Use applied AI safely by treating it as a governed business process, not as a plug-in that is safe by default. Define the outcome and affected people, assign accountable owners, minimize and control the data, set meaningful human approval, test for accuracy, bias, privacy and security failures, then monitor the workflow and stop it when risk becomes unacceptable.
This approach works for a small company using an AI assistant to draft customer replies as well as for a larger organization using models in hiring, lending, healthcare, safety or access decisions. Controls should become stricter as the possible harm, data sensitivity, autonomy and irreversibility increase.

What does safe applied AI mean in a business workflow?
Applied AI is a model embedded in a real process: drafting text, recommending an action, supporting a decision or taking an action automatically. Safety means designing the surrounding workflow so that the model’s limits, the data it receives and the consequences of mistakes are controlled.
NIST’s AI Risk Management Framework (AI RMF 1.0), published January 26, 2023, is voluntary, rights-preserving, non-sector-specific and use-case agnostic. It describes trustworthy AI through several properties that should be considered together:
#1 Best Overall

| Trustworthy property | What to check in a workflow |
|---|---|
| Validity and reliability | Does the system produce accurate, repeatable results for the intended task and population? |
| Safety | Can an incorrect or unsafe output injure someone, cause loss or trigger a dangerous action? |
| Security and resilience | Can attackers manipulate inputs, extract information or disrupt the service, and does the process recover? |
| Accountability and transparency | Can the organization explain who approved the use, what the system did and who is responsible? |
| Explainability and interpretability | Can a reviewer understand the evidence and limitations well enough to challenge an output? |
| Privacy enhancement | Are personal and confidential data minimized, lawfully processed and protected throughout their life cycle? |
| Fairness with harmful bias managed | Does performance differ unfairly between groups, and are those differences investigated and addressed? |
For generative AI, NIST’s Generative AI Profile, NIST-AI-600-1, released July 26, 2024, adds controls for risks such as prompt injection, fabricated content and data leakage. Apply those controls to the particular use case rather than assuming that a general checklist proves safety.
How should we define an AI use before building or buying it?
Write the intended use in one sentence before selecting a model or vendor. The sentence should name the business outcome and who could be affected; vague goals such as “use AI to improve efficiency” are not testable or governable.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- State the outcome and boundaries. For example: “Suggest responses to routine support tickets for an employee to approve.” Record what the system must not do, such as issuing refunds or making eligibility decisions.
- Classify the level of autonomy. Label the workflow as drafting, recommendation, decision support or autonomous action. The label determines how much approval, testing and logging are required.
- Identify affected people and consequences. Include customers, employees, applicants, suppliers and bystanders, not only the person operating the tool. Describe the worst credible result of an error.
- Set prohibited uses. Put disallowed purposes in the workflow specification and vendor contract. A tool approved for summarizing internal notes should not silently be reused for hiring or credit decisions.
- Define acceptance criteria. Specify accuracy, refusal, response-time, privacy and security requirements before testing. Include conditions that require the workflow to be paused.
ISO/IEC 23894:2023 recommends integrating AI risk management into AI-related activities rather than treating it as a one-time review. Australian implementation guidance similarly emphasizes clear roles, an AI register, supply-chain accountability, testing, monitoring and records.
Who owns the risk and how should it be recorded?
Every workflow needs named people with authority to approve, operate, challenge and stop it. A practical ownership model includes:
- Business owner: accountable for the purpose, expected benefit and impact on customers or staff.
- Technical owner: responsible for configuration, integrations, model changes, reliability and rollback.
- Data or privacy owner: checks lawful processing, minimization, retention, access and rights handling.
- Security contact: assesses attack paths, secrets, identity, logging and incident response.
- Final approver: accepts residual risk and confirms that controls match the consequences.
Keep an AI register entry before launch. Record the purpose, intended users, affected people, data sources, model and vendor, prohibited uses, approval thresholds, test results, owner, escalation route, retention period and rollback or shutdown procedure. Keep a separate vendor record with contract terms, service dependencies, update practices and the information needed to investigate an incident.
Set a risk appetite in operational terms: what is the maximum consequence this workflow may impose without a human decision? A draft that an employee can edit has a different tolerance from an automated account closure, payment, employment action or safety intervention.
What data and permissions must we map?
Make a data-flow map from the first input to the final output. List every source, transformation, transfer and destination instead of assuming that a model prompt is the only data movement.

- Input fields and their source systems, including hidden metadata and uploaded files.
- Personal data, protected health information, payment data, trade secrets, credentials and other confidential material.
- Who may submit inputs, view prompts and outputs, change instructions or export results.
- Where data is processed, how long it is retained, and whether the provider uses it for model training or service improvement.
- Transfers to plug-ins, retrieval systems, agents, subcontractors and downstream business systems.
- Where outputs are stored, who receives them and whether they can trigger another system.
Minimize inputs to what the approved purpose needs. Remove secrets, direct identifiers and unnecessary history; use masking, aggregation or synthetic test data where those preserve the task. Review permissions for least privilege and separate the ability to view data from the ability to approve an action.
The UK Information Commissioner’s Office (ICO) says AI processing of personal data requires lawful, fair, transparent and accountable controls, with a risk-based approach and data protection by design and default. Apply the privacy assessment before deployment, not after a complaint. Document the lawful basis, notices, retention and process for handling access, correction or deletion requests where applicable.
How can we protect confidential data in ChatGPT or other AI tools?
Do not paste confidential business information into an unapproved generative-AI service. CMS guidance specifically warns against entering program data, protected health information, personally identifiable information, sensitive non-public information or intellectual property into an unapproved tool.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minute- Create an approved-tools list. Specify which accounts, workspaces and integrations employees may use, and what data classes each one permits.
- Check the contract and settings. Confirm retention, training use, subprocessors, geographic processing, deletion, breach notice and access controls. A “do not train” setting is useful but does not replace contractual and technical review.
- Use a data gateway where practical. Apply automated redaction, classification and policy checks before prompts leave company systems. Block credentials, payment numbers and other prohibited patterns.
- Control identity and export. Require individual accounts, strong authentication, role-based access and auditable exports. Do not share a single login or place unrestricted model output in a public channel.
- Give staff safe alternatives. Provide approved templates, anonymized examples and a clear route for questions so that policy does not push work into hidden “shadow AI.”
Even when a provider has strong security, the organization remains responsible for deciding whether a particular disclosure is lawful and proportionate. Treat prompts, retrieved documents, generated outputs and conversation history as records that may need retention and deletion controls.
When does a human need to review an AI decision?
Human oversight is meaningful only when the reviewer can understand the recommendation, examine relevant evidence, consider other factors, ask for more information and override the result without penalty. The ICO warns that reviewers must actively check a recommendation rather than routinely applying it; a nominal reviewer who always accepts the output can create an effectively solely automated decision.
Require explicit approval before an output can:
- affect employment, hiring, promotion, discipline or termination;
- determine lending, insurance, pricing, eligibility or access to an important service;
- influence healthcare, safety, safeguarding or legal outcomes;
- send an external commitment, make a payment, delete data or change an account irreversibly; or
- publish a statement that could create regulatory, contractual or reputational liability.
Give reviewers enough time, training and context to do the job. Show the source material, uncertainty and known limitations where those cues help, but never treat a confidence score as proof of correctness. Measure whether reviewers actually override, investigate and document outputs; a zero-override rate may indicate automation bias rather than perfect performance.
For agents and automations, add a pause button, transaction limits, rollback capability and an escalation route. Make the safe failure state explicit: for example, queue a ticket for manual handling instead of sending an unverified answer when the model or a required data source is unavailable.

How do we test an AI workflow for hallucinations, bias and security problems?
Test the complete workflow, including prompts, retrieval, permissions, human approvals and downstream actions. A model benchmark alone cannot show that the business process is safe.
- Build a representative test set. Include ordinary cases, rare cases, ambiguous inputs, different languages or formats used by customers, and cases involving each affected group.
- Check factual behavior. Measure unsupported claims, citation or source fidelity, omissions, inconsistent answers and refusal when required information is missing. For generative systems, label fabricated content and test whether the workflow prevents it from reaching a customer.
- Check fairness. Compare error rates, recommendations, refusal rates and processing times across relevant groups. Investigate disparate impact; do not rely on a single average score.
- Check privacy leakage. Try to recover training examples, another user’s data, hidden instructions or confidential retrieval content. Verify that logs and error messages do not expose sensitive information.
- Check adversarial behavior. Test prompt injection, malicious documents, data poisoning, instruction conflicts, unauthorized tool calls and attempts to bypass approval gates.
- Check access and resilience. Verify role boundaries, secret handling, logging, rate limits, latency, cost controls, provider outages and behavior when a dependency returns an error.
- Record the decision. Store test data versions, metrics, acceptance thresholds, failed cases, unresolved limitations and the person who approved release.
Repeat relevant tests after a model, prompt, retrieval source, tool, policy, data set or workflow change. Keep a small set of “known hard cases” for regression testing so that a fix in one area does not quietly break another.
What risks are specific to generative AI and AI agents?
Generative systems can produce plausible but false content, follow instructions hidden in a document, reveal information from context or be manipulated into using connected tools. NIST-AI-600-1 is intended to help organizations add controls for these risks to the general AI RMF process.
Prompt injection and instruction conflicts
Separate trusted instructions from untrusted content, label retrieved text as data, restrict tools by allow-list and require approval for sensitive actions. Test direct attacks in prompts and indirect attacks embedded in documents, web pages or support tickets.
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Fabricated or unverifiable content
Require source grounding for claims that matter, display the supporting record to a reviewer and route uncertain cases to manual handling. Do not let fluent wording substitute for evidence.
Data leakage and over-broad context
Limit retrieval to the user’s authorization, filter fields before context assembly and prevent one customer’s records from appearing in another customer’s answer. Review conversation history and cached context as carefully as the original prompt.
Unsafe autonomous actions
Keep agents in draft or simulation mode until action tests pass. Use narrow permissions, spending and volume limits, two-person approval for high-impact actions, and an immediate shutdown path.

How should we pilot and release an AI workflow?
Release in stages so that mistakes are observed before they become irreversible.

- Use shadow mode. Generate recommendations without showing them to customers or changing records, then compare them with expert work.
- Move to draft-only output. Let trained staff edit and approve every external message or transaction while you measure quality and overrides.
- Gate high-impact actions. Put explicit approval in front of communications, payments, deletions, employment actions, eligibility decisions and other irreversible operations.
- Expand only against written criteria. Increase volume or autonomy only after the owner confirms that acceptance thresholds, incident procedures and staffing remain adequate.
Publish an internal operating procedure covering permitted inputs, review steps, escalation, outage handling and records. Training should use realistic examples of hallucinations, data disclosure and prompt attacks rather than only a policy summary.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How do we monitor and improve the workflow after launch?
Monitoring should show whether the system remains useful, lawful and controlled as data, users and vendors change. Track quality samples, human overrides, incidents, complaints, near misses, drift, latency, cost and availability. Review whether the population or task has changed from the approved purpose.
Watch for model, prompt, retrieval, tool and vendor updates. Require change notices where possible, identify which changes trigger re-approval, and rerun regression, privacy and security tests before enabling them in production. Keep records of versions, approvals, incidents, corrective actions and the rationale for continuing or restricting use.
Suspend or limit the workflow when harm occurs, uncertainty becomes unacceptable, a control fails or the approved data conditions no longer hold. Preserve relevant logs, notify the responsible business, privacy and security contacts, assess affected people, correct or retract outputs where necessary, and document what must be fixed and tested before resuming.

A small organization can apply the same discipline with fewer forms. Use this checklist for each workflow:
- Purpose, affected people, autonomy level and prohibited uses are written down.
- Business, technical, privacy and security owners are named, with one final approver.
- The workflow and vendor appear in an AI register.
- Inputs, outputs, permissions, transfers, retention and training settings are mapped.
- Confidential, personal and regulated data are blocked or minimized.
- Human approval, override, escalation, pause and rollback paths are tested.
- Representative and edge-case tests cover accuracy, bias, privacy, prompt injection, access and outages.
- Acceptance criteria, unresolved limitations and release evidence are recorded.
- Production monitoring covers quality, overrides, incidents, complaints, drift and vendor changes.
- A documented stop-and-correct procedure exists, and staff know how to invoke it.
For a very small team, one person may hold more than one role, but the decisions and conflicts should still be documented. Independent review is especially important when the owner benefits from launching quickly or when the workflow affects workers, customers or vulnerable people.

How do different workflow designs compare by risk?
Choose controls according to consequence, reversibility, data sensitivity, autonomy, auditability, vendor dependence, security exposure and operating cost. The following patterns are practical starting points, not automatic approvals.
| Design | Typical consequence | Recommended controls | Suitable starting point |
|---|---|---|---|
| Drafting assistant | Usually reversible; an employee edits before use | Approved data classes, source checks, staff training, sampling and output logging | Internal summaries or first drafts with no automatic sending |
| Recommendation engine | Can steer a person toward an action | Evidence display, uncertainty cues, group-level testing, reviewer challenge and override metrics | Prioritizing tickets or suggesting next steps |
| Decision support | May affect rights, money, employment or access | Qualified human decision-maker, documented reasons, appeal or correction route, stricter validation and audit trail | Analyst support where the person decides independently |
| Autonomous action | Immediate or irreversible external effect | Narrow permissions, transaction limits, approval gates, simulation, real-time monitoring, rollback and shutdown | Only after lower-autonomy stages meet written criteria |
A low-impact drafting use still needs an owner, defined purpose, data controls, testing and monitoring. Higher impact does not merely require a better model; it requires stronger process controls and a clear person who remains accountable.
What should we do when an AI output causes harm?
Make stopping the workflow a normal control, not an admission of failure. First pause the affected automation or move it to draft-only mode. Preserve prompts, inputs, outputs, model and tool versions, reviewer actions and relevant access logs so the event can be reconstructed.
Notify the designated business, privacy and security contacts, determine who may have been affected, and meet any contractual or legal reporting duties. Correct the immediate result, inform affected people when appropriate, and investigate whether the cause was data quality, a model change, an unsafe instruction, an access failure or inadequate human review. Resume only after a fix has been tested against the triggering case and related edge cases, the owner has accepted residual risk and the decision has been recorded.
Which standards and guidance should inform an AI safety program?
Use NIST AI RMF 1.0 as a flexible, risk-based structure; add NIST-AI-600-1 for generative-AI failure modes. ISO/IEC 23894:2023 supports integrating risk management into ordinary AI activities. The ICO’s guidance is relevant when personal data is processed, and the UK Code of Practice for the Cyber Security of AI calls for threat evaluation, documentation of data, models and prompts, testing and evaluation, monitoring, security updates and proper disposal.
These sources do not replace sector rules, contracts or professional judgment. They provide a vocabulary and a repeatable life cycle: govern and assign ownership, map purpose and harms, control data and permissions, test, release with approval, monitor, document and improve.
Frequently Asked Questions
Can employees use a public AI chatbot for harmless internal drafts?
Only if the organization has approved that service and the input falls within its permitted data class. “Harmless” text can still contain customer identifiers, confidential context or instructions that reveal internal operations, so use anonymized material and the approved account rather than a personal login.
How should we handle employees who already use unapproved AI tools?
Treat this as a discoverable operating risk, not solely a disciplinary issue. Publish a short approved-use policy, provide a workable alternative, review network or identity controls where appropriate, and invite confidential reporting so risky uses can be assessed and migrated.
What logs are important when the model is hosted by a vendor?
Retain a request identifier, timestamp, user or service identity, relevant model and prompt versions, tools called, output, reviewer action and resulting business action. Contract for access to the records needed to investigate incidents, while applying your own retention and redaction rules.
When should an AI workflow be retired rather than improved?
Retire it when the approved purpose disappears, the data or legal basis changes, the provider can no longer meet security or audit requirements, or repeated testing shows that harm cannot be reduced to an acceptable level. Revoke access, preserve required records, notify users and document the replacement or manual process.
