During an industrial cyber incident, protect people and the physical process before protecting data or restoring ordinary IT service. Activate an incident-response plan that includes operations, control engineers, safety, maintenance, IT, security, legal, communications, vendors and leadership. Establish what changed, which assets and process functions are affected, and whether the event threatens safety, environmental controls, product quality or continuity of the service being delivered. Contain through operations-approved measures, preserve evidence without destabilizing equipment, remove the cause, then recover from known-good states and validate control logic, instrumentation, alarms, communications and safety functions before declaring the plant recovered.
This approach reflects the way NIST defines operational technology (OT): programmable systems and devices that interact with the physical environment. OT security must account for performance, reliability and safety, not only confidentiality and data loss.

What should I do first during an OT incident?
The first objective is a safe, controlled response—not the fastest possible disconnection. Use the following immediate sequence while keeping the process under qualified operational supervision.
- Protect people and the process. Check for abnormal equipment behavior, unsafe readings, environmental releases, loss of interlocks or inability to control the process. Follow emergency operating procedures if safety is in doubt.
- Activate the OT-capable response team. Bring in the control-room lead, engineering, safety, maintenance, IT/security, management and any required vendor or external responder. Give one person authority to approve containment actions.
- Stop uncoordinated changes. Pause routine maintenance, patching, remote troubleshooting and configuration edits that could destroy evidence or alter process behavior. Continue only actions required for immediate safety.
- Establish an out-of-band channel. Use a communication method that does not depend on potentially compromised email, chat or phone systems. Record who is authorized to issue operational instructions.
- Describe the event in process terms. Record affected units, controllers, HMIs, engineering workstations, historians, safety systems, remote-access paths and IT/OT links. Note alarms, set-point changes, controller modes, loss of visibility and physical observations.
- Start a time-stamped incident log. Capture symptoms, decisions, user actions, network changes, containment steps and their observed process effects. This log supports safe handoffs, evidence handling and later recovery decisions.
Do not reboot, power off or unplug a controller, switch, engineering workstation or safety device simply because it appears suspicious. First determine what physical function it controls, what happens when communications are lost, and whether the action is reversible.

How should an industrial facility prepare before an incident?
Preparation determines whether responders can act safely when information is incomplete. A usable plan describes authority, technical steps and process hold points in language that operators can follow under pressure.
Maintain an OT asset and dependency inventory
Keep current records for control networks, safety systems, programmable controllers, HMIs, engineering workstations, historians, servers, network devices, remote-access routes, vendor connections and every IT/OT interconnection. Include ownership, location, firmware and software versions, backups, dependencies, normal operating modes and the physical process each asset influences. Architecture diagrams should show trust boundaries and paths between business, supervisory and control environments.
Define classifications and decision authority
Set incident categories that distinguish a suspicious endpoint from a loss of control, a safety-system event or an environmental threat. Identify who can approve network isolation, a controlled operating mode, a planned shutdown, restoration from backup and return to service. Make safety and operations authority explicit; security personnel should not make process-changing decisions without the designated operational approver.
Prepare communications and evidence procedures
Document out-of-band contacts, escalation paths, vendor numbers, legal and communications contacts, and alternates for each role. Specify how volatile data, logs, malware samples, controller configurations and photographs are collected, time-stamped, stored and transferred. Use chain-of-custody records and write-protected or otherwise controlled evidence storage.
Exercise the plan with the whole operating community
Run exercises involving operations, engineering, safety, maintenance, IT, security, legal, communications, vendors and leadership. Test scenarios such as ransomware on an engineering workstation, a compromised remote-access account, historian loss or unexplained controller-mode changes. Exercises should expose missing diagrams, unclear authority, unavailable backups and unsafe assumptions about what fails safe.
CISA’s ICS recommendations emphasize incident-response capability, cyber-forensics planning, defense in depth, remote-access controls and patch management. Treat those capabilities as routine operating requirements rather than emergency-only measures.

How do I detect and triage a possible industrial cyberattack?
Triage combines cyber indicators with process behavior. A single technical symptom can have a benign explanation, but a pattern spanning accounts, logs, network traffic and physical operations warrants escalation.
Technical indicators that deserve investigation
- Unusual network traffic or connections between zones that do not normally communicate.
- A sharp reduction in available disk space, unexpectedly high CPU use or rapidly growing logs.
- New accounts, administrator-account activity, locked accounts or accounts becoming active outside their normal role.
- Cleared logs, missing audit records or unusually full logs.
- Unexpected software, scripts, remote tools or changes to endpoint security controls.
Process and control indicators
- Set-point changes, controller mode changes or unexplained operator actions.
- Historian gaps that do not match a planned maintenance window.
- Safety-system events, unexplained alarm floods or loss of alarm annunciation.
- Control-room displays that disagree with field readings or physical observations.
- Unexpected loss of remote visibility, communications or engineering access.
Correlate each cyber indicator with time, operator activity and process state. Ask whether a maintenance job, automatic log rotation, capacity problem or planned configuration change explains it. Escalate when several systems show related anomalies, when process behavior is unexplained, or when safety, environmental control, product quality or service continuity may be affected.
Should I shut down the plant or isolate the network?
There is no universal shutdown rule. Choose the least disruptive action that keeps the process safe, and have operations and safety authority approve it after assessing what the affected systems control and how the process behaves when communications or control are lost.
Before disconnecting a switch, controller, engineering workstation, remote-access path or plant segment, answer these questions:
Recommended Free Tools
- What physical process, equipment or safety function does the asset control or monitor?
- Does the process fail safe, fail dangerous or enter an unknown state when the asset is isolated?
- Can trained personnel maintain stable manual control, and for how long?
- Will isolation remove alarms, interlocks, historian data or operator visibility?
- Could the action trigger a trip, loss of containment, environmental release, equipment damage or service outage?
- Can the action be reversed, and what evidence will be lost?
- Are vendor support, regulatory notification or management-of-change approvals required?
NIST’s ICS guidance states: “Response should be measured first and foremost against the ‘service being provided,’ not just the system that was compromised.” A technically clean isolation can therefore create greater operational risk than a controlled, monitored continuation.
Containment options and their trade-offs
| Option | When it may fit | Primary safety and process concern | Evidence and recovery considerations |
|---|---|---|---|
| Disable a compromised remote-access route | Suspected vendor, VPN or remote-support path is being abused and local control remains stable | Loss of legitimate remote support or visibility | Usually reversible; preserve access logs and session records before disabling when safe |
| Block malicious traffic at a segmentation boundary | Known hostile connections can be separated without interrupting required control traffic | Incorrect rules may block essential control, alarm or historian communications | Record rules and timestamps; validate affected flows after the change |
| Move affected systems to a controlled mode | Operators can maintain the process with approved manual or local procedures | Higher workload, reduced automation and possible loss of interlocks or visibility | Document the mode change and retain the running system for forensics if safe |
| Preserve a running system for forensics | Evidence is volatile and the process is stable enough to continue | Malicious activity may persist while investigators collect data | Requires close monitoring, isolation boundaries and a clear escalation trigger |
| Execute a planned shutdown | Safety, environmental or equipment risk cannot be controlled while operating | Service interruption, equipment stress and restart hazards | Use approved shutdown and restart procedures; capture state before power changes when safe |
Document the expected process impact before each action and record the actual result afterward. If facts change, reconvene the operational decision-maker rather than allowing an improvised technical response to continue.

How do I contain the incident while preserving evidence?
Containment should limit attacker access and prevent spread without destroying information needed to understand the event. Assign one person to maintain the incident timeline and another to coordinate evidence handling.
Capture the state before changing it
Record controller and HMI states, active users, administrator actions, network connections, alarms, set points, operating modes, historian status and relevant physical readings. Preserve volatile data when this can be done safely. Note the exact time zone and clock source used by each system because inconsistent time can make event correlation unreliable.

Control evidence handling
Use approved collection procedures, chain-of-custody forms and write-protected or otherwise controlled storage. Hashing, imaging or malware collection should follow the site’s forensic procedure and the equipment manufacturer’s instructions. Keep originals protected and perform analysis on working copies.
Coordinate before touching proprietary systems
Consult equipment vendors and external responders before altering firmware, proprietary databases, controller memory or specialized engineering tools. A well-intentioned cleanup can remove diagnostic data, invalidate support or leave the controller in an unapproved state.
Apply the smallest safe containment measure
Prefer a narrowly scoped block, account disablement or remote-access change over a plant-wide disconnection when the process remains safe. Verify that the measure actually stops the suspected path, and monitor for loss of control, alarms, communications or other unexpected effects.
How do I eradicate malware and remediate the cause?
Eradication starts with understanding initial access and persistence, not simply deleting a visible file. Keep operations informed because remediation can change system behavior or availability.

Identify and close the entry path
Determine whether access came through remote support, an exposed service, a compromised account, removable media, a workstation or an IT/OT connection. Remove unauthorized persistence, malicious accounts and tooling, then close or restrict the exploited route. Rotate credentials that may have been exposed, including service and remote-access credentials, using a controlled sequence that does not strand operators.
Rebuild affected systems with tested procedures
Use approved remediation steps for endpoints, servers, HMIs and engineering workstations. Validate backups and golden images before deployment; a backup is not known-good merely because it completed successfully. Preserve required configurations, licenses and vendor-specific dependencies before reimaging.
Patch through management of change
Patch or upgrade only through the site’s maintenance and management-of-change process. Check compatibility, test in an appropriate environment, schedule the work with operations and obtain vendor support where required. CISA emphasizes assessing ICS vulnerabilities and implementing protective measures as part of capability development; emergency patching that bypasses process controls can create a different operational hazard.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How do I recover an ICS safely after ransomware or malware?
Recover in controlled stages, restoring the least risky services first and the process-critical control functions only after their dependencies are understood. Keep a rollback plan and heightened monitoring throughout restart.

List systems that were affected, systems that were isolated, and systems that remain trusted. Verify that the suspected access path is closed before reconnecting restored assets.
- Restore supporting services first. Bring back the infrastructure required for authentication, time, engineering tools, historians or communications according to the site’s approved dependency order. Do not reconnect a restored service merely because it is technically online.
- Verify integrity. Check firmware, operating-system images, application versions, controller logic, configurations and security settings against approved baselines. Investigate any unexplained difference.
- Validate control and safety functions. Test controller modes, set points, instrumentation, alarms, interlocks, safety functions, communications and historian recording. Include field confirmation where displays alone cannot prove the process is correct.
- Restart in stages. Use an approved sequence with clear hold points, assigned observers and criteria for stopping or rolling back. Watch process values, alarm behavior, network traffic and user activity during each stage.
- Confirm the service, not just the computers. Recovery is incomplete until the physical process is stable and operating within approved limits, product quality is acceptable and the required service can be delivered.
Do not declare recovery complete solely because business IT systems are available. An online server, HMI or historian does not prove that control logic, instrumentation, safety functions or operating limits are correct.
What should happen after the incident?
Hold a blameless after-action review while the timeline and decisions are still fresh. Examine detection time, decision points, safety and environmental effects, evidence quality, communications, vendor support, restoration assumptions and control gaps.
Best Value
Turn findings into assigned changes with owners and dates. Update architecture diagrams, inventories, playbooks, training, segmentation, monitoring, backups and exercises. Re-test restoration and remote-access procedures after material changes. CISA describes the capability as learning from previous incidents, preparing a plan, assessing vulnerabilities, detecting and containing incidents, removing the threat and restoring normal operations; the review should cover each of those capabilities.
Which standards and guidance apply to industrial incident response?
Use OT-specific guidance for the physical-process and control-system context, and general incident-response guidance for organization-wide coordination. ISA/IEC 62443 adds requirements and processes for electronically secure industrial automation and control systems across sectors such as energy, manufacturing, transportation and process industries.
| Guidance | Scope | Status and date | How to use it |
|---|---|---|---|
| NIST SP 800-82 Rev. 3 | OT security, including performance, reliability and safety considerations | Final, published September 2023 | Use as the current finalized NIST OT security guide for architecture, safeguards and response planning |
| NIST SP 800-82 Rev. 4 | Draft OT security guidance reorganized around Cybersecurity Framework 2.0, with expanded asset management, monitoring and detection, zero-trust and sector coverage | Initial public draft published September 21, 2026; comments due November 30, 2026 | Treat as a draft for awareness and comment, not as a final requirement |
| NIST SP 800-61 Rev. 3 | General incident-response recommendations integrated with Cybersecurity Framework 2.0 risk management | Final, published April 2025 | Use for enterprise response governance, coordination and lifecycle activities, adapting procedures to OT safety and availability needs |
| ISA/IEC 62443 | Requirements and processes for secure industrial automation and control systems | Standards family | Use to align governance, lifecycle responsibilities and security practices between operations and information technology |
Standards do not replace site-specific operating procedures. Translate them into approved hold points, authority matrices, restoration criteria and exercises for the particular process and jurisdiction.
What common response mistakes make an industrial incident worse?
- Disconnecting first and asking questions later: Isolation can remove control, alarms or visibility and may trigger an unsafe state.
- Treating OT like an office network: Reimaging, rebooting or aggressive scanning can interrupt deterministic control or destroy volatile evidence.
- Letting security act without operations: Technical containment must be approved by the people responsible for process safety and service continuity.
- Assuming a clean backup is a safe backup: Validate images, controller logic, configurations and dependencies before restoration.
- Ignoring physical observations: A normal dashboard does not disprove a field problem, and a cyber alert does not explain every process alarm.
- Restoring IT and stopping there: Confirm the physical process, safety functions, alarms, interlocks, communications and product or service output.
- Failing to record decisions: Without times, owners, expected effects and actual outcomes, responders cannot safely hand off work or learn from it.
What should an OT incident-response checklist contain?
A practical checklist should be short enough for the control room and detailed enough for the response team. Include these items:
- Current OT asset inventory and architecture diagrams.
- Incident classifications and named operational decision authority.
- Out-of-band communications and escalation contacts.
- Safety, environmental and service-impact questions for containment.
- Approved evidence collection, chain-of-custody and storage procedures.
- Vendor contacts and rules for proprietary equipment or firmware.
- Known-good backup and golden-image validation steps.
- Management-of-change and maintenance requirements for remediation.
- Recovery order, validation tests, rollback triggers and restart hold points.
- After-action review method and a process for tracking improvements.
Frequently Asked Questions
Can an IT incident-response plan be used for an industrial control system?
It can provide useful governance, communications and evidence practices, but it cannot stand alone. OT procedures must add process-safety authority, physical consequences, manual-control limits, equipment dependencies and staged restart tests.
Should I turn off a suspected infected PLC immediately?
Only when the responsible operations and safety authority determine that power removal is safer than continued operation. First establish what the PLC controls, what happens when it stops, whether manual control exists and what evidence can be captured without destabilizing the process.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
How long should heightened monitoring continue after recovery?
There is no universal duration. Keep enhanced monitoring until the incident owner confirms that access paths are closed, restored systems match approved baselines, process behavior is stable and the documented recovery exit criteria have been met.
What if the facility cannot prove that any backup is clean?
Do not treat an unverified image as a recovery source. Isolate it for validation, involve the relevant vendor or qualified responder, and use the approved rebuild or restoration path while maintaining the safest available operating mode.
