Evaluate an AI agent as a governed system for a specific pharmaceutical workflow, not as a general-purpose chatbot. Define the intended decision, users, data, tools, permitted actions, human approvals and risk tier first. Then test four layers: scientific knowledge and retrieval; domain tasks such as target identification, retrosynthesis, screening, toxicity or repurposing; agent-process behavior such as planning, tool use, coding, recovery and uncertainty reporting; and prospective experimental or translational value. A benchmark score can show that an agent performs a task under defined conditions, but it cannot by itself establish experimental success, regulatory suitability or patient benefit.

The January 2026 FDA–EMA principles for AI used across drug development provide the governance baseline: human-centric design, risk-based validation and oversight, compliance with legal, ethical, technical, scientific, cybersecurity and GxP expectations, and a clearly documented context of use.

What does a proper evaluation of a pharmaceutical AI agent include?

A defensible evaluation connects every test to the way the agent will actually be used. An agent that summarizes literature for scientists has a different risk profile from one that prioritizes compounds, writes analysis code against proprietary data or recommends dose-related experiments. The same model can be acceptable for one context and unsuitable for another.

Write a context-of-use statement before selecting a benchmark. It should identify:

  • The research phase, such as discovery, nonclinical development, clinical development, post-marketing or manufacturing.
  • The decision or workflow the agent supports, including the output that a scientist or operator will act on.
  • The intended users, their training and the human review or sign-off points.
  • The data classes involved, including public, confidential, personal, patient, regulated and proprietary data.
  • The tools the agent may call, such as literature databases, cheminformatics software, laboratory information systems, simulation packages or internal code repositories.
  • Whether the agent is exploratory, provides decision support or may trigger an external action.
  • The failure consequences and the acceptance criteria that must be met before use.

This statement prevents a common evaluation error: declaring an agent safe because it performs well on a convenient benchmark that does not resemble the proposed workflow.

Which four layers should you measure?

1. Scientific knowledge and biomedical retrieval

Start with held-out biomedical questions and retrieval tasks. Measure factual accuracy, whether cited papers actually support the claim, coverage of relevant evidence, handling of contradictory findings and calibration of uncertainty. Include recent papers, older foundational work and deliberately conflicting records so that the agent must distinguish evidence quality rather than simply retrieve familiar language.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Generic language-model scores are only a screening layer. They do not show whether an agent can select a valid assay, interpret a dose–response relationship, identify a confounder or preserve provenance through a multi-step analysis. Require the agent to expose source documents, passages or database records for consequential statements, and score citation fidelity separately from answer fluency.

2. Domain-task performance

Select tasks that match the proposed workflow. Representative discovery uses identified by the European Medicines Agency in its 2024 horizon scan include target identification, drug selection and prioritization, compound screening and design, and drug repurposing. Other relevant tasks include retrosynthesis, structure–activity relationship analysis, toxicity prediction and protocol planning.

Define the unit of success before testing. For target identification, that might include biological plausibility, evidence triangulation and correct attribution of supporting data. For retrosynthesis, it may require chemically valid routes that satisfy specified constraints. For screening or prioritization, evaluate ranking quality on withheld compounds and the usefulness of uncertainty estimates, not only whether the top-ranked item looks plausible.

3. Agent-specific process capability

An agent can produce a scientifically plausible final answer through an unreliable process. Evaluate how it decomposes a question, chooses and sequences tools, writes and runs code, checks intermediate results, handles contradictory records, stops when evidence is insufficient, recovers from failed calls and communicates uncertainty.

The 2026 npj review groups evaluation into general cognition, task-specific workflows, agent-exclusive process capabilities and embodied scientific intelligence. In practice, record the full trajectory: prompts, plans, tool calls, query results, code, intermediate files, errors, retries, approvals and the final answer. Score whether the process is reproducible and auditable, not merely whether the last paragraph is correct.

4. Prospective and translational value

After retrospective tests, run prospective evaluations on newly generated or withheld cases. In chemistry and biology, confirm high-impact predictions with appropriate assays or orthogonal evidence when the context warrants it. A useful conceptual ladder has four tiers:

  1. Internal reproducibility: the same inputs, tools and versions produce consistent outputs across repeated runs.
  2. Molecular benchmark robustness: performance survives realistic data splits, distribution shifts and adversarial or contradictory cases.
  3. Prospective biophysical or biochemical validation: new predictions are tested experimentally with suitable controls.
  4. Clinical or translational calibration: predictions are assessed in settings such as physiologically based pharmacokinetic or quantitative systems pharmacology models and, where appropriate, clinical or translational datasets.

This ladder is conceptual, not a regulatory requirement. Moving upward requires more evidence, stronger controls and a clearer connection to the decision the agent is meant to support.

What do current benchmarks actually show?

Published benchmarks are useful for discovering strengths and failure modes, but their scores are not interchangeable. They use different tasks, datasets, environments, graders and definitions of success. Report repeated-run variability or confidence intervals where possible, and publish representative failures rather than a single aggregate number.

Benchmark What it tests Reported result Important boundary
ScienceAgentBench Bioinformatics and computational-chemistry tasks 102 tasks; the best reported independent-agent success rate was 34.3% Result reported in the 2026 npj Drug Discovery review; task success depends on the benchmark environment and grader.
BixBench Real-world biomedical data-analysis scenarios 61 scenarios and 205 questions; agents achieved about 21% accuracy on open-ended questions Open-ended accuracy is not equivalent to end-to-end laboratory or clinical utility.
RExBench Hypothesis-driven research expansion The best reported agent success rate was 25% The score reflects the benchmark’s definition of successful research expansion.
LARC Retrosynthesis under multiple constraints 72.9% success rate Reported success is specific to the stated retrosynthesis constraints and evaluation setup.
DrugDiscoveryBench Multi-step early-discovery workflows 82 tasks; no overall score is stated in the Scale Labs benchmark documentation accessed in 2026 Covers target identification and validation, hit identification across patents, databases and literature, hit-to-lead and structure–activity analysis, and lead optimization. It stops before candidate selection and excludes DMPK, toxicology, formulation and clinical work.

Use these results to ask what an agent can do under a defined test, not to rank systems globally. A 72.9% retrosynthesis result cannot be compared directly with a 21% open-ended analysis score, and neither establishes a probability of approval or patient benefit.

How do you design an evaluation protocol step by step?

  1. Define the context of use. State the R&D phase, decision, users, data, tools, permitted actions, human sign-offs and whether the system is exploratory, advisory or action-capable. Specify what the agent must never do, such as release an unreviewed result into a regulated record.
  2. Set risk-matched acceptance criteria. For low-risk literature triage, emphasize recall, citation correctness, uncertainty calibration and reviewer time. For compound prioritization, toxicity-related work or dose-informed decisions, require stronger prospective and translational evidence. Set thresholds with the scientific, quality, safety, security and regulatory owners of the workflow rather than borrowing a generic benchmark cutoff.
  3. Build a representative test set. Include recent and historical cases, positive and negative examples, rare edge cases, distribution shifts, contradictory records, missing data, proprietary-data access boundaries and adversarial prompts. Keep a locked test portion that the agent or its developers cannot tune against.
  4. Choose comparators. Compare with a validated baseline model, the existing production workflow and qualified human experts. Record the expertise and time available to each comparator. Fix data splits, tool versions, prompts and evaluation conditions so that differences reflect the systems rather than changing test environments.
  5. Test process reliability. Repeat each case to measure variance. Inject tool outages, malformed files, missing fields, rate limits and conflicting evidence. Score whether the agent retries safely, switches tools appropriately, escalates to a person, abstains when necessary and leaves a usable error record.
  6. Audit provenance and security. Preserve source documents, model and tool versions, prompts, code, random seeds, intermediate actions, approvals and final outputs. Check privacy, access control, cybersecurity, data-retention rules and GxP record expectations where applicable.
  7. Validate prospectively. Test chemistry or biology claims on newly generated or withheld cases. Confirm high-impact predictions with appropriate assays, replication or orthogonal evidence. A retrospective score should not be the only evidence supporting a change to experimental priorities.
  8. Monitor after deployment. Track data and model drift, recurring error patterns, user overrides, near misses, abstentions, tool failures, latency, cost and changes in the surrounding workflow. Define revalidation triggers before launch, such as a model update, a new data source, a changed tool version or a shift in the decision being supported.

Which metrics matter when comparing two agents?

Use a scorecard that reflects the intended workflow. A single leaderboard number hides trade-offs between accuracy, safety, operating burden and scientific usefulness.

  • Scientific correctness: factual accuracy, evidence quality and citation fidelity.
  • Domain-task performance: performance on the actual target-identification, design, screening, retrosynthesis, toxicity, repurposing or planning task.
  • Planning and long-horizon execution: whether the agent decomposes work into valid steps and maintains state across a long task.
  • Tool-use reliability and code quality: valid queries, correct parameter choices, executable code, tests and safe handling of errors.
  • Reproducibility: consistency across repeated runs, seeds, workers and tool versions.
  • Calibration and safe abstention: whether confidence tracks correctness and the agent declines unsupported conclusions.
  • Provenance, privacy and cybersecurity: traceability of inputs and outputs, access boundaries and resistance to data leakage or prompt injection.
  • Integration effort, latency and cost: infrastructure work, run time and operating expense measured in the intended environment.
  • Prospective utility: experimentally confirmed hits, better-designed analyses, useful hypotheses or measurable workflow improvement.
  • Auditability and regulatory fit: review controls, immutable records where needed, role separation and compatibility with quality systems.

Report distributions, not just means. Include per-task results, failure categories, abstention rates, reviewer overrides and the number of repeated runs. If experts disagree about grading, document the adjudication rule and the unresolved cases.

How should an agent be validated for a regulated workflow?

The January 2026 FDA–EMA document applies to AI that generates or analyzes evidence across nonclinical, clinical, post-marketing and manufacturing phases. Its principles are a practical baseline for an evaluation plan, even where no universal numerical pass/fail threshold exists.

Human-centric operation

Assign responsibility to named roles. Reviewers should be able to inspect evidence, challenge an output, correct it and stop the workflow. The interface should make model-generated content distinguishable from source data and approved records.

Risk-based validation and oversight

Scale validation to the potential harm of an error. A literature triage assistant may need strong retrieval and citation checks, while an agent influencing a dose-related analysis requires tighter controls, prospective evidence, documented approvals and a defined escalation path.

Standards and GxP alignment

Account for legal, ethical, technical, scientific, cybersecurity and regulatory standards, including applicable GxP requirements. Document data integrity, access control, change management, versioning, training, deviations and corrective actions. The agent’s logs should support reconstruction of what happened, with which inputs, tools and approvals.

Clear boundaries of authority

State which outputs are advisory and which actions, if any, the agent may initiate. Require explicit human authorization before actions that create regulated records, alter experimental priorities, communicate externally or affect a subject, patient or manufacturing process.

FDA’s current drug-development AI page describes a significant increase in applications containing AI components and coordination with EMA around ten guiding principles. Those principles support governance requirements, but they do not provide a universal benchmark score that automatically qualifies an agent.

Why can a high benchmark score fail to predict drug-development value?

Benchmarks often measure software execution, answer grading or molecular ranking under controlled conditions. Drug development also depends on assay quality, biological variability, manufacturability, pharmacokinetics, toxicology, formulation, clinical design, site execution and many other factors outside a benchmark. An agent may be excellent at generating hypotheses yet add little value if its suggestions are not testable, reproducible or better than the existing workflow.

A 2026 translational-practice review therefore recommends matching each intended use to suitable comparators, evaluation designs and acceptance criteria. Treat claims about faster discovery, lower cost or higher clinical success as hypotheses until prospective, reproducible evidence supports them. No authoritative figure currently isolates the causal effect of agentic AI on approval probability or patient outcomes.

What failure modes should you test before deployment?

Confidently wrong or weakly supported claims

Use cases with fabricated citations, misread tables, unsupported causal language and conclusions that exceed the evidence. Require source-level citation checks, calibrated confidence and mandatory human review for consequential outputs.

Free tools Windows power users keep installed

One-click scans. No signup required.

Tool and code failures

Test malformed queries, unavailable databases, incompatible package versions, partial execution and silent data truncation. The recovery policy should preserve the error, avoid inventing a result and either retry within limits or escalate.

Contradictory or shifting evidence

Present conflicting papers, changing database annotations and records with different dates. Score whether the agent identifies the conflict, explains why sources differ and avoids collapsing uncertainty into one answer.

Data-boundary violations

Probe attempts to expose proprietary, personal or restricted data through prompts, retrieved documents or generated code. Verify least-privilege access, tenant separation, redaction and logging before allowing the agent to handle sensitive material.

Uncontrolled autonomy

Simulate requests that would trigger experiments, modify records or send external communications. The agent should stop at the configured approval gate and make the pending action explicit rather than treating a user’s broad instruction as unlimited authorization.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Irreproducible results

Repeat identical cases after changing seeds, workers, model snapshots or tool versions. If outputs vary materially, expose that variance to reviewers and set a workflow rule for when a second run or human adjudication is required.

What should the evaluation report contain?

A useful report lets another qualified reviewer reconstruct the test and understand its limits. Include:

  • The context-of-use statement and risk classification.
  • Model, agent framework, retrieval system, tools, versions and configuration.
  • Dataset origin, inclusion criteria, time windows, splits and access permissions.
  • Prompts, system instructions, code, random seeds and environmental dependencies.
  • Task definitions, grading rubrics, human adjudication rules and acceptance criteria.
  • Per-task results, confidence intervals or repeated-run variability, abstentions and failure categories.
  • Comparator results, reviewer time and any differences in available tools or data.
  • Prospective experiments or orthogonal checks for high-impact predictions.
  • Security, privacy, provenance, GxP and human-oversight controls.
  • Known limitations, residual risks, launch conditions and revalidation triggers.

Publishing failure cases is especially important. It shows whether the agent fails safely, reveals where a benchmark is permissive and gives users concrete signals for when to seek expert review.

When is an agent ready for a pharmaceutical workflow?

Readiness is a decision about a defined use, not a permanent property of a model. Approve deployment only when the agent meets the agreed scientific and process criteria, performs acceptably against the existing workflow and qualified experts, passes security and provenance checks, and has a documented human-oversight plan. For higher-risk uses, require prospective evidence and appropriate experimental or translational confirmation before expanding authority.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Keep the initial scope narrow, log every consequential run and review real-world performance at scheduled intervals. Expand the context of use only after new data, tools, users or decisions have been evaluated rather than assuming that success in one workflow transfers automatically to another.

Frequently Asked Questions

How many test cases are enough to evaluate a pharmaceutical AI agent?

There is no universal sample-size rule. The set should cover the workflow’s important subcases, rare failures, distribution shifts and negative examples, with a locked holdout and enough repeated runs to estimate variability; the required size depends on the decision risk and the precision needed for the acceptance criteria.

Should human experts grade every agent output?

Experts should grade representative and high-impact outputs, while automated checks can handle deterministic measures such as citation matching, code execution or chemical validity. Use blinded review where practical, record reviewer expertise and adjudicate disagreements instead of treating one opinion as ground truth.

How often does an approved agent need revalidation?

Revalidate when the model, retrieval corpus, tool, prompt, data access, user population or supported decision changes, and when monitoring shows drift, near misses or a new error pattern. A fixed periodic review can supplement these event-triggered checks, but it does not replace them.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can an agent use proprietary pharmaceutical data safely?

Only within an explicitly controlled data boundary. Verify least-privilege access, encryption and retention controls, tenant isolation, prompt-injection defenses, audit logs and rules preventing confidential content from entering training or external services without authorization.
