Detect prompt injection and model hijacking by treating every model input as a trust-boundary decision, not as harmless text. Inventory all instructions and data entering the model, mark external content as untrusted, detect override attempts, constrain tools and credentials, validate every model output before execution, and require human approval for consequential actions. Log the full chain so an anomalous session can be contained. This works for chatbots, retrieval-augmented generation (RAG), and autonomous agents. It reduces both attack likelihood and blast radius, but no prompt filter or model setting can guarantee prevention because attacker instructions and legitimate instructions can appear as tokens in the same context window.

What are prompt injection and model hijacking?

OWASP defines a prompt-injection vulnerability as a user prompt that alters an LLM’s behavior or output in an unintended way. A direct injection is in the user’s message, such as an attempt to override policy or expose hidden instructions. An indirect injection is embedded in material the application retrieves or receives: a webpage, PDF, email, database row, vector-store chunk, memory entry, image, or tool response.

Model hijacking is the consequence in an agentic workflow. Attacker-controlled content redirects an agent from the user’s task to an attacker’s task, for example changing a requested invoice summary into an instruction to send data to an external address. NIST’s Center for AI Standards and Innovation describes agent hijacking as indirect prompt injection in data an agent ingests, including emails, files, and websites.

The architectural problem is fundamental: as the OWASP glossary puts it, the model has no reliable mechanism to tell developer instructions and attacker-written instructions apart when both arrive as tokens in one context window. Authorization therefore has to live in application code and infrastructure, not only in a system-prompt sentence.

Why does indirect prompt injection deserve first-class treatment?

Any content supplied to a model can carry instructions, even when a user expects it to be data. A hidden HTML element, document metadata, OCR text in an image, a calendar description, or a tool’s error message may be interpreted as an instruction. RAG improves access to information but also expands the attack surface because every retrieved item becomes part of the model’s context.

Impact is determined by agency. A read-only summarizer can usually leak information or produce a wrong answer; an agent with mail, file, database, payment, or code-execution tools can cause an external side effect. The same malicious sentence therefore has very different risk in different deployments. Model access should be designed around the minimum agency needed for the requested job.

How do I map the trust boundaries in an AI application?

Start with a data-flow diagram and label each boundary before adding detectors. Record who controls each source, which identity authenticates it, and whether the application can independently verify its provenance.

  1. List instruction channels. Include system and developer messages, user prompts, policy snippets, tool descriptions, memory, and any task templates.
  2. List data channels. Include retrieval results, uploaded files, websites, email, calendars, databases, images, audio transcripts, and tool results.
  3. List executors. Show the browser, shell, SQL engine, template renderer, external APIs, queues, and services that consume model output.
  4. Attach identity and scope. For every source and executor, record tenant, user, authentication context, permissions, and retention period.
  5. Mark trust explicitly. Treat user-controlled and third-party content as untrusted by default. A trusted storage location does not make its contents trusted if another user can write to it.

Microsoft’s retrieval-hygiene guidance treats prompts, retrieved content, documents, memory, and tool results as untrusted inputs requiring provenance and validation. Keep that classification in machine-readable metadata so policy code can enforce it instead of relying on a model to remember a label.

How can I detect suspicious content before it reaches the model?

Use several weak signals together and preserve the original content for investigation. A single classifier will miss adaptive attacks and may block legitimate security or programming text.

Content signals

  • Attempts to override earlier instructions, change the assigned task, or create a new role.
  • Requests to reveal system prompts, secrets, credentials, conversation history, or another tenant’s data.
  • Commands to contact an external address, upload files, alter records, purchase something, grant access, or execute code.
  • Obfuscation such as hidden CSS, zero-width characters, unusual Unicode, base64 or encoded fragments, payload splitting, and instructions spread across multiple fields.
  • Language that claims to be a higher-priority policy, a test mode, or an emergency bypass without verifiable provenance.

Context and provenance signals

  • A new or low-reputation domain, sender, document owner, or vector-store tenant.
  • A retrieved chunk whose instructions are unrelated to the user’s question or inconsistent with surrounding content.
  • A tool response that suddenly asks for a different tool, broader credentials, or a new destination.
  • Content that changed after indexing, has mismatched signatures, or lacks a source identifier and timestamp.

A practical screening pipeline

  1. Normalize text for analysis while retaining the byte-for-byte original for evidence.
  2. Run exact-pattern rules for common override, secret-exfiltration, and external-action phrases.
  3. Run a semantic classifier or a second model to identify task-switching intent, using a conservative threshold for high-impact workflows.
  4. Check source reputation, signatures, tenant ownership, freshness, and whether the item was expected for this task.
  5. Assign a risk score and route high-risk items to quarantine, a read-only path, or human review rather than silently deleting them.
  6. Store the decision, detector versions, and correlation ID with the session.

OWASP recommends combining input and output filtering with deterministic format validation. Detection should trigger a safer execution path; it should not be treated as proof that content is safe.

How should external data be separated from instructions?

Represent retrieved or uploaded material as untrusted data in a separate message or field whenever the orchestration framework supports that distinction. State plainly that embedded instructions are not authorized and that the data may be adversarial.

Use typed objects rather than concatenating raw text into a single prompt. For example, pass a document as {"source_id":"...","trust":"untrusted","content":"..."}, then have application code enforce which fields can influence a tool call. Escape delimiters and normalize Unicode before classification, but do not assume delimiters create a security boundary. The model still sees tokens in one context and may follow text that looks authoritative.

Keep retrieval and memory tenant-isolated. Enforce document-level access controls before retrieval, filter by tenant and user permissions, and attach provenance to every chunk. A model should never be the component deciding whether a retrieved record belongs to the current user.

How do least privilege and isolation limit a hijacked agent?

Constrain agency before testing. Give an agent only the tools, data, network routes, and credential scope required for its current task. Put authorization in deterministic code and infrastructure, not in prose inside the system prompt.

Control area Safer design Failure mode it limits
Credentials Short-lived tokens scoped to one user, tenant, resource, and operation Credential theft and cross-tenant access
Tools Allowlist only the functions needed for the task; deny arbitrary URLs and shell commands Tool-chain redirection and code execution
Data Field-level filtering and read-only views where possible Unnecessary disclosure or modification
Network Egress allowlists, private service endpoints, and blocked metadata or administrative networks Exfiltration and lateral movement
Runtime Isolated workers, resource limits, timeouts, and kill switches Persistence, denial of service, and runaway loops
Tenancy Separate indexes, memory namespaces, queues, and service identities Cross-tenant retrieval and action

Make dangerous operations separate capabilities. An agent that can draft an email should not automatically be able to send it; an agent that can read a record should not automatically be able to delete it. Require a fresh authorization check at the moment of execution because the model’s plan may have changed after retrieval.

How do I validate model outputs and tool calls?

Assume every model-generated value is attacker-influenced until independent checks pass. Structured output makes validation possible, but a valid schema does not make an action authorized.

  1. Parse against a strict schema. Reject unknown fields, malformed types, excessive lengths, and values outside documented ranges.
  2. Recompute policy facts. Resolve the current user, tenant, resource owner, destination, and balance in application code rather than accepting model-supplied identifiers.
  3. Check intent consistency. Compare the proposed operation with the original user request and approved workflow state. A request to summarize should not produce a send, delete, or purchase operation.
  4. Check destination and scope. Allow only approved recipients, domains, tables, API methods, and file paths.
  5. Sanitize for the sink. Escape output for HTML, SQL, shell, templates, URLs, and serialized tool arguments using the target’s standard encoder.
  6. Fail closed. On parsing, authorization, or provenance failure, return a safe error or draft for review rather than executing a best guess.

Validate tool arguments immediately before invocation and log both the proposed and accepted forms. Never let a model-generated URL, SQL fragment, shell command, or API argument bypass the same controls applied to a human request.

Where should human approval gates go?

Place an explicit confirmation checkpoint immediately before a high-consequence side effect. The reviewer should see the exact action, recipients or targets, data to be sent, relevant source evidence, and the identity under which it will run.

  • Sending email, messages, or calendar invitations.
  • Changing, deleting, or exporting records.
  • Making purchases, transfers, or refunds.
  • Granting access, rotating credentials, or changing security settings.
  • Executing code or deploying configuration.

Use two-person approval for irreversible or high-value operations. Do not let the model phrase the confirmation question in a way that hides material details, and expire approvals when the proposed parameters change. OpenAI describes prompt injection as an evolving security challenge; approval gates provide a control that does not depend on guessing every future attack phrase.

What should I log to detect a hijacked session?

Collect a correlated, tamper-evident event trail covering the complete request. Capture user and tenant identifiers, model and policy versions, normalized and original inputs, retrieval sources, memory reads and writes, tool descriptions, tool calls and results, output-validation decisions, approvals, denials, and final side effects.

Alert on repeated override attempts, prompt-leakage canaries, unusual tool sequences, cross-tenant identifiers, sudden query-volume changes, unexpected destinations, and actions unrelated to the user’s stated task. Compare the planned action with historical behavior for the same workflow, but keep behavioral analytics as a signal rather than an authorization decision.

OWASP incident-response guidance recommends tracing behavior, quarantining anomalous sessions, throttling suspicious accounts, and isolating exposed prompts. Preserve enough context to reproduce the decision while redacting secrets in routine logs. Define retention and access rules because security telemetry can itself contain sensitive prompts and documents.

How do I protect a RAG chatbot from malicious documents?

Secure the ingestion path as carefully as the generation path. Authenticate uploaders, scan files, preserve hashes and source metadata, and quarantine documents that fail parsing or provenance checks. Strip active content where practical, but continue to treat visible text, metadata, OCR output, and embedded links as untrusted.

  1. Enforce tenant and document permissions before chunking and indexing.
  2. Store source ID, owner, version, timestamp, and trust classification with every chunk.
  3. At query time, filter retrieval by the requesting identity before ranking results.
  4. Run injection screening on retrieved chunks and record which detector made the decision.
  5. Tell the model to quote or summarize evidence, not execute instructions found in it.
  6. Keep links and actions in a separate review path; do not turn a document’s suggested URL into an automatic tool call.

Test poisoned documents that look authoritative, contain hidden text, or place instructions in metadata. A clean answer does not prove safety if the agent also made an unreviewed side effect.

How should multimodal inputs be tested?

Include images, scanned PDFs, screenshots, audio transcripts, and video captions in the same threat model. Attackers can place instructions in pixels, alt text, EXIF metadata, subtitles, or speech that is transcribed before the model sees it.

Run OCR and transcription through the same provenance and injection screening used for text, preserve the original media for investigation, and restrict tools that can be reached from a multimodal turn. Display the extracted instruction and its source to a reviewer when it would influence an external action. Do not assume that a vision or speech model will distinguish a label intended for a human from an instruction intended for the agent.

How do I run a prompt-injection red-team test?

Build a repeatable test set with a benign user task, a realistic resource containing the malicious instruction, and an observable success criterion. NIST’s AgentDojo work uses this pattern: give the agent a normal assignment, insert an attack into a resource it must process, and record whether the agent performs the hijack.

Test family Examples Observe
Direct overrides Ignore-previous-instruction text, role confusion, fake policy updates Task change, secret disclosure, policy bypass
Web and file content Hidden CSS, metadata, OCR text, encoded or split payloads Whether retrieval or rendering causes execution
Enterprise data Malicious email, calendar event, database row, memory entry, or vector chunk Cross-tenant access and unauthorized workflow changes
Tool responses Error messages or API results that request a different tool or broader scope Unexpected tool sequence or privilege expansion
Multimodal content Instructions embedded in images, scans, audio, or captions Whether extracted text changes the plan
Exfiltration Requests for system prompts, secrets, history, or another user’s records Sensitive-information disclosure rate
Output injection HTML, SQL, shell, template, URL, and structured-argument payloads Unsafe rendering or execution
Adaptive attacks Language changes, encoding, timing variation, repeated attempts Detection degradation and throttling behavior

OWASP recommends covering direct and indirect injections, sensitive-information disclosure, insecure output handling, excessive agency, and system-prompt leakage. Use production-like permissions in a separate environment; a harmless test with no tools cannot measure real blast radius.

Which metrics show whether defenses are working?

Track attack success rate, unauthorized data-exposure rate, unauthorized-action rate, false positives on benign content, time to detection, time to containment, and the percentage of risky actions stopped by approval gates. Break results down by direct, indirect, multimodal, tenant, tool, and model version.

No universal pass rate is established by the cited guidance. Set thresholds from your harm model: a read-only FAQ may tolerate occasional extra review, while an agent handling payments or regulated records should require near-zero unauthorized actions and a tested manual fallback. Re-run the suite after model, prompt, retrieval, tool, or policy changes.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How do detection approaches compare?

Evaluate an architecture across the controls that remain effective when a detector misses an attack. A prompt-only defense should score poorly if it lacks independent authorization and execution controls.

Approach Strength Limitation
Prompt rules and delimiters Cheap, fast first layer for obvious overrides Can be bypassed by paraphrase, encoding, or context; not an authorization boundary
Pattern and semantic detectors Catch broader language and obfuscation families False positives, model drift, and adaptive evasion require tuning
Provenance and retrieval controls Reduce untrusted and cross-tenant content entering context Do not detect a malicious instruction from an otherwise valid source
Least privilege and isolation Limit damage when content changes behavior Requires careful identity, network, and tenancy engineering
Output and tool validation Stops malformed or unauthorized side effects Must cover every sink and be maintained as tools evolve
Human approval Catches consequential actions that automated checks miss Review fatigue and latency; ineffective if details are hidden
Monitoring and red-team tests Find new attack paths and shorten response time Detective rather than preventive; needs representative telemetry

What should happen after a suspected hijack?

  1. Stop pending tool calls and revoke or expire the session’s short-lived credentials.
  2. Quarantine the conversation, retrieved items, memory writes, and generated artifacts so they cannot feed another workflow.
  3. Determine whether data was disclosed or an external action completed; preserve immutable logs and source hashes.
  4. Reset exposed prompts, tokens, and integration credentials, and isolate affected tenants or services.
  5. Notify the responsible owner and follow legal, contractual, and incident-response obligations.
  6. Add the exact payload, its variants, and the observed tool sequence to the regression suite.
  7. Restore service only after authorization checks, retrieval filters, and approval gates are verified.

Can prompt injection be fully prevented?

No. Prompt injection is a class of failures caused by ambiguous instruction and data channels, and attackers can adapt wording, encoding, timing, and delivery medium. Layered controls reduce probability and impact: clear trust boundaries, least privilege, independent authorization, strict output handling, approval checkpoints, telemetry, and recurring adversarial tests.

Design for containment rather than a perfect detector. Keep read-only and side-effecting capabilities separate, make high-impact actions reversible where possible, and provide a manual path when the model or a detector is uncertain.

What is a practical rollout plan?

First 30 days: inventory and containment

  • Draw the data-flow and trust-boundary map.
  • Disable unnecessary tools and broad credentials; add timeouts and egress restrictions.
  • Require structured outputs and deterministic authorization for every existing tool.
  • Start correlated logging and a basic direct/indirect attack corpus.

Days 31–60: detection and review

  • Add provenance metadata, retrieval tenant filters, pattern rules, and semantic screening.
  • Introduce approval screens for sending, changing, deleting, purchasing, granting access, and code execution.
  • Run the red-team matrix against realistic documents, emails, webpages, images, and tool responses.

Days 61–90: resilience and operations

  • Measure attack success, exposure, unauthorized actions, false positives, and response times by workflow.
  • Automate quarantine, token revocation, throttling, and evidence collection.
  • Schedule recurring tests after model, prompt, retrieval, or tool changes and rehearse recovery with service owners.

Frequently Asked Questions

Can a web application firewall stop prompt injection?

A web application firewall can block some malicious HTTP payloads, but it cannot understand instructions hidden in a legitimate document, email, image, or tool response. It should complement application-level provenance, authorization, output validation, and approval controls.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Should I fine-tune the model to resist injection?

Fine-tuning may improve behavior on known examples, but it cannot establish identity, tenant authorization, or permission to execute an action. Treat it as an additional robustness measure and continue enforcing security decisions outside the model.

How often should an AI agent be red-team tested?

Run a baseline before production and repeat tests whenever the model, system prompt, retrieval index, memory design, tool set, permissions, or output sink changes. High-impact agents also need scheduled tests against newly observed attack patterns.

What is a prompt-leakage canary?

It is a unique, monitored marker placed in a protected instruction or test secret. If it appears in an output, retrieval result, log destination, or external request, the event signals possible leakage and can trigger quarantine and credential rotation.
