Do not treat an AI answer as verified just because it sounds confident or includes citations. Check it claim by claim: identify the source behind each important statement, open the original document, confirm that it supports the exact wording and date, compare high-impact facts with an independent authority, and look for omitted perspectives or stereotypes. If a link, quotation, study, court case, statistic, or DOI cannot be independently found and matched to the claim, label it unverified rather than repeating it. For medical, legal, financial, safety, policy, or scientific decisions, add qualified human review and documented testing.
Why can a fluent AI answer still be wrong?
Generative AI systems produce a likely sequence of words; they do not automatically retrieve and validate each fact before writing it. Google Search Central therefore says all AI-generated content should be manually fact-checked for accuracy and trustworthiness before publication. A polished explanation, precise number, or long reference list is not proof.

OpenAI’s current guidance puts the practical warning plainly: “ChatGPT can be helpful—but it’s not always right.” NIST’s 2025 terminology calls confidently presented false or erroneous output confabulation. That includes invented reasoning, quotations, citations, and references to sources that do not exist. The UK House of Commons Library’s 2026 guidance summarizes the right role: AI is an assistant, not an authority.
- Confidence is a style, not a reliability score. Models can use assertive language when evidence is missing.
- Specificity can be fabricated. Exact dates, page numbers, journal titles, legal citations, and DOI-like strings may be generated because they look plausible.
- A real source can still be misused. The link may be genuine but irrelevant, outdated, outside the stated jurisdiction, or unable to support the sentence attached to it.
Your goal is not to decide whether an answer “sounds right.” It is to build a traceable chain from each material claim to suitable evidence and to expose what the answer leaves out.

What is the fastest reliable workflow for checking an AI answer?
Use the following sequence for any answer that matters. You can skim low-risk brainstorming, but apply every step to content you will publish, submit, act on, or pass to someone else.
- Define the task and its risk. Decide what decision the answer will influence and what could go wrong. A casual explanation of a hobby needs less scrutiny than dosage advice, a tax interpretation, a safety procedure, a legal deadline, or a policy recommendation. For high-consequence uses, plan expert review before asking the model.
- Atomize the response into claims. Break paragraphs into separately checkable facts, dates, figures, quotations, causal explanations, interpretations, and opinions. Treat a sentence containing several numbers or a compound conclusion as multiple claims. This prevents one correct detail from making an unsupported conclusion appear sound.
- Request and trace the originals. Ask the model to identify the author, organization, title, publication date, jurisdiction, URL or DOI, and the specific passage behind each important claim. Open the link yourself. Follow summaries back to the original study, government record, standard, court decision, dataset, or first-party documentation.
- Match evidence to the exact wording. Read enough surrounding context to test the number, quote, definition, population, method, and limitation. A source that says a treatment “may help” does not support “is proven.” A national statistic does not automatically describe your city, year, or demographic group. A court decision from one jurisdiction does not establish a rule everywhere.
- Check currency and independence. Confirm the publication and update dates, version number, effective date, and jurisdiction. Compare consequential claims with a second authoritative or genuinely independent source. Independence matters: ten pages repeating the same press release are not ten confirmations.
- Record the result. Mark each claim as verified, contradicted, outdated, ambiguous, or unverified. Keep the source, access date, and a short note explaining the match or problem. Do not silently turn an uncertain statement into a fact while editing.
- Revise or remove. Replace unsupported precision with a qualified statement, obtain better evidence, or delete the claim. Preserve meaningful uncertainty instead of hiding it in confident prose.
How should I split an answer into checkable claims?
Highlight every assertion that could be true or false. Separate “The law changed in 2024, so employers must…” into the change date, the legal text, the covered employers, and the consequence. Mark value judgments and recommendations separately; they require an explicit rationale and may depend on goals rather than a factual citation.
Pay special attention to numbers, superlatives (“largest,” “only,” “always”), causal verbs (“caused,” “prevents”), and quotations. These are easy for a model to overstate and easy for a reader to repeat without checking.
How do I check whether an AI citation or source is real?
A citation is evidence only after the cited work exists and supports the claim. Check the source itself, not merely the formatting of the reference.

Use this source-authentication checklist
- Resolve the link. Make sure the URL loads the claimed organization or publication rather than a redirect, parked domain, unrelated page, or dead link. AI-generated links can be outdated, broken, or unrelated.
- Match bibliographic details. Search the organization’s own site, library catalog, journal index, court database, or DOI registry for the exact title, author, date, volume, and identifier. Plausible-looking journal names, case numbers, and DOI strings can be invented.
- Locate the quoted passage. Use the document’s search function, then read the surrounding paragraphs, footnotes, table notes, and appendices. If the wording does not appear, do not present it as a quotation.
- Check authority and scope. Identify who produced the material, how it was reviewed, what population or jurisdiction it covers, and whether it is a primary record, a secondary summary, an opinion, or marketing copy.
- Test the date and version. An old policy, superseded technical standard, retracted paper, or preliminary estimate may not answer a current question. Record the effective date, not just the page’s creation date.
- Compare the claim with the source’s conclusion. Read methods, limitations, and conflict-of-interest statements. A statistically significant association is not automatically a causal effect; a model projection is not an observed result.
| What the AI gives you | What to verify | Common failure |
|---|---|---|
| Journal article and DOI | Exact title, authors, DOI record, publication status, quoted result, corrections or retraction | Invented article or a real paper whose result is reversed or overstated |
| Government or agency report | Official domain, edition, release date, jurisdiction, table and footnote definitions | Correct report attached to the wrong country, year, population, or measure |
| Law, regulation, or court case | Primary text, court or legislature, effective date, current amendments, controlling jurisdiction | Fabricated case citation or a real ruling generalized beyond its jurisdiction |
| Web article or statistic | Original dataset or study, methodology, update date, independent confirmation | Secondary copy repeated as if it were primary evidence |
What if a citation cannot be found?
Run several independent searches using the title, author, distinctive phrase, and identifier. Search the alleged publisher’s own archive and a reputable index rather than relying on another AI system to confirm it. If no authoritative record appears, label the citation unverified and remove any quotation or precise claim that depends on it. Do not “repair” an invented reference by guessing a similar real source.
How can I tell whether the answer is current?
Currentness is a separate check from truth. A statement may have been accurate when written and still be wrong for your decision today.

- For news and fast-moving events, establish a cutoff time and compare reports from independent, reputable organizations.

- For technical instructions, test the steps against the current official documentation and your actual version. Interfaces and command options change.
If sources disagree, show the disagreement and explain why: different dates, definitions, samples, methods, or jurisdictions often account for the difference. Do not choose the newest number automatically if it is provisional and the older one is the final audited figure.
How do I spot bias, stereotypes, or missing perspectives?
Bias review covers both wording and selection. An answer can contain individually true sentences while presenting a distorted picture because it omits affected groups, treats one viewpoint as the default, or applies different standards to similar people.

Review the language
- Are some identities or communities described more positively, sympathetically, or competently than others?
- Does the answer use stereotypes, clichés, deficit language, or broad generalizations about a group?
- Are labels defined, or does the response treat a contested term as neutral fact?
- Does it distinguish an individual case from a claim about an entire population?
- Are uncertainty and limitations applied consistently, or only when discussing one side?
Review what is missing
- Which demographic, regional, disability, language, socioeconomic, or stakeholder perspectives are absent?
- Who benefits from the recommendation, and who bears its risks or costs?
- Does the evidence include people representative of the setting where you will use the answer?
- Are counterarguments, adverse effects, or reasonable alternative explanations addressed?
Re-prompting can expose hidden assumptions. Ask for neutral wording, a list of affected groups, the strongest counterargument, and evidence that would change the conclusion. Canada’s guidance recommends neutral prompts, attention to population diversity, ongoing bias testing, trusted-source checks, expert review, and disclosure of AI use. A second prompt is a diagnostic aid, not a substitute for independent review.
How should I verify a high-stakes answer?
For medical, legal, financial, safety, policy, or scientific decisions, raise the evidence standard before relying on the output. If factual accuracy or data integrity cannot be assured, avoid using AI as the decision-maker.

- Check privacy and confidentiality before entering personal, proprietary, or regulated information.
- Provide a way to correct errors and monitor performance after deployment.
For scientific information, USGS describes quality validation, transparent methods, peer review, and public release practices as parts of trustworthy communication. Those practices matter because a model’s answer can appear logical even when its data, method, or citation is fabricated.

| Criterion | Questions to ask |
|---|---|
| Traceability | Can each important statement be followed to an identifiable original source? |
| Claim-level accuracy | Does the source support the exact number, quote, definition, and causal wording? |
| Currency | Are dates, versions, laws, and statistics current for the stated place and purpose? |
| Completeness | Are material limitations, alternatives, risks, and affected groups included? |
| Uncertainty | Does the answer distinguish verified facts, estimates, assumptions, and unknowns? |
| Balance | Are perspectives and standards applied consistently without stereotypes or default assumptions? |
| Test performance | Does it hold up on representative and adversarial cases, not just the example that prompted it? |
An answer with fewer claims but strong, relevant evidence is usually safer than a comprehensive-looking answer built on unverifiable references.
What should I do when the answer has no citations?
Absence of citations does not prove an answer false, but it removes a fast route to verification. Ask for claim-level sources and the date or version behind each material statement, then apply the same workflow. For basic explanations, consult a current authoritative reference yourself; for consequential decisions, pause until a qualified reviewer confirms the underlying facts.
Can prompting reduce errors and bias?
Better prompts improve the review surface, not the model’s authority. Ask the system to separate facts from interpretation, state uncertainty, identify assumptions, provide primary sources, quote only text it can locate, specify dates and jurisdictions, and list plausible counterarguments. Requesting “do not invent a citation; say you cannot verify it” may reduce unsupported references, but every citation still requires manual checking.
A printable final checklist
- Have I defined the decision and its risk?
- Did I split the response into individual claims?
- Can I identify and open the original source for each material claim?
- Does the source support the exact wording, number, quotation, context, and jurisdiction?
- Is the information current, and did I check the relevant version or effective date?
- Did an independent authoritative source confirm important facts?
- Did I test for stereotypes, missing groups, default assumptions, and unequal standards?
- Have I labeled claims as verified, contradicted, outdated, ambiguous, or unverified?
- For high-stakes use, did a qualified person review representative cases and the final decision?
Frequently Asked Questions
Can an AI answer be accurate if it has no sources?
Yes, a statement can happen to be correct without a citation, but you have no convenient evidence trail. Ask for the source, date, and scope of each material claim, then verify it independently before relying on or publishing the answer.
Best Value
Should I ask another chatbot to fact-check the first one?
A second model can identify questions, contradictions, or missing perspectives, but it can repeat the same kind of error. Use it as a review aid and confirm decisive claims against primary or authoritative human-maintained sources.
How do I document an AI fact-check for an editor or auditor?
Keep the original prompt and output, model/version and date, a claim-by-claim review sheet, source links or document identifiers, quoted supporting passages, unresolved issues, and the reviewer’s decision. Record changes when a source or model version is updated.

A hallucination, or confabulation, is fabricated or erroneous content presented as if true, such as an invented study or quotation. Bias can occur even when individual facts are real, through selective evidence, stereotypes, unequal wording, or omission of relevant perspectives.
