Human-in-the-loop (HITL) AI is a way to operate an AI agent so it pauses before a consequential action and waits for a person to approve, reject, edit, or stop it. The checkpoint belongs at the boundary between the agent’s proposal and an action that changes something outside the conversation: sending a message, deleting a record, spending money, changing access, or publishing content. Low-impact work that is reversible and easy to check can often proceed under monitoring instead. The right threshold depends on the action’s impact, reversibility, data sensitivity, uncertainty, and available recovery path—not on a universal rule or the agent’s own informal judgment.
What is human-in-the-loop AI?
Human-in-the-loop describes a workflow in which a person must review a proposed action before the system carries it out. The agent may still analyze a request, gather information, prepare a draft, or assemble a tool call; the human checkpoint controls whether the consequential step is executed.

That distinction matters because “AI assistance” and “AI action” are not the same thing. Summarizing a document generally produces an answer for a person to inspect. Sending that summary to a customer, changing the source record, or granting someone access alters the world outside the chat. HITL is a control on that transition, not simply a person being somewhere in the workflow.
OpenAI’s agent documentation describes an approval pause before sensitive tool calls and side effects such as cancellations, edits, shell commands, or sensitive MCP actions. The UK National Cyber Security Centre uses a compact distinction: in human-in-the-loop systems, humans approve actions before they happen.

How does it differ from other forms of oversight?
| Operating pattern | Human role | Typical fit |
|---|---|---|
| Human-in-the-loop | Approves, rejects, edits, or stops a proposed action before execution. | Actions with meaningful impact, uncertainty, sensitivity, or difficult recovery. |
| Human-on-the-loop | Monitors a running system and can intervene while it operates. | Bounded, monitored activity where intervention remains possible. |
| Human-out-of-the-loop | Does not review or intervene in the system’s actions during operation. | Only work whose risk and controls justify operation without human review. |
These labels describe where oversight sits; they do not by themselves establish that a system is safe. A required approval can still be weak if the reviewer lacks context, is rushed, or cannot stop execution.
When should an AI agent ask for approval?
Require approval when the proposed action could materially affect a person, system, organization, or legal position, especially if it is hard to reverse. Government of Canada guidance recommends review for actions that send, publish, approve, spend, or update records unless the expected impact is demonstrably low and the action is trivially reversible.
Use the action’s consequence—not merely the tool name—as the trigger. A tool that edits records may be safe for a disposable draft and high-risk when it changes a customer’s account. Conversely, a seemingly ordinary send operation may create a contractual or reputational commitment depending on its recipient and content.
- State changes: deleting, editing, approving, or resetting systems or records.
- External effects: sending messages, publishing material, submitting forms, or contacting a customer or third party.
- Money and commitments: purchases, payments, budget approvals, or actions that create legal or compliance obligations.
- Access and sensitive information: changing permissions, accessing or disclosing sensitive data, or sending information beyond its intended audience.
- Hard-to-reverse operations: actions whose effects cannot be reliably undone, including deletion of critical records or network egress.
- Unclear or suspect inputs: ambiguous instructions, uncertain evidence, or untrusted content that may try to manipulate the agent’s tool use.
The Canadian Centre for Cyber Security specifically advises that designers or operators—not the agent—set approval thresholds. It identifies system resets, network egress, and deletion of critical records as examples of high-impact checkpoints. This is important because an agent’s confidence or its own interpretation of policy is not an adequate substitute for a control defined outside the model.

How can a team decide which actions need review?
Classify actions by risk before enabling the agent, then make the decision rule explicit in the system’s policy or orchestration layer. Microsoft’s task framework offers four useful axes—repeatability, impact, error detectability, and time sensitivity—and the comparison should also account for reversibility, data sensitivity, ambiguity, reviewer capacity, and the quality of the stop or recovery path.
| Question | Lower-risk signal | Higher-risk signal |
|---|---|---|
| What happens if it is wrong? | Limited inconvenience; no meaningful effect on people or operations. | Financial, safety, privacy, legal, reputational, or service impact. |
| Can the result be undone? | Simple, reliable rollback or correction. | Irreversible, costly, or uncertain recovery. |
| Can a mistake be detected? | Easy to spot before harm occurs. | Hard to detect, or likely to remain unnoticed. |
| How clear are the request and evidence? | Specific instruction and reliable supporting information. | Ambiguity, missing facts, conflicting instructions, or untrusted content. |
| What access does the action require? | Read-only or draft access with limited scope. | Sensitive data, broad permissions, external communication, or destructive capability. |
| Can the operation be stopped? | A tested stop control and clear recovery route exist. | Stopping is difficult, delayed, or dependent on the agent behaving correctly. |
Repeatability matters because a predictable task is easier to constrain and monitor. Time sensitivity matters because a slow approval queue may itself create operational risk. Neither axis cancels out the others: a routine action can still merit review if it exposes sensitive information, while a time-critical task may need a carefully bounded fallback rather than an improvised bypass.

Use a deterministic trigger for high-risk categories. For instance, the policy can require approval whenever an operation sends externally, changes permissions, deletes a protected record, or exceeds an approved spending boundary. The agent may explain a pause, but should not be able to waive the requirement based on its own assessment.
How much autonomy should an agent receive?
Autonomy should increase only as the potential impact stays bounded and the system’s monitoring and recovery controls remain credible. The following ladder turns that principle into operating choices; it is a practical synthesis of official guidance on risk, bounded autonomy, least privilege, and reversibility.

- Assist: Let the agent read, summarize, classify, or draft. A person uses the result, with ordinary review appropriate to the work.
- Suggest: Let the agent prepare a plan or tool call, but require a person to approve before execution.
- Act with a checkpoint: Permit bounded work while pausing for sends, publications, purchases, permission changes, sensitive-data access, or irreversible operations.
- Use monitored autonomy: Allow low-impact, reversible actions to run with alerts, logs, rate limits, and a tested stop control.
- Prohibit autonomy: Do not permit the agent to bypass policy, erase audit records, exfiltrate data, or expand its own permissions.
Microsoft’s examples help illustrate the difference: drafting an internal update or summarizing notes is generally easier to delegate than approving a budget, sending a customer-facing proposal, or publishing external communications. The Australian National AI Centre describes the same proportional approach: automated monitoring may fit low-stakes applications, while high-stakes decisions need mandatory human review.
What should an approval screen show?
A reviewer needs enough concrete information to judge the proposed operation without reconstructing the run from scratch. Show what the agent intends to do, where it will do it, what tool and arguments it will use, what effect is expected, what evidence supports the action, and why policy escalated it.
Recommended Free Tools
- Proposed operation: a plain-language description plus the exact target and relevant parameters.
- Expected effect: what will change, who or what will be affected, and whether the change is reversible.
- Context and evidence: the relevant user request, records, and facts needed to evaluate the proposal.
- Reason for escalation: the policy condition or risk that caused the pause.
- Usable controls: clear options to approve, reject, edit, or stop, with no ambiguous default that executes the action.
After the decision, record the reviewer, decision, timestamp, action actually taken, and outcome. Microsoft, Canadian, and OpenAI guidance all emphasize visibility, accountability, and auditability. A recorded approval should correspond to the operation that ran; if editing the proposal changes its material effect, the reviewer should assess the changed action rather than relying on an earlier approval.
How should approval fit into the agent’s technical design?
Keep the decision to require approval in policy or orchestrator logic outside the model’s informal judgment. The model can propose a tool call and explain why it is needed, while the surrounding system checks whether that class of action is allowed, whether approval is required, and whether the reviewer’s decision authorizes the exact operation.

Apply least privilege so an agent cannot do more than its task requires. Government of Canada guidance recommends “read-only by default” where possible: allow viewing and drafting before granting write or execution rights. Pair that default with explicit action schemas, scoped tool access, monitoring, and a pause or disable mechanism external to the agent. An external stop matters because the system must remain stoppable even if the agent misbehaves.
Approval is one layer, not a guarantee. A person can approve a harmful action if the interface hides the consequences, the reviewer is overloaded, or the request looks routine. Deterministic rules, narrow permissions, useful context, and a reliable stop path reduce reliance on a single click.
How should teams test and maintain oversight?
Test the complete action path, including cases where the agent is wrong, the reviewer rejects a proposal, or an operation has partly completed when a problem appears. The Canadian Centre for Cyber Security and Government of Canada guidance call for testing realistic edge cases, worst-case inputs, prompt injection, and recovery from rejected or partially completed actions.
- Verify that each high-risk action pauses before its side effect, not after it.
- Try ambiguous requests, conflicting instructions, and untrusted content that could steer tool use.
- Check what a reviewer sees and whether approve, reject, edit, and stop controls produce the intended result.
- Test failure handling when a tool times out, a run is interrupted, or only part of a multi-step operation succeeds.
- Confirm that logs capture prompts, tool calls, approvals, overrides, and outcomes without allowing the agent to erase its audit trail.
- Exercise the external pause or disable mechanism and confirm that operators can use it independently of the agent.
Reassess the approval policy whenever the model, tools, data, permissions, task scope, or organizational policy changes. A threshold calibrated for one tool or data set may no longer fit after the agent receives broader access or begins affecting a different group of people.
Best Value
Is there one approval threshold that works for every agent?
No universal threshold follows from the cited guidance. The appropriate checkpoint depends on the domain, likely impact, reversibility, sensitivity, policy, and whether a mistake can be detected and recovered from; requirements may also vary by jurisdiction and application. ISO/IEC DIS 42105 describes the broader process of ensuring appropriate human oversight, but that does not itself establish one trigger for every tool call.
Frequently Asked Questions
Does every AI action need a person to approve it?
No. A workflow can reserve mandatory approval for defined risk categories while letting lower-stakes work proceed with monitoring. The key is to classify actions in advance rather than make the agent improvise the boundary each time.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsDoes human approval mean a company has met every legal requirement?
Not by itself. Approval is an operational safeguard, not proof of compliance with every rule that may apply to a particular decision, sector, or jurisdiction.
Should an AI agent ask a human to approve its reasoning?
The useful checkpoint is normally the proposed operation and its likely effects, supported by relevant evidence. Reviewing an action boundary gives the operator something concrete to approve or reject.
What happens if a reviewer rejects an agent’s proposed action?
The system should not execute that proposal, and the workflow should make rejection a tested outcome rather than an exceptional dead end. Teams should also decide how the agent reports the refusal and how to recover if an earlier step in a multi-step task already changed something.
