To check what an AI answer leaves out, break it into important claims, trace those claims to suitable original or authoritative sources, and look for missing scope, qualifications, exceptions, methods, and disagreement. Treat citations as leads to inspect—not proof—and spend the most time checking claims where an error could cause harm or a costly decision.
No quick check can prove an answer is complete. A repeatable evidence check can, however, reveal unsupported statements and important context the answer did not include.

An AI answer can sound certain and read smoothly without having checked every statement against evidence. Likely wording, repeated wording, and confident wording are not the same as verified wording, as the Technical University of Munich’s AI Literacy guide explains.
Omissions are harder to notice than obvious factual mistakes. A statement may be accurate for one date, population, jurisdiction, definition, or version but misleading when those limits are left unstated. A summary can also report a study’s conclusion while omitting its methods, caveats, or contrary evidence. Checking only whether an answer sounds plausible—or whether it includes links—does not uncover those gaps.

In a 2025 study published in Nature Communications, the SourceCheckup framework evaluated seven popular large language models on 800 medical questions and 58,000 statement-source pairs. The authors reported that 50% to 90% of evaluated responses were not fully supported by their cited sources, and sometimes were contradicted by them. For GPT-4o with Web Search, they reported that approximately 30% of individual statements were unsupported and nearly half of responses were not fully supported. These results apply to the models and medical questions evaluated; they are not error rates for every AI system or subject. They show why citation presence alone is not a guarantee, not how often any particular answer you receive will be wrong. Read the study.
How do I turn an AI answer into claims I can check?
Start with the statements that carry the answer’s argument or could change what you decide. The University of Florida Business Library recommends identifying claims before trying to verify them. Its fact-checking guide offers a useful starting point.

Mark the parts that need evidence
Read the answer sentence by sentence and flag:
- Factual statements, especially names, dates, quantities, and cause-and-effect claims.
- Quotations and attributions: who said or wrote something, and where.
- References, links, and claims that a source establishes a particular result.
- Calculations, comparisons, and recommendations built from factual premises.
- Assumptions about what the question means, what circumstances apply, or what is typical.
- Conclusions that combine several claims or imply that a general rule has no exceptions.
Do not treat every sentence as equally urgent. A minor background detail usually matters less than a claim that supports the answer’s recommendation, affects someone’s rights or health, or changes a significant financial or technical decision.
Split bundled statements
One sentence can contain several claims that need different evidence. For example, an illustrative answer might say, “The policy took effect in 2024, covers every employer, and allows appeals within 30 days.” Check the effective date, who is covered, and the appeal deadline separately. A source that confirms one part does not automatically support the others.
It helps to record each claim in plain language, without the answer’s persuasive framing. If a sentence says a product is “the safest option,” the checkable questions may be which risks were compared, for whom, under what conditions, and according to what evidence. If you cannot state what evidence would confirm or disconfirm a claim, it may be too vague to verify as written; clarify it or treat it as an opinion rather than an established fact.
Use a short claim log
A simple table prevents a long answer from turning into a vague impression. Record the claim, its importance, the best source to check, what that source actually supports, and your decision. A log can be as simple as:

| Claim | Why it matters | Evidence to inspect | Result |

| One precise statement from the answer | What could change if it is wrong? | Original document, dataset, standard, or other suitable source | Keep, qualify, revise, reject, or investigate |
For a short, low-stakes answer, a few notes may be enough. For a high-stakes answer or one with many linked conclusions, the log makes gaps and unsupported steps easier to see.
How do I find evidence independent of the AI answer?
Set the AI response aside long enough to search for evidence appropriate to each claim. Look for the original research, official document, dataset, standard, or other authoritative material—not just another page repeating the same summary.
- Choose the right kind of source. A claim about what a rule says calls for the relevant official text and jurisdiction; a claim about a study calls for the publication itself; a statistic may require its underlying dataset, definition, and collection method. The best source depends on the claim.
- Search distinctive terms from the claim. Use a named report, author, document title, quoted phrase, or specific figure to locate the source. Searching for the whole AI answer can return pages that merely echo its wording.
- Open the source and check the exact passage or data. Confirm that it exists and says what the answer attributes to it. A result snippet, abstract, search summary, or page title is not enough when the details matter.
- Check recency and applicability. Note the source’s date and the claim’s relevant version, population, place, or time period. A reliable source can still be outdated or about a different case.
- Look for corroboration or meaningful disagreement. Check whether independent authoritative sources support the same point or identify a different interpretation. Independence matters: multiple pages repeating one original source do not provide multiple confirmations.
The University of Illinois Urbana-Champaign Library advises evaluating AI output for factors such as bias and context, while Purdue University Northwest’s Verify AI guide supports tracing claims to evidence. These checks are especially useful when a polished answer makes a broad claim from a narrow source.
How do I check whether a citation really supports its claim?
Check citations in two passes: first confirm that the referenced source exists and its details are accurate; then read enough of the source to see whether it supports the attached statement. A genuine citation can still be irrelevant, incomplete, or misrepresented.
Verify the reference itself
- Does the link open the named source, rather than a similarly titled or unrelated page?
- Do the author, title, publication, date, and other available details match the source?
- Is it the original work or an intermediary summary? If the answer makes a claim about the original, inspect the original where possible.
- Is the source current enough for a claim that may change over time?
The University of Oxford Bodleian Libraries’ guide to evaluating AI-generated references focuses on evaluating references and the information they point to. Even a correctly formatted citation should not be accepted without checking its contents.
Match the source to the precise wording
Ask whether the cited source supports the whole claim, not merely the same subject. If an answer says a study proves a treatment works for all adults, check the study’s actual participants, result, uncertainty, and limits. Evidence for one group or outcome cannot silently establish a broader statement. If a source mentions a fact only in passing, that may not support a stronger causal or general conclusion.

Read beyond the abstract when the stakes warrant it. The methods, definitions, limitations, and results may change how a headline or summary should be interpreted. If you cannot access the full source, say that your check is limited rather than treating an abstract or snippet as complete confirmation.
What context should I look for in the original source?
Look for details that define how far the evidence can be applied. These include scope, definitions, dates, populations, jurisdiction, version, uncertainty, exceptions, methods, limitations, and counterevidence.
Recommended Free Tools
Scope, definitions, and applicability
Check who or what the source covers, and how it defines key terms. “Users,” “effective,” “safe,” or “significant” may have a specific meaning in a study, policy, or technical document. Ask whether the source concerns the same population, setting, location, product version, or legal jurisdiction as the AI answer implies.
Dates and changes over time
Distinguish when an event happened, when evidence was collected, and when the source was published. For claims about current rules, recommendations, software, or statistics, verify that the relevant source has not been superseded. An answer can accurately describe an older source and still mislead if it presents that information as current.
Methods, uncertainty, and limitations
Find out how a result was measured, which assumptions were used, and what uncertainty the source reports. A numerical finding without its denominator, measurement definition, or study design may create a false impression of precision. Look for stated limitations and avoid turning cautious language such as “may,” “associated with,” or “in this sample” into a guarantee or universal claim.
Exceptions and competing evidence
Search for exclusions, edge cases, dissenting findings, or official interpretations that could change the answer. This does not mean every disagreement deserves equal weight; assess whether the counterevidence is credible, relevant, and based on evidence comparable to the source being checked. The aim is to see whether the answer omitted a material qualification, not to collect objections indiscriminately.

The University of Maryland Health Sciences and Human Services Library’s content-assessment guidance is a relevant resource for assessing AI-generated information. In health-related cases, a summary should not be treated as a substitute for the underlying evidence or qualified advice appropriate to the person’s circumstances.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How can I compare two AI answers without confusing agreement with proof?
Compare the answers claim by claim, but use their differences to identify what to investigate—not as a vote on what is true. Agreement between two AI tools is not independent proof; both may omit the same context or rely on similar material.
For each answer, compare:
- Claim-level support: Which statements have sources that actually back the precise wording?
- Source quality and independence: Are the sources authoritative and genuinely separate, or do they repeat one another?
- Scope: Do dates, populations, jurisdiction, definitions, and versions match the question?
- Uncertainty: Does either answer mention qualifications, limitations, or exceptions?
- Counterevidence: Does either answer address a credible alternative interpretation or material disagreement?
These are practical comparison questions synthesized from institutional guidance, not a validated scoring instrument. A numerical score can conceal important differences: one unsupported claim may matter more than several well-supported background facts. Write down the concrete difference and check it against sources that are suitable to the claim.
Can I ask another AI tool to find omissions?
Yes—as a way to generate leads, not as an independent verification. Rephrase the question, ask what assumptions the answer makes, request its limitations, or ask what credible counterarguments could matter. Then check any new claims or references against appropriate source material.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
A second answer can surface a missing group, exception, or interpretation you had not considered. But the second system may also be wrong, and agreement between tools does not establish that a claim is correct. Do not let an AI-generated list of sources replace opening and evaluating the sources themselves.
How much fact-checking does an AI answer need?
Scale the effort to the consequences of being wrong. A small, reversible choice may need a quick check of the key factual claim; a decision that could affect health, finances, legal rights, safety, or other people deserves a more careful review of the original evidence and important limitations.
Best Value
- Low consequence: Verify the central fact and any details that would change the practical answer. You may not need to audit every incidental sentence.
- Moderate consequence: Check the main claims, the cited sources, and the applicability of the evidence to your situation. Look for at least one independent authoritative source where available.
- High consequence: Inspect primary documents or studies, review relevant methods and limitations, and seek qualified human judgment when the decision calls for it. Do not rely on an AI answer as the sole basis for action.
Do not confuse a faster check with a complete one. When evidence is unavailable or ambiguous, record what remains uncertain and avoid treating the answer’s confidence as a reason to proceed.
What should I do after checking a claim?
Choose a clear outcome for each important claim: keep it if the evidence supports its exact scope, qualify it if the evidence is narrower, revise it if the wording is inaccurate, reject it if it is contradicted, or investigate further if the evidence is insufficient.
- Keep: The source supports the claim as written and applies to the relevant case.
- Qualify: Preserve the supported part, but add the missing condition, date, population, uncertainty, or exception.
- Revise: Correct a date, number, attribution, definition, or degree of certainty.
- Reject: Remove a claim that lacks suitable support or conflicts with stronger relevant evidence.
- Investigate: Leave it unresolved when sources are inaccessible, genuinely conflicting, or too limited to answer the question.
When you share a corrected answer, keep the evidence trail attached to the claims it supports. A link next to a paragraph is not enough if readers cannot tell which source supports which statement. State material uncertainty plainly rather than smoothing it away.
Can any checklist prove an AI answer is complete?
No. The checks above can help identify unsupported claims and important omissions, but they cannot guarantee that every relevant gap has been found. The University of Illinois Urbana-Champaign Library, Technical University of Munich AI Literacy, and Oxford Bodleian Libraries provide practical evaluation guidance; none establishes a completeness test that proves an answer has no important omissions.
That limit matters most when the answer is broad or the stakes are high. Treat a successful check as evidence that the claims you examined survived those checks—not as proof that every possible exception, source, or interpretation has been considered.
Frequently Asked Questions
What should I do if an AI citation is broken?
Search the exact title, author, or DOI in a library catalog or the publisher’s site to see whether the work can be located elsewhere. If you cannot identify the source, mark that citation as unverified; do not assume the cited claim is false solely because a link fails.
How do I check an AI-generated quotation?
Find the original transcript, publication, recording, or document and compare the exact words and surrounding context. If you can only find the quote repeated in secondary pages, treat the wording and attribution as unconfirmed.
What if reliable sources disagree about an AI answer?
First check whether the sources use different definitions, dates, populations, or methods; apparent disagreement may reflect different questions. If they address the same question and remain in conflict, describe the disagreement and avoid presenting one interpretation as settled without a strong basis.
Is an AI answer safe to use if it has citations?
Not on that fact alone: citations can be inaccurate, irrelevant, or fail to support the attached wording. For consequential use, verify the key claims in the sources and get appropriate qualified advice where needed.
