Evaluate a hospital AI tool as a clinical intervention, not as a software purchase. Before approving it, define the exact use case, verify the regulatory pathway, inspect independent and local validation, test safety and workflow effects, protect patient data, set contractual controls, and establish monitoring with clear human override. A vendor’s headline accuracy or benchmark score is only one input and cannot show that patients benefit in your setting.

The checklist below turns those requirements into questions clinicians, safety leaders, information-governance teams and procurement officers can use together. It applies to generative AI, clinical decision-support models, prediction systems and other tools that influence care, documentation or patient communication.

What should a hospital define before evaluating an AI tool?

Start by writing a precise statement of the clinical problem and the conditions in which the system will be used. If the problem, users and decision boundary are vague, vendor claims cannot be tested and a safe fallback cannot be designed.

Write the intended use and users

Record the task in operational terms: for example, flagging patients for a clinician review, summarizing records for a consultation, drafting documentation, or supporting a diagnostic decision. Name every intended user group, including physicians, nurses, allied health professionals, pharmacists, administrators and patients if they will see an output. A model designed for specialist review may be unsafe when treated as an autonomous tool by a different role.

Specify the population and care setting

Define age ranges, conditions, acuity, language, disability considerations and other inclusion boundaries. State whether the system will operate in emergency, inpatient, outpatient, community or home-care settings, and identify the sites and devices involved. Performance that appears acceptable in one ward or health system may not transfer to another because workflows, coding practices, equipment and patient mix differ.

Describe inputs, outputs and affected decisions

List each input the model requires, such as structured observations, free-text notes, images, waveforms, laboratory values or patient questions. Describe the output format, timing, confidence information and the clinical action it may influence. Make explicit what the system is not allowed to decide. Also document what happens when data are missing or the service is unavailable; downtime is part of the intended use, not an exception.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Set measurable success criteria before reading marketing material

Choose outcomes that matter to patients, clinicians and the organization. Depending on the use case, these may include diagnostic errors, treatment delays, avoidable admissions, documentation completeness, time to review, workload, patient understanding or equity measures. Define acceptable false-positive and false-negative rates, escalation requirements and a minimum level of benefit compared with current practice. NHS procurement guidance treats problem definition and suitability as the starting point because a technically impressive system may not be appropriate for the problem.

How do we verify regulatory status and accountability?

Regulatory clearance, registration or classification must be checked for the jurisdiction and intended use, but it does not prove that the product is useful or safe in your hospital. Pair the regulatory review with named local owners who can act when evidence or performance changes.

Ask which regulatory category applies

Request the product’s classification in every geography where it will be deployed, the authority involved, approval or registration identifiers, approved indications and any restrictions. Confirm whether a change in intended use, model capability or patient population would alter that status. General-purpose or multimodal systems may fall under different rules from a device-specific clinical model. The World Health Organization’s 25 March 2025 guidance addresses large multimodal models, but it does not replace device-specific or jurisdiction-specific regulation.

Separate compliance from clinical suitability

Ask what evidence supported the authorization and what it does not cover. A regulatory decision may establish that a product meets a defined pathway; it does not establish local workflow fit, subgroup performance or patient benefit. The NHS buyer guide treats regulatory compliance as a separate assessment from performance, ethics, data protection, maintenance, procurement and contracts.

Assign accountable owners

Before a pilot, name the people responsible for clinical safety, information governance, security, procurement, legal review, vendor management and operational support. Give each owner authority to pause use, request evidence and approve a change. In the UK, involve the data protection officer and Caldicott Guardian where applicable. Clinicians should know the management route for reporting false, inconsistent or unsafe outputs.

What validation evidence should an AI vendor provide?

Require enough documentation to reproduce the boundaries of the claimed performance. Independent testing on data that were not used for training, followed by local clinical validation, is more informative than a single benchmark number.

Request technical documentation

Ask for a model card or equivalent document covering the model’s purpose, architecture at an appropriate level, training-data provenance, test-data provenance, inclusion and exclusion criteria, preprocessing, labels, missing-data handling and known limitations. Obtain the version history, release dates and a description of material changes. For a generative system, request information about retrieval sources, prompt or policy controls, output filtering and how hallucinations are assessed.

Look for external validation

External data must be distinct from training and internal tuning data. Check whether validation sites, equipment, coding conventions and patient populations resemble yours. FUTURE-AI recommends external datasets and local clinical validation across relevant populations and workflows. A study performed in one institution can still be useful, but its transportability must be tested rather than assumed.

Demand subgroup and edge-case results

Request accuracy, calibration or other appropriate metrics for age, sex, race or ethnicity where lawful and relevant, language, comorbidity, disability, device, site and data-quality strata. Ask for performance on rare conditions, atypical presentations, incomplete records, out-of-range values and conflicting inputs. AHRQ recommends asking specifically about diverse-population validation, subgroup limitations, hallucination or bias risk and edge-case performance.

Understand uncertainty and failure modes

Find out whether the output includes a confidence estimate, prediction interval, abstention option or other uncertainty signal, and test whether that signal is calibrated. Obtain a catalog of known failure modes and examples of unsafe outputs. A high average score can conceal a small but consequential group of failures; the hospital needs to know how those failures present and what users should do.

How can a hospital test clinical utility and safety?

Clinical utility is demonstrated by better or safer care in the real decision, not by accuracy, area-under-the-curve or a benchmark result alone. Compare the tool with current practice using outcomes that reflect patient and operational consequences.

Define the comparator

Document the current standard of care, including existing decision-support alerts, manual review, staffing and escalation. Compare the AI-assisted pathway with that baseline rather than with an idealized workflow. AHRQ advises comparing real-world performance with vendor-reported metrics so that differences in data, users and operating conditions are visible.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Measure benefits and harms together

Track intended benefits such as earlier recognition, fewer errors, reduced delays or lower documentation burden. At the same time, record false alarms, missed cases, inappropriate treatment suggestions, privacy incidents, extra workload and downstream tests. FUTURE-AI states that clinical evaluations should demonstrate benefit and safety and may require randomized trials, depending on risk and context.

Build a prospective pilot

Use a limited, time-bounded pilot with representative clinicians and patients. Predefine the sample, endpoints, stopping rules, review cadence and who can suspend the tool. If the system is initially silent, compare its recommendations with clinician decisions and outcomes; if it is visible, measure how users respond and whether decisions change. Document deviations from the protocol and investigate near misses rather than counting only successful cases.

Assess the downstream consequence

A recommendation can be accurate yet harmful if it arrives too late, is difficult to interpret or triggers unnecessary work. Examine changes in time to treatment, handoffs, referrals, testing, length of stay, patient communication and clinician workload. Require evidence of the benefits, safety profile and downstream consequences, as emphasized by the National Commission into the Regulation of AI in Healthcare’s 2026 framework recommendation; verify which recommendations are binding in your jurisdiction.

How do we check fairness, robustness and failure behavior?

Test the model under the variation it will encounter, not only on clean development data. Fairness and robustness require both subgroup analysis and operational safeguards when inputs or outputs are unreliable.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Compare performance across relevant groups

Analyze the same clinically meaningful metrics across age, sex, race or ethnicity where lawful and relevant, language, comorbidity, disability, device, site and data quality. Explain any differences, determine whether they reflect measurement or access disparities, and set thresholds for action. Do not infer fairness from an overall average.

Run stress and repeatability tests

Test missing inputs, out-of-range values, contradictory records, low-quality images or audio, unusual terminology, language changes and interrupted connections. Repeat identical or near-identical prompts to identify unstable outputs, especially in generative systems. Test whether a small formatting change produces a clinically different recommendation without a valid reason.

Plan for drift

Performance can change when disease prevalence, clinical practice, coding, devices or populations change. Establish drift indicators and review intervals before deployment. FUTURE-AI recommends robustness evaluation under real-world variation, risk files, quality controls and periodic audits.

Define stop, fallback and escalation rules

Specify the conditions that make an output unusable, such as missing critical data, an out-of-distribution case, a confidence signal below threshold or a service outage. The interface should direct users to a documented fallback, such as the current manual process or a second clinical review. Record who must be notified, how quickly and who can suspend the system.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What human oversight and workflow checks are required?

Pilot the tool in the workflow where decisions occur and preserve meaningful human control. Oversight must be designed into the interface, staffing and procedures rather than left to individual vigilance.

Observe representative users

Include the clinicians who will use the system during busy and routine periods. Measure time, cognitive burden, learnability, accessibility, false alarms and whether users can inspect the input data and the reasoning or evidence behind an output. Test handoffs between professions and shifts, because an alert that is clear to one role may be ambiguous to another.

Define the human decision boundary

Write which decisions remain human, when a second review is mandatory and how a clinician records an override. Require users to be able to reject, correct or annotate an output without fighting the interface. Check for automation bias: the tendency to accept a machine suggestion because it appears authoritative or saves time.

Train for limitations, not just buttons

Training should cover intended use, uncertainty, common errors, bias, automation bias, escalation and downtime. Use realistic examples, including plausible but wrong outputs. Confirm competence before access and provide refresher training after major updates. NHS professional guidance states that final care decisions should be made with the patient or service user using professional judgement.

How should hospitals protect privacy, security and data rights?

Privacy and security controls are clinical-safety controls because unauthorized access, inappropriate reuse or a silent data failure can change care. Involve information-governance and security specialists before any patient data enter a vendor environment.

Map the data lifecycle

List what data are collected, where they are processed, where they are stored, how long they are retained and every subcontractor that can access them. Confirm the lawful basis, data minimization, data location, access controls, encryption, audit logs and breach-notification process. Ask whether prompts, records, images or outputs are reused to train or improve a shared model.

Control access and deletion

Use role-based access, strong authentication and least-privilege service accounts. Require logs that show who viewed, entered, changed or exported information. Contract for deletion at the end of the relationship and for a verifiable response to patient or institutional deletion requests. Provide patient-facing privacy information when AI contributes to care, documentation or use of personal information.

Test security in operation

Ask about vulnerability management, penetration testing, incident response, backup and recovery, business continuity and the handling of malicious or untrusted inputs. Verify that an integration cannot expose records through prompts, logs or support channels. Include the vendor’s security obligations and notification timeframes in the contract.

What should procurement and the contract require?

Use a transparent, competitive process with written requirements and acceptance tests. Commercial terms should preserve the hospital’s ability to monitor, challenge, update or leave the service.

Write acceptance tests

Turn the use-case, safety, subgroup, interoperability and uptime requirements into tests that must pass before production access. Include representative data, failure cases, response times, audit-log checks and downtime behavior. Do not accept a general promise to meet performance without a measurable test and a remedy.

Specify operational commitments

Contract for uptime, support hours, incident response, severity definitions, model-version control, change notification and validation after updates. Require the vendor to disclose material changes to training data, prompts, algorithms, hosting, subprocessors or intended use. Define who monitors performance and who pays for revalidation.

Protect data, audit and exit rights

State data ownership, permitted processing, retention, deletion, audit rights, indemnity and responsibilities for regulatory reporting. Require export of records, configurations, logs and relevant outputs in a usable format. Include an exit plan that covers transition assistance, continuity of care and secure destruction; portability is a safety requirement when clinicians must preserve a complete record.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Include maintenance and liability

Set responsibilities for bug fixes, cybersecurity patches, recalibration, rollback and retirement. Clarify liability for defective outputs, service failures, privacy incidents and delayed notification. A contract that omits maintenance or update validation can turn a safe pilot into an unmanaged clinical risk.

How should clinicians and patients be prepared?

People need an honest explanation of what the system does, where it can fail and who remains responsible. Preparation should cover both staff practice and patient communication.

Give clinicians a practical operating guide

Provide a short reference showing approved uses, prohibited uses, required checks, uncertainty cues, escalation contacts, override documentation and downtime steps. Explain how to report an error, near miss, bias concern or privacy issue. Incorporate the procedure into orientation and clinical governance rather than treating it as optional software training.

Explain AI involvement to patients

Tell patients when AI contributes to care, documentation or handling of their personal information, using language suited to the setting and decision. Explain the role of the clinician, the opportunity to ask questions and how to raise a concern. Patient-facing information should not imply that the system makes the final decision.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Protect professional judgement

Clinicians retain responsibility for contextualizing outputs and deciding care with the patient. The NHS professional guidance states: “The final decision about the care that people receive should be made in consultation with the patient or service user, using your professional judgement.” Build that principle into policy, training and the user interface.

What must be monitored after go-live?

Approval is the beginning of surveillance, not the end of evaluation. Create an AI-specific monitoring plan with privacy-preserving logs, review thresholds and authority to roll back or retire the tool.

Log the complete decision path

Where lawful and necessary, record relevant inputs, outputs, model version, user actions, clinical decisions, overrides, complaints and near misses. Minimize identifiable data and restrict access to the monitoring team. Logs should allow an investigator to reconstruct what the system showed and what the clinician did without creating a new exposure risk.

Create incident categories and reporting routes

Add categories for hallucination, unsafe recommendation, missed case, subgroup disparity, automation bias, downtime, data leakage and unexpected model behavior. Make reporting easy inside the clinical workflow and route urgent concerns to the accountable safety owner. NHS guidance directs concerns about false or inconsistent outputs through clinical management routes; your local policy should name the exact route and response time.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Compare real-world performance with vendor metrics

Review the same clinically relevant measures used in validation, broken down by important subgroups and sites. Look for changes in calibration, false alarms, overrides, workload, outcomes and equity. AHRQ recommends comparing real-world performance with vendor-reported metrics rather than assuming the original results still apply.

Set thresholds for action

Define triggers for recalibration, additional training, rollback, suspension or retirement before launch. Review at scheduled intervals and after major changes in population, workflow, data source or model version. FUTURE-AI recommends continuous quality control, periodic auditing, updating and privacy-preserving logging.

How should two remaining AI tools be compared?

Use a weighted, documented comparison tied to the defined use case; do not rank products by a single accuracy figure. A tool that performs slightly better in a laboratory test may be the worse choice if it lacks local validation, safe escalation or an exit path.

Comparison area Questions to document
Intended use and population Does the approved use match the hospital’s patients, setting, users and decision?
Validation quality Are training and test data distinct, externally validated and locally tested?
Subgroup fairness Are results reported for relevant demographic, clinical, language, device, site and data-quality groups?
Clinical outcomes and safety Is there evidence of patient benefit, foreseeable harms and downstream consequences?
Workflow and usability Can representative users inspect, override and escalate outputs without unacceptable burden?
Interoperability Does it exchange the required data reliably and preserve records during downtime or exit?
Explainability and uncertainty Does it show supporting inputs, evidence or calibrated uncertainty appropriate to the decision?
Privacy and security Are lawful basis, retention, location, subprocessors, access, logs, breach response and deletion clear?
Regulatory status Is the classification, authorization and intended-use boundary documented for the deployment geography?
Updates and drift control Are version changes notified, revalidated and reversible, with monitoring after release?
Training and support Are user training, accessibility, incident response and support commitments adequate?
Cost and contract Do acceptance tests, uptime, audit, liability, data ownership and exit portability protect the hospital?

What does a defensible approval record contain?

A governance file should allow a new reviewer to understand why the tool was selected, what evidence supports it and what conditions keep it safe. Keep the following artifacts together:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Use-case statement, intended users, population, setting, inputs, outputs, affected decisions and downtime workflow.
  • Regulatory classification, approvals or registrations, restrictions and accountable owners.
  • Model documentation, data provenance, external validation, local validation, subgroup results and known failure modes.
  • Clinical-utility protocol, comparator, outcomes, stopping rules, harms assessment and pilot findings.
  • Workflow assessment covering usability, accessibility, cognitive burden, automation bias, override and escalation.
  • Privacy impact assessment, security review, data map, retention schedule, access design and patient information.
  • Procurement record, acceptance tests, contract, service levels, update controls, audit rights and exit plan.
  • Training materials, competency records, incident categories, monitoring dashboard, review schedule and rollback criteria.

This record makes the decision auditable and gives clinicians a practical basis for questioning a result. It also prevents a change in model version, staffing or patient population from quietly changing the risk profile.

Frequently Asked Questions

Can a hospital rely on a CE mark, FDA clearance or another regulator’s authorization alone?

No. Authorization addresses a defined regulatory pathway and intended use, while local usefulness, workflow fit, subgroup performance and patient benefit still require evaluation. Record the jurisdiction, indication and evidence scope, then perform local validation before routine use.

What is the minimum pilot size for a clinical AI tool?

There is no universal number in the available guidance. Set the sample and duration from the use case, expected event rate, risk, subgroup coverage and outcomes you must estimate, and define stopping rules before the pilot starts.

How should clinicians report an AI output that looks wrong but causes no harm?

Use an AI-specific near-miss or unsafe-output category, preserve the relevant version and context in a privacy-protective log, and route the report to the named clinical-safety owner. Trend these reports by site, subgroup and model version instead of treating them as isolated user mistakes.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What should happen when a vendor releases a new model version?

Treat it as a controlled change: obtain the change description, repeat the agreed acceptance and subgroup tests, assess workflow effects, train users if needed and retain a rollback option. Do not allow automatic production updates without a documented validation and approval decision.

How can patients challenge an AI-assisted decision?

Tell patients which part of the process involved AI, who made the final clinical decision and how to ask questions or request a review. Provide a human contact and a route for privacy or fairness concerns, and ensure the clinical record identifies the responsible professional.
