Reduce AI-generated rework by treating code generation as a controlled loop, not a one-shot request. Give the model a precise outcome, repository context, constraints, examples, acceptance criteria and edge cases; ask for assumptions and a test plan before it writes a patch. Then review in layers: run objective checks, verify the change against the ticket and architecture, inspect maintainability and dependencies, look for AI-specific mistakes, and obtain qualified human approval before production. This approach shifts errors toward the cheaper stages of development without pretending that any prompt guarantees a correct first draft.
What causes AI-generated code to need rework?
Most rewrites start with an underspecified request or an incomplete understanding of the codebase. A model can produce syntactically polished code while solving the wrong problem, assuming an API that does not exist, violating a security rule, or overlooking an edge case that was obvious to the team.

“Add retries” does not say which operations are safe to retry, how many attempts are allowed, what backoff to use, or how callers observe failure. Without a definition of done, the model optimizes for a plausible implementation rather than your actual product behavior.
Missing repository context
Generated code is only as correct as the interfaces, conventions and documentation it can see. If the prompt omits the relevant README, API contract, data model, recent migration or existing middleware, the model may invent a parallel pattern or call a nonexistent function.

Hidden constraints and edge cases
Language and framework versions, latency budgets, licensing rules, privacy requirements and files that must not change all affect a valid solution. So do failure examples: a normal input rarely exposes the null, duplicate, timeout, permission or partial-write case that later forces a rewrite.
False confidence
Generated lines often look equally certain even when their risks differ. JetBrains Research and Lund University’s 2026 study, based on 17 practitioners and 43 software professionals, identifies trust calibration as the central review problem: reviewers need risk and confidence information where they are focusing, rather than relying on polished appearance.
What should a prompt contain before the model writes code?
A useful prompt is a compact engineering brief. It defines the outcome, supplies trusted context, states boundaries, demonstrates behavior, requests a plan first, and specifies the required output. These elements prevent misunderstandings before they become code.
1. State the outcome and acceptance criteria
Describe the user or system result, not merely the mechanism. Add observable acceptance criteria such as response codes, persistence behavior, performance limits, compatibility requirements and what “done” means. If the request is to change an endpoint, identify the caller-visible behavior that must remain unchanged.
2. Provide grounded context and trusted sources
Attach or quote the relevant README sections, interfaces, API contracts, configuration, existing implementations and recent changes. Tell the model which materials are authoritative when they conflict, and identify the files or modules that own the behavior. Context should be focused enough to be read completely; a large, unfiltered dump can hide the important rule.
3. Make boundaries explicit
Name the language, framework and runtime versions; permitted APIs; security and data-handling rules; performance or availability targets; dependency policy; and files that must not change. State whether schema changes, public response changes, logging changes or migrations are prohibited. Constraints that exist only in a reviewer’s head will not reliably reach the first draft.

4. Show representative examples
Include normal inputs and expected outputs, plus at least one failure example. Examples clarify formatting, validation order, error semantics and idempotency better than abstract adjectives such as “robust.” Include a case that previously caused a defect when one is known.

5. Ask for assumptions, a plan and tests first
Have the model list ambiguities, assumptions, a short implementation plan, threat cases and a test plan before producing a patch. This creates a checkpoint where you can correct a wrong interpretation cheaply. OpenAI guidance emphasizes role and workflow definition, structured tool use, testing and iterative validation; Microsoft guidance similarly stresses clear instructions, decomposition, output structure and grounded context, while noting that techniques vary by model and that some prompting patterns are unsuitable for reasoning models.
6. Define an output contract
Require a predictable response such as: assumptions; plan; files to change; smallest patch; tests added or updated; commands to run; expected results; and unresolved questions. A fixed structure makes omissions visible and lets a reviewer compare the answer with the request without hunting through prose.

How do I move from a prompt to a reviewable patch?
Use a staged exchange rather than asking for a complete feature in one turn. Each stage narrows uncertainty and leaves an artifact a human can inspect.
- Brief the model. Supply the outcome, context, boundaries, examples and output contract.
- Review the assumptions and plan. Correct wrong files, misunderstood business rules or missing threat cases before implementation.
- Request a minimal change. Ask for only the files and logic required by the accepted plan. Smaller patches are easier to test, review and revert.
- Require tests with the change. Ask for tests that exercise acceptance criteria and failure cases, not only the happy path. The model should explain what each test proves.
- Run the repository’s checks locally or in CI. Compile or build, run unit and integration tests, lint, type-check, dependency checks and security or static-analysis tools used by the project.
- Return failures to the model as evidence. Paste the exact failing test or diagnostic and ask for an explanation and a narrow fix. Do not let the model silently weaken or delete a failing test.
- Open a traceable change for human review. Include the original intent, generated or edited files, checks run, results and unresolved risks.
Which review steps catch the most expensive rework?
Review in a deliberate order: objective evidence first, then intent and architecture, then maintainability and dependencies, then AI-specific hazards, and finally human approval proportional to risk.
1. Run objective checks first
Confirm that the code builds or compiles, unit and integration tests pass, lint and type checks are clean, and warnings or errors have been investigated. Run the project’s dependency and security scanners and static-analysis rules; GitHub’s review guidance cites CodeQL, Dependabot and code-quality checks as examples. A green pipeline does not prove the feature is right, but a failing check gives a concrete defect to resolve before a subjective review.
2. Verify context and intent
Compare the patch with the ticket, architecture documentation, README, API contracts and established patterns. Ask whether it solves the intended user problem, preserves required behavior and avoids assumptions about business rules or user behavior. This is where a technically valid implementation that addresses the wrong requirement is rejected.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problems3. Inspect maintainability and dependencies
Check naming, structure, modularity, documentation and error handling. Look for code that is harder to maintain than the original change would be to rewrite. Verify that every new dependency exists, is maintained, appropriately licensed and actually needed; generated manifests can contain misspelled, obsolete or unnecessary packages.
4. Hunt AI-specific failure modes
- Hallucinated APIs, methods, configuration keys or version features.
- Constraints that were acknowledged in prose but ignored in code.
- Incorrect boundary, concurrency, retry, authorization or validation logic.
- Tests skipped, weakened, deleted or made meaningless by broad mocks.
- Insecure defaults, leaked secrets, unsafe deserialization or excessive logging.
- Polished code whose behavior does not match the ticket or domain model.
5. Obtain qualified human approval
Security, authentication, data migration, money movement, privacy and production-infrastructure changes need a reviewer with the relevant expertise, not merely a passing pipeline. UK Home Office Engineering Guidance and Standards states that “AI-assisted outputs MUST be reviewed and approved by a human before reaching production” and that AI tools cannot replace human judgment, understanding, ownership or responsibility.
How should automation and human judgment be divided?
Automation is best for repeatable, objective checks; people must decide intent, architecture, risk and acceptable trade-offs. Treat the boundary as a design choice rather than assuming that more automation means more assurance.

| Decision area | Automation is good at | Human reviewers must decide |
|---|---|---|
| Build and tests | Repeating compilation, unit and integration suites and reporting failures | Whether the tests represent the real acceptance criteria and important failure modes |
| Static analysis and security | Finding known patterns, tainted flows, policy violations and dependency alerts | Whether a flagged path is exploitable in this system and which mitigation fits the threat model |
| Formatting and style | Applying deterministic lint and formatting rules | Whether a design is understandable, modular and consistent with local architecture |
| Intent and product behavior | Checking machine-readable contracts and snapshots | Confirming that the change solves the user problem and preserves business semantics |
| Approval | Enforcing required status checks and reviewer counts | Accepting residual risk and taking ownership of the release decision |
OWASP SAMM recommends explicit governance and controls because AI can generate code, tests, configuration and infrastructure at a speed and volume existing processes were not designed to absorb. Keep secrets and signing keys away from AI tools, enforce static-analysis and architectural rules in the build, and verify dependencies and misuse cases.

How deep should review be for a small change versus a high-risk change?
Increase review depth and approver expertise as the impact of failure rises. A low-risk, isolated presentation change can rely on a narrow test and owner review; a change that can move money or expose personal data requires layered checks and specialist approval.
| Change profile | Minimum practical review | Escalate when |
|---|---|---|
| Small, isolated, reversible change | Focused prompt context, targeted tests, lint or type checks and an owner review | The patch touches shared interfaces, persistence or authentication |
| Shared service or public API | Contract tests, integration coverage, dependency and security scans, architecture review and a second reviewer | Backward compatibility, rollout or migration behavior is uncertain |
| Security, privacy, money, migration or production infrastructure | Threat-focused tests, static analysis, dependency verification, staged rollout or rollback plan and qualified peer approval | Any unresolved risk, data exposure possibility or irreversible operation remains |
Risk is not determined by diff size alone. A five-line permission change can deserve more scrutiny than a large isolated refactor.
How do I make AI-assisted changes traceable and auditable?
Use the same engineering records as any other change, with enough extra context to show where AI assistance entered the workflow. Mark the pull request or commit according to team policy, preserve the prompt or design context when policy and privacy rules allow, record checks and results, and document review decisions.
Traceability helps investigators distinguish a model mistake from a process gap and lets teams improve prompts and guardrails. The UK Home Office guidance requires AI-assisted changes to be visible and auditable through normal engineering processes; it does not transfer accountability to the tool.
How can a team measure whether rework is actually falling?
Do not claim a universal percentage improvement from a particular prompt format: no authoritative source establishes one. Measure your own workflow over comparable changes and treat the results as signals, not industry benchmarks.
- Failed or missing tests discovered after generation.
- Static-analysis, security and dependency findings per change.
- Number and severity of review comments.
- Pull requests reopened after “ready” status.
- Defects traced to generated code after merge or release.
- Security alerts and dependency corrections.
- Elapsed time from generated draft to approved merge.
Segment the data by change type and risk. A rise in review comments can mean prompts are worse, or that reviewers are catching more issues earlier; pair counts with defect severity and time to approval before drawing a conclusion.
Best Value
What should a reviewer ask before approving generated code?
Use a short, repeatable checklist so speed does not erase judgment.
- Does the patch implement the stated outcome and every acceptance criterion?
- Are assumptions about APIs, versions, data and user behavior supported by repository evidence?
- Do tests cover normal, failure, boundary, concurrency and authorization cases?
- Did the change preserve required response codes, schemas, compatibility and performance?
- Are dependencies necessary, maintained and licensed for this project?
- Could secrets, personal data, payment data or signing material be exposed?
- Did any test, warning, scanner finding or reviewer question get suppressed rather than resolved?
- Is the approver qualified for the change’s security, privacy, financial or operational risk?
- Can the team explain how to roll back or contain the change?
What are the practical trade-offs of a stronger workflow?
Richer prompts and more context reduce misunderstandings but take preparation. Staged checks cost time up front and usually reduce downstream rewrites; one-pass generation is faster only when the first draft is already close to correct. Generic templates improve consistency, while repository-specific contracts and examples determine correctness. CI repeats objective checks cheaply, but only people can judge intent and architectural fit.

The right target is not maximum ceremony for every edit. Match the prompt detail, test depth, scanner set and reviewer expertise to the consequences of being wrong, and make the minimum safe process automatic where possible.
What should teams do when the model and the repository disagree?
Stop generation and resolve the authority conflict before accepting a patch. Prefer the current, explicitly designated contract or implementation pattern; if documentation and code disagree, open a human decision or issue rather than asking the model to choose silently.
Record the decision in the change description and update the stale source when appropriate. This prevents a locally correct patch from institutionalizing an undocumented exception.
Frequently Asked Questions
Should we keep a shared library of prompts?
Yes, keep versioned prompt patterns for recurring work, but treat them as living engineering documents rather than universal recipes. Include the model version, required context, output contract and examples, then let each repository add its own interfaces and constraints.
How should confidential code be handled in an AI workflow?
Follow your organization’s data-classification and tool-approval rules before sending source, logs or customer data to a model. Remove secrets and unnecessary personal information, use approved private deployments when required, and document what context was shared so reviewers can assess exposure.
What if generated tests pass but the feature is still wrong?
Passing tests may only show that the implementation satisfies its own examples. Ask a domain owner to review acceptance criteria, abuse cases and user workflows, and add a test that captures any missed behavior before merging.
Can a model review its own code safely?
A second model pass can find formatting or obvious omissions, but it is not an independent approval. Use it as an additional signal after deterministic checks, then rely on a qualified human for intent, architecture and risk decisions.
