Monitor an AI agent as a distributed, probabilistic workflow rather than as a single API call. Give every run a stable ID, trace model generations through tool calls and approvals to downstream services, and combine logs, metrics, traces, policy events, and quality evaluations. This lets you distinguish a model failure from a broken dependency, detect an unsafe or novel action before it spreads, and reconstruct an outage from evidence instead of guesses.

A useful monitoring system covers four questions: did the run complete, was each decision and tool call allowed, did the result meet quality and safety requirements, and did the system stay within its latency and cost budget? The guide below shows what to collect, how to alert, and how to investigate incidents without exposing more user data than necessary.

What does AI-agent monitoring include?

Agent monitoring is the combination of observability, safety controls, and quality measurement for an application that can reason, call tools, delegate work, and take actions. Traditional uptime checks cannot explain why an agent selected a tool, repeated a request, ignored an approval, or produced a plausible but unsupported answer.

Think of one run as a trace with nested spans:

  • Session or conversation: the long-lived user or workflow context.
  • Turn or run: one request and its complete execution.
  • Model generation: each prompt, response, token count, and model version.
  • Tool invocation: the tool name, arguments, permissions, approval result, response, and duration.
  • Handoff: any delegated agent or orchestration step.
  • Dependency request: calls to databases, search systems, payment services, queues, or other APIs.

Logs preserve event details, metrics show aggregate health, and traces connect the events into an end-to-end path. Quality and safety evaluators add a fourth perspective: whether the behavior was grounded, factually adequate, policy-compliant, and useful even when all requests returned HTTP 200.

Why are traces essential for nondeterministic agents?

An agent can make a different decision on two otherwise identical runs. A trace is therefore the only practical way to inspect the decision path that led to a user-visible result. It should let an operator move from an alert to the exact generation, tool call, approval, or downstream request that failed.

Google Cloud describes telemetry as necessary for inspecting nondeterministic decisions and diagnosing failed API requests, infinite execution loops, and latency bottlenecks. Microsoft’s guidance similarly treats logs, metrics, and traces as AI-native evidence that must support incident reconstruction. A single success-rate metric cannot show whether the model hallucinated an endpoint, whether a tool rejected valid arguments, or whether a retry loop exhausted a quota.

What should you capture for every agent run?

Define a data contract before instrumenting production. Capture enough context to replay the control flow and explain the outcome, while applying redaction, access controls, retention limits, and regional storage rules to user content.

Area Fields to record Why it matters
Identity and context Stable conversation, session, and run IDs; tenant or workspace ID; request identity; environment; region Correlates events across services and separates customers or deployments during an incident.
Versioning Model name and version; system and developer prompt versions; agent code release; tool schema version; policy version Shows whether a regression followed a model, prompt, code, or policy change.
Model calls Input and output, when permitted; start and end timestamps; duration; status; finish reason; input and output tokens; retries; provider error Explains quality changes, latency, rate limits, and spend.
Tool calls Tool name and version; normalized arguments; caller identity; requested permissions; approval decision; result status; result size; duration Reveals unexpected actions, denied operations, malformed arguments, and slow tools.
Orchestration Parent span; handoffs; loop and retry counters; cancellation reason; queue wait time Finds infinite loops, duplicate work, and bottlenecks between steps.
Dependencies Downstream service; endpoint or operation class; status code; timeout; circuit-breaker state; dependency trace ID Separates agent failures from outages in search, databases, identity, or third-party APIs.
Evaluation Groundedness, factuality, tool-use correctness, safety, policy outcome, task-success score, evaluator version Detects bad answers that transport-level health checks miss.
Security and governance Policy decisions; redaction actions; data classification; user consent or approval; access-control result Provides evidence that sensitive actions were authorized and handled under policy.

Use normalized values for high-cardinality fields. For example, store a hashed or classified user identifier rather than an email address, and record an operation class such as refund.create instead of retaining an entire URL with query parameters.

How do I instrument an agent end to end?

Start with a stable correlation model

Create a run ID at the API edge and propagate it through every internal request. Keep a separate session or conversation ID for multi-turn context, and a parent span ID for each nested operation. Put those identifiers in structured logs and trace attributes so an alert can open the complete timeline.

Represent the workflow as nested spans

Create spans for the session, turn, model generation, tool call, approval, handoff, and dependency request. Record start time, end time, status, and a bounded error object on every span. A model span should link to the prompt and response versions; a tool span should link to the exact validated argument object and the approval decision.

Adopt OpenTelemetry GenAI conventions

OpenTelemetry GenAI semantic conventions provide portable names for model, agent, tool, and orchestration telemetry. Google recommends OpenTelemetry-based instrumentation, Microsoft recommends the GenAI conventions, and AWS services can ingest OpenTelemetry traces. Cloudflare exposes compatible agent attributes and export paths. Portability matters when a team changes model providers or moves workloads between clouds.

Separate payloads from control metadata

Keep the trace usable even when message content is redacted. Store content references, hashes, classification labels, and sizes in the trace, while placing sensitive payloads in an encrypted, access-controlled store with its own retention policy. This preserves timing and causal relationships for most investigations without making every operator a reader of private conversations.

Which metrics reveal errors and outages?

Build dashboards at both fleet and run level. Fleet metrics show whether an incident is broad; trace drill-down shows which step is responsible.

Metric family Measure Interpretation
Traffic Requests, completed runs, cancellations, queue depth A sudden volume drop can indicate routing or authentication failure; a spike can exhaust quotas.
Reliability Overall success rate, model-call errors, tool-call errors, dependency errors, timeout rate Breaks a generic failure rate into the component most likely to need remediation.
Latency End-to-end duration, time to first token, model duration, tool duration, queue wait, dependency duration Shows whether users are waiting on reasoning, a tool, or an external service.
Control flow Retries, loop iterations, handoffs, cancellations, approval wait time Finds runaway plans, duplicate side effects, and approval bottlenecks.
Resources Input and output tokens, context size, rate-limit responses, concurrency, memory or CPU use Explains cost growth, throttling, and capacity-related outages.
Quality and safety Groundedness, factuality, tool-use correctness, policy denials, unsafe-action blocks, task outcome Detects silent degradation when infrastructure remains available.

Set alert thresholds from a known-good baseline for each agent and workload. Use separate alerts for a fast increase in errors, a sustained latency shift, a dependency-specific failure, and a rise in unsafe or denied actions. Avoid one global threshold: a retrieval-heavy workflow and a short classification workflow have different normal durations and token profiles.

How can I tell whether an outage is the model or a downstream API?

Compare the trace’s span statuses and timing in sequence. If model generations fail before any tool span starts, investigate provider availability, credentials, model limits, or prompt-size errors. If generations succeed but a specific tool or dependency has elevated timeouts or non-success responses, the downstream service is the more likely source.

Model-side symptoms

  • Provider errors or rate-limit responses across otherwise healthy tools.
  • Prompt or context-length errors after a change in retrieval volume or conversation history.
  • Long model spans with normal dependency timings.
  • Malformed structured output that fails schema validation before a tool call.

Tool or dependency symptoms

  • Normal model completion followed by repeated failures for one operation class.
  • Dependency latency or timeout increases isolated to one region or endpoint.
  • Valid arguments and approvals, but a downstream 4xx, 5xx, or circuit-breaker event.
  • Other agents using the same dependency showing the same error signature.

Tag each error with a suspected fault domain only after the trace is inspected. A model may generate a bad argument that causes a tool’s 4xx response, so preserve both spans rather than assigning blame to the last component that returned an error.

How do I detect unexpected or unsafe agent actions?

Record the complete action sequence and compare it with a behavioral baseline. The baseline can be built from approved production runs, replayed test cases, or a golden trajectory for a task. Alert when the sequence, arguments, permissions, or resource use deviates materially.

Useful deviation signals

  • A tool appears that the agent has not previously used for this task or tenant.
  • Arguments contain a new destination, unusually broad scope, or an amount outside the user’s normal range.
  • A denied or risky operation is retried, renamed, or reached through an alternate tool.
  • Retries, loop iterations, token use, or latency rise sharply without a corresponding workload change.
  • An agent skips a required approval, invokes a tool before authorization, or calls tools in an impossible order.

Enforce controls before execution

Put policy evaluation between the model’s proposed action and the real side effect. Validate arguments against a schema, enforce least-privilege credentials, require human approval for irreversible operations, apply rate and amount limits, and provide a deny rule or kill switch for a tool. Log the policy input, decision, rule version, and approval identity so an operator can prove why an action was allowed or blocked.

Evaluate the result, not only the action

Add automated evaluators for tool-use correctness, groundedness, factuality, safety, and task outcome. A syntactically valid call can still select the wrong record or use stale evidence. Store evaluator scores with their version and input references, then compare scores across releases and cohorts.

What is the incident-response workflow for an agent failure?

  1. Alert on a user-visible symptom. Start with elevated errors, timeouts, latency, dependency failures, unsafe-action blocks, or a quality-score drop rather than waiting for a complaint.
  2. Open the affected trace. Follow the timeline from session and turn to model generation, tool invocation, approval, handoff, and downstream service. Confirm whether the failure is isolated or repeated across runs.
  3. Compare with a known-good run. Examine action order, prompt and model versions, arguments, dependency responses, token use, and evaluation scores. A golden trajectory is useful for spotting the first divergence.
  4. Contain risk. Require approval, disable the affected tool, tighten deny rules, reduce rate limits, or route traffic to a safer model or workflow. Choose the smallest control that stops irreversible side effects.
  5. Preserve governed evidence. Retain the relevant logs, trace IDs, policy decisions, and deployment metadata under your approved retention and access rules. Do not copy sensitive payloads into tickets or chat rooms.
  6. Turn the incident into a regression test. Add the failing input and expected safe behavior to an evaluation dataset. Run the check against prompt, model, tool, and policy changes before restoring or expanding traffic.

Document the first observable symptom, the earliest divergent span, the containment action, and the evidence that service recovered. This makes a later post-incident review about system changes rather than speculation about model intent.

How should I monitor latency, token usage, and cost?

Measure latency at each boundary, not only at the HTTP edge. Record queue wait, time to first token, model generation time, tool execution time, dependency time, approval wait, and total run duration. Percentiles are more useful than averages because a small number of long plans can dominate user experience.

Track input and output tokens per model call and per completed run, plus retries and failed calls. Break usage down by agent, workflow, model version, tenant, and environment. A sudden context-size increase often signals retrieval duplication or an unbounded conversation history; a token increase with stable task volume can indicate a loop or prompt regression.

For cost accounting, attach the provider’s current unit rates outside the trace and calculate cost from recorded token quantities, tool invocations, and infrastructure usage. Keep the rate card version and currency with the calculation. Do not treat a failed call as free: retries and partial generations can consume quota and money even when a run ultimately fails.

Practical budget controls

  • Set maximum loop iterations, wall-clock duration, and token budgets per workflow.
  • Require approval before high-cost tools or large fan-out operations.
  • Cancel orphaned runs when a client disconnects or a parent workflow expires.
  • Alert on cost per successful task, not only total spend, so quality regressions are visible.

How do sampling, privacy, and retention affect trace reliability?

Full payload capture is expensive and may violate privacy, residency, or contractual requirements. Define a data contract that specifies what is captured, who can read it, where it is stored, how long it is retained, and how it is encrypted. Redact secrets and personal data before export, and restrict raw prompts and tool results to a small incident-response group.

Use unsampled or higher-rate traces for security-critical actions, policy denials, and a small set of golden workflows. Sample routine successful runs for fleet health, while keeping complete control metadata such as IDs, statuses, durations, and error classes. Sampling must be visible on dashboards so operators do not mistake a partial view for a complete conversation history.

Cloudflare warns that agent traces are sampled, subject to span-size limits, and not a complete or lossless conversation record. Large prompts or tool results may therefore be truncated or omitted. Store a governed payload reference and byte count, and provide a separate retrieval path for authorized forensic work.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How do monitoring platforms differ?

Products vary less by whether they can draw a trace and more by the depth and governance of that trace. Evaluate them against the same workload and failure scenarios.

Comparison axis Questions to ask
Trace depth Can you see model generations, tool arguments and results, handoffs, approvals, retries, and dependency spans in one timeline?
Metric coverage Are errors, latency, tokens, cost, volume, queue time, and loop counts available by agent and workflow?
Evaluation Can the system run online checks for groundedness, factuality, safety, tool correctness, and task outcome, then compare releases?
Portability Does it ingest and export OpenTelemetry GenAI data without proprietary fields becoming mandatory?
Alerting and SLOs Can alerts combine service symptoms with quality or safety signals and link directly to an affected trace?
Privacy controls Are redaction, field-level access, regional storage, encryption, retention, and deletion policies configurable?
Sampling and completeness Are sampling rules, span-size limits, dropped spans, and payload truncation visible to operators?
Fleet views and export Can teams compare agents, tenants, models, and versions, and retrieve data through an API for incident tooling?

OpenAI tracing provides concepts such as sessions, turns, model generations, tool calls, and delegated agents. Google Cloud emphasizes agent observability across LLM interactions, tool usage, behavior, performance, security, safety, and quality. Amazon CloudWatch Omni combines trace analysis, agent topology, evaluations, datasets, experiments, and aggregate production health views. Microsoft’s approach centers on AI-native logs, metrics, traces, behavioral baselines, and continuous evaluation. Cloudflare’s agent tracing exposes attributes such as agent name and ID, conversation ID, tool arguments, and tool results. Treat these as capability examples, then verify current retention, regional availability, pricing, and export behavior for your deployment.

What is a practical rollout plan?

Phase 1: establish control-plane visibility

Instrument run IDs, statuses, durations, model versions, tool names, dependency outcomes, and error classes. Build a dashboard for volume, success rate, timeout rate, end-to-end latency, and top failing components.

Phase 2: add action and quality evidence

Capture normalized arguments, approvals, policy decisions, retries, loop counts, token usage, and evaluator scores. Create a trace view that follows one run from user request to final response.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Phase 3: enforce safety and regression gates

Define behavioral baselines, alert on deviations, add approval gates and kill switches, and place representative failures in an evaluation dataset. Block releases that regress safety, tool correctness, or task outcome beyond your agreed tolerance.

Phase 4: optimize operations

Tune sampling and retention by risk, correlate cost with successful tasks, add dependency-level SLOs, and export OpenTelemetry data so observability remains portable as models and infrastructure change.

What should be on an operator’s checklist?

  • Every run has a stable ID propagated to logs, traces, and downstream requests.
  • Model, prompt, code, tool-schema, and policy versions are recorded.
  • Tool arguments, permissions, approvals, results, and side-effect status are traceable.
  • Dashboards separate model, tool, dependency, queue, and policy failures.
  • Alerts cover errors, timeouts, latency shifts, retries, loops, token spikes, and safety deviations.
  • Golden trajectories and evaluation datasets run on every material change.
  • Raw payload access is minimized, encrypted, region-aware, and retention-controlled.
  • Sampling and span limits are visible, with a governed path for forensic retrieval.
  • High-impact tools have schema validation, least privilege, approval, rate limits, and a kill switch.
  • Incident evidence is preserved and converted into a regression test before traffic is restored.

Frequently Asked Questions

Can monitoring prove that an AI answer is true?

No. Telemetry can show which evidence was retrieved, which tools were called, and how evaluators scored the response, but it cannot establish truth by itself. For high-impact domains, require authoritative source checks or human review in addition to automated groundedness and factuality tests.

How long should agent traces be retained?

Use a risk-based schedule rather than one universal period. Keep control metadata and incident records long enough for your security, contractual, and regulatory obligations, while deleting or anonymizing raw prompts and tool results sooner when they are not needed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What if my agent framework cannot emit OpenTelemetry?

Add an adapter at the orchestration boundary that creates spans from lifecycle callbacks, then propagate the run and parent-span IDs into tools and dependencies. Preserve the same semantic fields so you can replace the adapter later without redesigning dashboards or alerts.

How do I reduce alert fatigue from normal agent variability?

Alert on deviations from workload-specific baselines and combine several signals, such as a new tool plus a policy denial or a latency shift plus dependency errors. Route low-confidence quality findings to review queues and reserve paging for user impact, safety risk, or sustained service degradation.

Should successful runs be sampled or stored in full?

Sample routine successes for fleet-level metrics, but retain complete traces for golden workflows, high-impact actions, policy violations, and a statistically useful review set. Always keep enough identifiers and timing metadata to detect gaps and to locate an authorized payload copy when an incident requires it.
