Keep people at the points where an AI mistake becomes costly, irreversible, or externally binding—not on every low-risk step. Classify each action by consequence and reversibility, then use the lightest gate that controls that risk: notify for safe automatic work, confirm for moderate-risk actions, draft for content that represents the organisation, and mandatory review for regulated, safety, legal, clinical, or financial decisions. Route low-confidence or high-impact cases to a named reviewer, show the evidence and changes in one compact screen, and measure reviewer time, queue age, overrides, severe-error escapes, and rejection handling. This design preserves accountability while allowing routine work to flow at machine speed.
What does human review mean in an AI workflow?
Human review is a deliberate control point at which a person can inspect an AI output, correct it, approve it, or stop the workflow before (or immediately after) a consequential action. It is not the same as placing a human somewhere in the process and asking them to click “continue.” Microsoft’s runbook puts the standard plainly: “The reviewer must be able to evaluate, not just click.”

A useful workflow distinguishes three states that are often confused:
- Generated: the model produced an output, but no authorised person has accepted responsibility for it.
- Reviewed: a person inspected the relevant evidence and either accepted or edited the output.
- Committed: the approved action was written to the destination system, sent externally, or otherwise became operational.
Keep those states visible in the CRM, ticketing system, document library, or other destination. If a draft or unreviewed record looks identical to a final one, downstream users will treat machine text as settled fact.

How do I decide which AI outputs need approval?
Start with the consequence of being wrong and the reversibility of the action. The model’s confidence is useful, but it is not a substitute for this risk classification.
Classify consequence
Ask who could be harmed, what obligations could be triggered, and how widely an error could propagate. A typo in an internal tag is usually low consequence; a wrong payment instruction, clinical recommendation, legal filing, safety control, or public statement is high consequence. Include privacy, security, financial loss, discrimination, contractual exposure, and reputational damage in the assessment.
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Clear out junk files and repair common Windows errorsFree Scan →Classify reversibility
An action is reversible only if you can reliably undo it before harm occurs. Deleting a duplicate draft may be reversible; sending an email to thousands of customers, changing access permissions, or releasing a payment may not be. “Undo” must be tested in the real destination system, not assumed because an API offers a delete call.
Assign a gate
| Gate | Use it when | What the workflow does |
|---|---|---|
| Notify | Low consequence, reversible, and well understood | Executes automatically, then records a notification for monitoring or sampling. |
| Confirm | Moderate consequence or limited reversibility | Pauses and asks an authorised person to approve the specific action before execution. |
| Draft | The output represents the organisation or contains important numbers | Creates a clearly labelled draft; a person edits and commits it in the destination system. |
| Mandatory review | Regulated, safety, legal, clinical, financial, or otherwise irreversible work | Blocks continuation until a named, qualified role records a decision. |
Choose the least intensive gate that still controls the consequence. Microsoft’s runbook expresses the economic test as “Review must be cheaper than doing it manually.” That means reducing the amount each reviewer must read, not removing the reviewer from a high-risk decision.
Where should a checkpoint sit in the process?
Place the checkpoint immediately before commitment, when the reviewer still has a meaningful ability to approve, correct, or stop the action. Google Cloud describes this human-in-the-loop pattern as pausing an agent at a predefined checkpoint so a person can provide input before the agent continues.
Before commitment
For an outbound message, the gate belongs before sending; for a permission change, before the API call; for a purchase, before the order is submitted. Present the exact payload that will be committed, not a paraphrase generated for the reviewer.

After safe execution
Low-risk, reversible actions can run first and notify a person afterward. Examples include adding a non-authoritative label, creating a duplicate-safe internal task, or proposing a search query. Record the event and sample it so automatic passage does not become invisible.

At handoffs
Insert a gate when an AI output crosses from one team or system to another. The receiving system should carry the status, approver identity, timestamp, and any unresolved questions. A handoff is a risk boundary even when the model itself has not changed.
How should I route uncertain or high-impact cases?
Use selective escalation: defer cases with low confidence, high impact, or both, while allowing demonstrably safe cases to proceed. Research in the ACM literature finds that confidence-based deferral can save time with little performance loss when confidence estimates are well calibrated.
Combine confidence with impact
Do not let a high-confidence score override a high-consequence category. A model can be certain and wrong. A practical policy is:
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- High impact, any confidence: send to mandatory review.
- Moderate impact, low confidence: send to a specialist queue or confirmation gate.
- Low impact, high confidence: permit automatic execution, with monitoring samples.
- Low impact, low confidence: ask for clarification, retry with better context, or route to a general queue.
Set thresholds from a labelled local sample. Calibrate scores separately for each task, language, customer segment, and model version; a single global threshold hides uneven error rates.
Use a queue policy reviewers can understand
Every escalation should include a reason code such as “low confidence,” “high-value transaction,” “conflicting source records,” or “policy category.” Sort first by potential harm and deadline, then by arrival time. This prevents a large volume of trivial uncertain items from burying a small number of urgent cases.
Keep an escape hatch for novel cases
Route records with missing evidence, contradictory sources, or an unfamiliar category to a specialist rather than forcing a binary approve/reject decision. A reviewer should be able to request more information without losing the item’s place in the workflow.
What should a reviewer see on one screen?
A reviewer should be able to evaluate the decision without opening a chain of unrelated tools. Microsoft guidance says to make clear how well the system can do what it can do; UK government cyber-security guidance similarly calls for systems that make it easy for humans to assess outputs for which they are responsible.

Minimum review packet
- Proposed action: the exact text, fields, parameters, or API payload that will be committed.
- Evidence: the source passage, record fields, calculations, and timestamps used to produce it.
- Confidence and rationale: a calibrated score or band, the trigger for escalation, and material assumptions. Do not present fabricated explanations as proof of correctness.
- Diff or before/after: highlight every changed word, number, permission, or field value.
- Policy context: the rule, threshold, or approval authority that applies.
- Controls: approve, edit, reject, request information, and route to a specialist.
- Audit details: item ID, model and prompt version, source versions, reviewer identity, decision, reason, and timestamp.
Keep review units small enough to read completely. A long report with one approve button encourages rubber-stamping; split it into claims, transactions, or paragraphs that can be checked independently.
Make numbers verifiable
For totals, dates, rates, and thresholds, show the source value beside the generated value and expose the calculation. If a value was transformed, display the formula or conversion. Important numbers should never be accepted solely because the surrounding prose sounds plausible.
How do I design rejection, correction, and recovery?
A rejection is a workflow branch, not an error message. Before production, define where a rejected item goes and what happens to downstream work.
- Capture the decision and reason. Require a short reason code plus optional notes; record who made the decision and when.
- Choose the next owner. Route to the agent for a constrained retry, a specialist queue, the requester for missing information, or disposal when the task is no longer valid.
- Preserve the original. Keep the rejected output and evidence linked to the new attempt so changes can be audited.
- Prevent duplicate side effects. Use an idempotency key or equivalent check before retries so a second attempt cannot send, charge, or create the same action twice.
- Set a deadline. Escalate items that remain unresolved beyond the service-level target; do not let them disappear in a dead-letter queue.
- Test rollback. For actions marked reversible, rehearse the undo path and verify that linked systems return to a safe state.
Microsoft security guidance recommends deterministic human-in-the-loop enforcement for high-risk or irreversible actions through orchestrator logic rather than model reasoning. In practice, the model may suggest an action, but only policy code should be able to release it.
How can I measure whether review is helping or becoming a bottleneck?
Measure both safety and flow. A shorter queue is not an improvement if severe errors escape; a perfect review rate is not sustainable if reviewers spend more time than a manual process would require.
| Metric | What it reveals | Useful breakdowns |
|---|---|---|
| Reviewer minutes per item | Whether the review unit and evidence are efficient | Gate, task type, reviewer role, and model version |
| Queue age and oldest item | Whether work is waiting too long | Priority, region, shift, and specialist queue |
| Acceptance and override rates | How often reviewers agree with the output and where they correct it | Confidence band, category, and source quality |
| Severe-error escapes | Whether harmful mistakes reached users or systems | Impact class, failure mode, and detection point |
| Escalation rate | How much work is being deferred to people | Threshold, model, and workflow branch |
| Reject-path completion | Whether rejected work receives a documented outcome | Reason code, destination, retry count, and age |
Track distributions, not just averages. A mean of five minutes can conceal a small set of cases that consume an hour. Monitor the 95th-percentile review time, the oldest queue item, and the share of items awaiting specialist input.
Establish a baseline before changing gates
Run a pilot in review-all mode for a representative period. Measure local accuracy, reviewer time, disagreement reasons, and the frequency of severe errors. Then enable selective passage for one low-risk category and compare it with the baseline. Relax a gate only after that category shows stable accuracy over a meaningful period, including changes in data and workload.

Sample automatic passes
Review a random sample of notify-gate items and compare them with escalated items. Sampling detects silent failures, confidence drift, and systematic errors that acceptance rates cannot show because no person saw the automatically passed cases.

What evidence shows that selective review can reduce work?
Selective review is promising but domain-specific. Tahmo and colleagues reported in the Journal of Clinical Epidemiology (2026) that model-assisted title and abstract screening reduced workload by 48.3% while reporting 86.7% sensitivity. In the same study, model-assisted full-text screening reduced workload by 81.3% while reporting 82.6% sensitivity.
Those figures describe a machine-assisted knowledge-synthesis workflow, not a universal target for customer service, finance, software operations, or clinical care. Reproduce the measurement with your own labels, harm thresholds, languages, and reviewer population before using similar gates.
NIST’s 2024 AI Use Taxonomy defines 16 activities for describing human-AI work in terms of outcomes. Use that outcome-centered framing when writing requirements: specify what the person must decide and what evidence they need, rather than asking only whether a model feature is available. Microsoft Research’s 2019 guidance contains 18 interaction practices grouped around initial interaction, interaction in progress, handling errors, and change over time; these are useful prompts for designing reviewer controls and feedback loops.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How do I prevent reviewers from rubber-stamping?
Design the interface and incentives so approval requires a real check. Show the evidence and diff before the approval control, require a reason for overrides and high-impact approvals, and make the reviewer accountable by role rather than by an anonymous button.
- Hide the approve control until required evidence has loaded.
- Require an explicit selection when a reviewer accepts an exception or overrides a policy warning.
- Use periodic blind samples in which a second reviewer checks accepted items.
- Rotate difficult cases and provide escalation support instead of rewarding speed alone.
- Alert on impossible patterns, such as a reviewer approving every item in seconds or never using correction controls.
These checks should identify process problems, not punish a reviewer who follows a poor system design. If evidence is incomplete or the task is intrinsically ambiguous, improve the workflow rather than demanding faster approval.
How should corrections improve the AI workflow?
Treat every correction as structured feedback. Store the original output, the edited result, the reason, the policy category, and the relevant source data. Review recurring corrections in a governance meeting and decide whether the remedy is better retrieval, a clearer policy, a prompt change, a model update, a new threshold, or a new mandatory gate.
Best Value
Evaluate changes against a fixed holdout set and a fresh sample from production. A lower overall error rate can still conceal a new failure concentrated in a vulnerable group or high-impact category. Version the model, prompts, policies, thresholds, and reviewer instructions together so an incident can be reconstructed.
What is a practical rollout plan?
Implement the controls in stages so you can find bottlenecks before they affect customers or regulated records.
- Map the workflow. List every model output, tool call, external communication, data write, and handoff.
- Assign consequence and reversibility. Document the affected people, possible harm, undo procedure, and accountable owner for each step.
- Select gates. Apply notify, confirm, draft, or mandatory review; record the policy reason for each choice.
- Build the review packet. Add source evidence, confidence, diff, policy context, controls, and audit fields to one surface.
- Define queues and rejection routes. Name the reviewer role, priority rules, retry limits, specialist path, and service-level target.
- Run review-all mode. Establish accuracy, minutes per item, queue age, override reasons, and severe-error baselines.
- Enable selective passage. Start with one low-risk, reversible category and a conservative confidence threshold.
- Sample and stress-test. Inspect automatic passes, contradictory records, missing data, peak volume, and model-version changes.
- Adjust only with evidence. Recalibrate thresholds or relax gates when safety and flow metrics remain acceptable over time.
What are the most common bottleneck causes?
Most queues slow down because the review task is poorly scoped, not because humans are inherently too slow.
- Oversized review units: split a long document into claims or decisions with local evidence.
- Missing context: fetch the source record automatically and show it beside the proposed change.
- Ambiguous authority: assign a named role and backup rather than a generic team inbox.
- One threshold for every task: calibrate by category and impact.
- Hidden specialist work: count time spent waiting for legal, clinical, security, or finance input.
- Unbounded retries: cap retries and escalate recurring failures with their history attached.
- Invisible drafts: preserve draft status in the destination so downstream users do not create duplicate manual checks.
Which design principles should govern a final review policy?
A durable policy makes accountability explicit while keeping routine work moving.
- Gate consequences, not model novelty: a familiar-looking output can still trigger an irreversible action.
- Use deterministic orchestration for high-risk decisions; do not rely on the model to decide whether it needs a human.
- Give reviewers enough evidence to evaluate, not merely click.
- Escalate uncertainty and impact, and sample what passes automatically.
- Keep generated, reviewed, and committed states distinct in every destination system.
- Make rejection, rollback, and incident response executable procedures.
- Change thresholds only after measuring safety, workload, and drift together.
Frequently Asked Questions
What is the difference between a human-in-the-loop and a human-on-the-loop design?
In a human-in-the-loop design, the workflow pauses and requires a person’s decision before a specified action continues. In a human-on-the-loop design, low-risk actions run automatically while a person monitors events, samples results, and can intervene; it is appropriate only when the action is reversible and the monitoring response is fast enough to limit harm.
How many reviewers should approve a high-risk AI action?
The correct number depends on the consequence, independence requirements, and applicable policy; there is no universal percentage or staffing formula. Define whether one qualified approver is sufficient, when a second-person check is required, and how conflicts are resolved before deployment.
Should reviewers see the model’s chain-of-thought or hidden reasoning?
No. Reviewers need source evidence, assumptions, calculations, confidence information, and an inspectable diff—not private internal reasoning that may be incomplete or fabricated. A concise, testable rationale is safer and easier to audit.
When should an AI workflow stop instead of escalating to a person?
Stop when required evidence is unavailable, a policy conflict cannot be resolved, or the action could cause harm before a qualified reviewer can respond. Preserve the item and its context, notify the owner, and create an incident or exception record rather than silently retrying.
