To audit an AI agent and undo an unwanted change, connect three records: the agent’s trace of model and tool activity, the external system’s audit log of what changed, and a change ledger such as commits, snapshots, backups, or reversible transactions. If you spot a harmful action, pause the agent, find the affected change using its request or correlation ID, restore the prior state with the system’s native recovery method, verify the result, and document the rollback. A trace alone can show what the agent attempted; it may not prove what an external system ultimately changed.
What should an AI agent audit record show?
A useful audit trail must let an operator follow an action from its trigger to its result. Record the agent’s activity and the external system’s outcome together, rather than treating a chat transcript or a list of tool calls as a complete account.

OpenAI’s Tracing documentation describes a trace as the steps within one turn, including model responses, tool calls, and work delegated to other agents. Its dashboard shows the recorded inputs, outputs, duration, and status for each step. Those details help explain the agent’s path, but they should be connected to the identity, authority, approval, and resource involved in the action.
- Trace details: model steps, tool calls, delegated work, inputs, outputs, duration, and status.
- Identity and authority: a stable request ID, agent identity, tool name and arguments, and the authority granted to the agent.
- Decision record: the applicable approval or policy decision, including which action it allowed, denied, modified, or held for review.
- External result: the target resource and the resulting change identifier, configuration, or data change recorded by the system the agent acted on.
Anthropic’s guidance recommends logging tool invocations, data access, and external communications with timestamps and request context, and propagating unique request IDs through resulting actions. It also distinguishes a log of an individual action from traceability: traceability connects an action to its triggering event and can include intermediate tool outputs and provenance. In practice, preserve both the individual records and the links between them.

These layers answer different questions. A trace describes what the agent and its tools did during a turn, including intermediate steps. The target system’s audit log records the actor, authority, request context, resource, and resulting configuration or data changes. A change ledger supplies a recoverable version or transaction history. A complete investigation needs to reconcile all three: a recorded tool call may have failed, partially succeeded, or produced a change different from the agent’s intended result.
Make sure the records identify both the attempted operation and its outcome. Keep tool arguments and outputs where appropriate, along with the target resource, approval or policy decision, and resulting change identifier. Those details make it possible to distinguish an authorized action from an adjacent one and to determine whether the system actually accepted the request. When an action is delegated, preserve the connection between the original request and each downstream agent or tool rather than treating a parent trace as the whole story.

Start with the external system that owns the resource, then use the request or correlation ID to connect its audit entry to the agent’s trace. The goal is to identify the actual side effect—not merely the agent’s stated intent or attempted tool call.
- Locate the triggering request. Find the request or correlation ID in the user-facing request record, agent trace, or monitoring system. Check that the same ID was carried into the resulting agent actions.
- Inspect the trace. Follow the recorded model steps, tool calls, and delegated work. Review each step’s inputs, outputs, duration, and status to understand what the agent tried to do and what information it used.
- Check the target system’s audit log. Search for the relevant actor, resource, time, and request context. Confirm whether a change was accepted and identify its resulting configuration, data state, or change identifier.
- Follow delegated actions. Include work performed by other agents or downstream tools. A top-level request can lead to several actions, and reviewing only the first agent’s trace can leave part of the chain unexplained.
- Match the outcome to a change ledger. Look for a commit, snapshot, backup, transaction record, or other versioned state that identifies what can be restored.
A trace answers questions such as “Which tool did the agent call?” and “What did it send?” An external-system audit log answers “Which resource changed, and what did the system record?” A ledger helps answer “What was the previous state, and how can it be recovered?” If those records cannot be joined, treat the result as uncertain until you can independently establish the affected state.
Request IDs are useful only when they are stable across the systems involved and actually propagated into the resulting actions. Search results should be checked against timestamps, actor identity, target resource, and change identifiers. That cross-check helps avoid attributing a neighboring event to the agent merely because it happened around the same time. If an action has no usable link back to its trigger, note the gap and investigate the relevant system’s own records rather than assuming the agent trace is conclusive.
How can I roll back an unwanted agent action?
Stop further activity first, preserve the evidence, and then choose a recovery method that matches the type of side effect. A single undo operation may not reverse a chain of actions or an effect outside the system where the agent was running.

- Stop or pause the agent session, or revoke its authority. Preserve the current trace before taking steps that could remove useful context. If an agent has already started multiple actions, stopping it prevents additional activity but does not automatically reverse changes already made.
- Build the action list. Search by request or correlation ID and record each tool call, delegated step, target resource, approval, and result. Use the external system’s own audit records to confirm which attempted actions took effect.
- Classify each side effect. Determine whether it involved code or configuration, a data mutation, an external message, a credential action, or a transaction. The resource type determines whether a direct restore is available or whether containment and follow-up are necessary.
- Use that system’s recovery mechanism. Revert a commit, restore a snapshot or backup, reverse a transaction, or apply a compensating change. Choose the recovery point that corresponds to the state before the unwanted change, and account for any later valid changes that would be overwritten by a broader restore.
- Verify the restored state. Re-run appropriate validation and compare the result with the pre-change record. Check the affected resource in its owning system; do not assume that a successful rollback command proves every related effect was undone.
- Append a corrective-action record. Record who performed the rollback, why, the restored version or state, any residual impact, and follow-up controls. Link that record to the original request and change identifiers.
Some effects cannot be taken back. An external message may already have been delivered, and Anthropic’s guidance treats irreversible external effects as incident-response work requiring containment and notification. A compensating action can address an outcome, but it does not make the original event disappear; keep both actions in the record.

Recovery is not simply a matter of choosing the broadest restore. A snapshot or backup may include valid changes made after the agent’s action, while a narrower reversal may leave related effects untouched. Identify the affected resource and the state to restore, then validate against the pre-change record. If there are several linked actions, assess each one; rolling back a code change will not by itself undo a transaction or message produced by another tool.
What if the agent changed a repository?
Repository history can make code changes easier to identify and recover. GitHub documents that session logs expose the tools an agent used, that commits can be traced to session logs, and that a session can be stopped while preserving commits already pushed. Use the trace and session log to identify the relevant work, then assess the commits and revert or otherwise restore the repository as appropriate.
Stopping a session is not the same as undoing its work: already-pushed commits may remain. Nor does repository recovery reverse a separate action the agent took through another tool, such as a data mutation or external communication. Audit each target system independently and retain the links between the actions.
This is why repository history is one part of a change ledger, not a universal undo mechanism. Use the session-to-commit relationship to find which work belongs to the agent, then confirm the repository’s current state after recovery. If the agent touched configuration or other resources outside the repository, return to the external audit logs and identify those effects separately. GitHub’s documented session capabilities improve traceability and recovery for repository work; they do not make external side effects reversible.
How do I prevent a dangerous agent action before it happens?
Logging is for understanding and investigating actions after they occur; policy gates can intervene before an action runs. Put stronger controls on actions with greater potential impact, and record the decision alongside the action it governs.
The Agent Control Standard defines hooks that run before actions and can return allow, deny, modify, ask, or defer. Anthropic also describes permission controls and configuration-change hooks. These mechanisms support a practical separation: routine, bounded work can follow an allowed policy, while actions that need a person or more context can be denied, modified, asked about, or deferred.
Require explicit approval for destructive actions, external communications, credential operations, payments, and production changes. The approval record should identify the gated action and its decision; otherwise, it may be difficult to demonstrate later which operation a person authorized. Do not treat a successful tool call as evidence that an appropriate approval occurred.

A gate is most useful when its decision is linked to the specific action that follows. Record the action it evaluated and whether it was allowed, denied, modified, asked about, or deferred. If an operation is changed after review, the record should make clear which version was approved and which operation ultimately ran. This is also why the audit trail should capture the granted authority: an investigator needs to understand not just what an agent did, but what it was permitted to do.
Prevention and recovery solve different problems. An approval control can stop or pause a high-impact action before execution; a commit, backup, transaction reversal, or compensating change can help recover after an effect has occurred. Neither replaces the other. A strong operating approach combines the pre-action decision with records of the action’s eventual result, so a later review can tell whether the gate worked and what recovery remains possible.
How should agent audit logs be stored and retained?
Audit records are useful only if investigators can retrieve them and have reason to trust that they have not been silently altered or deleted. Protect the records separately from the agent’s ordinary working state.
- Protect integrity: use append-only storage or another integrity-protected design, replicate records, and restrict deletion.
- Make events actionable: stream high-value events to a security monitoring system so they can be correlated and reviewed.
- Preserve the chain: keep request IDs and change identifiers with the event records so that trace, audit log, approval, and result remain connectable.
- Plan for export: ensure the records can be copied to systems you control if your retention needs exceed a service’s policy.
Anthropic describes immutable audit trails with integrity verification and real-time SIEM correlation as higher-capability controls. OpenAI’s Audit Logs API documents user actions and configuration changes. OpenAI’s help documentation says API-platform audit logs have no fixed guaranteed retention period and recommends that customers who need long-term security, compliance, or eDiscovery export and store copies in their own systems. Do not assume a provider retains audit records for a period that meets your requirements; establish and test an export process.

Streaming selected high-value events to a security monitoring system supports correlation and review as events happen. These controls are more useful when the records retain the request and change identifiers needed to connect the agent trace, external audit entry, approval, and recovery event.
OpenAI’s Audit Logs API documents user actions and configuration changes, but that record is not a replacement for the agent’s tool trace or the target system’s record of a resource change. Consider what each source covers and where it is stored. The API-platform audit-log retention guidance does not promise a fixed retention period, so organizations with longer-term security, compliance, or eDiscovery needs should export and store copies themselves. Confirm that the export includes the fields and links needed for an investigation, not just a summary of events.

- Coverage: Are tool calls, data access, messages, and delegated actions all captured?
- Traceability: Can a request ID connect the user request to the agent decision, tool call, approval, and resulting change?
- Integrity and retention: Are logs append-only or tamper-evident, replicated, exportable, and retained for the period you need?
- Prevention: Can high-impact actions be allowed, denied, modified, or held for approval before execution?
- Reversibility: Does each resource type have a commit, snapshot, transaction reversal, backup, or compensating-action path?
- Operations: Can the people responsible search, filter, alert on, review, and export the records?
Judge reversibility per resource, not per agent. An agent may edit version-controlled code and also perform actions in systems with different audit and recovery capabilities. The weakest link may be an external action whose result is logged but cannot be directly undone.
The Tool Desk
Outbyte Driver Updater FREEScan for outdated or missing drivers - takes under a minuteDriver Scan →Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →These comparison axes help separate a visible interface from an operationally useful audit setup. Coverage asks whether all meaningful actions are recorded, including data access and external communication rather than only tool calls shown in one trace. Traceability asks whether an investigator can move from a user request through the decision and tool call to the external result. Integrity and retention ask whether those records are protected, available, exportable, and kept long enough for the organization’s needs.
Prevention and reversibility should also be evaluated separately. A tool can have a reliable approval gate but no direct rollback, or a reversible change ledger but weak controls before execution. Operations determines whether people can actually use the records: search and filtering help locate events, alerting can surface high-value actions, and review and export workflows support investigation and retention. There is no single numerical benchmark in the cited product documentation for declaring a setup sufficient; judge it against the agent’s granted authority and the evidence needed to account for each action.
Where are AI agent tool calls logged?
They may appear in the agent’s trace, but the target system’s audit log is needed to establish what the external system recorded as a result. OpenAI Tracing documentation describes model responses, tool calls, and delegated work in a trace, with inputs, outputs, duration, and status shown for each step. For a complete account, connect that trace to the external system’s actor, resource, request context, and change record.
Do not assume that one dashboard contains every relevant event. Use the trace to understand the agent’s steps, the external audit log to confirm the resulting action, and a change ledger to find the recovery point. Request IDs and change identifiers help join those records.
Can every AI agent action be undone?
No. Commits, snapshots, backups, reversible transactions, and compensating changes can provide recovery paths for some effects. An external message may already have been delivered, and other irreversible effects require containment and notification rather than a simple rollback. Stop further activity, confirm what happened, and keep a record of any corrective action and residual impact.
Even where a native recovery mechanism exists, it may restore only one resource or version. Check for linked changes in other systems and verify each restored state. Recovery should be confirmed against the owning system’s record and the pre-change state, not inferred from a successful command alone.

Frequently Asked Questions
What is a correlation ID in an AI agent audit trail?
It is a stable identifier carried across records for a request and the actions it triggers. Its value is practical: investigators can use it to connect events that would otherwise be split across the agent trace, approval record, and external system.
Can an audit log prove that a person authorized an agent action?
A log can document an approval decision and link it to the action it gated, which is stronger evidence than a successful tool call alone. The record is only persuasive if it identifies the relevant action and remains connected to the resulting change.
Free tools Windows power users keep installed
One-click scans. No signup required.
Does stopping an AI agent undo its changes?
No. Stopping limits further activity, but completed changes may remain; recovery requires a separate action in the system that owns the changed resource.
Should I rely on a provider’s built-in audit-log retention?
Only if its documented retention meets your organization’s needs. OpenAI’s API-platform audit-log help documentation says there is no fixed guaranteed retention period, so customers with longer-term needs are advised to export and store their own copies.
How do I see what an AI agent changed?
Find the request or correlation ID, inspect the agent trace and delegated steps, then check the target system’s audit log for the actor, resource, and resulting change. Match that result to a commit, snapshot, backup, transaction, or other change ledger to identify the prior state and recovery options.
Where are agent tool calls logged?
They can appear in the agent trace, which records model responses, tool calls, and delegated work. Check the external system’s audit log as well to confirm whether a call changed a resource and what result the system recorded.
How do I stop an agent before it makes a dangerous change?
Pause or stop the session or revoke its authority when you detect a problem. For prevention, use pre-action policy hooks or permission controls to allow, deny, modify, ask about, or defer actions, and require explicit approval for high-impact operations such as destructive changes, external communications, credentials, payments, or production work.
How can I prove who authorized an agent action?
Keep an approval or policy-decision record tied to the specific gated action, then connect it to the agent trace and resulting change using request and change identifiers. A successful tool call alone does not show that a person approved it.
