Reliable production detection uses two separate controls: evidence checks for factual claims and deterministic validation for the output contract your application expects. Trace each material claim to trusted, relevant evidence; parse every response and enforce its schema, types, allowed values, and business rules. Evaluate those controls on representative, versioned traffic, measure false positives and misses, and keep the same telemetry running after launch. Treat detector scores as decision signals, not proof of truth, and keep permission checks and human approval outside the model for consequential actions.
What is the difference between a hallucination and an invalid output?
A hallucination is a content failure: the system states something unsupported, contradicted, or fabricated. An invalid output violates the interface or policy that consumes the response, even when the words happen to be true. A response can be valid JSON and still hallucinate, or contain a true answer that is unusable because a required field is missing.

| Failure class | Typical example | Primary test | Typical response |
|---|---|---|---|
| Unsupported claim | An answer cites a policy that is absent from the retrieved documents | Compare each material claim with trusted evidence | Clarify, qualify, retrieve again, or send to review |
| Contradicted claim | The answer says a product is approved while the authoritative record says it is rejected | Check evidence for a direct conflict | Block or escalate according to risk |
| Malformed structure | Truncated JSON or extra prose where JSON is required | Parser and syntax validation | Retry through a measured repair path or reject |
| Schema violation | A numeric field is returned as a string, or a required property is absent | Schema, type, range, and enumeration checks | Reject, repair, or request clarification |
| Business-rule violation | A refund amount exceeds the account balance or an action targets another tenant | Deterministic application invariants and authorization checks | Block and log; require approval where appropriate |
Keep these definitions stable enough to compare releases. Version the definitions alongside the model, prompt, retrieval configuration, tools, and policy so a change in a score can be traced to a change in the system.
How should I define failure categories and the output contract?
Write an operational definition before choosing a detector. “Hallucination” might mean an unsupported answer, a contradiction, a fabricated citation, or a claim that is not supported by retrieved documents; each definition produces a different test set and threshold. Define “invalid” in terms of what the downstream consumer actually requires.

Specify factual expectations
- State which claims require citations or source passages.
- Define whether an answer may say “unknown” when evidence is absent.
- Record how to handle conflicting authorities, stale documents, and false-premise questions.
- Set the unit of judgment: a sentence, atomic claim, tool decision, or complete answer.
Specify the contract
Document required fields, types, nullable fields, enumerated values, numeric ranges, maximum lengths, and cross-field invariants. Include authorization rules and tenant or session constraints when an output can trigger an action. A contract should state what happens on timeout, detector failure, or an unanswerable request; otherwise teams silently make inconsistent choices in production.
How do I build an evaluation set that predicts production behavior?
Use examples drawn from expected traffic, then add known failure modes. Keep labels, trusted references, retrieved context, expected application outcomes, and the versions of every relevant component. Reserve a held-out portion for measuring changes rather than tuning against every example.
Include difficult and ordinary cases
- Answerable and unanswerable questions.
- Ambiguous prompts and prompts built on false premises.
- Relevant, irrelevant, stale, and conflicting retrieved documents.
- Malformed-output cases, missing fields, wrong types, out-of-range values, and illegal enum values.
- Tool-use scenarios, including denied permissions, invalid arguments, and partial tool failures.
- Long-context and short-context requests, plus languages and user journeys that actually occur in your service.
The Singapore Responsible AI Playbook recommends held-out examples representing expected traffic and known failure modes. Its production guidance calls for measuring precision and recall, false-positive and false-negative rates, over-refusal, latency, cost, completion after warnings or redirections, and reductions in downstream failures. It also recommends testing the complete system with and without a guardrail where possible, because an isolated detector can behave differently when prompts, retrieval, tools, and other controls are present. See the production integration guidance.
Label with enough context to audit
For each example, retain the input, relevant retrieved passages, model output, expected outcome, claim-level labels, contract-validation result, and reviewer rationale. Mark uncertainty and reviewer disagreement instead of forcing a binary label. Split data by time or user journey when leakage is possible; a random split can make a system look better than it is when near-duplicates appear in both sets.
How can I test whether an answer is factually grounded?
Judge material claims against evidence that is relevant to the task, and preserve the evidence trail. For retrieval-augmented systems, store the exact documents and passages shown to the model, then assess whether each claim is supported, contradicted, or not addressed. For agents, connect claims and actions to the document and tool result that led to them.
Use a claim-to-evidence workflow
- Identify material claims, including numbers, dates, identities, causal statements, and citations.
- Retrieve or select authoritative evidence for each claim. Record document identifiers, versions, timestamps, and passages rather than only a search score.
- Classify the relationship as supported, contradicted, not addressed, or unresolved because the evidence conflicts.
- Store the verdict, evidence, evaluator version, and confidence with the claim.
- Sample cases for human review and add disagreements or newly discovered failures to the evaluation set.
NIST describes evaluation probes that scrutinize factual claims against a human-curated corpus and build audit trails mapping decisions to source evidence. Its stated goal is to move beyond “the AI said so” to “here is what the AI found, where it found it, and how the evidence supports the conclusions.” This is an evidence-tracing pattern, not a guarantee that an automated probe establishes truth. Details are in the NIST evaluation-probes project.

Do not mistake fluency or agreement for truth
Well-written prose is not evidence. Sampling several answers and seeing the same claim can reveal uncertainty, but agreement can also reproduce the same error. A detector should therefore inspect sources and task-specific expectations rather than rely on confidence language, style, or repeated generations alone.
How do I validate an LLM output contract deterministically?
Put parsing and business validation in application code at the boundary where the response is consumed. Check the cheapest, most deterministic conditions first, and fail closed for high-impact actions.
Validate in layers
- Parseability: confirm that the complete response is valid UTF-8 and parses as the required format.
- Schema: enforce required properties, object and array shapes, and nesting limits.
- Types: reject strings where numbers, booleans, dates, or identifiers are required.
- Values: enforce enumerations, ranges, length limits, and normalization rules.
- Invariants: check relationships such as totals equaling line items, start dates preceding end dates, and identifiers belonging to the current tenant.
- Authorization: verify that the requested operation and arguments are permitted for the authenticated user and session.
For example, an order service might require a JSON object with an order_id, an action from a fixed enumeration, and an array of line items whose quantities are positive integers. A schema validator can enforce shape and types; only application code can verify inventory, ownership, current price, or whether the action is allowed at that moment.
Make repair observable
If you permit a retry or a constrained repair, record the original failure, retry count, final response, added latency, and user-visible result. Do not let retries erase a regression in the first response. Set a maximum retry budget and define what happens when the validator, parser, or repair service times out.

A perfectly parseable object can contain a fabricated account balance or an unsafe command, so contract checks and evidence checks remain separate.
Which detection approaches should I combine?
No single detector covers syntax, factual support, policy, and human context. Choose methods by the failure they can observe, then compare them on held-out examples and realistic latency and cost budgets.

| Approach | What it can check | Limitations | Useful comparison measures |
|---|---|---|---|
| Deterministic contract validation | Parseability, schema, types, enumerations, ranges, and business rules | Cannot establish factual support or resolve ambiguous evidence | Contract coverage, failure handling, latency, and downstream impact |
| Evidence-grounding evaluator | Whether answer claims are supported by retrieved or curated sources | Depends on evidence quality, claim extraction, and evaluator reliability | Grounded-claim recall, false alarms, traceability, and domain performance |
| Model-as-judge or guardrail | Policy, relevance, semantic support, and rubric-based checks | Adds cost and latency; can miss issues or over-block; itself needs testing | Precision, recall, over-refusal, cost, latency, and adversarial robustness |
| Self-consistency sampling | Disagreement among multiple sampled answers as an uncertainty signal | Agreement does not prove truth; extra generations increase cost | Detection lift per added inference and task-specific held-out performance |
| Human review | Ambiguous, novel, or high-impact cases requiring context | Does not scale to every request and reviewer judgments can vary | Reviewer agreement, escalation yield, turnaround time, and harm avoided |
Documentation for the SelfCheckGPT-style hallucination rail in NeMo Guardrails describes sampling answers and checking consistency. Its examples use manually curated false-premise questions and model runs evaluated in 2023 and March 2024. Those historical results illustrate a method, not a current, general production benchmark; reproduce the evaluation on your own models and traffic before relying on it.
How should I choose thresholds and responses?
Keep detection separate from response. The Singapore guidance uses that exact principle because the same signal can justify different actions in different journeys. A low-risk drafting assistant may warn and continue, while a payment or account-change workflow may block and require approval.

| Signal or condition | Possible response | When it fits |
|---|---|---|
| Contract passes; evidence supports claims | Allow and record | Routine, reversible work with ordinary impact |
| Missing evidence or ambiguous intent | Ask a clarifying question or qualify the answer | The user can resolve uncertainty without harm |
| Low-confidence detector result | Warn, redact the affected claim, or retrieve again | Partial service is better than silent failure |
| Deterministic contract failure | Retry through a bounded path or reject | The downstream interface cannot safely consume the result |
| Permission, invariant, or high-impact failure | Block and request human approval | Irreversible, regulated, financial, or safety-relevant actions |
Tune for harm, not a pleasing score
Set thresholds against the cost of false positives and false negatives, considering impact, reversibility, review availability, and user abandonment. Document which input, output, retrieval result, or tool action each check inspects; possible outcomes; timeout behavior; and the operational owner. A detector that blocks too aggressively can drive users to unsafe workarounds, while a detector that misses rare severe failures can create disproportionate harm.
How do security and tool permissions change the design?
Prompt injection can influence generated text, retrieved-context handling, and tool use. A model-based guardrail is itself exposed to these attacks, so it cannot replace deterministic permission checks or human approval.
Use defense in depth as recommended by OWASP’s prompt-injection prevention guidance:
- Screen inputs and outputs where appropriate, while treating both as untrusted.
- Validate tool names, arguments, resource identifiers, and session context in application code.
- Apply least privilege, narrow scopes, rate limits, and separate credentials for tools.
- Require explicit approval for consequential or irreversible actions.
- Keep retrieved instructions and user content distinguishable from system policy, and test whether untrusted text can alter the tool plan.
OpenAI’s misalignment-monitoring documentation likewise cautions that monitoring can miss issues or flag legitimate activity, and that application safeguards and human approval remain important for consequential actions. Coverage depends on API mode and preserved conversation context, so teams using that feature should verify the current coverage documentation.

What should I monitor after launch?
Establish a baseline during a stable period, then watch rates and distributions by model version, prompt version, retrieval configuration, tenant, journey, and severity. Alert on actionable changes rather than every ordinary fluctuation.
Track detector and user outcomes
- Trigger rates by failure category.
- Warnings, redactions, retries, blocks, and escalations.
- Sampled false positives and false negatives, including reviewer disagreement.
- Over-refusal, abandonment after a guardrail action, and completion after a warning or clarification.
- Latency, timeouts, evaluator errors, provider availability, and retry counts.
- Tool-call denials, authorization failures, and downstream transaction errors.
- Shifts in input distributions, detector scores, retrieved-document mix, and trace length.
- User reports and rare high-impact events.
The Singapore monitoring and incident-response guidance recommends preparing owners and escalation routes before incidents. Preserve relevant traces and configuration versions, route cases to review, switch to a safer fallback, and maintain a scoped way to restrict or disable a guardrail, tool, or journey. Protect records because prompts, documents, outputs, and tool results can contain sensitive data.
The UK government’s voluntary AI Cyber Security Code of Practice, published 31 January 2025, calls for logging system and user actions and analyzing logs for anomalies, unexpected behavior, and changes in model performance. See the UK Code of Practice.
Re-evaluate after every material change
Run the held-out suite after a model, provider, prompt, retrieval index, tool, policy, or detector change. Compare not only detector metrics but the complete user journey: successful completion, downstream failures, review volume, latency, and cost. Include rare severe cases even when their frequency is too low to move an aggregate rate.
How do I measure whether a detector is actually useful?
Report the metric definition, model and version, dataset, organization, and evaluation year with every number. There is no current, broadly generalizable production hallucination rate that can be applied to all systems.
Use a confusion matrix
- True positive: the detector flags a labeled failure.
- False positive: it flags an acceptable result, creating unnecessary friction.
- True negative: it allows an acceptable result.
- False negative: it allows a labeled failure.
Precision is the share of flagged cases that are genuine failures; recall is the share of genuine failures that are caught. Report both with false-positive and false-negative rates, then add over-refusal, latency, cost, user completion, and downstream failures. A higher recall can be unacceptable if it blocks most legitimate work, while a high precision detector can still miss dangerous cases.
Best Value
Evaluate the whole system
Measure retrieval quality, claim extraction, judge behavior, retries, and response routing together. An evaluator may perform well on isolated outputs but fail when retrieval returns irrelevant passages or when another guardrail rewrites the answer. Compare configurations with and without the detector where safe, and record the effect on the final user outcome rather than treating a detector score as the product’s success metric.
What is a practical production architecture?
A robust path keeps evidence, contract validation, authorization, and response policy as observable stages around the model rather than hidden instructions inside it.
- Receive and classify: authenticate the request, identify the user journey and risk tier, and attach a correlation identifier.
- Retrieve and preserve: fetch permitted sources, store document versions and passages, and mark conflicting or stale evidence.
- Generate: call the model with a versioned prompt and tool policy; capture the raw response and tool attempts.
- Validate structure: parse, apply schema and business rules, and verify authorization in code.
- Evaluate claims: map material claims to evidence and record support, contradiction, or absence.
- Decide: apply the risk-tier response policy—allow, clarify, warn, redact, retry, block, or escalate.
- Record and learn: emit privacy-protected telemetry, sample cases for review, and feed confirmed misses into the next held-out evaluation set.
Keep the raw trace, detector versions, and policy decision linked by the correlation identifier, with access controls and retention limits appropriate to the data. This makes an incident reproducible without giving every operator unrestricted access to sensitive prompts or documents.
What should I check before enabling a new model or guardrail?
- Failure definitions and contract versions are approved and tied to the release.
- Evaluation examples cover normal traffic, unanswerable requests, false premises, conflicting evidence, malformed outputs, and tool misuse.
- A held-out set exists and has not been used for tuning.
- Claim-level evidence and reviewer rationale can be audited.
- Schema, invariant, authorization, and timeout behavior are tested in application code.
- Thresholds are mapped to explicit responses and risk tiers.
- Precision, recall, false-positive and false-negative rates, over-refusal, latency, cost, completion, and downstream failures have baselines.
- Monitoring dashboards, alert thresholds, owners, escalation routes, and rollback or disable procedures are ready.
- Logs and traces are protected, retained only as long as needed, and redacted where required.
- High-impact tool actions require deterministic permission checks and, where warranted, human approval.
Frequently Asked Questions
How often should a production LLM evaluation set be refreshed?
Refresh it whenever confirmed incidents, user reports, new tool paths, or major traffic changes reveal a case the suite missed. Keep older examples so trend comparisons remain meaningful, and add a time-based slice to detect drift in terminology, documents, or user behavior.
Should hallucination checks run synchronously on every request?
Run low-latency deterministic checks synchronously when the downstream action cannot wait, and use sampled or asynchronous human review for broader quality measurement. For high-impact journeys, synchronous evidence and permission checks are usually preferable to relying on a later audit.
What information is needed to investigate a false negative?
You need the original input, model and prompt versions, retrieved passages, tool results, raw and transformed outputs, detector decisions, policy response, and timestamps. Link these records with a correlation identifier and restrict access because they may contain personal or confidential data.

The mechanism may be reusable, but thresholds and labels are not automatically portable. Evidence quality, output contracts, user harm, and acceptable false-positive rates differ by domain, so calibrate and validate each application on its own held-out traffic.
