Prepare for the scan your facility scheduled—not for the word “AI.” An AI-assisted cardiac CT or MRI follows the same safety checks, fasting or caffeine rules, contrast screening, medication review and breath-hold instructions as the underlying CT or MRI protocol. Before the appointment, confirm whether you are having coronary CT angiography (CCTA) or cardiac MRI, obtain the imaging center’s written instructions, and tell staff about possible pregnancy, kidney disease, allergies or previous contrast reactions, medicines, implants, claustrophobia and difficulty holding your breath. Never start, stop or reschedule a medicine on your own. The radiology team decides whether your heart rate, kidney function, implant status and other factors allow the examination to proceed.
What should I do first when an AI-assisted cardiac scan is scheduled?
Confirm the examination name and ask the imaging center for its current written preparation sheet. “Cardiac CT” may mean coronary CT angiography, while “cardiac MRI” uses a different scanner, different contrast decisions and different safety screening. AI can be used to reconstruct images or analyze them, but the preparation is set by the CT or MRI protocol and the facility’s equipment.

- Read the appointment letter rather than relying on generic fasting advice from another facility.
- Tell the center about pregnancy or possible pregnancy, kidney disease, allergies, any prior reaction to contrast, and every prescription, over-the-counter medicine and supplement you take.
- For MRI, report every implant or possible metal fragment before arrival, even if another clinician previously called it “MRI safe.”
- Explain claustrophobia, involuntary movement or difficulty following breath-hold instructions early enough for the team to plan.
RadiologyInfo, the American Heart Association (AHA), the Society of Cardiovascular Computed Tomography (SCCT), Mayo Clinic and StatPearls all emphasize that local instructions take precedence because protocols, scanners, medicines and patient risks differ.
How are cardiac CT and cardiac MRI preparation different?
Both examinations require screening and cooperation, but the practical priorities are not the same. CT uses x-rays and usually an iodine-based contrast injection to show coronary arteries; MRI uses a strong magnetic field and radio waves, does not use ionizing radiation, and may use gadolinium contrast.

| Preparation issue | Coronary cardiac CT (CCTA) | Cardiac MRI |
|---|---|---|
| Radiation | Uses x-rays. SCCT’s 2022 consensus uses approximately 3 mSv as an annual U.S. background-radiation comparison for communication; that figure is not a prediction of an individual scan dose. | Uses a magnetic field and radio waves, not ionizing radiation. |
| Contrast | Commonly an iodinated contrast injection; allergy history and kidney function matter. | Gadolinium may be used; kidney disease can change or prevent its use. |
| Heart-rate preparation | Often important for clear coronary images. Staff may measure your pulse and blood pressure and decide whether a beta blocker or nitroglycerin is appropriate. | Heart-rate slowing is not the usual preparation focus described in the patient guidance. |
| Fasting and drinks | Many protocols restrict food for several hours and restrict caffeine; the exact interval varies. | Some centers request four to six hours without food or drink, while others allow normal food, fluids and medicines. |
| Implant screening | Standard medical screening still applies, but the magnetic-field restrictions of MRI are not present. | Central to safety. Pacemakers, defibrillators, pumps, clips, cochlear implants, vascular devices and metal fragments require formal compatibility review. |
| Breath-holds and stillness | Short, coached breath-holds help prevent motion and may be practiced before scanning. | Repeated short breath-holds and lying still are commonly required throughout multiple image sequences. |
| Appointment duration | Exact timing depends on the scanner, heart-rate control, contrast and local workflow. | Exact timing depends on the sequences, breath-holds, contrast and local workflow; ask the facility for its estimate. |
How should I prepare for a coronary cardiac CT?
Follow the center’s fasting, caffeine and medication instructions exactly. Typical CCTA preparation combines several hours without solid food, caffeine avoidance, a review of medicines and kidney function, and coaching for a brief breath-hold.
Do I need to fast for a cardiac CT?
Often, but the interval is facility-specific. SCCT guidance describes no food for about three to four hours, while Mayo Clinic says patients usually fast at least eight hours; StatPearls summarizes no solid food for four hours. These differences reflect local protocols rather than a universal rule.
Recommended Free Tools
When the center permits fluids, it may allow water or other clear, non-caffeinated drinks. Do not assume that a “fast” means no water, and do not substitute a website’s advice for the written instructions from your imaging facility. If you have diabetes or another condition affected by missed meals, tell the center when scheduling so it can give a safe plan.
Can I drink coffee, tea or an energy drink before CCTA?
Usually you will be asked to avoid caffeine for approximately 12 hours. SCCT, Mayo Clinic and other clinical guidance cite this interval because caffeine can interfere with the heart-rate conditions the team is trying to achieve. The restriction can include coffee, tea, cola, energy drinks, pre-workout products and other caffeinated items.
Check labels and ask about decaffeinated products if the instructions are unclear. Do not shorten or extend the interval on your own; the center may use a different cutoff for its scanner and protocol.
Should I stop my regular medicines before a coronary CT?
Do not stop routine medicines unless the ordering clinician or imaging team specifically tells you to. SCCT describes continuation of regular medicines unless instructed otherwise, but the staff must review your blood pressure, rhythm, kidney function, contraindications and the scanner protocol.

What should I tell the CT team about contrast, pregnancy and kidney disease?
Tell staff about any previous reaction to iodinated contrast, allergies, kidney disease, dialysis, pregnancy or possible pregnancy, and other significant illnesses. RadiologyInfo specifically advises discussing pregnancy possibility, illnesses, medicines and allergies before a contrast CT.
The team decides whether kidney-function testing, a modified protocol or a different examination is needed. A previous mild reaction does not automatically answer what is safe for the next scan, so describe what happened and when. Never conceal a prior reaction because the injection is planned in advance.
Why does the CT team measure my heart rate and practice a breath-hold?
Coronary arteries move with each heartbeat. The technologist or nurse may measure your heart rate and blood pressure, provide a beta blocker if the clinician considers it safe, and coach you through a short breath-hold. StatPearls and SCCT identify heart-rate assessment, possible medication control, intravenous access and breath-hold practice as typical parts of CCTA preparation.
Practice only the technique the staff gives you. Tell them if you cannot hold your breath, become short of breath or feel faint; they can decide whether to adjust coaching, timing or the examination plan. Clear images depend on staying still and following the breathing command, not on an AI setting that can compensate for unlimited motion.
What happens when I arrive for a coronary CT?
- The staff verify the examination, your identity, symptoms, allergies, medicines, pregnancy possibility and relevant medical history.
- They place an intravenous line if contrast is planned and may check pulse, blood pressure and recent kidney-function information.
- If the protocol calls for heart-rate control or nitroglycerin, a qualified clinician decides whether and when to give it.
- You receive positioning and breath-hold instructions, then the scanner acquires the images during the instructed window.
- After contrast, staff may observe you briefly according to local policy and tell you what symptoms require immediate attention.
What if I ate, drank caffeine or took a medicine that the instructions prohibited?
Call the imaging center before traveling, and tell the staff the exact time and amount rather than trying to hide it. They may proceed, change the timing or reschedule depending on the heart-rate target, contrast plan and fasting rule. Do not take extra medication to “correct” the mistake.
How should I prepare for a cardiac MRI?
MRI preparation centers on implant safety, metal screening, contrast decisions, stillness and repeated breath-holds. Eating and drinking rules vary more widely than they do for CCTA, so the appointment letter is especially important.
Do I need to fast before a cardiac MRI?
There is no single rule. The AHA says some patients may be asked not to eat or drink for four to six hours, while other facilities permit normal food, fluids and medicines. Follow the instructions for your appointment and ask the center if the letter is unclear.

If sedation is being considered because of severe claustrophobia or inability to remain still, the facility may issue additional fasting instructions. Do not assume that a routine, non-sedated MRI and a sedated MRI have the same requirements.
Is my pacemaker or defibrillator safe for cardiac MRI?
Only the MRI service can confirm that after reviewing the exact device, leads, settings and local scanning conditions. Report pacemakers, implantable defibrillators, infusion pumps, neurostimulators, cochlear implants, aneurysm clips, vascular devices, tissue expanders and any other implant. Bring the device identification card or documentation if you have it.
“MRI conditional” does not mean that every scanner setting and every body position is automatically acceptable. The team may need information from the implant manufacturer or your cardiology service, and the device may require monitoring or reprogramming. Do not enter the MRI area until screening is complete.
What metal and electronics must I remove?
Before entering the scanner room, remove jewelry, watches, hearing aids, phones, bank or access cards, hairpins, metal clothing and other electronic items. RadiologyInfo notes that the technologist provides screening and a gown. Tell staff about metal fragments in the eyes or elsewhere, including fragments from an old injury or workplace exposure.
Do not rely on appearance: a small object can still be hazardous or distort the images. Ask before bringing personal equipment, mobility aids or monitoring devices into the controlled MRI area.
Can I receive gadolinium if I have kidney disease?
Tell the MRI team about kidney disease, dialysis, a kidney transplant or previously abnormal kidney tests before the appointment. The AHA and RadiologyInfo explain that kidney function may need checking and that gadolinium is generally avoided or modified in advanced kidney failure.
The decision depends on the clinical question, the specific contrast agent and your current kidney function. Do not cancel or accept contrast based only on a past test; ask the facility what information it needs and whether a non-contrast sequence or another approach is suitable.

What if I am claustrophobic or cannot stay still?
Tell the center before the appointment, not when you are already on the table. Cardiac MRI requires you to lie still while the scanner acquires repeated sequences and to follow short breath-hold commands. RadiologyInfo advises raising claustrophobia or inability to remain still in advance because sedation planning may be needed.
What’s actually slowing this PC down?

Describe what has happened during previous scans, including panic, involuntary movement or an inability to complete the examination. The team can then decide whether additional planning, a different schedule or another clinically appropriate examination is needed.
What happens during a cardiac MRI visit?
- The staff complete the MRI safety questionnaire and verify every implant, metal fragment, pregnancy possibility, medicine and relevant illness.
- You change into MRI-safe clothing and remove all jewelry, electronics, cards and other screened items.
- An intravenous line may be placed if gadolinium is planned, and kidney-function information is reviewed when indicated.
- You lie on the table and receive instructions for keeping still and holding your breath for repeated short periods.
- The technologist communicates with you throughout the scan. Tell them immediately if you feel unwell, panicked or unable to follow the breathing instructions.
What does AI change about scan preparation?
The patient guidance reviewed for cardiac CT and MRI does not identify a separate diet, medication, implant or equipment requirement for an AI-assisted examination. AI may reconstruct raw CT or MRI data, reduce image noise, or help analyze images after acquisition; it does not make iodine or gadolinium risk-free, make an implant automatically compatible, or eliminate motion and breath-hold requirements.
Ask the imaging center three practical questions:
- Is AI being used for image reconstruction, interpretation support or both?
- Does the local workflow change the expected appointment length, number of breath-holds or timing of contrast?
- Will a technologist give the same preparation and safety instructions as for a non-AI cardiac CT or MRI?
The answers are workflow-dependent. A facility may use different software versions, scanner protocols or review processes, so only that center can give an accurate timing or preparation answer.
What safety issues should I resolve before the appointment?
Pregnancy or possible pregnancy
Notify the ordering clinician and imaging facility as soon as you know or suspect you may be pregnant. RadiologyInfo specifically includes pregnancy possibility in the pre-CT discussion, and MRI teams also need that information when deciding on sequences and any contrast.
Free tools Windows power users keep installed
One-click scans. No signup required.
Allergy or prior contrast reaction
Describe the previous reaction, the contrast type if known, symptoms and treatment. “Allergy” can refer to different reactions, and the radiologist needs the details to decide whether to modify, substitute or avoid contrast.
Kidney disease or dialysis
Provide the most recent kidney information the center requests and identify whether you receive dialysis. Iodinated CT contrast and gadolinium MRI contrast are evaluated differently, but both require the team to know your kidney status before injection.
Best Value
Medication uncertainty
Bring or have available an accurate medication list, including diabetes medicines, blood-pressure drugs, erectile-dysfunction medicines, supplements and as-needed drugs. Never take an unprescribed beta blocker or nitroglycerin for the scan, and never stop metformin or another medicine without explicit clinical instructions.
Difficulty following instructions
Tell staff if hearing, language, cognitive, respiratory or movement problems could affect breath-holds or stillness. Clear communication lets the team arrange appropriate coaching and decide whether the planned protocol is feasible.

- Use the facility’s date-specific instructions, especially for fasting, caffeine and medicines.
- Confirm that the center has received implant details before an MRI appointment.
- Report kidney disease, pregnancy possibility and previous contrast reactions when scheduling, not only at check-in.
- Wear or bring only items the staff have cleared for the scanner environment.
- Arrive prepared to follow short breath-holds and remain still; tell the technologist immediately if you cannot.
- If you break a fasting or caffeine rule, call before leaving home so the center can make the decision.
The most reliable preparation is a direct conversation with the imaging facility. AI assistance does not replace that conversation or the technologist’s safety screening.
Frequently Asked Questions
What should I bring to a cardiac CT or MRI appointment?
Bring the appointment letter, a current medication list, and any implant identification card or device documentation. If you have had a contrast reaction or recent kidney testing, bring those details or confirm that the facility can access them.
What if I accidentally drink coffee or eat before the scan?
Call the imaging center and report exactly what you consumed and when. The team may keep the appointment, change its timing or reschedule it based on the protocol; do not take extra medicine to compensate.
Can the radiologist use AI to complete the scan if I cannot hold my breath?
AI reconstruction or analysis does not guarantee usable images when there is excessive motion. Tell the technologist before scanning begins so the team can assess whether coaching or a different protocol is appropriate.
Why does the MRI service need the exact model of my implant?
MRI conditions can differ by device model, lead configuration, scanner strength and programmed settings. The service needs that information to verify compatibility and arrange any required monitoring or device management before you enter the scanner room.
