part of engineering judgment, not a failure to take review seriously.

What happened in John Lee’s 209-review case?

Lee describes one large pull request that propagated secret taint across workflow steps. The code and review were both produced with Codex. The figures below are the author’s report of that incident, not an independently audited benchmark or a representative sample of AI reviews.

Measure Author-reported detail How to interpret it
Files changed 83 files The review covered a broad change surface, increasing the chance that local suggestions would interact.
Lines added 21,962 This is the pull request’s reported addition count, not a general measure of AI-generated code quality.
Commits 36 The branch evolved through many revisions before the review converged.
Review conversations 209 across two days A count from this single pull request; it does not establish a normal review volume.
Comments judged accurate About 90%, “by my feel” A subjective estimate by Lee, not a measured accuracy rate or controlled study.
Fix-and-review rounds 12 rounds in roughly two hours The author’s account of repeated iteration as each response exposed more edge cases.
Convergence after a written contract One small follow-up, then two comments in a final review request An anecdotal outcome after the team clarified intended behavior.

Lee says that reviewing each comment in isolation caused the changes to drift toward finer edge cases. The team then wrote a two-sentence feature contract: credentials used only for authentication must remain usable, while secret values and anything derived from them must be blocked automatically. Reworking the pull request around that contract produced the much shorter final review described above.

That sequence is useful as a case study, not as a promise that every project will shrink its review queue in the same way. One anecdote cannot establish a universal AI accuracy rate or prove a productivity gain.

Why can correct comments create an endless fix-and-review loop?

The loop starts when the team treats the review queue as a list of required edits rather than evidence to evaluate against a feature goal. Four mechanisms make the problem worse.

Local fixes change the global behavior

A comment may be valid for one function while its proposed fix changes assumptions elsewhere. After the fix, the model reviews a new version of the system and identifies consequences created by the previous response. The comments are not necessarily contradictory; the sequence is producing a moving target.

Edge cases displace the user-visible requirement

Large changes contain unusual states, naming inconsistencies, and theoretical failure paths. Those may deserve attention eventually, but they can consume the same decision budget as a defect that changes what users can do. Without a contract, the review gradually optimizes for local neatness instead of the feature’s promised outcome.

Severity is flattened into a single queue

A crash, an authorization bypass, and a stylistic naming concern may all appear as comments with similar wording. If the process does not distinguish user impact, reviewers spend time debating low-value findings while a merge-blocking risk receives no special treatment.

Fast responses hide cumulative scope growth

Addressing a comment immediately feels efficient. Across many comments, however, each “small” change can add tests, abstractions, and new branches that were never needed for the release. The pull request becomes harder to reason about precisely because every response was individually defensible.

How do I stop AI code review from turning into endless fix cycles?

Use a written behavior contract, triage comments by user and merge impact, and review proposed fixes in the context of the complete change. The following workflow preserves useful findings without making every observation a blocker.

  1. Write the feature contract before accepting review changes. State what users must be able to do, what must be prevented, and any deliberate exceptions. Keep it short enough to test every comment against it.
  2. Classify each finding before editing code. Mark it as a blocker, a required contract correction, a follow-up issue, or an intentional non-action. Do not let the tool’s confidence or the comment count decide priority.
  3. Ask what changes for the user. For each proposed fix, describe the observable effect: access denied, data exposed, workflow broken, latency changed, or no product-visible difference. If the effect cannot be explained, pause rather than patch automatically.
  4. Check interactions with the current diff. Read the surrounding code, tests, and related comments before implementing a fix. A technically sound change can be wrong if it reverses an intentional exception or broadens the feature’s scope.
  5. Batch related fixes. Group comments that address the same contract rule and run the relevant tests once. Batching reduces the chance that each tiny edit triggers a fresh interpretation of the whole pull request.
  6. Record non-blocking work separately. Create an issue with the finding, affected area, and reason it is deferred. This preserves the signal without forcing unrelated work into the merge.
  7. Ask for a focused final review. Tell the reviewer which contract points changed and which findings were intentionally deferred. A final pass should verify the agreed behavior, not reopen every possible improvement.

How should I triage an AI review comment?

A simple impact matrix keeps technical correctness and merge necessity visible at the same time. Use the highest applicable category, and document the reason when choosing not to act.

Category Typical effect Action in the current PR
Contract blocker Violates the behavior promised to users or breaks an explicit security or data rule. Fix, add or update a regression test, and re-review the affected path.
Merge-risk defect Likely crash, data loss, authorization failure, or incompatible integration even if the main feature still appears to work. Investigate before merge; involve the owner who can assess operational impact.
Context-dependent concern Real edge case whose importance depends on deployment, workload, compatibility policy, or a deliberate product trade-off. Decide with product and system context; do not auto-fix from the comment alone.
Non-blocking improvement Readability, naming, refactoring, or a defensive check that does not alter the promised behavior now. File a follow-up issue or leave a rationale for deferral.
Intentional difference The code differs from a common pattern because the feature contract requires an exception. Explain the exception in the code or review and leave the implementation unchanged.

Severity should be based on consequences, not on how confidently the model phrases the comment. A precise explanation of a low-impact issue is still low impact.

Should I fix every accurate AI code-review comment?

No. Fix every comment that reveals a contract violation or a material merge risk; defer or decline comments that do not justify changing this pull request. The decision should be explicit rather than silent, because an “ignore” is a judgment about scope and priority.

Lee puts that human role plainly: “An ignore is a judgment that this comment isn’t our mountain, and that judgment is why a human is in the loop at all.” A useful review response names the reason: outside the feature contract, no user-visible effect, already covered by a separate issue, or intentionally rejected to preserve compatibility.

Do not use deferral to hide uncertainty about a possible security or data-integrity problem. If the impact is unclear and the cost of being wrong is high, escalate the question to the person responsible for the product or system boundary before merging.

What should a two-sentence feature contract contain?

The contract should state the allowed behavior and the prohibited behavior in terms that a reviewer can apply to a concrete code path. It should also identify important exceptions, because those are where generic review advice most often conflicts with product intent.

Lee’s example concerns secret-taint propagation. In plain language, credentials that are used only to authenticate remain usable; secret values and values derived from those secrets are blocked automatically. That wording gives reviewers a test for both false positives and false negatives: blocking an authentication-only credential violates the first sentence, while allowing a derived secret violates the second.

For another feature, a contract might specify that a migration must preserve existing customer-visible identifiers while rejecting malformed records, or that a cache may serve stale results for a bounded period but must never return data across tenants. The exact content must come from the product owner or domain expert; an agent cannot infer the intended trade-off reliably from syntax alone.

Allowed: [user-visible behavior that must continue or become possible]
Blocked: [behavior, data flow, or failure mode the feature must prevent]
Exceptions: [deliberate cases that look similar but are treated differently]

Keep the contract next to the pull request description or acceptance tests. A reviewer should be able to point from a comment to the sentence it affects, or explain why it does not.

What does the human reviewer still need to do?

The human supplies product purpose, acceptable risk, and scope boundaries. Lee writes, “The model can’t know that,” referring to the product essence behind a judgment about necessity. Even a highly capable model can inspect code and identify patterns without knowing which trade-off the release is intentionally making.

  • Set the priority rule: decide whether user harm, security exposure, reliability, compatibility, or delivery timing dominates for this change.
  • Recognize deliberate exceptions: understand why a credential, API shape, or workflow branch is treated differently from superficially similar data.
  • Own the merge decision: accept responsibility for what is fixed, deferred, or rejected; do not outsource that decision to a comment count.
  • Close the feedback loop: when a repeated comment reflects missing context, update the contract, tests, or documentation so future reviews have a clearer target.

This does not make AI review useless. It assigns the tool a tractable job: surface possibilities, explain affected code, and check implementation against criteria that a human has defined.

How can teams operationalize this approach in a pull request?

Put the decision rules in the review template so they are visible before comments accumulate. A lightweight template can ask for the contract, known non-goals, and disposition of findings.

  • Comment disposition: fixed, investigated and accepted, follow-up issue, or intentional ignore.
  • Evidence: regression test, reproduction, log, or reasoning supporting each blocker decision.

When asking an AI reviewer for a pass, provide the contract and ask it to label findings by impact and confidence separately. Confidence answers “How likely is this observation to be true?” Impact answers “What happens if we leave it?” Keeping those labels separate prevents a high-confidence, low-impact suggestion from outranking a less certain but potentially severe failure that needs human investigation.

After changes are made, rerun only the checks relevant to the contract first. Then perform a broader review if the diff expanded materially. This preserves a stable target and makes it easier to spot scope growth.

What failure modes should I watch for?

Using “accurate” as a synonym for “blocking”

Correction: require the commenter to state the user or operational consequence. If none is material for this release, record the item for later rather than changing code reflexively.

Writing a contract that is too vague

“Handle secrets safely” cannot resolve a dispute about authentication-only credentials. Name the data classes, allowed transitions, and prohibited outcomes that distinguish the edge cases.

Deferring without an owner

A deferred issue with no description or owner will disappear. Capture the affected path, why it is non-blocking now, and what condition should trigger follow-up.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Letting the contract excuse real defects

A non-goal is not a waiver for crashes, data exposure, or broken integrations. Escalate any finding whose downside is severe even when it is outside the feature’s headline behavior.

Assuming fewer comments proves better engineering

Lee’s shorter final review followed a clarified contract in one reported case. Comment counts alone do not measure quality; a small review can miss defects, and a large one can reflect a genuinely broad change.

What does this case prove—and what does it not prove?

It demonstrates a practical distinction and a plausible workflow: define intended behavior, sort findings by user impact and merge relevance, understand interactions before editing, and track non-blockers separately. It also illustrates how a large, AI-assisted pull request can consume time when every local observation becomes an immediate task.

It does not establish that AI reviewers are 90% accurate in general, that twelve rounds are typical, or that writing a contract will always reduce review effort. The 90% figure is Lee’s personal “by my feel” estimate, and all incident counts are self-reported. No independent statistical study or benchmark is cited for the essay.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use the case as a process prompt: make the product rule explicit, require a consequence for every blocker, and preserve a human decision about scope. Those controls remain useful regardless of which review model or coding workflow a team uses.

What should you remember when an AI comment is technically right?

Ask whether the comment changes the behavior this pull request is meant to deliver, prevents a serious merge risk, or belongs in a later improvement. Accurate observations are valuable inputs, but necessity is a product and engineering decision. Keeping that distinction visible is how a human reviewer prevents a plausible review from becoming an unbounded rewrite.

Frequently Asked Questions

How do I write a good feature contract for an AI reviewer?

Describe the user-visible behavior that must remain possible, the behavior the change must prevent, and any deliberate exceptions. Use concrete data classes or workflow states rather than broad phrases such as “handle securely.”

Where should deferred AI review findings be tracked?

Use the team’s normal issue tracker and include the affected code path, the reason it is non-blocking for this release, and an owner or trigger for revisiting it. Linking that issue from the pull request preserves context without expanding the merge scope.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can an AI model determine whether a code change is necessary?

It can estimate technical impact when given explicit criteria, but it cannot reliably infer unstated product trade-offs. A human still needs to define priorities, exceptions, and the acceptable risk for the release.

What is the difference between review confidence and review severity?

Confidence is the likelihood that the reported condition is real. Severity is the consequence of leaving it unchanged; a highly certain style issue can be less urgent than a low-confidence warning about a possible authorization failure.
