Monitor an AI agent as a complete, auditable run—not as a final answer or HTTP status. Capture every model generation, tool call and argument, handoff, guardrail decision, retry, error, timestamp, latency value, token count, cost estimate, and real-world outcome. Keep one trace and session identifier across subagents and services, then review the data through four lenses: reliability, safety, performance, and economics.
This approach catches failures that look successful on the surface, such as selecting the wrong tool, passing incomplete data, looping through retries, or claiming that a ticket, payment, or deployment succeeded when it did not. It also gives you the controls needed to block dangerous actions and stop an unexpectedly expensive run before it consumes your budget.

What should an AI-agent monitoring system record?
A useful monitoring system records the entire causal chain from request to verified result. A single response string is too small a unit: an agent may make several model calls, invoke tools, transfer control to another agent, trigger guardrails, and retry before producing that response.
| Record | What it tells you | Useful dimensions |
|---|---|---|
| Run and trace identity | Which execution produced an event and how events relate to one another | Run ID, trace ID, session ID, parent span, workflow, tenant, user |
| Model generation | What the model was asked to do and what it produced | Model, prompt or policy version, response, finish reason, timestamp |
| Tool activity | Which capability was selected and whether it returned a valid result | Tool name, argument hash or redacted arguments, result status, duration, error |
| Agent handoff | Where control moved in a multi-agent workflow | Source agent, destination agent, reason, parent and child spans |
| Guardrail event | Whether a request or result was allowed, blocked, or sent for approval | Rule, decision, policy version, reviewer, timestamp |
| Operational outcome | Whether the intended task actually happened | External system ID, state before and after, verification result |
| Usage and cost | What the run consumed and what it is expected to cost | Requests, input and output tokens, cached and reasoning tokens where available, estimated provider charge |
OpenAI’s Agents SDK tracing model is a practical reference: it creates spans for runner operations, model turns, agent execution, function tools, guardrails, handoffs, and custom events, and aggregates token usage for a run. A vendor-neutral schema can expose the same concepts even when the runtime is different.

Preserve context across boundaries
Propagate the trace and session identifiers in every service-to-service request, queue message, tool invocation, and subagent handoff. If a retrieval service, payment adapter, or background worker starts a new trace without the parent identifiers, the most important evidence will be split across unrelated logs. Store a parent-span reference so an investigator can move from the top-level request to the exact model turn or state change that caused it.
Protect sensitive telemetry
Full prompts, tool arguments, and returned documents can contain personal, financial, or proprietary data. Redact or tokenize sensitive fields before exporting telemetry, retain a cryptographic hash when you need to prove that a value was unchanged, and restrict raw payload access to the people who need it. Keep the audit record of a decision even when the underlying content is redacted.

1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsHow do I instrument every agent run?
Instrumentation is most reliable when it is built into the runner rather than added only at the API gateway. The runner knows when a model turn starts, when a tool is selected, when a guardrail fires, and when a handoff or retry occurs.
- Create a trace at the entry point. Generate a run ID and trace ID when a user request, scheduled job, or webhook enters the workflow. Attach the tenant, authenticated user or service identity, workflow name, model policy version, and environment.
- Open a span for each meaningful operation. Record model turns, agent execution, function tools, retrieval, custom business logic, guardrails, approvals, handoffs, and state changes as separate spans. Include start and end timestamps so queueing and execution time can be distinguished.
- Capture inputs safely. Store the tool name and a normalized argument representation. Use a redacted payload or argument hash when the raw value is sensitive. Record the prompt or policy version so a later change can be correlated with a change in behavior.
- Capture outputs and statuses. Record whether a call completed, timed out, returned malformed data, was rejected by validation, or raised an exception. Preserve provider finish reasons and error codes where available.
- Aggregate usage at run level. Add request count, input tokens, output tokens, cached-token counts, reasoning-token counts when a provider reports them, and an estimated charge. Include per-span usage as well as the total, because a single retry loop may be responsible for most of the spend.
- Attach the external outcome. After a write operation, query the system that was supposed to change. Record the resulting object ID, state transition, and verification status. Do not treat the agent’s statement that an action is complete as proof.
- Close the trace with a classified result. Use a controlled status such as succeeded, failed, blocked, awaiting approval, cancelled, or inconclusive. Keep the reason and the component that assigned it.
Arize recommends preserving trace and session context across services, turns, and subagents, then adding manual spans around custom tools, retrieval, application logic, and state changes. That pattern is especially important when your framework automatically instruments only model calls.
How can I tell that an agent failed when its final answer looks successful?
Separate transport success from task success. An HTTP 200 response or a fluent final message proves only that the request completed at the interface; it does not prove that the agent chose the right tool, supplied valid arguments, or changed the intended system.
Detect hard failures
Hard failures are explicit and usually easy to alert on: uncaught exceptions, provider errors, authentication failures, tool timeouts, schema violations, cancelled runs, and exceeded loop or rate limits. Group them by workflow, model, tool, deployment, and version so a rising error rate points to a change rather than a vague system-wide problem.
Detect soft failures
Soft failures produce syntactically valid output while violating the task. Examples include selecting a read-only search tool instead of the update tool, omitting a required field, passing stale retrieved data, handing a task to an agent without the necessary context, repeating a tool call after a successful result, or returning an answer that contradicts the source record. Create explicit checks for these conditions instead of relying on a generic success flag.
- Compare the selected tool with the tools allowed for the current state and user role.
- Validate required arguments, ranges, data types, and cross-field relationships before execution.
- Check that returned data contains the fields and freshness indicators needed by the next step.
- Verify that a handoff includes the task, relevant context, constraints, and expected output contract.
- Detect repeated calls with the same normalized arguments and growing trace length.
- Reconcile the final response with the authoritative system state, not merely with the model’s text.
Use outcome checks at the system of record
For a ticketing action, query the ticket after the tool returns and compare its status, assignee, and update timestamp with the requested change. For a payment, verify the transaction ID and settled state through the payment system. For a deployment, check the deployment controller and health checks. If verification is unavailable, classify the run as inconclusive rather than successful.

Evaluate sampled traces
Automated evaluations can score sampled traces for tool choice, argument correctness, policy adherence, answer groundedness, and outcome accuracy. Run the same evaluation set after a model, prompt, policy, retrieval index, or tool implementation changes. Store the evaluator version and rubric with each result so a score change can be interpreted correctly.
How do I detect wrong tool calls and broken handoffs?
Tool and handoff monitoring needs both a record of what happened and a model of what was permitted to happen. A trace shows the sequence; a workflow policy defines whether that sequence was valid.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Represent expected transitions
For each workflow state, define the tools and destinations that are allowed, the required approvals, and the data contract for the next step. A customer-support agent might be allowed to read an account immediately, draft a reply, and request approval before changing billing details. A transition from a read operation directly to a destructive write should be rejected or escalated, even if the model’s arguments are well formed.
Compare actual and expected sequences
Build alerts for impossible transitions, repeated handoffs, a child agent that lacks a parent context, and a tool result consumed by an agent that was not authorized to see it. Record the reason for each handoff and whether the destination agent accepted the supplied context. When a sequence diverges, preserve the preceding spans; the first incorrect decision is often several steps before the visible error.
Validate tool results before reuse
Treat tool output as untrusted input. Check its schema, authorization scope, freshness, and consistency before passing it to another model turn. A successful database query can still return an empty, partial, or unauthorized result. Mark validation failures explicitly so they are not confused with provider errors.
How can I stop an agent from taking unsafe actions?
Detection alone is not a safety control. Put limits around the capabilities an agent can exercise, require approval for consequential actions, and make every decision auditable.
Apply least privilege
Separate read tools from write tools and issue credentials with only the permissions required for the current workflow. Use different identities for staging and production. Scope access by tenant, resource, operation, and time where the underlying system permits it. Do not give a general-purpose agent a credential that can perform unrelated administrative actions.
Require approval for irreversible or high-impact operations
Insert a human approval step before destructive, financial, legal, externally visible, or difficult-to-reverse actions. Show the reviewer the normalized arguments, affected resource, expected side effects, evidence used by the agent, and the identity of the requesting user. Record approval, rejection, expiration, and the policy version that required the gate. An approval should authorize one defined operation, not an unrestricted continuation of the agent.

Validate arguments and outputs
Use strict schemas with enumerated operations, bounded quantities, permitted destinations, and required confirmation fields. Reject unknown fields and ambiguous identifiers. Validate returned data before it is used in a write or sent to another agent. For sensitive workflows, perform a second independent check against the authoritative source or a deterministic policy engine.
Defend against prompt injection
Inspect retrieved documents, web content, files, and tool results for instructions that attempt to override policy or redirect the agent. Keep untrusted content clearly separated from system and policy instructions, and prevent retrieved text from directly authorizing a tool call. Datadog’s agent observability capabilities include safety and quality evaluations, prompt-injection detection, and sensitive-data redaction signals; equivalent checks can also be implemented in your own guardrail layer.
Recommended Free Tools
Limit loops, speed, and blast radius
Set a maximum number of model turns, tool calls, handoffs, and wall-clock duration for each workflow. Apply per-user and per-tenant rate limits, and pause a workflow when it repeatedly fails the same validation. Use idempotency keys for writes so a retry cannot create duplicate tickets, orders, or payments. Keep an emergency kill switch that disables a tool or workflow without redeploying every agent.
Keep immutable audit records
Write guardrail decisions, approvals, rejected arguments, executed operations, and resulting resource IDs to append-only storage or another system with tamper-evident controls. Include the actor identity, policy version, and timestamps. Operational logs can be rotated; the audit trail must remain sufficient to reconstruct who authorized an action and what actually happened.
How do I control runaway LLM costs?
Cost control requires attribution before it requires optimization. Count usage and estimate spend for every run, then enforce budgets at the same boundaries where you report them.
Measure the full cost of a run
Track request count, input tokens, output tokens, cached tokens, reasoning tokens where available, model selection, latency, and tool-related retries. Attribute the totals to the run, user, tenant, workflow, model, and tool. Include calls made by child agents and aggregate them to the parent trace. An estimate is not an invoice: provider pricing, discounts, cached-token treatment, and delayed billing can differ, so reconcile estimates with provider usage statements.
Free tools Windows power users keep installed
One-click scans. No signup required.
Set layered budgets
Use a per-run ceiling to stop a single loop, a per-user or per-tenant allowance to contain abuse, a per-workflow budget to protect expensive automations, and a daily or monthly organizational budget. Decide what happens at each limit: stop immediately, switch to a cheaper approved model, require approval, or return a partial result. Record the budget decision in the trace.
Alert on deviations, not only totals
Alert when token growth exceeds the workflow’s normal baseline, when retries multiply, when a trace becomes unusually long, when a model changes unexpectedly, or when spend accelerates faster than request volume. A low-volume workflow can still be costly if it invokes a large model repeatedly. Compare like with like by segmenting alerts by model, workflow version, tenant, and time of day.

Control retries and model escalation
Use bounded, categorized retries. Do not retry authentication failures or invalid arguments as if they were transient network errors. Require an explicit policy for escalating to a more capable model, and log every escalation with its reason. Cache deterministic retrieval and tool results where safe, but never reuse data across tenants or beyond its freshness period.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How do I investigate an agent incident?
Use a repeatable sequence so the investigation does not stop at the first error message.
- Freeze the evidence. Record the run, trace, session, deployment, model, prompt or policy version, tool versions, and relevant configuration. Preserve the event order and prevent retention jobs from deleting the trace.
- Classify the impact. Determine whether the issue was incorrect output, failed execution, unauthorized access, an unsafe action, data exposure, service degradation, or excess cost. Identify affected tenants and external resources.
- Find the first divergence. Walk from the root span through model turns, tool calls, guardrails, and handoffs. Compare each transition with the workflow policy and locate the first invalid choice, missing validation, or stale result.
- Verify the real-world state. Query the system of record for writes, payments, deployments, messages, or permissions. Record what changed, what did not, and whether a compensating action is safe.
- Contain the issue. Disable the affected tool or workflow, revoke an overbroad credential, pause queued runs, or lower a budget. Use the smallest control that stops further impact while preserving evidence.
- Correct and replay safely. Fix the policy, schema, prompt, tool, or model routing. Replay a redacted trace in a test environment, then run evaluations on representative traces before restoring production access.
- Document the prevention. Add a regression test, a new alert, a guardrail, or an outcome check. Link the change to the incident trace and retain the decision record.
Which observability approach fits an AI-agent stack?
The right choice depends on where the agent runs, how much control you need over data, and whether evaluations and safety signals are first-class requirements.
| Approach | Strengths | Best fit | Questions to verify |
|---|---|---|---|
| OpenAI Agents SDK tracing | Native spans for model generations, tools, guardrails, handoffs, runner operations, and custom events; run-level token aggregation | Applications already using the SDK runtime and wanting instrumentation close to execution | How will non-SDK services, retention, access control, and cross-provider calls be represented? |
| Datadog Agent Observability | Hosted end-to-end traces, operational dashboards, safety and quality evaluations, prompt-injection and privacy signals, and cost monitoring | Teams standardizing agent telemetry with broader production monitoring | Data residency, retention, integration coverage, permissions, and current plan limits |
| Arize Phoenix and agent observability | Tracing and evaluation that connect model calls, retrieved context, tool activity, handoffs, and state changes | Teams evaluating open-source or self-managed observability and regression workflows | Deployment effort, storage and retention, enterprise controls, alert delivery, and integration maintenance |
Compare products on trace granularity, evaluation and regression workflows, safety-event coverage, data residency, retention, integrations, access controls, alerting, deployment model, and cost attribution. Features, retention policies, and pricing change, so confirm current terms for your deployment rather than treating a feature list as permanent.
How should I roll out monitoring without overwhelming operators?
Start with complete event coverage for a narrow set of high-value workflows, then expand the controls and analysis.
Phase 1: establish the contract
Define the trace schema, redaction rules, status vocabulary, ownership, retention period, and outcome checks. Instrument one representative workflow end to end, including at least one tool, one handoff, and one write or approval path.
Best Value
Phase 2: make failures visible
Build reliability and performance dashboards, classify hard and soft failures, and route alerts to the team that can act. Sample traces for evaluation and record the versions of models, prompts, policies, tools, and evaluators.
Phase 3: enforce safety
Separate read and write credentials, add strict argument validation, introduce approval gates for high-impact operations, set loop and rate limits, and write immutable audit events. Test rejected, expired, duplicated, and adversarial requests deliberately.

Add token and spend attribution, layered budgets, retry categories, model-escalation policies, and deviation alerts. Review cost per verified successful task with finance or platform owners so an estimate can be reconciled to actual provider charges.
Phase 5: operate a regression loop
After every material model, prompt, tool, retrieval, or policy change, replay a representative trace set and inspect both automated scores and real-world outcome checks. Keep failed examples as permanent regression cases rather than deleting them after the incident is closed.
What mistakes make agent monitoring unreliable?
- Logging only the final answer: you lose the tool, handoff, and guardrail decisions that explain the result.
- Using HTTP status as task status: transport success can hide an incorrect or incomplete external action.
- Starting a new trace in every service: broken context prevents end-to-end diagnosis.
- Capturing raw data without controls: unrestricted prompts and tool payloads create a second privacy risk.
- Counting only successful requests: retries, child-agent calls, cached tokens, and reasoning tokens can dominate cost.
- Alerting on totals alone: aggregate spend can look normal while one tenant or workflow is looping.
- Allowing unrestricted retries: transient recovery logic can become a cost and safety failure.
- Relying on a model to police itself: independent schemas, policy checks, approval gates, and system-of-record verification are stronger controls.
- Ignoring retention and access design: a trace that cannot be retrieved, or that everyone can read, is not dependable operational evidence.
Frequently Asked Questions
Can logs alone prove that an AI agent was compliant?
No. Logs show what the system recorded, but compliance also depends on trustworthy identity, tamper-evident storage, access controls, retention, and an authoritative record of the resulting action. Pair telemetry with policy versions and independent outcome verification.
Should I store complete prompts and tool arguments?
Store the minimum content needed for debugging and evaluation. Redact secrets and personal data, retain hashes or structured fields for correlation, and provide a controlled path for authorized investigators to retrieve protected payloads when a serious incident requires it.
Crashes, No Sound, or Screen Glitches?

Run evaluations continuously on a sample of production traces and rerun a broader regression set after any model, prompt, retrieval, tool, policy, or routing change. Keep the evaluator version and rubric with each score so results remain comparable.
Can one monitoring design cover hosted and self-managed agents?
Yes, if the event schema is independent of a particular vendor. Standardize run and span IDs, model and tool events, guardrail decisions, outcome checks, usage fields, and redaction rules, then adapt exporters for each deployment.
Why does estimated spend differ from my provider invoice?
Telemetry estimates may not include negotiated rates, delayed adjustments, cache treatment, taxes, or provider-specific accounting. Reconcile estimates with provider usage data and preserve the pricing context used for each calculation.
