An AI-feature audit is a repeatable evidence trail, not a one-time accuracy check. It should let an independent reader determine what the feature is intended to do, whose data and decisions it can affect, how outputs are produced, where it fails, whether a qualified person can intervene, and how the controls remain effective after release. Start by fixing the feature’s scope and version, map its full data-to-action path, test normal and hostile conditions, verify user disclosures and explanations, exercise the human override, and preserve the records that prove each control worked. Scale the depth to the stakes: a system influencing employment, credit, health, safety or public services needs substantially stronger evidence than a low-impact convenience feature.
What does an AI-feature audit actually establish?
A defensible audit answers six practical questions: What is this feature for? Who can be affected? What information and external tools does it use? What happens when it is uncertain or wrong? Who is authorised to intervene? How will the team know if performance or risk changes after launch?

It combines a system description, data and model inventory, test results, user-facing transparency copy, oversight procedures, monitoring records, incidents, remediation and sign-off. A statement such as “the model is accurate” is not evidence unless it names the test population, metric, threshold and date.
How do I set the audit scope and intended use?
Write a one-page scope record before testing. It prevents a team from quietly expanding a feature’s purpose or treating a vendor’s general claims as proof for your deployment.

Record the feature and decision context
- Feature name, owner, release or service version, model version, prompt or policy version, and deployment date.
- Business purpose and the decisions or actions the output may influence.
- Intended users, affected people, and any groups who may be indirectly affected.
- Jurisdictions, languages, accessibility needs and sector rules that apply.
- Prohibited uses, unacceptable outcomes and decisions that the system must never make autonomously.
- Impact level: describe what a wrong output can cost a person, organisation or the public.
- Who owns the feature and who can approve a material change, pause it or retire it.
Define the boundary in operational terms. “Assists customer support” is too broad; “suggests a draft reply for a trained agent, who sends every message” is testable. State whether the output is advisory, automatically executed, or used to rank or filter people.
Set change-control rules
List changes that require a new or partial audit: model or vendor substitution, prompt or retrieval changes, new data sources, altered retention, a new country or language, a new user group, a changed decision purpose, or a material incident. Keep the old evidence linked to the old version so a reviewer can reconstruct what was actually deployed.

How do I map an AI feature from input to action?
Draw the complete path rather than documenting only the model. The map should show where data enters, how it is transformed, what the model and tools do, and where a person or downstream system can change the result.
- Inputs and sources: identify user-entered data, sensors, uploaded files, databases, third-party feeds and inferred attributes. Mark sensitive or regulated categories, provenance, lawful basis and retention.
- Preprocessing: record cleaning, filtering, chunking, redaction, language detection, feature engineering and defaults for missing values.
- Model and configuration: capture provider, model identifier, version, system and developer prompts, temperature or other settings, fine-tunes, safety filters and fallback models.
- Retrieval and tools: document search indexes, grounding sources, APIs, calculators, code execution, plug-ins and the permissions granted to each tool.
- Output handling: show validation, ranking, formatting, confidence or abstention logic, content filters and any post-processing.
- Downstream action: specify whether output is displayed, stored, sent to a person, used to trigger an automated action or fed into another model.
- Escalation and records: mark every human review point, alert, override, stop control, audit log and retention period.
For each arrow, name the responsible team and the relevant vendor. Record what is logged, who can access it, how long it is retained and what is deliberately not captured because of privacy or security constraints. A data-flow diagram plus a versioned configuration export is more useful than a marketing description.
How transparent is the AI tool at the point of use?
Transparency starts where a person encounters the feature. Tell people when they are interacting directly with AI unless that fact is obvious in context, and do not hide the disclosure in a general privacy policy. Use plain language, an accessible presentation and a route to ask questions or challenge an outcome.
Direct-interaction disclosure
Label a chatbot, voice agent, recommendation panel or generated document at the moment of interaction. The label should remain visible when content is copied or exported where practical. Test the disclosure with screen readers, keyboard navigation, mobile layouts and translated versions.

Generated-content marking
For generative systems, check whether outputs carry effective, reliable, robust and interoperable machine-readable marks when technically feasible. Keep the mark through your own export and transformation steps, and document cases in which a downstream format strips it.
The European Commission’s Guidelines on transparency obligations for providers and deployers of AI systems, published 20 July 2026, state: “These transparency obligations apply from 2 August 2026.” The same EU guidance describes additional disclosure duties for deployers handling deepfakes, biometric or emotion-recognition outputs and certain text on matters of public interest. Confirm which duty applies to your role and use case rather than assuming every generated image or paragraph has the same treatment.
What should users be told about an AI output?
Give a person enough information to understand the feature’s role and decide what to do next. The explanation need not expose trade secrets or every internal parameter; it must be understandable, accessible and useful for the particular decision.
- Purpose and role: what the feature is designed to assist, and what it is not authorised to decide.
- Data categories: the kinds of information considered, including important inferred or retrieved attributes.
- Influential factors: the main inputs or evidence that can change an output, expressed in terms a user can act on.
- Uncertainty: confidence, a score range, an abstention message or another honest signal where available. Explain what the signal does and does not mean.
- Known limitations: languages, populations, conditions, document types or time periods in which performance is weaker.
- Human contact: the team or role that can answer questions about a result.
- Challenge and correction: how to request review, correct inaccurate data, supply missing context and receive a decision.
The UK Government Digital Service Data and AI Ethics Framework, updated 18 December 2025, treats transparency as “foundational to all other ethical principles.” Its transparency guidance includes understandable communication and explainability of an individual decision. Audit the actual screen, message or letter a person receives, not merely an internal explanation document.

How do I test the limitations and harms of an AI system?
Build a test matrix before looking at results. Each test case needs an expected behaviour, observed output, severity, reproducibility status and remediation owner. Preserve inputs and outputs under an appropriate privacy and security regime so another reviewer can repeat the test.
Test categories to include
- Representative cases: normal inputs across the real user population, languages, devices and workflows.
- Edge cases: rare but consequential situations, ambiguous requests, unusual formats and boundary values.
- Missing or corrupted data: blanks, contradictory fields, truncated files, bad encodings and stale records.
- Distribution shift: new populations, seasons, geographies, suppliers, policies or writing styles that differ from development data.
- Adversarial prompts and manipulation: prompt injection, conflicting instructions, jailbreak attempts, poisoned retrieval content and deliberate misclassification.
- Privacy leakage: memorised personal data, cross-user retrieval, sensitive inference and exposure through logs or error messages.
- Fairness and accessibility: subgroup error, refusal, ranking and quality differences; assistive-technology and language variants.
- Unsafe automation: whether a plausible but wrong output can trigger a harmful action without an effective pause.
Choose metrics that match the harm. Depending on the feature, that may include false-positive and false-negative rates, calibration, abstention rate, ranking quality, latency, incident frequency or subgroup comparisons. Define the population, sampling method, confidence limits and acceptance threshold. If no reliable ground truth exists, use structured expert review and disclose that limitation instead of presenting a subjective score as accuracy.

Log failures as actionable findings
For every material failure, record the exact version, input conditions, output, impact, severity, likelihood, containment, owner and due date. Mark whether the issue is fixed, accepted with a documented rationale, mitigated by human review or still open. Re-run the original case after remediation and retain both results.
Is there a human in the loop, and can a person override the model?
A human-in-the-loop claim is meaningful only when a named person has the information, authority, time and interface needed to intervene. Merely placing a human after an automated decision, without showing them the relevant evidence or allowing a real alternative, is not effective oversight.
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Clear out junk files and repair common Windows errorsFree Scan →Scan for outdated or missing drivers - takes under a minuteDriver Scan →Specify the oversight supervisor
Name the responsible or authorised human oversight supervisor for each workflow. ISO/IEC DIS 42105 defines human oversight as “activities by a human intended to control an AI system” and says the document provides guidance on human control and monitoring of AI systems. Use that idea operationally:
- State which outputs require mandatory review and which alerts trigger sampling or immediate escalation.
- Show the evidence, uncertainty signal, input provenance and known limitations available to the reviewer.
- Give the reviewer a clear approve, edit, reject, defer, override and stop option where appropriate.
- Define response times, staffing levels, backup coverage and what happens when no reviewer is available.
- Train reviewers on automation bias, common failure modes, privacy, accessibility and the escalation route.
- Log the original output, reviewer identity or role, action, reason, changed result and time.
Exercise the controls
Run a live or simulated drill: present a known failure, verify that the alert appears, ask the reviewer to stop or override the action, and confirm that the downstream system honours the decision. Test an outage, a queue of simultaneous alerts and an attempt by an unauthorised user to change the result. Measure whether the reviewer can act before harm occurs; do not infer control from a policy document alone.
How do I monitor an AI feature after launch?
Release is the start of the evidence cycle. Establish a baseline from the approved version and set review triggers tied to risk rather than an arbitrary universal interval. No authoritative source establishes a single audit frequency or a universal pass rate for all AI features.
Monitor the signals that can change risk
- Input and output drift, missing-data rates and changes in upstream sources.
- Error, abstention, refusal, override and appeal rates, with meaningful subgroup views.
- Incidents, complaints, unsafe outputs, security events, latency and outages.
- Human-review workload, response time and cases in which reviewers routinely disagree with the model.
- Model, prompt, retrieval-index, vendor, policy and dependency versions.
Define thresholds that trigger investigation, heightened sampling, a rollback or a stop. A material change or incident should reopen the relevant tests even if the calendar review is not due. Keep an on-call route for urgent suspension and verify that rollback restores a known-good configuration, permissions and transparency copy.

What evidence do I need for an AI audit?
Assemble an audit pack that lets a competent reviewer reproduce the decision and verify control operation. Keep access restricted where records contain personal or security-sensitive information, but preserve an auditable index.
| Evidence | What it should show | Owner |
|---|---|---|
| System or feature card | Purpose, users, affected people, prohibited uses, limitations and version | Product and engineering |

The European Commission’s overview for high-risk systems specifically identifies risk management, data quality, logging, documentation, human oversight, accuracy, cybersecurity and robustness. It also expects deployers to monitor operation and assign sufficiently equipped oversight personnel. Treat those items as a practical minimum for high-impact deployments, while applying proportional controls to lower-risk features.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How should proportionality change the audit?
Use the likely impact, scale, reversibility and vulnerability of affected people to set the evidence burden. A low-risk drafting aid may need clear disclosure, basic misuse tests, logging and a user correction path. A feature that ranks applicants, recommends medical action, controls access or allocates public benefits needs representative subgroup evaluation, stronger abstention and override controls, formal incident response and independent review.
The EU trustworthy-AI guidance groups the core requirements as human agency and oversight; technical robustness and safety; privacy and data governance; transparency; fairness; societal and environmental well-being; and accountability. Use these as a coverage check, not as a substitute for feature-specific tests. Record why a control is proportionate and what residual risk remains.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsHow do I compare two AI features or vendors?
Compare evidence and operating controls, not headline model scores. Use the same intended use, test population and acceptance criteria for both candidates.
| Comparison axis | Questions to ask |
|---|---|
| Intended-use fit | Does the documented purpose match your workflow, users, jurisdictions and prohibited uses? |
| Disclosure quality | Are AI interaction notices, generated-content marks and accessibility support present at the point of use? |
| Explanation and limitations | Can users see influential factors, uncertainty, known weak cases and a correction route? |
| Performance evidence | Are representative, edge-case, shift and subgroup results reproducible for your population? |
| Privacy and fairness | What data is retained or inferred, and how are leakage and disparate impacts tested? |
| Human intervention | Can an authorised person inspect, edit, override, stop and document an outcome? |
| Traceability | Are prompts, model versions, tool calls, outputs, decisions and changes logged? |
| Monitoring and incidents | What dashboards, alerts, support response and rollback mechanisms exist? |
| Appeal and correction | Can an affected person challenge an output and obtain a timely human review? |
| Change control and cost | How are vendor updates announced, tested and approved, and what staffing and storage do controls require? |
Ask vendors for versioned documentation and test evidence that matches your deployment. A generic model card cannot establish that your prompts, retrieval sources, user interface and downstream actions are safe.
Best Value
What are the most common audit failures, and how do I fix them?
- Testing only average accuracy: add harm-based metrics, edge cases, subgroup analysis and abstention behaviour.
- Documenting the model but not the workflow: map preprocessing, retrieval, tools, downstream actions and human escalation.
- Calling a reviewer a safeguard without authority: provide evidence, a usable override, staffing, training and a logged decision.
- Using a buried disclosure: move the AI notice and challenge route to the point where the person encounters the feature.
- Ignoring version drift: tie every result to model, prompt, data, vendor and policy versions, then retest material changes.
- Keeping no failure history: maintain an incident and remediation register, including accepted residual risks and reasons.
- Assuming a vendor’s certification covers your use: verify your population, configuration, jurisdiction and downstream action independently.
What does a repeatable audit workflow look like?
Use this sequence for a new feature and repeat the relevant stages after changes or incidents:
- Approve the scope record and risk level.
- Freeze and archive the configuration being assessed.
- Review the data-flow, dependency and permissions map.
- Inspect point-of-use notices, explanations, uncertainty wording and challenge routes.
- Run the test matrix, record failures and assign remediation owners.
- Drill human alerts, override, stop and rollback paths.
- Approve launch only when open risks have an owner, deadline and explicit acceptance.
- Start monitoring, retain decision records and apply the defined change triggers.
For every stage, record who performed it, when, against which version, what evidence was inspected and what decision followed. That chain is the audit’s value: it makes control claims checkable months after release.
What should a final audit decision say?
Use a specific disposition rather than “passed.” State the approved purpose and version, tests completed, material limitations, required human controls, unresolved risks, monitoring triggers, expiry or review conditions, and the person authorised to stop the feature. If evidence is insufficient, restrict the use, add safeguards or do not deploy. A transparent “not approved for this use” is a valid audit outcome.
Frequently Asked Questions
How often should an AI feature be audited?
There is no authoritative one-size-fits-all interval. Set a baseline review for the feature’s risk level, then trigger an earlier review after a material model, prompt, data, vendor, jurisdiction or intended-use change, or after a serious incident or drift signal.
Can a small team audit an AI feature without a dedicated compliance department?
Yes, if responsibilities are explicit and independent checks are possible. Assign owners for product, technical evaluation, privacy and human operations; use a risk-based scope; preserve versioned evidence; and obtain an outside review for high-impact decisions when internal reviewers cannot challenge the deployment.
What counts as a real human override?
The authorised person must be able to see relevant evidence and uncertainty, choose a documented alternative, stop the downstream action and have that choice honoured by the system. A button that merely records disagreement, or a reviewer who cannot delay an automatic decision, is not meaningful control.
How should we audit a vendor’s black-box model?
Treat the vendor as one component in your system, not as the auditor. Obtain version and change information, retention and incident terms, available evaluation evidence, logging interfaces and support commitments; then test the configured workflow on your own representative and edge cases and document any evidence the vendor cannot provide.
