An LLM can safely help write cold emails only when it is kept away from the decisions that create commercial or reputational consequences. In a nine-week deployment for a food exporter recruiting international distributors, an LLM drafted language and selected from controlled options, while deterministic code and human approval decided which person could be contacted, when a message could go out, and when outreach had to stop. The author, mhk sameera, reports 678 delivered messages, 154 inbound messages and 96 human replies, but the account is a single-organization case study with no control group. It does not show that AI personalization improves sales results. It does show how to put a language model inside an outbound system without giving it operational authority.
What is the safe division of labor in an LLM cold-email system?
The practical boundary is simple: let the model produce language inside constraints, and let deterministic software enforce policy. As mhk sameera puts it, “The model writes prose and picks from lists. It never decides whether, when, or to whom a message is sent.” In this deployment, every message still required human approval.

The reported stack used Next.js App Router with TypeScript, MongoDB, one VPS service, Apollo.io for people search and enrichment, an OpenAI-compatible model described as GPT-4o class for language tasks, and Microsoft Graph for sending and reading each representative’s mailbox. The application had 57 API routes, 17 pages and about 31,000 lines of TypeScript. Those implementation details matter less than the authority boundary: the model supplied suggestions; application policy supplied permission.
| Decision | Who made it | Why that division matters |
|---|---|---|
| Email wording | LLM, constrained by an approved template | Language can be varied without allowing unsupported claims. |
| Industry and search choices | LLM selects from an application-owned closed list; code resolves provider IDs | Free-form identifiers can be misspelled, omitted or silently narrow a search. |
| Identity, ownership and suppression | Deterministic policy functions | These decisions affect privacy, reputation and duplicate outreach. |
| Send time and pacing | Sending routine and scheduler | One enforced path prevents an unattended job from bypassing safeguards. |
| Final release | Human approver | All 678 reported messages remained subject to review. |
How do you stop the model from choosing recipients?
Recipient selection should be an auditable pipeline, not a prompt outcome. The case study moved consequential checks into shared policy modules called by both interactive routes and scheduled jobs. That avoided an earlier failure in which a duplicate queue-condition implementation let later safety rules be missed by the unattended path.

Use closed lists for operational values
The model selected industry names from a fixed list. Application code then translated those names into provider identifiers. This is safer than allowing the model to invent an API value. It does not eliminate omission: a confectionery-distributor search once omitted “food & beverages,” silently reducing the result set. An operator-owned override was added so a person could correct the model’s selection.
Verify identity before enrichment is trusted
The described enrichment guard accepted an exact email match and rejected a different confirmed address. When the address was hidden, it required both a company-domain match and surname agreement. LinkedIn profile slugs were also checked against the person’s name. These checks are intentionally conservative: a plausible-looking record is not enough evidence that the intended individual owns the mailbox.
Make ownership a company-level rule
Contact-only suppression is inadequate in business-to-business outreach. The author’s operating principle is, “In B2B, the unit that experiences your outreach is the company, not the contact.” The system prevented different representatives from approaching separate contacts at the same company and stopped automation for the company after a reply. It excluded free-mail domains from domain-ownership claims, where a domain cannot reliably identify one organization. In the nine-week account, 129 second approaches were held back.
How should generated copy be prevented from inventing sales claims?
The approved sender template must be the source of truth. The model may rephrase what the template offers, but it must not add products, prices, credentials, statistics, delivery timelines or other claims absent from that template. Once a message was approved, the outgoing copy was frozen so later model output could not change it.

A workable content-control pattern
- Define the offer in an approved template. Include only claims the business is prepared to substantiate.
- Pass the template and permitted fields to the model. Treat recipient facts and allowed options as data, not as an invitation to improvise.
- Validate the returned structure. Reject missing required fields, unknown options and text that exceeds policy limits.
- Show the exact rendered email to a human. Approval should cover the final subject, body, recipient and mailbox.
- Freeze the approved version. The send routine should use the stored approved copy rather than regenerate it.
Closed lists and templates reduce the surface area for hallucination, but they do not guarantee completeness or correctness. The omitted industry in the case study is a reminder that a model can fail by leaving something out, not only by making something up. Operator overrides and review queues remain necessary.
What should happen when an inbound message arrives?
Inbound mail needs ordered classification because an automatic response is not a buying signal. The reported classifier checked bounces, automatic replies, opt-outs and human replies in that order. It separated new text from quoted history so an old newsletter footer containing “unsubscribe” would not be mistaken for a fresh opt-out.
Recommended Free Tools
Recommended decision sequence
- Bounce: mark the address undeliverable and prevent another attempt.
- Automatic reply: record the out-of-office or system notice without treating it as interest.
- Opt-out: suppress the person and apply the organization’s required retention and compliance process.
- Human reply: stop the lead and company sequence, then assign the conversation to a representative.
In this deployment, 154 inbound messages were processed: 35 were reported as autoresponders (22.7%), and 23 were bounces. The author reports 96 human replies from 42 contacts. Those categories must not be collapsed into one “reply rate,” because each triggers a different operational action.
How do pacing and scheduling keep automated outreach from running wild?
Sending controls belong inside the sending routine, not only in a dashboard or scheduler. The system used randomized gaps and enforced a sending window within the routine itself. That way, a manual call, retry or scheduled job still encountered the same time and pacing checks.

Monitor the execution path, not just the formula
The scheduler was initially not running, so automatic sends did not happen during the first weeks. The author reports six sends in August and 567 in September. Two pacing calculations also overstated achievable volume: one used the wrong time window, and another ignored scheduler-tick rounding. A mathematically correct interval can still produce the wrong daily volume when jobs run on discrete ticks.
- Record every enqueue, approval, attempted send, successful send and failure.
- Compare planned volume with the number actually executed in each scheduler window.
- Alert when the scheduler has not ticked for an expected interval.
- Recalculate capacity using the real tick granularity and the enforced sending window.
- Apply the same suppression and ownership policy to retries as to first attempts.
These controls are operational safeguards, not a promise that a particular volume is safe in every jurisdiction or mailbox environment. Cold email remains subject to the laws and rules applicable to the deploying organization; the case study leaves lawful basis and consent as the organization’s responsibility.

What did 678 real sends reveal about this deployment?
The following figures are the author’s report for one organization over nine weeks, not industry benchmarks and not evidence of causal lift from AI personalization.
| Measure | Reported result | How to interpret it |
|---|---|---|
| Lead records | 2,088 | Records in the system, not necessarily unique people reached. |
| Distinct recipients | 527 | People who received at least one message. |
| Messages delivered | 678 | Delivered messages, including more than one message to some recipients. |
| Inbound messages processed | 154 | All classified inbound mail, not only interested prospects. |
| Human replies | 96 from 42 contacts | Human responses, separate from autoresponders and bounces. |
| Autoresponders | 35, reported as 22.7% of inbound | Automatic notices that should not be counted as buying interest. |
| Bounces | 23 | Delivery failures requiring suppression or data correction. |
| Second approaches held back | 129 | Contacts prevented from receiving another company-level approach. |
| Messages in a language other than English | 72.7% of sent leads | A deployment characteristic, not proof that multilingual personalization caused replies. |
| High score tier | 89% of leads; mean score 77.8 | The score had weak discrimination because Apollo returned no email-status value for 1,166 enriched contacts. |
The author describes an open rate of 57.7% as an upper bound because mail clients can preload tracking images. The reply classifier was not evaluated against a labeled sample. Those limitations make the figures useful for understanding system behavior, but insufficient for claiming campaign effectiveness.

What failed, and what should engineers change?
Duplicated queue logic
When interactive and unattended paths implement similar filters separately, one path eventually diverges. A shared policy module makes recipient, ownership, approval and stopping rules reusable and reviewable.
Missing scheduler execution
A queue can be perfectly populated while no messages are sent. Monitor scheduler heartbeats and reconcile expected jobs with actual sends before interpreting campaign results.

Window length, tick rounding and retries all affect capacity. Measure observed intervals and completed sends rather than trusting a single formula.
Mailbox and attachment limitations
The author chose Mail.Read rather than Mail.ReadWrite to keep permissions narrower and scoped sending to a sales-mailbox group. In that specific Microsoft Graph/Exchange implementation, 28 sends failed because attachments exceeded a 2.5 MB inline limit, and follow-ups could not be threaded as replies. These are reported implementation constraints, not universal current limits for every Graph configuration. Design a fallback for oversized files and make the inability to thread visible to reviewers.

Overconfident lead scoring
A transparent score is still misleading if an upstream feature is absent. With email-status data missing for all 1,166 enriched contacts, 89% of leads landed in the high tier and the mean was 77.8. Track feature availability, score distributions and empty-value rates alongside the scoring formula.
How can a team implement this boundary step by step?
- List irreversible or reputational decisions. Recipient identity, company ownership, send timing, claims, suppression and stop conditions should be outside model authority.
- Create policy functions first. Put identity checks, domain ownership, approval state, pacing, opt-outs and company stops in deterministic modules.
- Define model contracts. Use schemas, closed lists and approved fields for every model response.
- Build one send path. Interactive actions, retries and scheduled jobs should call the same policy filter and sending routine.
- Require human approval. Display the exact recipient, mailbox, subject, body, language and evidence used for identity matching.
- Freeze approved content. Store a version or hash so the sent message is the one that was reviewed.
- Classify inbound mail in order. Separate bounces, automated notices, opt-outs and human replies before assigning follow-up.
- Instrument reality. Capture scheduler health, pacing, missing data, suppression decisions, failures and overrides.
- Review outcomes cautiously. Distinguish delivered messages, unique recipients, human replies and qualified opportunities; use a control group before making causal claims.
Does this case study prove that AI personalization works?
No. It demonstrates a safer architecture for using an LLM in outbound operations, not a measured improvement in sales performance. The deployment had no control group, the reply classifier lacked labeled evaluation, and the reported open rate may be inflated by image preloading. The 96 human replies are an operational count from this organization, not a benchmark or a causal estimate.

The durable lesson is governance: keep generative flexibility at the wording layer and keep authority over people, timing and stopping in code that can be inspected, tested and overridden by humans.
Frequently Asked Questions
Can an LLM choose the best prospects if a human approves the final email?
Human review of wording does not fix an unsafe recipient decision. Prospect eligibility, identity evidence, company ownership and suppression should be determined before drafting, with the model limited to controlled attributes and suggestions.
How should out-of-office messages affect a cold-email sequence?
Treat them as automatic notices, record any stated return date if your workflow supports it, and avoid counting them as interest. Resume only under a policy that checks the date, suppression state and any explicit opt-out language.
What audit trail should an AI outreach system keep?
Store the data used for identity matching, policy decisions, model version and structured output, approver, approved message version, scheduler event, send result and inbound classification. This lets an operator reconstruct why a message was or was not sent.
