Yes. The 18 campaign achievements in Call of Duty: Modern Warfare 4 remain earnable after the launch-week updates through October 29, 2026. Infinity Ward’s patch notes for those updates describe Multiplayer and DMZ fixes, not changes that remove or disable campaign achievements. Nothing is permanently missable: after the story, Mission Select lets you return to completed missions. Plan on at least two campaign runs, however, because Realism unlocks only after a first completion and two Assassins achievements require opposite choices.
What changed in the October update?
The word “update” can mean several launch-week drops. The official Call of Duty: Modern Warfare 4 Launch Patch Notes page records changes on October 21, 28 and 29, 2026; a press recap covers a live drop on October 25–26. Across those entries, the listed work concerns Multiplayer, DMZ, stability, audio, weapon tuning, challenges and playlists. None says that a campaign achievement was removed, renamed or made unobtainable.

- October 21: UI/UX, Multiplayer crashes, audio and DMZ progression.
- October 25–26: Search and Destroy and Collateral XP, plus DMZ mission and vehicle issues.
- October 28: a first weapon-tuning pass and Multiplayer challenge, killstreak and perk fixes, alongside DMZ exploit and Exfil fixes.
- October 29: further Multiplayer and DMZ work, and the Halloween Possession playlist.
Infinity Ward characterized the October 29 drop as addressing issues “in Multiplayer and DMZ.” Its October 28 notes likewise described fixes and improvements across those two modes. That scope matters: the existence of a patch does not by itself mean campaign progress or achievements changed.
There is a separate tracker issue that can cause confusion. The patch page lists a known problem with the Patriot XMR Platinum Camo Challenge’s UI tracking, while noting the challenge can still be completed. That is a Multiplayer weapon-camo challenge, not one of the 18 campaign achievements. Multiplayer camo challenges and DMZ progression do not count toward this campaign set.

This answer covers the official patch information through October 29, 2026. Season 1 is dated November 6 in launch-week guides, but there is no confirmed official statement that it adds, removes or replaces any of these 18 achievements. Treat a future change to the list as unconfirmed unless Activision or a platform’s achievement list shows one.
How many achievements are in Modern Warfare 4?
The campaign list has 18 unlocks worth 1,000 Gamerscore on Xbox. PlayStation tracking sites list eight Gold and ten Silver trophies, with no Bronze and no Platinum. The campaign has 16 missions; some achievements are tied to individual missions, while others ask for campaign-wide progress or a full-game completion.

Steam groups the achievements under “Modern Warfare 4 – Campaign” in the Call of Duty app. The platform lists and trophy guides are the working references: Activision has not published its own numbered PlayStation or Xbox achievement table on its site. The table uses the names and requirements reported by Steam and trophy guides, with Xbox Gamerscore values and the reported PlayStation tier.

The Xbox total is two 100-point achievements plus sixteen 50-point achievements, or 1,000 Gamerscore. “Trench Broom” is the spelling used by Steam and PowerPyx; AllThingsHow also uses “Trench Boom” in one place. If your tracker shows the alternate spelling, it refers to the same shotgun challenge. The consistent requirement across PowerPyx, KeenGamer, GameRant, TechWiser and RetroGems is 15 kills, not a 50-kill variant.
For the crate challenge, three guides report 26 Yellow Supply Crates, while Grindnstrat reports 25; use 26 as the safer target. For Noteworthy, the 50-Note threshold is consistent. Stranded alone contains 58 Notes according to guides, while the total campaign count and whether Notes outside Stranded also count are disputed. Collecting and reading notes throughout the campaign is sensible, but clearing Stranded is the main route to the threshold.
The missions without a dedicated achievement are Traffic, Gone Dark, Survivors, Defection/Deflection, Decapitation and Greater Good. That does not mean those missions are optional for the full-campaign completion achievements.

Are MW4 achievements missable or glitched after the patch?
No permanent missables or patch-disabled campaign achievements have been reported through October 29. PowerPyx’s October 18 roadmap lists zero missable and zero glitched trophies; KeenGamer and Grindnstrat also report zero missables because Mission Select is available. VGTimes reported the day after launch that players could revisit missions after the credits and that no trophy refusing to unlock had been reported at that point.
A challenge can still fail during a particular attempt. In that sense, a run can be spoiled even though the achievement itself remains obtainable. If a condition is missed, reload an appropriate checkpoint or replay the mission from Mission Select rather than assuming the update deleted the unlock.
- Incoming!: destroy each SAM Site vehicle reinforcement before troops unload. If they do, restart the sequence.
- Long Shot: take out all four mortar operators before the mission ends. If a crew escapes, reload promptly; destroying the third tank can end the mission early.
- Skeet Shooting: kill ten paratroopers in one wave before they land. Reload the stadium checkpoint if the wave is lost.
- Raining Thunder: the 15 kills must come from three consecutive mortar rounds. PowerPyx advises reloading if the achievement does not appear after the third opportunity.
- Can’t Touch This: any hit in the alley fight voids that attempt. The red screen outline signals a hit; a reload from a mid-fight checkpoint can provide another attempt.
For a non-popping achievement, first check the exact condition and difficulty requirement, then confirm the mission or campaign completion registered. Common reasons include a civilian being hurt, an enemy escaping, a stray kill in Assassins, a troop unloading, or being hit in the alley. Restarting the relevant checkpoint is usually the quickest fix; Mission Select is the fallback after completing the story.
Do I need Realism, and how many playthroughs are required?
For all 18 achievements, plan for at least two campaign completions. Realism is locked until you finish the campaign once, and Pacifist and Revenge cannot be earned in the same Assassins run because one permits only the scripted kill while the other requires killing everyone after Shin dies.
Four achievements check difficulty:
- Safety’s On: Hardened or higher, in the kitchen; do not fire your weapon.
- Revenge: Hardened or higher in Assassins.
- Can’t Touch This: Veteran or higher for the alley fight.
- Was it worth it?: finish the entire campaign on Realism.
You do not need to play an entire mission on Hardened or Veteran just to satisfy the two encounter challenges. PowerPyx, Grindnstrat and RetroGems describe changing difficulty in the corridor before the kitchen and again before Makarov’s alley, then lowering it after the encounter. Check the difficulty before entering each fight so the relevant section is completed under the required setting.
Realism becomes available after any first completion, including Recruit. PowerPyx describes its rules as Veteran combat without a HUD, with friendly fire enabled and no teammate nameplates. Sources differ on whether to call it the top or second-highest difficulty; the practical point for completion is that it is the specific setting required for Was it worth it?, not a debate over its place in an unpublished difficulty ranking.

Completion guides estimate roughly 15 or more hours for all 18, with PowerPyx splitting that into about eight hours for a first pass and seven for Realism. AllThingsHow estimates a straightforward Recruit story at about nine hours and a full achievement clear at 16–18 hours. PowerPyx, KeenGamer and Grindnstrat estimate trophy difficulty at 6/10. These are guide estimates, not a guarantee: exploration, retries and how quickly the combat challenges click affect the time.
What is the most difficult MW4 achievement?
There are two reasonable answers, depending on what “hardest” means. Was it worth it? is the longest commitment because it requires a whole Realism campaign; Can’t Touch This is the most demanding single encounter because one hit ruins the alley attempt. Safety’s On adds a different kind of pressure: it requires clearing the kitchen without firing, and only on Hardened or higher.
A launch-week Steam rarity snapshot copied by How To Video Game on October 25, 2026, puts Was it worth it? at 0.5% unlocked, Revenge and Bullet Train at 0.7%, Safety’s On at 1.0%, and Can’t Touch This at 1.1%. These are Steam global unlock rates on that date, two days after full launch—not current rates, and not proof that the rarest achievement is mechanically hardest. How To Video Game calls the Realism clear and several challenge achievements especially brutal; TechWiser and Modern Warfare Wiki emphasize the alley fight as the key skill check.
What is the most efficient route to 100%?
A practical route is to collect mission-specific challenges during a comfortable first campaign, reserve the contradictory Assassins outcomes for separate replays, then finish the Realism run. This keeps the first playthrough from becoming an unnecessarily punishing attempt to stack every condition at once.
- Complete all 16 missions on a comfortable setting. Work on the mission challenges as they appear: Skeet Shooting, Bullet Train, Raining Thunder, Trench Broom, the Following Orders achievements, Stranded challenges, Long Shot and Never Forgotten. Read Notes as you find them. In Out in the Cold, raise difficulty before the kitchen for Safety’s On and before the alley for Can’t Touch This; lower it afterward if desired.
- Replay Assassins for Pacifist. Use Mission Select and kill only the one scripted enemy. Trophy guides report fixed keypad codes across playthroughs: 5523 for the library, 7632 for the kitchen and 9103 for the bunker.
- Replay Assassins for Revenge. Set Hardened or higher. After Shin dies, kill everyone before touching the bunker ladder; the extra four enemies must be dead for the condition.
- Finish the campaign on Realism. This earns Was it worth it? and also satisfies Hold the Line, which accepts any difficulty. If you already earned Hold the Line, the Realism run still supplies the required completion.
- Use Mission Select for cleanup. Revisit any missed objective, crate, note or encounter. For Never Forgotten, trophy guides give 8410 as the Dog Tags safe code; some Following Orders guides give 214 for a lockbox.
The first campaign boot requires an internet connection; after that, the campaign can be played offline and its trophies still unlock. The campaign uses one autosave rather than manual save slots, so checkpoint reloads and Mission Select are the practical recovery tools. Multiplayer and DMZ progress is server-side and does not contribute to this achievement set.
A community tip from comments on an Incredilags guide suggests Settings > Interface > Readability > High Contrast Mode to make enemies stand out during combat challenges; that is a reported accessibility aid, not an official trophy requirement. For Skeet Shooting, PowerPyx recommends setting FOV to 60 and using single-fire, which can help with target control.

Is there a Platinum trophy in Modern Warfare 4?
No. On PS5, MW4 has a trophy list but no Platinum. Push Square reported that because the game is not technically a standalone app, it is counted as downloadable content and does not receive a full trophy list. ProvenGamer likewise reports that MW4 sits inside Call of Duty HQ, whose existing list already has a Platinum. The list is reported as eight Gold and ten Silver trophies.
There is no PS4 version, so the old workaround of playing a separate PS4 list on PS5 is not available. Older Call of Duty titles were decoupled from HQ in summer 2025 and later gained PS5 Platinums, but that does not establish that MW4 will receive a standalone Platinum. No date for such a change is confirmed.
On Xbox, the 18 achievements are worth 1,000 Gamerscore. On Xbox and PC, RetroGems reports that the unlocks use an “MW4 –” prefix inside the HQ bundle; they are a 1,000-point set, not a separate MW4 application completion. Steam also places the campaign group under the Call of Duty app.
Are there multiplayer or DMZ achievements in MW4?
Not in this 18-achievement campaign set. Multiplayer prestige, camo, killstreak, Ground War, Kill Block and Battle Pass progress do not count toward its 1,000 Gamerscore or trophy list. DMZ has a separate 70-level and dog-tag progression, but it is not part of these campaign achievements.
Campaign completion also feeds a separate 16-unlock reward track for Multiplayer and DMZ operators. That reward track is independent of the 18 achievements, so a campaign reward does not mean a trophy has been added, and a Multiplayer unlock is not needed for campaign completion.
Do I need a particular edition or platform?
You need access to the campaign, but not a premium edition. The official MW4 product page lists Standard/Cross-Gen and Vault editions; Vault extras include BlackCell for one season, operator packs, a weapon collection, a DMZ deployment bonus, a skin, a camo pack and a weapon blueprint. None is required for the 18 campaign unlocks. Any edition that includes the campaign can complete the list.

The full launch was October 23, 2026, for PS5, Xbox Series, PC and Nintendo Switch 2; digital pre-orders received campaign early access October 16 at 1 p.m. ET. There is no PS4 version. On PC, the official site says TPM 2.0 and Secure Boot are required for online play. The first campaign boot also requires internet, but subsequent campaign play can be offline.

Why are MW4 achievements listed under Call of Duty HQ?
MW4 is delivered inside the Call of Duty HQ app rather than as a separate standalone application. That packaging is also the reason PlayStation’s MW4 list is treated as an add-on trophy list instead of a separate Platinum-bearing game list.

Not by itself. A low percentage can reflect how recently the game launched, how many players have reached the relevant mission, or how many have attempted the condition; it is not a controlled difficulty measure.
Can I earn campaign achievements without buying the Vault Edition?
Yes. The paid extras listed for Vault are cosmetic, seasonal or mode-related bonuses, not campaign achievement requirements. Make sure the edition you select includes the MW4 campaign.
Do MW4 trophies unlock offline?
After the first campaign boot, the campaign can be played offline and its trophies still unlock. Multiplayer and DMZ use server-side progression, so that offline rule applies to campaign achievement progress rather than those modes.
