Prepare an underwriting AI system as a regulated operating capability, not as a model that can be switched on after procurement. Before it sees a live application, define the decision and risk limits, assign accountable owners, trace and test every data source, complete independent validation, build fairness and explainability evidence, secure the full data and model supply chain, and preserve meaningful human review and consumer appeal. The insurer remains responsible for an AI-supported underwriting outcome even when a vendor supplies the algorithm or external consumer data. A controlled pilot, continuous monitoring, documented change management and a tested rollback path are the practical conditions for scaling safely.
What does it mean to prepare AI for insurance underwriting?
Preparation is a lifecycle program that covers design, development, validation, implementation, use, monitoring, updating and retirement. It applies to a rules engine, a statistical score, machine learning, a generative component used by an underwriter, or a vendor service that supplies data or recommendations.

The governing principle is accountability. NAIC guidance says insurers remain responsible for complying with insurance laws, regulations, standards and consumer-protection rules when they use AI. New York State Department of Financial Services (NY DFS) likewise states that an insurer remains responsible for understanding tools, external data and AI systems used in underwriting or pricing, including those developed or deployed by third parties. EIOPA principles place similar emphasis on governance, transparency, data quality and human oversight in the European supervisory context. Requirements still vary by jurisdiction, line of business and use case, so map the program to each state, federal and EU obligation that applies to your book.
Readiness has four tests: the system must make a defensible risk assessment, operate within an approved risk appetite, produce evidence that a supervisor or customer can understand, and fail safely when data, performance or security conditions deteriorate.

How should we define the underwriting use case and risk appetite?
Write the decision specification before selecting a model or buying data. A precise specification determines the controls, validation depth and human involvement required.
Record the decision boundary
- Line of business, product, state or country and legal entity making the decision.
- Exact decision: eligibility, pricing tier, limit, referral, document request, fraud flag or another underwriting action.
- People affected, including applicants, policyholders, additional insureds and any protected or vulnerable groups.
- Required response time and the consequences of a false accept, false decline or missed referral.
- Whether AI recommends, augments or automatically makes the decision.
- Data categories used, including internal history, public information and external consumer data.
Set measurable limits
Approve acceptable error, calibration, disparity, override and complaint thresholds before deployment. Define prohibited uses, escalation triggers, who can pause automated decisions and who can authorize a return to manual processing. NY DFS calls for an approach that is reasonable and appropriate to the insurer’s business model and the complexity and materiality of the risks. A low-impact document-classification aid and an automated decline therefore should not pass through the same approval gate.
The Tool Desk
Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →Classify materiality
Assign a documented materiality tier based on financial exposure, customer impact, regulatory sensitivity, automation level, data sensitivity and concentration risk. The tier should determine validation independence, monitoring frequency, approval authority, evidence retention and the amount of human review. Reassess the tier when the model, data, product, geography or decision role changes.
What governance is needed before procurement or model training?
Governance should exist before a business unit signs with a vendor or a data-science team trains a model. It converts an experiment into an accountable insurance process.
Create an accountable reporting path
Give the board or a delegated committee visibility into risk appetite, material systems, incidents, exceptions and remediation. Establish a cross-functional operating group with underwriting, actuarial, data science, compliance, legal, enterprise risk, information security, operations and internal audit. Include product and claims specialists where the model’s outputs affect those functions.
Approve written policies that define roles, approval gates, training, issue escalation, exception handling, documentation standards and annual policy review. A business owner should be accountable for the decision; a model owner for performance and change control; data owners for provenance and quality; compliance and legal for applicable obligations; security for the operating environment; and internal audit for independent assurance.
Keep a live AI and model inventory
Maintain one inventory for systems in production, under development, in pilot, supplied by vendors or recently retired. NY DFS specifically calls for an up-to-date inventory of those states. At minimum, record:
- System name, unique identifier, owner and business sponsor.
- Purpose, decision role, products, jurisdictions and affected populations.
- Inputs, outputs, protected or sensitive attributes, external data and known proxies.
- Model type, version, training date, deployment date, vendor and subcontractors.
- Materiality tier, validation status, approvals, restrictions and prohibited uses.
- Performance, fairness, drift, override, complaint and incident metrics.
- Open findings, remediation deadlines, change history, next review and retirement date.
Link inventory records to source data definitions, validation reports, contracts, approvals, monitoring dashboards and incident tickets so an examiner can reconstruct what was used and why.
How do we prepare underwriting data for AI?
Make every feature traceable, legally usable and actuarially defensible. A model cannot be made compliant by adding a disclaimer after training.

Build a feature-level data register
For each variable, document the original source, collection date, geographic scope, consent or other legal basis where relevant, transformations, missing-value treatment, retention period, permitted use, sharing path and vendor involvement. Preserve the value presented to the model, not only the raw record, so an investigation can reproduce a decision.
Test accuracy, completeness, timeliness, stability and representativeness for the population and territory in which the model will operate. Identify sampling bias, label leakage, duplicate records, stale values, unexplained missingness and changes in collection practice. Establish data-quality rules that block or route an application to a human when critical fields fail.
Demonstrate actuarial validity
For each underwriting variable, retain evidence of a clear empirical, statistically significant, rational and not unfairly discriminatory relationship to the relevant risk. Document the population, observation window, outcome definition, statistical method, limitations and approving actuary. A feature that improves a predictive metric but lacks a defensible relationship to insured risk should not be used merely because it is available.
Control external consumer data
Treat purchased, brokered or otherwise external consumer information as a controlled dependency. Before use, assess collection practices, geographic coverage, update frequency, lawful use, error rates, proxies for protected classes and the vendor’s correction process. Establish a way for a consumer or underwriter to challenge inaccurate information; investigate the source, remove or quarantine bad data from decision systems, correct downstream records and document each step from acquisition through storage, use, sharing, archiving and destruction.
Keep training, validation and production datasets separated where practical. Restrict access by least privilege, version datasets and transformations, and retain immutable snapshots of the data used for each material model release.

Fairness is an evidence program, not a single score. Evaluate the complete decision process, including data selection, feature engineering, thresholds, referrals, overrides and adverse-action communications.
Test outcomes across relevant groups
- Compare approval, decline, referral and information-request rates by legally relevant and operationally relevant groups.
- Measure error rates, calibration, lift and loss outcomes within groups, not only for the portfolio as a whole.
- Investigate direct use of protected characteristics and indirect proxies such as location, occupation, language, household structure or digital behavior.
- Check intersectional groups and small segments where aggregate averages can conceal harm.
- Analyze whether missing-data rules or manual overrides create a different result for one group.
Predefine disparity tolerances, confidence requirements and escalation rules with compliance, actuarial and legal owners. Where a metric signals a problem, investigate the mechanism and document the decision to remediate, restrict, add human review or not deploy. Keep code, data extracts, test definitions, results and approvals so the analysis is reproducible.
Make the risk relationship explainable
Explainability should match the decision and audience. An underwriter needs the influential factors and data quality warnings needed to review a recommendation; a consumer receiving an adverse action needs specific, accurate reasons connected to the information used; a supervisor needs the underlying data, rationale, validation and control evidence.
Do not rely on a vendor’s statement that the model is proprietary. NY DFS says proprietary algorithms do not justify a lack of specificity in adverse-action explanations. EIOPA identifies disclosure of AI use, datasets, reasons for selected criteria and influential rating factors or counterfactual explanations as relevant underwriting transparency information.

What independent validation and model-risk controls are required?
Validation must be independent of the team that built or implemented the system and proportionate to materiality. The Federal Reserve defines model risk as “the potential for adverse consequences from decisions based on incorrect or misused model outputs and reports.” Its supervisory framework emphasizes an accurate inventory, appropriate development and use, effective validation, continuous performance monitoring, governance, resources and internal audit.
Validate before first use
- Challenge the intended use, assumptions, target definition, sampling and feature construction.
- Reperform data-quality, leakage, stability and representativeness tests.
- Test discrimination, calibration, ranking, sensitivity, robustness and explainability for the intended population.
- Review security, access controls, implementation logic, integrations, logging and fallback behavior.
- Compare the implemented artifact with the approved version and verify that thresholds, labels and adverse-action reason codes are correct.
- Issue a written finding with limitations, conditions, owners and a decision to approve, restrict, remediate or reject.
Test at least annually after deployment and sooner after a material change. Material changes include retraining, a new data source, a changed threshold, a new jurisdiction or product, a vendor model update, a new decision role or a significant drift signal.
Monitor the live decision system
Monitoring should combine model, data, customer, operational and security signals:
- Accuracy, calibration, loss performance and confidence or uncertainty.
- Input completeness, validity, distribution shifts and missingness.
- Approval, decline, referral, manual-review and override rates.
- Subgroup outcomes, error rates and disparity indicators.
- Application volume, latency, queue age and human-review workload.
- Complaints, correction requests, appeals, adverse-action reason quality and incidents.
- Access anomalies, failed controls, vulnerabilities and unauthorized data disclosures.
Set thresholds that automatically create an issue, pause automated decisions, increase sampling or require human review. Monitoring owners should document the trigger, investigation, customer impact, temporary control, root cause and closure evidence.
Can a human underwriter override an AI decision?
Yes, but “human in the loop” is meaningful only when the reviewer has authority, information, time and a documented path to disagree. A person who can merely accept a score without seeing its basis is not an effective control.
Design the review workflow
- Route low-confidence, out-of-distribution, high-impact, exception and data-quality cases to a qualified underwriter.
- Show the recommendation, influential factors, source dates, missing-data warnings and applicable policy rules.
- Require a reason code and free-text rationale for an override, along with the final decision and reviewer identity.
- Prevent retaliation against a reviewer who challenges the system; escalate repeated disagreement to model governance.
- Sample non-overridden decisions to detect automation bias and rubber-stamping.
- Give customers a correction, complaint, appeal and human-review route with service-level targets.
Publish the conditions that trigger review and test that the queue can be staffed during outages or sudden volume spikes. Preserve manual-continuation procedures so underwriting can continue when the model or a data supplier is unavailable.
What should cybersecurity and privacy controls cover?
Underwriting can involve financial, health and other personal information. NAIC materials identify sensitive underwriting and claims information and describe potential breach impacts including identity theft, business interruption, reputational damage, data repair and litigation.
Protect the model and its environment
- Use least-privilege identity management, strong authentication, encryption in transit and at rest, and managed secrets.
- Segment training, validation and production environments; restrict exports and administrative access.
- Log data access, model calls, prompts or inputs, outputs, overrides, configuration changes and administrator actions.
- Apply secure development, dependency scanning, vulnerability management, patching and penetration testing.
- Define retention and deletion schedules for raw data, features, prompts, outputs, logs and backups.
- Maintain tested incident response, backup, recovery and business-continuity procedures.
- Test for prompt or input abuse, model extraction, poisoning, unauthorized disclosure and insecure integrations when generative or externally hosted components are used.
Security review should include the data and model supply chain, subcontractors, hosting locations and support access. A model that is statistically sound but exposes applicant information is not ready for underwriting.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesWhat must we require from an underwriting AI vendor?
Vendor due diligence should be risk-proportionate and completed before production access. The contract must preserve the insurer’s ability to understand, test, monitor, explain and stop the service.

Ask for evidence, not marketing claims
- Training-data sources, collection methods, geographic coverage, update practices and known limitations.
- Development methodology, performance by segment, fairness testing, explainability method and validation reports.
- Model and data versioning, release process, change-notification lead time and rollback capability.
- Security architecture, access controls, encryption, vulnerability program, incident history and breach-notification timing.
- Subcontractors, processing locations, cross-border transfers and support personnel access.
- Service levels, uptime, latency, support escalation and manual fallback arrangements.
- Correction, deletion, portability and restrictions on using insurer or consumer data for unrelated training.
- Audit rights or qualified independent audit reports, regulator cooperation and examination support.
Require prompt notice of material changes, incidents, data-source corrections and regulatory inquiries. Allocate responsibility for inaccurate external data, preserve records needed for adverse-action explanations and prohibit terms that prevent the insurer from meeting legal disclosure duties. A confidentiality clause cannot override the insurer’s obligation to provide specific reasons to a consumer or evidence to a supervisor.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How should we pilot and scale an underwriting AI system?
Use a bounded pilot rather than a portfolio-wide switch. The pilot should generate evidence under realistic conditions while limiting customer and financial exposure.
- Select a narrow workflow. Choose one product, jurisdiction, decision role and representative sample. Exclude prohibited or high-severity cases until controls are proven.
- Freeze the baseline. Record current underwriting outcomes, turnaround time, referral volume, loss indicators, complaints, overrides and manual effort.
- Predefine success and stop metrics. Include accuracy, calibration, subgroup fairness, customer outcomes, operational capacity, security events and financial impact.
- Run independent validation and approvals. Obtain model-risk, actuarial, compliance, legal, security and business sign-off with conditions documented.
- Operate with enhanced human review. Sample decisions, compare AI-assisted outcomes with the existing process and investigate every material variance.
- Exercise failure paths. Test stale or missing data, vendor outage, model timeout, drift alert, security incident, adverse-action explanation and rollback to manual processing.
- Review the evidence before expansion. Approve a larger scope only when thresholds are met, findings are closed or accepted by the proper authority, and staffing and customer communications are ready.
Repeat the materiality assessment after each scope, data, model, vendor or jurisdiction change. Keep a versioned decision log showing who approved expansion, under what conditions and with which unresolved risks.

- Training completed for underwriters, operations, compliance, complaints and incident responders.
As of March 2026, the NAIC reported that its AI Systems Evaluation Tool was being piloted by 12 participating states. That figure describes a regulator-program status, not the level of insurer adoption, and it does not replace state-specific obligations.
How do regulatory expectations differ by location?
NY DFS Circular Letter No. 7 (2024) applies to New York-authorized insurers and addresses external consumer data and artificial intelligence in underwriting and pricing. NAIC materials are model or guidance documents whose adoption and implementation can differ by state. Federal Reserve model-risk principles are supervisory expectations for organizations within its remit. EIOPA materials address the European supervisory context. Confirm the current requirements for the legal entity, product, customer location, data flow and decision before launch, and reassess them when the operating footprint changes.
What are the most common failure modes?
Buying a black box and documenting later
Without a defined decision, owner, data register and explanation design, the insurer may be unable to validate or answer a consumer. Require evidence and contractual access before procurement approval.
Best Value
Assuming a vendor owns compliance
Third-party development does not transfer the insurer’s accountability. Include the supplier in inventory, validation, monitoring, incident response and examination preparation.
Using accurate data that is not valid for the risk
A clean data field can still be an unjustified proxy or have no rational relationship to the insured peril. Retain actuarial analysis and remove features that cannot meet the validity and discrimination standard.
Recommended Free Tools
Testing once and stopping
Portfolio mix, data collection, economic conditions and vendor versions change. Schedule annual testing at minimum, monitor continuously and trigger additional validation after material changes or drift.
Calling a rubber stamp human review
Reviewers need authority, reasons, source dates and time to challenge a recommendation. Measure overrides and disagreement, and route uncertain or high-impact cases to people.
Giving vague adverse-action reasons
“The model scored you high risk” is not a specific explanation. Map influential factors and relevant data sources to accurate reason codes that a consumer can understand and correct.
Ignoring operational failure
Outages, stale feeds, queue overload and security incidents can make a valid model unsafe. Test fallback, staffing, rollback and customer communication before expanding scope.
Frequently Asked Questions
How long should an insurance underwriting AI pilot run?
There is no universal duration. Run it long enough to observe the relevant underwriting cycle, seasonal mix, data refreshes and enough outcomes for subgroup and calibration checks; define that evidence window in the approval plan rather than choosing an arbitrary number of days.
What records must be kept for an AI underwriting decision?
Retain the model and data versions, input values and source dates, transformations, output, thresholds, reason codes, human overrides, approvals and relevant monitoring results. Retention periods should follow the insurer’s legal, regulatory, complaint and litigation requirements for the product and jurisdiction.
How should an insurer handle a consumer who disputes external data?
Pause or route the affected decision for human review, identify the originating provider, investigate the alleged error and prevent the disputed value from continuing to drive decisions while it is unresolved. Correct downstream records, notify the consumer of the outcome and document the investigation and remediation.
Can generative AI draft an underwriter’s rationale?
It can assist with drafting only when the insurer controls the inputs, prevents unsupported reasons, records the source facts and requires a qualified underwriter to verify and approve the text. The final explanation must describe the actual factors used in the decision, not a plausible narrative generated after the fact.
