Evaluate a connected medical device as a safety-critical system, not as a standalone appliance. Before signing a purchase order, map every device, cloud service, mobile app, gateway, workstation, network connection and remote-support path; require the vendor’s threat model, cybersecurity risk assessment, machine-readable SBOM, testing evidence, vulnerability-response plan and security-support end date; then price the staffing, downtime and validation work your facility will carry. A device with excellent clinical capabilities can still be a poor purchase if its supplier cannot explain how vulnerabilities are found, patched and communicated throughout the product’s service life.

What does medical-device cybersecurity include?

Cybersecurity covers the complete technology and operating environment that can affect a device’s confidentiality, integrity, availability or safe clinical function. That usually extends beyond the hardware in the treatment room.

  • Device and embedded software: sensors, firmware, operating systems, interfaces, storage and local user accounts.
  • Connected services: vendor clouds, analytics portals, licensing servers, update servers and APIs.
  • Supporting components: mobile applications, gateways, bedside workstations, servers and network appliances.
  • People and privileges: clinicians, biomedical engineers, administrators, vendor technicians and service accounts.
  • Data paths: telemetry, patient records, diagnostics, logs, backups and remote-maintenance traffic.

The buyer’s question is therefore not simply “Does this device have encryption?” It is whether the whole system can be operated safely when a component is compromised, a vulnerability is disclosed or a supplier stops supporting an element.

The U.S. Food and Drug Administration (FDA) describes the problem plainly: “Threats and vulnerabilities cannot be eliminated and reducing cybersecurity risks is especially challenging.” Treat residual risk as part of device safety and total lifecycle cost.

How do I define the system boundary before asking for security evidence?

Start with a one-page boundary and data-flow diagram. Make the vendor identify what it controls, what your facility controls and what is inherited from another supplier. Do not accept a diagram that shows only the product’s enclosure.

  1. Inventory every component. List the clinical device, firmware, optional modules, cloud tenancy, mobile app, gateway, workstation software, network interfaces, storage and backup targets.
  2. Draw trust boundaries. Mark hospital networks, internet connections, wireless links, vendor-managed segments, removable media and remote-service tunnels.
  3. Identify data and commands. Show where patient data, configuration files, credentials, alarms and treatment commands originate, travel and reside.
  4. Map identities. Record each role, authentication method, privileged account, service account and emergency-access mechanism.
  5. Assign ownership. For every component, record the party responsible for hardening, monitoring, patching, incident response, backup and retirement.
  6. Document dependencies. Include operating systems, databases, third-party libraries, network services and cloud providers that could bring a vulnerability into the system.

Ask the supplier to confirm the diagram in writing. If a cloud service or remote-support component is omitted, your later risk assessment and contract will be incomplete.

What security documents should a vendor provide?

Request a security risk package rather than isolated marketing statements. FDA’s February 2026 final premarket guidance says a security risk report should connect the threat model, cybersecurity risk assessment, SBOM and testing documentation. Ask for the following, with the product version and date on each document.

Threat model and architecture views

Look for trust boundaries, attack surfaces, authentication flows, update paths, abuse cases and assumptions about the facility network. A useful threat model explains why controls were selected and which threats remain plausible.

Cybersecurity risk assessment

Require risks to be tied to clinical consequences, not just technical severity. The assessment should distinguish an event that merely interrupts a dashboard from one that could alter therapy, suppress an alarm or expose sensitive records.

Residual-risk and unresolved-anomaly conclusions

Ask which risks remain open, why they are acceptable, what compensating controls are required and who must implement them. Request the supplier’s unresolved-anomaly assessment, including known limitations that were not corrected before release.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Mitigation traceability

Request a matrix linking each identified risk to a requirement, design control, verification activity and test result. Health Canada recommends testing evidence and traceability for higher-risk device classes; the same discipline is useful in any procurement.

Security testing summary

Ask for the scope, dates, version tested, methods, significant findings, retest status and remaining limitations for penetration testing, fuzzing, code analysis or other security testing. Confidentiality agreements may be reasonable, but “tested” without scope or results is not meaningful evidence.

How can an SBOM reveal supply-chain risk?

Request a current, machine-readable software bill of materials (SBOM) covering commercial, open-source and off-the-shelf components. A PDF list of product names is difficult to search during an incident and may omit transitive dependencies.

SBOM item to verify What to ask Why it matters
Component identity Does each entry include a standard name, version and supplier or origin? Accurate identification is needed to match vulnerability advisories.
Dependency relationships Can the vendor show direct and transitive dependencies? A flaw in a library several layers down can still affect the device.
Maintenance status Is each component actively maintained, no longer maintained or abandoned? Unmaintained code may have no practical patch path.
End-of-support date When will the component’s supplier stop security fixes? A component can outlive the vendor’s stated product-support window.
Update cadence How often is the SBOM refreshed, and how is it versioned? You need the SBOM that matches the exact build in service.
Format and delivery Can your tools ingest it automatically, and will revisions be supplied after updates? Machine-readable data accelerates triage during a vulnerability.

Ask whether the vendor scans the SBOM against CVE advisories and CISA’s Known Exploited Vulnerabilities Catalog. Confirm that a component marked “commercial” is not being treated as exempt from maintenance obligations.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What should I ask about vulnerability disclosure and response?

A credible supplier can explain how it finds vulnerabilities, decides whether they affect patient safety and communicates with customers. FDA’s section 524B FAQ identifies monitoring, coordinated disclosure, cybersecure processes, patches and SBOMs among expectations for cyber devices.

  1. Monitoring: Does the vendor continuously monitor CVE feeds, the CISA Known Exploited Vulnerabilities Catalog, supplier notices, security researchers and its own telemetry?
  2. Triage: How are exploitability, device reachability, privilege required and clinical impact assessed? Is there a separate patient-safety review?
  3. Disclosure: Is there a public security contact, coordinated-vulnerability-disclosure process and acknowledgement target?
  4. Customer communication: Will the vendor issue advisories that name affected versions, symptoms, mitigations and required actions?
  5. Remediation: What target times apply to workarounds, patches and permanent fixes? Are targets contractual or merely aspirational?
  6. Compensating controls: If a patch is delayed, can the vendor provide segmentation, configuration changes, feature disablement or monitoring guidance?

FDA’s postmarket guidance warns that exploitation “may represent a risk to health” and typically requires continual maintenance throughout the product life cycle. Ask for examples of how the supplier handled a past vulnerability, with sensitive details redacted if necessary.

How should security updates be delivered and validated?

An update is safe only when its authenticity, clinical effect and operational procedure are controlled. Ask the vendor to walk through a normal patch and an emergency patch.

Authenticity and access control

Updates should be signed, verified before installation and restricted to authorized administrators. Clarify certificate rotation, offline update options and how the system prevents downgrade to a vulnerable version.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Testing and clinical validation

Require evidence that updates were tested for safety and effectiveness on the same hardware, firmware and integrations you will operate. Ask whether your facility must repeat validation, and which tests are supplied by the manufacturer.

Deployment and downtime

Get the expected maintenance window, reboot behavior, alarm handling, data preservation and staffing requirements. Ask whether a redundant unit or approved safe mode is needed while the update runs.

Rollback and recovery

Confirm whether rollback is supported, how a failed update is detected, how configuration is restored and how backups are verified. A rollback that requires a vendor visit can create a very different risk from one your biomedical team can execute.

Cost responsibility

Put labor, travel, specialized tools, network changes, validation and after-hours work into the total-cost model. Clarify whether security updates are included for the entire support period or billed as service events.

How do I judge independent testing and traceability?

Ask for testing appropriate to the device’s risk and connectivity. Useful evidence may include penetration-testing summaries, fuzzing results, vulnerability scans, secure-code analysis, authentication tests, update-integrity tests and recovery exercises.

  • Verify the test date and exact software and hardware versions.
  • Check whether internet-facing services, wireless interfaces, cloud APIs and remote-support paths were included.
  • Ask how findings were rated, corrected and retested.
  • Look for tests of denial of service, malformed input, privilege escalation, credential handling and logging.
  • Require a traceability matrix connecting risks to requirements, controls and verification results.

Testing is evidence of what was examined, not proof that no vulnerabilities exist. A supplier that cannot disclose scope, limitations or retest status leaves you unable to judge residual risk.

How long will the manufacturer support the device?

Obtain a written service-life and security-support commitment before purchase. Ask for separate dates for hardware service, operating-system support, application updates, component support and cloud availability.

Lifecycle question Evidence to obtain
Expected service life Planned years of operation and conditions that shorten it.
Security-support end date A dated commitment for patches and vulnerability advisories.
Component end of support Dates for operating systems, databases, libraries and appliances listed in the SBOM.
End-of-life plan Replacement, migration, data export, secure wiping and network-disconnection procedures.
Cloud or remote-service dependency Notice period and alternatives if a hosted service is retired.
Post-support responsibilities Controls, isolation and incident duties that transfer to your facility.

FDA’s postmarket position treats cybersecurity as continual lifecycle maintenance because exploitation can affect health. A device that is inexpensive to buy but unsupported halfway through its clinical life may cost more to replace, isolate or defend.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How should I compare two clinically comparable devices?

Use the same evidence requests and scoring rules for every finalist. Clinical equivalence does not imply cybersecurity equivalence.

Comparison axis Questions for both vendors
Architecture and attack surface How many interfaces, services, remote paths and external dependencies exist?
SBOM and component support Is the SBOM complete, machine-readable and tied to maintenance and end-of-support dates?
Vulnerability response How are CVEs and exploited vulnerabilities monitored, disclosed and remediated?
Patch safety and delivery Are updates signed, clinically validated, reversible and deployable within your maintenance windows?
Testing and traceability What independent testing and risk-to-test evidence can be shared?
Support horizon What is the security-support end date and retirement path?
Facility integration What segmentation, identity, logging, backup and alerting changes are required?
Contractual commitments Which response times, notices, update obligations and costs are enforceable?

Score evidence, not promises. A simple 0–3 scale works: 0 for absent, 1 for verbal or incomplete, 2 for documented but limited, and 3 for documented, current and contractually committed. Set minimum scores for patient-safety, support and vulnerability response categories; do not let a low purchase price compensate for a failed minimum.

What operational burden belongs in the purchase decision?

Estimate the work your facility must perform every month and during an incident. Include biomedical engineering, clinical engineering, IT, privacy, security operations and vendor-management time.

  • Segmentation: VLANs, firewalls, allow-lists and isolation procedures.
  • Identity: directory integration, multifactor authentication, privileged-access reviews and account termination.
  • Logging: retention, time synchronization, alert routing and integration with your security-monitoring platform.
  • Remote access: approval, session recording, time-limited credentials and emergency disablement.
  • Backup and recovery: configuration backups, restoration tests and replacement-device readiness.
  • Patch deployment: inventory, scheduling, clinical validation, downtime coordination and post-update checks.
  • Incident response: contact trees, clinical escalation, evidence preservation and alternate-care procedures.

Ask vendors to describe the staffing and maintenance-window assumptions behind their proposal. A system that requires a specialist visit for every update can be more expensive and less resilient than one your trained staff can patch safely.

What should the contract require?

Translate the evaluation into obligations that survive turnover and product changes. Legal and clinical teams should review the final language.

  • Delivery of a current machine-readable SBOM at acceptance and after each material update.
  • Notice of newly discovered vulnerabilities, affected versions, exploit status and compensating controls.
  • A named coordinated-vulnerability-disclosure channel and measurable acknowledgement, assessment and remediation targets.
  • Security-support dates, component-support commitments and advance end-of-life notice.
  • Signed updates, clinical-safety validation evidence, rollback instructions and emergency-patch procedures.
  • Access to testing summaries, traceability evidence and audit information under reasonable confidentiality terms.
  • Defined responsibilities for cloud services, remote support, backups, logs, network changes and post-support operation.
  • Data export, secure deletion and transition assistance at termination or retirement.

Specify what happens when the vendor misses a commitment: escalation, compensating controls, service credits, replacement assistance or the right to suspend a connection. A policy statement without an owner, deadline and remedy is difficult to use during an incident.

What are warning signs that a device is a poor cybersecurity purchase?

  • The supplier describes the product as “secure” but will not show architecture, risk conclusions or testing scope.
  • The SBOM is unavailable, static, unreadable by tools or missing component versions and support status.
  • Remote service uses permanent shared accounts, unrestricted inbound connections or undocumented vendor access.
  • Updates are unsigned, cannot be rolled back or require unsafe downtime.
  • The vendor gives no acknowledgement target, disclosure process or distinction between technical and patient-safety impact.
  • Security support ends before the stated clinical service life, or component end-of-support dates are unknown.
  • The facility is expected to accept all residual risk without a mitigation list or operational instructions.
  • Security evidence applies to a different model, firmware release or cloud edition than the one being purchased.

One warning sign may be fixable; several usually indicate that the supplier’s lifecycle process is immature. Pause procurement until the gap has an owner and a documented remedy.

What should I do if the vendor will not answer?

Record the unanswered question, the business impact and the compensating control that would be required. Escalate through procurement, clinical engineering, information security and the supplier’s security or regulatory contact.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Give the vendor a written evidence list with a response deadline.
  2. Separate confidential material that can be reviewed under a nondisclosure agreement from information that must be delivered to operators.
  3. Ask for a time-limited exception only when the clinical benefit is compelling and controls are practical.
  4. Require senior approval of residual risk, an expiration date and a review trigger for new vulnerabilities or product changes.
  5. Keep a competing product or replacement plan if the supplier misses the deadline.

Do not substitute generic consumer security accessories for vendor evidence or a facility risk assessment. They cannot repair an unsupported component, validate a clinical patch or define who responds to an exploited vulnerability.

What changes for different device types?

The same questions apply, but the emphasis changes with connectivity and clinical consequence.

Networked monitoring and diagnostic systems

Focus on data integrity, alarm availability, workstation hardening, segmentation and integration with electronic health-record systems.

Therapy-delivery or life-support equipment

Prioritize command integrity, safe-state behavior, authenticated updates, rollback, manual backup procedures and evidence that cybersecurity controls cannot introduce unsafe delays.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Home-use and mobile-connected devices

Examine patient-owned networks, mobile-app permissions, account recovery, cloud dependence, privacy exposure and how a patient receives urgent security instructions.

Cloud-managed fleets

Review tenant isolation, API authentication, provider dependencies, data location, outage behavior and the process for exporting records if the service ends.

What is the practical procurement sequence?

  1. Set the clinical and service-life requirements. Define acceptable downtime, replacement timing, data retention and safety-critical functions.
  2. Issue a common cybersecurity questionnaire. Require the same boundary, SBOM, vulnerability, update, testing and support evidence from every bidder.
  3. Review evidence with the right owners. Include clinical engineering, IT/network, security operations, privacy, legal, procurement and front-line users.
  4. Score and test finalists. Validate segmentation, authentication, logging, remote access, backup restoration and update procedures in a representative environment.
  5. Negotiate commitments. Put support dates, disclosure targets, SBOM refreshes, patch validation and end-of-life duties in the contract.
  6. Approve residual risk explicitly. Document open items, compensating controls, owners, deadlines and the conditions that would stop deployment.
  7. Maintain the record after purchase. Keep the boundary diagram, SBOM version, risk assessment, test results, update history, advisories and retirement plan current.

FDA’s section 524B cybersecurity submission requirements began on March 29, 2023. FDA’s February 2026 final premarket guidance superseded its 2025 guidance, and ANSI AAMI SW96:2023 entered FDA’s recognized consensus-standards list on October 9, 2023. These dates matter when you ask a supplier which regulatory expectations and standards version underpin its evidence.

Frequently Asked Questions

Is an SBOM enough to prove a medical device is secure?

No. An SBOM identifies software components, but it does not show whether the architecture is resilient, vulnerabilities are monitored, patches are safe or residual clinical risks are acceptable. Use it as an input to continuous vulnerability management and lifecycle review.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How often should a hospital review a device’s cybersecurity?

Review at onboarding, after every material software or network change, when a relevant vulnerability is disclosed and before major contract or support milestones. Keep the boundary diagram, SBOM, risk assessment and operating procedures synchronized rather than relying on an annual reminder alone.

Who is responsible for a vulnerability in a connected medical device?

Responsibility is shared according to the component and control: the manufacturer generally owns product design and vendor-issued fixes, while the facility owns its network, accounts, monitoring and deployment decisions. The contract should define handoffs, deadlines and what happens when support ends.

Can network segmentation compensate for an unpatched device?

Segmentation can reduce reachability and limit blast radius, but it does not remove vulnerabilities or guarantee safe operation. Treat it as a documented compensating control with monitoring, access restrictions and a deadline for a vendor fix.

What should happen when a device reaches end of life?

Stop treating it as an ordinary supported asset. Follow a documented replacement or isolation plan, export required records, revoke remote access, remove credentials and securely wipe or destroy storage according to your facility’s policy.
