Stop a search loop by tracing one complete failing run, finding the first repeated action or violated constraint, and enforcing a hard boundary on iterations, tool calls, time, tokens, or cost. Then make the agent recover with a compact progress summary and a changed strategy or fresh context. A loop limit alone hides the cause; a trace without a limit lets the failure consume resources. You need both, followed by single-step, full-trace, and multi-turn regression tests.

Why does an AI agent keep searching the same thing?

An agent search loop is a continuation path that keeps generating searches instead of producing a final answer or a measurable change in task state. In a runner such as the OpenAI agent loop, the system repeatedly calls the model, executes its tool calls, follows handoffs, and continues until the model returns with no further tool work. A loop occurs when that path never reaches a completion condition.

The repeated search can look different on the surface. Queries may be textually identical, or they may vary only by punctuation, ordering, or a synonym while retrieving the same documents. The agent may also call different tools with unchanged arguments, hand work between agents without progress, or receive a parser error that causes it to retry indefinitely. Microsoft Agent Framework documentation summarizes the safety requirement as: “Always bound autonomous loops.”

Common reasons include:

  • No hard bound: the runner has no maximum iteration, tool-call, time, token, or cost limit.
  • Uninspectable success: the model believes it is done, but the runner cannot validate a final answer or required subtask.
  • Semantic repetition: normalized queries, arguments, evidence, and task state remain unchanged even when the wording changes.
  • Tool or parser failure: an empty, malformed, or unexpected result is treated as a reason to try the same action again.
  • Self-reinforcing context: a long history repeatedly selects the same search strategy.
  • Handoff churn: agents pass the task among themselves without a whole-workflow budget.
  • Hidden waiting: approval or background-task state is invisible, so the agent keeps acting while an external step is pending.

How do I capture a complete failing run before changing the prompt?

Reproduce one failure with deterministic inputs where possible and save the end-to-end trace. Changing instructions first can remove the evidence needed to locate the actual fault.

  1. Freeze the input. Record the user request, system and developer instructions, model and tool versions, routing decisions, and any random seed or sampling settings your stack exposes.
  2. Run once to the failure boundary. Allow the agent to reach its existing limit, or stop it manually if none exists. Do not silently retry.
  3. Capture every span. For each model call, search, parser, guardrail, and handoff, save inputs, outputs, duration, status, and the invoked tool or generation. OpenAI describes this end-to-end record as a trace; its dashboard groups sessions, turns, and spans.
  4. Preserve external evidence. Store the normalized query, query hash, returned URLs or document IDs, response status, latency, and a hash of the result set. Keep the exact tool arguments, not only a rendered log message.
  5. Record state transitions. At every iteration, log the current hypothesis, unresolved subtasks, completion score, progress delta, token and cost counters, retry count, approval state, and the selected action.
  6. Mark the stop reason. Distinguish a model-generated final answer, a tool error, a guardrail block, a timeout, a budget exhaustion, a repeated-state stop, and a human interruption.

What should each search-iteration log contain?

Field Purpose
Iteration number and elapsed time Shows growth and identifies slow or accelerating loops.
Normalized query and query hash Reveals exact and semantic repetition after whitespace, case, ordering, and punctuation are normalized.
Tool name and canonical arguments Detects unchanged calls even when the model’s prose differs.
Response status, latency, and result hash Separates a new result from the same evidence returned repeatedly.
URLs or document IDs and novelty flag Shows whether each search adds any source not seen before.
Model message, selected action, and completion score Connects the decision to the available evidence and the runner’s success test.
Tokens, estimated cost, and retry counters Supports enforceable operational budgets.
Hypothesis, unresolved subtasks, and progress delta Tests whether the task state actually changes.
Stop reason, escalation target, and approval state Makes recovery and human intervention explicit.

How can I find the first action that caused the loop?

Compare adjacent iterations, not just the final transcript. The first point at which the agent stops gaining information is usually more useful than the last repeated search.

Check for exact and semantic repetition

Canonicalize query text by lowercasing, trimming whitespace, sorting unordered filters, and removing harmless punctuation. Apply the same canonicalization to tool arguments. Then compare hashes and returned evidence. A configurable no-progress threshold should count cycles in which the query, arguments, evidence hash, unresolved subtasks, and completion score all remain unchanged.

Locate the first divergence

Inspect the transition where behavior changes from productive to repetitive. Typical first divergences are bad query construction, a schema or parser error, stale context, a misleading search result, handoff churn, or a completion predicate that never recognizes success. If the tool returned a malformed payload, the parser may be the true origin even though the visible symptom is another search.

Test the completion predicate

Write success as a structured condition the runner can inspect, such as a validated answer object with every required field or a set of completed subtasks. A natural-language instruction such as “stop when you have enough information” is not an executable predicate. Log the predicate’s inputs and decision so you can see whether the agent had satisfied the task but the runner rejected it.

Trace handoffs and waiting states

Record every transfer between agents, including the sending agent’s claimed progress and the receiving agent’s initial state. Set both a per-agent budget and a whole-workflow budget. If an approval, browser task, or background job is pending, expose that state and route to a waiting or escalation outcome rather than permitting more autonomous searches.

How do I set a maximum number of tool calls and iterations?

Enforce multiple independent limits at the runner boundary. A model instruction is advisory; the executor must reject the next action when a limit is reached.

Boundary What it protects Recommended behavior at the boundary
Maximum iterations Unbounded model-tool continuation Stop and emit a trace-backed summary.
Maximum tool calls Search storms and expensive APIs Return a budget-exhausted event with the last useful evidence.
Wall-clock timeout Slow providers and hung background tasks Cancel in-flight work where supported, then preserve partial results.
Token budget Context growth and repeated reasoning Summarize progress or restart with a compact state packet.
Cost budget Unexpected provider or retrieval charges Require approval or switch to a lower-cost recovery path.
Repeated-state threshold Semantic loops that evade text matching Stop after a configurable number of no-progress cycles.
Per-agent and workflow handoff limits Multi-agent churn Escalate when either local or global allowance is consumed.

Microsoft Agent Framework uses finite loop limits and supports explicit max_iterations-style configuration. Match that control with budgets appropriate to your workload; a research task may need more searches than a simple lookup, but neither should be unlimited.

Use an executable success schema

Define the final object before the first search. For example, require a validated answer containing the requested conclusion, cited evidence identifiers, unresolved questions, and a confidence or limitation field. Reject incomplete objects as explicit failures and route them to targeted recovery rather than asking the model to “try again.”

Implement a repeated-state detector

Keep a compact fingerprint for each iteration: canonical query, canonical arguments, evidence hash, unresolved-subtask set, and completion score. If the fingerprint and progress delta repeat, increment a no-progress counter. Stop when the counter reaches your configured threshold, even if the model proposes a cosmetically different query.

state = load_task_state()
while True:
    if budget.exhausted() or state.no_progress_cycles >= LIMIT:
        return recover(state, reason='bounded_stop')
    action = model.choose_action(state)
    result = execute(action)
    progress = evaluate_progress(state, action, result)
    state = update_state(state, action, result, progress)
    if success_schema_valid(state):
        return finalize(state)

This is framework-neutral pseudocode, not a drop-in SDK call. The important property is that the check runs outside the model and before another tool call.

What should the agent do when the loop is stopped?

A stop should produce a useful failure outcome, not a silent timeout or another identical retry. Create a compact state packet containing:

  • the original task and required output;
  • searches tried, canonical forms, and tools used;
  • evidence found, evidence missing, and duplicate-result counts;
  • the first suspected cause and the trace span where it appeared;
  • iterations, tool calls, elapsed time, tokens, estimated cost, and retries consumed;
  • the exact stop reason and any pending approval or external task;
  • changed next options.

Offer one of four recovery paths: ask the user for a narrower query, switch to a different retrieval tool or query plan, request human approval, or restart from the original task plus the progress packet. A fresh context is appropriate when the existing history keeps selecting the same action. Do not append the same “try again” instruction without changing the query plan, tool, context window, model, or approval state.

When should I reset the context?

Reset after the trace shows self-reinforcing context: the agent cites its own prior failed searches, repeats an unresolved hypothesis, or spends tokens restating the same evidence. Preserve the progress packet outside the context, then start with the original task, verified evidence, known gaps, and a new strategy. Do not reset merely because a single search failed; first determine whether the failure is transient and whether a targeted retry changes a relevant variable.

What are the common root causes and their fixes?

No hard bound

Symptom: the run consumes resources until a provider or infrastructure timeout. Fix: enforce iteration, tool-call, time, token, cost, repeated-state, and handoff limits in the executor. Return a typed stop event that downstream code can handle.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Completion predicate mismatch

Symptom: the model has enough evidence, but the runner never accepts the result, or the model claims completion before required subtasks are done. Fix: define a machine-checkable success schema, validate it at the boundary, and log each predicate input and decision.

Semantic repetition

Symptom: queries differ in wording but retrieve the same documents and do not change the task state. Fix: normalize queries and arguments, hash evidence, track novelty, and stop after no-progress cycles.

Search-result or parser failure

Symptom: empty, malformed, or schema-incompatible tool output causes the model to repeat the call. Fix: validate every tool response, make empty and malformed results explicit failure types, and route them to a recovery policy that can change tools or ask for clarification.

Self-reinforcing context

Symptom: a long transcript repeatedly selects the same action despite new instructions. Fix: summarize verified progress, discard irrelevant history, and run the next attempt in a fresh context with a different strategy.

Hidden handoff churn

Symptom: agents transfer responsibility without adding evidence or closing a subtask. Fix: trace each handoff, require the sender to state measurable progress, and enforce local and global handoff budgets.

Approval or external-task stall

Symptom: the agent keeps searching while waiting for a human decision or background job. Fix: expose a pending state, pause autonomous execution, and resume only when the external state changes.

How do I verify that a loop fix really works?

Use three tests in sequence, then keep the failure in a regression set.

  1. Single-step test: replay the smallest failing transition with a fixed model response and tool result. Confirm that schema validation, progress calculation, and stop logic produce the expected event.
  2. Full-trace evaluation: replay the complete failure and inspect every span. Confirm that the agent reaches a valid success object or a bounded recovery outcome, and that no duplicate action occurs after the stop boundary.
  3. Multi-turn regression: run the original task across follow-up turns and altered wording. Check context retention, handoff behavior, budgets, and recovery choices.

Keep a small corpus of loop failures and replay it after changing prompts, tools, routing, models, or stop logic. OpenAI describes trace grading as “the fastest way to identify workflow-level issues while debugging.” Use graded traces to evaluate tool choice, evidence quality, completion, and policy compliance rather than relying only on a final-answer score.

Evaluate at three observability levels

LangChain’s observability model separates runs, traces, and threads. A run is one LLM or tool step, so use it to check tool selection and arguments. A trace is the complete execution, so use it to check termination, evidence, and handoffs. A thread is multi-turn context, so use it to check whether progress survives later user messages. A failure can pass one level and fail another.

Use evidence-backed constraint checks

AgentRx’s approach is to synthesize executable constraints from tool schemas and domain policies, then record evidence-backed violations step by step. The practical lesson is to attach each violation to the exact input, output, or state transition that proves it, rather than labeling an entire trajectory “bad.” Microsoft Research reported 115 manually annotated failed trajectories in 2026; its method improved failure localization by 23.6% and root-cause attribution by 22.9% over prompting baselines. Those figures describe that reported evaluation, not a guarantee for every agent stack.

Which debugging approach is safest for production?

No single control is sufficient. Choose a combination based on the failure you must diagnose and the risk of continued execution.

Approach Trace visibility Termination safety Recovery controls Evaluation granularity Reproducibility Operational cost
Dashboard only High after the fact Low; it may only display an ongoing loop Usually limited Session and span views Depends on captured inputs Moderate ingestion and storage cost
Loop limit only Low High for runaway duration Often a generic timeout Usually final outcome only Weak without a trace Low
Trace plus hard budgets High High Can emit targeted recovery Run, trace, and thread checks Strong when inputs and results are stored Moderate
Trace, budgets, and regression corpus High High Strategy changes and fresh-context restart Continuous dataset-based evaluation Strong Highest setup cost, lowest repeated-failure risk

A dashboard without explicit stop rules makes a loop visible but does not contain it. A loop limit without traces stops the symptom but leaves the cause unknown. Production systems should combine both and add a tested recovery outcome.

What is a practical production checklist?

  • Can you replay one complete failure with deterministic inputs?
  • Does every model call, tool call, parser, guardrail, and handoff have inputs, outputs, duration, and status?
  • Are queries, arguments, evidence, and task state normalized and fingerprinted?
  • Is success a validated object rather than an instruction to “know when done”?
  • Are iteration, tool-call, time, token, cost, repeated-state, and handoff budgets enforced outside the model?
  • Does a stop emit a state packet with evidence, gaps, suspected cause, and next options?
  • Does every retry change a relevant variable?
  • Have you tested a single transition, the full trace, and multi-turn behavior?
  • Is the failed trajectory stored in a regression corpus?
  • Can a human see and approve pending external work instead of an agent looping invisibly?

Frequently Asked Questions

Why does changing the prompt sometimes fail to stop a search loop?

The loop may originate in a parser, tool response, handoff policy, or completion predicate rather than the wording of the prompt. Trace the first repeated or invalid transition and change the component that produced it; otherwise a new prompt can leave the same executable failure intact.

How many searches should an AI agent be allowed to make?

There is no universal number because task complexity, retrieval quality, latency, and cost differ. Set a finite default, expose it as configuration, and tune it from replayed traces using evidence novelty and progress per call rather than choosing an arbitrary unlimited allowance.

What is the difference between a timeout and a repeated-state stop?

A timeout ends execution because elapsed time exceeded a wall-clock budget, even if the agent was making progress. A repeated-state stop ends it because normalized actions, evidence, and task state stopped changing; logging both reasons tells you whether the problem is slowness or stagnation.

Should I let an agent continue after an empty search result?

Treat an empty result as an explicit tool outcome and apply a recovery policy. The next action should alter the query, source, filters, or user request; automatically issuing the same call turns a transient retrieval problem into a loop.
