Do not treat an AI answer as verified. Fact-check it by breaking every factual sentence into separate claims, ranking those claims by the harm a mistake could cause, and checking the most important ones against the original authority: a law or regulator for a rule, the publisher of a paper for a study, or the agency that collected statistics for a number. Then confirm that each citation exists, read the cited passage in context, check its date and scope, and corroborate consequential findings independently. OpenAI’s Help Center warns that ChatGPT can produce incorrect or misleading content, including fabricated quotes, studies, citations and references. The House of Commons Library likewise says AI is an assistant, not an authority. The workflow below turns an apparently polished answer into a documented, auditable set of claims.
What does it mean to fact-check an AI answer?
Fact-checking means testing each proposition in the answer against evidence that is independent of the model. You are not judging whether the prose sounds confident or whether a link looks official. You are asking whether a specific source supports the exact date, number, quotation, definition and conclusion the answer presents.

Use this rule as your starting point: every factual statement generated by an AI system is unverified until you check it. A response can contain a mixture of accurate information, outdated information, a real citation used for a different purpose and a citation that does not exist. Treating the whole paragraph as one claim hides those differences.
- Primary source: the institution that issued the rule, the original research paper, the underlying dataset, legislation, a regulator’s notice or an official statistical release.
- Secondary source: a publication or analyst interpreting primary material. It can help you find the original evidence, but it is not a substitute when the original is available.
- Lead: an AI-provided title, link or name that helps you search. A lead becomes evidence only after you open and evaluate the source yourself.
The goal is not to prove that an answer is entirely right or entirely wrong. The useful result is claim-level: supported, contradicted or unresolved, with the source and date you checked recorded for each item.

How do I extract and split the claims?
Copy the answer into a working document and rewrite it as a numbered list of propositions. Preserve qualifiers instead of paraphrasing them away: “as of March 2025,” “for residents of Ontario,” “in a randomized trial,” and “may” all change what must be verified.
Separate facts from advice and interpretation
Mark statements that describe the world (facts), recommend an action (advice), explain a likely cause (interpretation), or express a value judgment. A recommendation can be sensible even when a supporting statistic is wrong, but you should not let an unverified statistic make the decision for you.

Break compound sentences apart
Consider the sentence: “The 2024 rule applies to small employers, requires a written notice within 30 days, and carries a $5,000 penalty.” This is at least three claims: who is covered, the deadline, and the penalty. A single citation may support one and not the others. Create separate rows for each proposition.
Capture the verification fields
For every claim, note the jurisdiction or location, relevant population, time period, version or edition, unit of measurement and conditions. Also record whether the statement is a direct quotation, a calculated number or a prediction. These details tell you what kind of source and search you need.
Which AI claims should I check first?
Start with claims that could change a decision or cause harm, not with the easiest sentence to verify. The House of Commons Library and public-sector guidance recommend prioritizing consequence, authority and currency.
- Deadlines and eligibility: dates, filing windows, visa requirements, benefit rules, product recalls and access conditions can make an action invalid if they are wrong.
- Legal, policy and regulatory statements: check the enacted text or current regulator guidance for the relevant jurisdiction, not a summary written for another place.
- Health and financial advice: verify the underlying clinical, regulatory or market evidence and obtain qualified professional advice where a mistake could affect safety or money.
- Exact numbers: statistics, prices, rates and penalties need a named data collector, date, method, denominator and unit.
- Quotations and highly specific claims: a precise quote, study result or historical detail is easy to repeat incorrectly and often easy to test directly.
- Surprising or contested assertions: unusual claims deserve independent confirmation even when they include a plausible-looking citation.
Lower-risk background statements can wait. If time is limited, verify the few claims on which your decision depends and label the rest as unverified rather than implying that the whole answer has been checked.
How do I find the primary source?
Search outward from the issuing authority, not inward from the AI’s wording. Identify who had the legal, scientific or administrative responsibility to create the evidence, then search that organization’s site, document repository or data portal.
Match the source to the claim
| Claim type | Preferred primary source | What to match |
|---|---|---|
| Law, regulation or deadline | Enacted legislation, official register, regulator or agency notice | Jurisdiction, effective date, covered parties, exceptions and current version |
| Research finding | Original journal article, conference paper, trial registry or author’s data | Study design, population, outcome definition, analysis and publication status |
| Statistic or trend | Official statistical agency, census, survey or dataset publisher | Collection date, sample, denominator, revisions and methodology |
| Company or product fact | Manufacturer documentation, filing, release note or contractual terms | Model, edition, region, date and whether the statement is a promise or a measured result |
| Historical quotation | Original transcript, recording, book or archival document | Exact wording, speaker, date, translation and surrounding passage |
Use the AI’s citation to extract search terms, then search the institution, title, author, date and identifier separately. A news article or reference page can point you to a document, but follow the trail until you can inspect the original.
Use stable, document-specific locations
Prefer a page or file that identifies the document, issuing body, publication date and version. A homepage, a search-results page or a link that redirects through several domains is weak provenance. Save the document-specific address and, when appropriate, a downloaded copy or archive permitted by the source’s terms.

How can I tell whether an AI citation is real?
Existence is the first test, not the last. Search the claimed title, author, publisher, date and DOI or other identifier in the issuing institution’s catalogue, a library catalogue or the relevant registry.

- Search the title in quotation marks, then search the author and publisher together if no exact match appears.
- Check that the document’s title, authors, date, publisher and identifier all match the AI response.
- Open the record from the publisher or issuing institution rather than relying on a scraped citation page.
- Look for corrections, retractions, superseded editions or a notice that the item is a proposal, preprint or commentary.
A DOI that resolves proves that an identifier points somewhere; it does not prove that the document supports the claim. Likewise, a real government page can be out of date or apply to a different jurisdiction. If you cannot locate the cited item in an authoritative catalogue or publisher site, record the citation as unresolved and do not repeat it as evidence.
How do I read a source in context?
Read the relevant passage and the material immediately around it. Your question is not “Does this page mention the same topic?” but “Does it support this exact proposition at this strength?”
Check wording and scope
Watch for shifts such as “associated with” becoming “causes,” “in this sample” becoming “for everyone,” or a preliminary estimate becoming a settled figure. Check footnotes, tables, appendices and definitions. A headline may summarize a result more broadly than the methods justify.
Check numbers and denominators
Reproduce simple calculations when possible. Confirm whether a percentage uses all respondents, only valid responses, a weighted population or a selected subgroup. Distinguish percentage points from percent change, nominal prices from inflation-adjusted values, and a one-time allowance from a recurring entitlement.
Check quotations
Locate the original transcript, page or timestamp. Verify that the words are exact, that a translation is identified, and that omitted sentences do not reverse the meaning. If an AI gives a quotation without a stable original location, treat it as unverified even when the wording sounds familiar.
How do I check currency, jurisdiction and scope?
Write the source’s date, jurisdiction, version, population and time period next to the claim. A source can be authentic and still fail to answer the question because one of those fields does not match.
- Date: distinguish publication, update, observation and effective dates. A current webpage may describe an older observation period.
- Jurisdiction: confirm country, state, province, city, regulator or court. Similar names do not imply identical rules.
- Version: check the model, software release, policy edition or amended statute cited.
- Population and setting: compare age, industry, income, sample selection, clinical setting or other inclusion criteria.
- Time period: ensure that a result from one month, season or economic cycle is not presented as a permanent trend.
Record a mismatch explicitly. “Accurate for the 2021 edition, not the current edition” is more useful than a vague statement that the AI was wrong.
What should I do when two authoritative sources disagree?
Do not average conflicting figures or write a blended claim that neither source makes. Compare the disagreement systematically and explain why one source is more applicable, or leave the issue unresolved.

| Comparison question | Why it matters |
|---|---|
| Which jurisdiction and date? | Rules and conditions can change across places and time. |
| Who authored each source and what mandate do they have? | A regulator, court, academic group and advocacy organization may answer different questions. |
| What underlying document or dataset is used? | Sources may appear independent while reproducing the same original error. |
| How are terms and outcomes defined? | Different definitions can produce different but internally correct numbers. |
| What population and method were used? | Sampling, exclusions, weighting and measurement change comparability. |
| Which version or update is current? | A superseded document should not override an effective revision. |
| What incentives or purpose are visible? | Sales, advocacy and public information have different risks of selective presentation. |
Use lateral reading: investigate the source’s authority and purpose, then consult another independent authoritative source. Independence means more than two pages repeating the same press release.
What does it mean if the AI changes its answer?
Instability is a warning to increase external checking, not evidence that one version is correct. Rephrase the prompt, ask the system to list assumptions and limitations, and compare the names, dates, figures and citations it produces.
Material changes reveal which parts are poorly grounded. If one run names a different study, changes a deadline or supplies a new quotation, preserve the variants and verify the underlying claim directly. Do not select the version that sounds most confident or contains the most links.
How should I judge the quality of a source?
Evaluate five axes together. A source with an impressive design can still be irrelevant or opaque; a plain administrative page can be highly authoritative for a narrow question.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsAccuracy
Does the source faithfully report the evidence, show calculations and correct errors? For research, inspect methods and results rather than relying on an abstract alone.
Authority
Who produced it, what expertise or legal mandate do they have, and are they responsible for the underlying record? Authority is claim-specific: a licensing agency is authoritative for its own licence rules, not necessarily for medical effects.
Relevance and currency
Does it fit the question’s place, population, date and version? An older source may remain historically valuable while failing to answer a current question.
Transparency
Can you see the methods, documents, data definitions, authorship and stable links? Missing provenance makes an otherwise plausible statement difficult to audit.
Purpose
Is the source informing, selling, campaigning or persuading? Purpose does not automatically disqualify a source, but it tells you what omissions or framing to test with independent evidence.

What red flags show that an AI answer needs extra checking?
These warning signs do not prove an answer is false; they tell you where to spend verification time.
- The citation cannot be found in a library catalogue, publisher site, DOI registry or issuing institution.
- The source exists, but its text lacks the quoted figure, date, wording or conclusion.
- A precise number has no data collector, year, method or denominator.
- A quotation has no original transcript, publication or stable document location.
- One commercial or advocacy source is treated as decisive while relevant counterevidence is absent.
- Different prompt runs produce materially different names, dates, citations or explanations.
- A polished page has weak provenance, no method, broken links or unexplained redirects.
When a red flag appears, narrow the claim, locate a primary source or mark the result unresolved. Do not repair a weak citation by adding more unsourced commentary.
How can I demonstrate a claim-to-passage check?
Use a small, explicit example whenever you are documenting a review. Suppose an AI answer says, “The agency requires all applicants to submit the form within 30 days.” Do not search only for the phrase “30 days.”
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- Split it into coverage (“all applicants”), action (“submit the form”), deadline (“within 30 days”) and authority (“the agency”).
- Find the agency’s current rule or official notice for the relevant jurisdiction.
- Read the operative section, definitions and exceptions. Determine whether “30 days” starts at application, notice or another event.
- Compare the source’s population with “all applicants.” If it covers only a category, downgrade the AI statement to that category.
- Record the exact passage, document version, date checked and any unresolved exception.
If the official text says “eligible applicants must submit within 30 calendar days of receiving notice,” the original sentence is overbroad. Your corrected note should preserve the trigger and eligibility condition rather than silently repeating the simpler wording.
What should an audit trail contain?
A compact log makes your conclusion reproducible and prevents a checked claim from being detached from its qualification.
| Field | What to record |
|---|---|
| Claim ID and exact wording | The smallest proposition you evaluated, including qualifiers. |
| Risk or consequence | Why an error matters and how urgently it must be checked. |
| Primary source | Issuing body, title, document-specific location and identifier. |
| Publication or update date | Plus effective date, observation period or version when relevant. |
| Passage or data element | Page, section, table, figure, paragraph or timestamp that bears on the claim. |
| Result | Supported, contradicted or unresolved. |
| Scope and uncertainty | Population, jurisdiction, exceptions, limitations and remaining disagreement. |
| Date checked | When you performed the verification. |
Cite the source you actually read, not a link copied from the AI. For high-consequence work, retain a permitted copy or snapshot because web pages, datasets and policies can change.
What if the primary source is missing, inaccessible or ambiguous?
Do not fill the gap with confidence. Search the issuing institution’s archive, a library catalogue, a registry or a later document that explicitly supersedes the missing item. A reputable secondary source may describe the evidence, but label it secondary and keep looking for the original when the decision warrants it.
Recommended Free Tools
If a document is paywalled, use its bibliographic record and abstract only for what they actually establish, then look for an author manuscript, institutional repository or official dataset. Do not infer the full methods or conclusion from an abstract. If the source remains unavailable, report the claim as unresolved and state what evidence could settle it.
When wording is genuinely ambiguous, quote the relevant language and describe the competing interpretations. Ask the responsible institution for clarification where practical; an AI-generated interpretation is not an authoritative ruling.
Best Value
How should I handle health, legal and financial answers?
These domains require a higher verification threshold because a plausible error can cause immediate harm. Confirm the current jurisdiction and date, check the responsible authority’s primary material, and obtain qualified professional advice for an individual decision.
- Health: verify recommendations against current guidance, trial evidence or medicine labels; check population, dose, contraindications and whether the evidence is observational or experimental.
- Law: read the enacted text, current regulation or court decision; distinguish general information from advice about your facts and jurisdiction.
- Finance: confirm fees, rates, eligibility, tax treatment and risk disclosures with the responsible institution or regulator; distinguish historical performance from a guarantee.
An AI answer can help you formulate questions for a clinician, lawyer or financial professional. It should not be the final authority for a decision where a wrong assumption carries serious consequences.
What is the fastest reliable workflow?
For a short answer, use this sequence without skipping the claim-level discipline:
- Copy the response and number every factual proposition.
- Mark the claims that affect safety, money, rights, deadlines or eligibility.
- Search for the responsible institution, original paper, legislation or official dataset.
- Verify that each citation exists and matches its author, title, date and identifier.
- Read the operative passage, table or method and compare wording, scope and definitions.
- Check currency, jurisdiction, version, population and time period.
- Corroborate consequential findings with an independent authoritative source.
- Record supported, contradicted and unresolved results with the date checked.
- Rewrite the answer so every surviving claim carries its necessary qualification.
This process is deliberately slower than accepting a fluent response, but it prevents a fabricated citation or a real source used out of context from becoming a false fact in your work.
Frequently Asked Questions
Can I trust a DOI just because it resolves?
No. A resolving DOI confirms that an identifier points to a record, not that the paper supports the AI’s claim. Check the paper’s methods and results, and look for corrections, retractions or a newer version.
What should I do when an AI gives a link that redirects or stops working?
Search the issuing organization by title, author and date, then locate a stable document-specific record or archive. If you cannot verify the original text, mark the claim unresolved rather than treating the dead link as evidence.
Is asking the AI to provide more citations a valid fact-check?
It can generate useful search leads, but it is not independent verification. Compare multiple runs only to identify unstable details; the final check must come from the original authority or evidence.
How long should I keep my fact-check notes?
Keep the claim, source, passage, version and date checked for as long as the answer could influence a decision. For regulated, legal or safety-related work, retain permitted source copies or snapshots so later readers can reproduce the check.
