Treat an AI agent’s security report as an unverified lead, not a vulnerability you can safely patch on sight. Verify the affected component and trust boundary, inspect the exact source and data flow, preserve raw evidence, and independently replay the claimed behavior. A qualified human should then classify the result before remediation. Make any change in an isolated branch or disposable environment, inspect the complete diff, rerun the original scanner and relevant tests, add adversarial cases, and check that the change creates no new alerts, syntax errors, regressions, deleted tests, or weakened assertions.
What does it mean to verify an AI-generated security finding?
Verification means establishing, with evidence independent of the discovering agent, that a specific security requirement is violated under a realistic threat model. A fluent explanation, a severity label, or a proof-of-concept script generated by the same agent is only a hypothesis until another reviewer can reproduce the effect and observe the result directly.

This distinction matters because automated tools can misunderstand intended public behavior, miss authorization context, or infer an attack path that is unreachable in deployment. Conversely, a clean scan or a passing AI-written test suite does not prove that business-logic and authorization flaws are absent. NISTIR 8397 (2021) recommends using several complementary techniques, including threat modeling, automated testing, static code scanning, fuzzing, web-application scanners where applicable, and dependency or service review.
How should I record the finding before investigating it?
Preserve the report exactly as it was produced, while clearly marking its conclusion as unverified. The summary can be useful for triage, but it cannot replace the underlying artifacts.

- Capture the discovering context. Save the prompt or task specification, agent and model version, tool versions, configuration, enabled plugins, and the repository revision or commit.
- Record location and timing. Include timestamps, affected file paths, line ranges, branch name, build or deployment configuration, and the exact command or request that produced the output.
- Preserve raw evidence. Store tool output, stack traces, HTTP exchanges, logs, screenshots, generated scripts, payloads, and any captured callback or database-side effect. Do not keep only a paraphrased narrative.
- Write down the claim without endorsing it. State the alleged weakness, preconditions, affected assets, claimed attacker capabilities, and proposed severity, but label each item as “agent-reported” until checked.
- Track provenance. Keep hashes or immutable copies of evidence where practical, and note who collected, transformed, or re-ran each artifact.
OWASP’s Application Security Verification and related testing guidance requires evidence artifacts and provenance; unverified observations belong outside the main findings section until they have been confirmed.
How do I reconstruct the scope and threat model?
Start with the system’s intended behavior, not with the agent’s label. Map what the component is supposed to expose, who may call it, and where trust changes as data moves through the system.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsIdentify the component and entry points
- Name the service, module, endpoint, job, command, or user interface involved.
- List every entry point: HTTP routes, message consumers, scheduled tasks, file imports, command-line arguments, webhooks, and administrative tools.
- Trace the input from ingestion through validation, authorization, storage, rendering, and external calls.
Define actors and trust boundaries
- Describe unauthenticated, ordinary-user, privileged-user, service-account, and administrator capabilities separately.
- Mark transitions between browser and server, tenant and shared infrastructure, application and database, and application and third-party service.
- State which identities, network locations, keys, and deployment controls an attacker realistically starts with.
Check intended use
Read API documentation, product requirements, authorization policies, and deployment configuration. A resource that is deliberately public, a diagnostic endpoint restricted to an internal network, or a field intentionally writable by a tenant may look suspicious to a scanner while satisfying the design. GitHub Security Lab has noted that missing threat modeling is a major source of false positives; impact cannot be judged without understanding the intended use.
What evidence should an AI agent provide before I accept its claim?
Require a source-grounded chain from input to security impact. A label such as “possible IDOR,” “SQL injection,” or “broken access control” is not evidence by itself.
- Exact source location: repository path, line range, function or route name, and the relevant version.
- Data or control flow: show how attacker-controlled data reaches a sensitive operation, or how an authorization decision can be bypassed.
- Attack preconditions: authentication state, role, tenant, network position, feature flags, required records, and any race or timing assumptions.
- Reproducible sequence: concrete requests, inputs, commands, fixture data, and expected output, with secrets removed or replaced by test credentials.
- Observed impact: the asset read, changed, executed, or disclosed, rather than a theoretical consequence.
- Violated requirement: the policy, access rule, confidentiality property, integrity constraint, or availability objective that the behavior breaks.
Ask the agent to separate facts it observed from inferences it made. If it cannot identify the exact sink, authorization check, or externally observable effect, keep the item in a triage queue rather than treating it as a confirmed vulnerability.
How can I independently replay the alleged vulnerability?
Use a fresh verifier context that contains the finding record, evidence, and relevant target code, but not the discovering agent’s hidden chain of thought. The verifier should be able to reach the same conclusion from the supplied artifacts.

- Choose an authorized environment. Use a local checkout, disposable test deployment, staging system, or intentionally vulnerable fixture. Never probe systems or data without authorization, and do not replay an exploit against production.
- Pin the version and configuration. Check out the recorded revision and reproduce feature flags, database schema, authentication settings, middleware, and dependency versions.
- Build a minimal harness. Reduce the claim to the smallest request, test, script, or fixture that exercises the suspected path. Remove unrelated agent-generated code.
- Use controlled identities and data. Create separate accounts, tenants, records, and roles so that a successful cross-boundary action is unambiguous.
- Observe an out-of-band effect. Confirm a captured callback, database or log side effect, authenticated response, file change, or other direct signal. A response that merely contains an error message is not proof of compromise.
- Repeat and vary conditions. Re-run the case with valid and invalid roles, missing objects, boundary values, and alternate routes to determine whether the behavior is consistent and what preconditions actually matter.
- Have another person reproduce it. A second reviewer should receive the written procedure and obtain the same observable result without coaching from the discovering agent.
Keep the harness and captured output with the finding. If the effect cannot be reproduced, record the exact environment and attempts; mark the claim as unsupported rather than silently deleting it.
How should I judge design intent and severity?
After reproducing the behavior, compare it with the documented security boundary and the demonstrated impact. Severity should describe what an attacker can actually achieve under stated preconditions, not the most dramatic scenario suggested by the model.
Questions to resolve
- Is the behavior documented, deliberately public, or required for a supported workflow?
- Does the attacker cross an authorization, tenant, confidentiality, integrity, or availability boundary?
- What privileges, user interaction, network access, or special configuration are required?
- What is the worst demonstrated result, and is it repeatable?
- Does the assigned severity match the evidence, or does it assume an unproved escalation?
Unsupported Critical or High ratings should be downgraded or flagged for further review. Do not inflate a finding because the code pattern resembles a well-known weakness; classify the reachable impact in this application.
How do I classify a finding before remediation?
Use a small, explicit status vocabulary so engineering queues distinguish confirmed work from uncertain observations.
| Status | Use it when | Queue treatment |
|---|---|---|
| VERIFIED | The source path, threat model, violated requirement, and observable impact are independently reproduced. | Create a remediation task with evidence, scope, calibrated severity, and an owner. |
| FLAGGED | The behavior is plausible, but a precondition, environment detail, or impact remains unconfirmed. | Keep it in a review queue with a specific question and an expiry or follow-up owner. |
| REJECTED | The behavior is intended, unreachable, outside the threat model, or disproved by reproduction. | Keep the evidence and reason for auditability, but remove it from the remediation queue. |
Record who made the classification, which evidence was considered, and when it was made. This prevents a rejected or speculative report from being reintroduced later as if it were a confirmed defect.
How should I prepare an AI-proposed fix safely?
Make the smallest change that enforces the verified requirement, and make it somewhere you can discard or reset. An isolated branch, disposable clone, or test environment protects production while the change is being evaluated.

Constrain the change
- State the exact invariant the patch must enforce, such as “a user may read records only within their tenant.”
- Prefer a narrow authorization, validation, encoding, or configuration change over broad refactoring.
- Pin or approve dependencies explicitly; do not accept an AI-suggested package merely because it makes the code compile.
- Separate generated code from hand-written changes so reviewers can see what the agent introduced.
Review the complete diff
Inspect application code, lockfiles, manifests, migrations, configuration, generated files, fixtures, and tests. Search for unrelated edits, new network calls, credential handling, disabled checks, changed defaults, and altered error behavior. OWASP guidance specifically warns that routine-looking AI changes can add risky packages or manipulate tests.
Audit the tests themselves
Look for deleted test cases, weaker assertions, broad snapshots that no longer check security properties, and mocks that replace the unit under test. A test suite can pass after an agent has removed the assertion that would have failed; passing output is meaningful only when the tests still exercise the real boundary.
The Tool Desk
Outbyte PC Repair FREEClear out junk files and repair common Windows errorsFree Scan →Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →What post-fix checks must I run?
A fix is not verified until the original behavior is gone and the surrounding system remains correct. Run checks independently of the agent that proposed the patch.
- Re-run the original detector. Confirm that the reported path no longer triggers, and investigate if the tool now reports a different location or variant.
- Replay the original exploit. The prior unauthorized action should fail with the expected status or error, while the legitimate workflow should still succeed.
- Run unit and integration tests. Include authorization, validation, serialization, database, and middleware coverage around the changed path.
- Run regression and end-to-end tests. Exercise adjacent roles, tenants, API versions, background jobs, and failure handling.
- Add negative and boundary cases. Test missing credentials, wrong tenant identifiers, expired sessions, malformed input, empty collections, maximum sizes, and alternate encodings.
- Use complementary security checks. Apply static analysis, dependency and service review, black-box or web-application scanning where appropriate, and fuzzing for parsers or input boundaries. These techniques cover different failure modes; none is a complete substitute for the others.
- Inspect the resulting output. Check for new alerts, syntax or type errors, changed snapshots, deleted tests, reduced assertion counts, unexpected warnings, performance regressions, and behavior outside the requested scope.
GitHub describes automated findings as advisory, and its Autofix guidance emphasizes reviewing the diff and rerunning tests rather than accepting a green result as proof. The same discipline applies to any AI-generated patch.
How do merge gates and separation of duties reduce risk?
Do not let the discovering agent author, select, and approve the evidence used to validate its own report. Independent verification is a control against confirmation bias and hidden assumptions.
- Require qualified human review for every security-related finding and proposed fix.
- Use two-person review or security-team sign-off for authentication, authorization, cryptography, sandboxing, secrets, and other security-critical files.
- Block merges for Critical findings unless an authorized human records a written exception, scope, compensating controls, and expiration.
- Make the merge request include the finding record, reproduction steps, threat model, test results, diff review, and classification.
- Ensure the verifier can inspect the original evidence and the post-fix run without relying on hidden agent context.
These gates should be enforced by repository or pipeline policy where possible, not by an instruction in an AI prompt that can be forgotten or bypassed.

How do manual review, conventional scanners, and AI-assisted verification differ?
No single approach supplies all of the evidence needed for a security decision. The practical choice is a layered process in which each method compensates for another’s blind spots.
| Approach | Evidence quality | Independence | Reproducibility | Threat-model coverage | Typical weakness |
|---|---|---|---|---|---|
| Manual review | Can connect design, source, and business rules; depends on reviewer notes. | High when a separate reviewer performs the analysis. | Usually good if the reviewer writes a harness; may be inconsistent. | Strong for intended use and authorization context. | Cost and latency; quality varies with expertise. |
| Conventional scanners | Repeatable alerts and locations; often limited exploit evidence. | Independent of an AI discovering agent. | High for deterministic rules and reruns. | Broad pattern coverage, weaker business-logic understanding. | False positives, missed runtime context, and severity that needs calibration. |
| AI-assisted verification | Can assemble flows and generate harnesses, but narrative may exceed evidence. | Low if the same agent discovers and validates its own claim; higher with a fresh verifier. | Variable until a minimal, replayable test and out-of-band effect exist. | Potentially broad, but dependent on supplied context and model assumptions. | Confident hallucinations, omitted preconditions, unsafe fixes, and test weakening. |
Use the combination that fits the risk: deterministic scanners and tests for repeatability, human threat modeling for intent, and AI for acceleration only when its output is independently checked.
How can recurring findings become permanent guardrails?
When the same class of issue appears repeatedly, convert the lesson into a control that works without an AI interpretation. Examples include a custom static-analysis rule for missing authorization, a policy-as-code check that rejects public storage, a regression test for tenant isolation, or a pipeline gate that requires security approval for sensitive paths.
- Define the rule in terms of an observable requirement and a clear failure condition.
- Keep a test fixture that proves the rule catches the original bug and accepts the intended design.
- Run the check in pull requests and scheduled builds, independently of any agent session.
- Version the rule, document exceptions, and link each exception to evidence and an approving person.
- Review false positives and false negatives periodically; a guardrail that teams routinely bypass is not effective.
OWASP SAMM’s position is that AI may run checks on its own output, but that does not replace independent verification. Deterministic controls provide the durable enforcement; humans still decide whether a design exception is justified.
What mistakes most often invalidate verification?
- Trusting fluency: polished reasoning is treated as proof even when no source path or effect is shown.
- Replaying against production: a risky test causes real data changes or contacts an external target.
- Skipping intent analysis: documented public behavior is reported as an access-control flaw.
- Accepting a clean SAST result: a pattern scan is mistaken for coverage of authorization or business logic.
- Letting one agent control the loop: the discoverer writes the exploit, edits the tests, and approves the patch.
- Reviewing only the edited function: lockfiles, configuration, generated code, and deleted tests are ignored.
- Repeating an unsupported severity: “Critical” is copied from the report despite unproved privileges or impact.
- Discarding rejected reports: the reason for rejection is lost, so the same claim returns without context.
What is a practical verification checklist?
Use this checklist in the issue or pull request so every reviewer can see which controls were completed.
- Finding context, revision, timestamps, tool versions, and raw artifacts are preserved.
- Component, entry points, actors, trust boundaries, intended behavior, and attacker starting privileges are documented.
- Exact source locations, data flow, attack sequence, violated requirement, and demonstrated impact are stated.
- A separate verifier reproduced the claim in an authorized non-production environment.
- An out-of-band signal confirms the alleged effect, or the item is marked unsupported.
- Severity reflects demonstrated impact and realistic preconditions.
- Status is VERIFIED, FLAGGED, or REJECTED, with a named reviewer and rationale.
- The fix is isolated, minimal, dependency changes are approved, and the complete diff is reviewed.
- Original scanner, exploit replay, unit, integration, regression, security, negative, and boundary tests have run.
- Deleted tests, weakened assertions, new alerts, syntax errors, and out-of-scope behavior have been checked.
- Required security or two-person approvals and any written exception are recorded before merge.
- Recurring lessons are encoded as deterministic rules, tests, policy checks, or pipeline gates.
The acceptance criterion is reproducible evidence and a safe, reviewed change—not the confidence of the model that first raised the alert.

Frequently Asked Questions
Can an AI agent verify its own security finding?
It can perform useful follow-up analysis, but its result should remain advisory when it shares the original context. Give the evidence to an independent reviewer or verifier and require a separate reproduction before marking the finding verified.
What should I do when the vulnerability cannot be reproduced?
Preserve the original artifacts, record the exact revision and environment you tested, and mark the item flagged or rejected with a reason. Ask for the missing precondition or a minimal fixture instead of silently patching code based on an unrepeatable claim.
Is a passing security test enough to merge an AI-generated fix?
No. Check that the test still exercises the real component, that assertions were not weakened, and that the original exploit fails while legitimate behavior remains. Review the full diff and run complementary checks for regressions and new alerts.
How can I verify an authorization finding across tenants?
Create separate test tenants with distinct records and identities, then attempt the operation using each role and tenant combination. Confirm both the denied response and the absence of side effects in the other tenant’s database, logs, or callbacks.
Where should rejected AI findings be kept?
Retain them in an auditable triage or security-evidence store with the source report, reproduction attempts, decision, reviewer, and date. Keep them out of the active remediation queue while preserving enough context to recognize a genuinely changed condition later.
