Assess an AI agent as a privileged, changeable system—not as a chatbot. Inventory its model, prompts, tools, data, identities, memory, integrations and execution environment; classify what it can do by impact and reversibility; threat-model realistic abuse paths; test the complete system under adversarial and changing conditions; then enforce least privilege, isolation, approval gates, monitoring and tested recovery. Use NIST AI RMF to organize governance, OWASP guidance for agent-specific failure modes and MITRE ATLAS to map adversary techniques. Reassess after every material change to the model, provider, prompts, tools, retrieval, memory or policies.

Why do AI agents create a different security problem?

An ordinary language-model interaction usually ends with text. An agent can interpret instructions, retrieve information, call APIs, run code, modify records, send messages and delegate work to other agents. That combination of autonomy and tool access expands the attack surface and can create privilege-escalation, accountability and blast-radius problems.

The model may produce an unsafe recommendation, but the agent architecture determines whether that recommendation becomes an external email, a changed database row or a production deployment. A secure assessment therefore covers the model and every surrounding component: orchestration code, tool wrappers, credentials, retrieval systems, memory stores, approval workflows, logs and connected services.

Do not treat a successful demo as evidence of safety. An agent can behave correctly in a scripted conversation while failing when a hostile document, poisoned web page, malformed tool response, stale memory item or second agent changes its context.

What should you inventory before assessing an agent?

Create one record for every deployed or piloted agent. The record should identify who is accountable, what the agent is allowed to do and which technical paths connect it to business systems.

  • Purpose and owner: business objective, named accountable owner, risk owner, support contact and approval authority.
  • Model and provider: model name and version, hosting location, fallback models, update policy, data-use terms and safety settings.
  • Instructions: system prompts, developer prompts, policy files, tool descriptions, routing rules, guardrails and configuration held outside source control.
  • Tools and APIs: every operation, endpoint, resource scope, write capability, network destination, plugin or dependency, including tools called indirectly by another service.
  • Identity and secrets: service accounts, tokens, certificates, delegated user credentials, secret stores, rotation schedules and the authority each credential confers.
  • Data and retrieval: source systems, indexes, document permissions, retention, classification, tenant boundaries, citations and data that can enter prompts or tool responses.
  • Memory: short-term context, persistent memory, conversation history, shared state, retention and deletion controls.
  • Execution environment: containers, virtual machines, browsers, code interpreters, file systems, network egress, operating-system privileges and isolation boundaries.
  • Human controls: approvers, separation of duties, escalation routes, timeout behavior, override rules and what happens when no person responds.
  • Observability and response: decision and tool-call logs, alerting, cost telemetry, kill switches, backups, rollback paths and incident ownership.

A centralized baseline should cover ownership, identity, lifecycle, observability, data governance, security and development standards. Record the inventory in a system that can be updated when a prompt, tool, retrieval source or provider changes; a static spreadsheet becomes unreliable as soon as the agent evolves.

How should you classify an agent’s actions?

List each action separately and score it for business impact, likelihood of misuse and reversibility. An agent that can read payroll data but cannot change it has a different risk profile from one that can issue payments, even if both use the same model.

Dimension Questions to answer Examples of higher risk
Impact What is the worst credible consequence if this action is wrong or abused? Financial transfer, production change, legal communication, safety decision or disclosure of regulated data
Likelihood How easily can a user, document, attacker or software defect trigger the action? Action reachable from untrusted email, public web content or a broadly shared chat
Reversibility Can the result be undone completely, quickly and without residual harm? Deletion, external publication, credential rotation or an irreversible transaction
Scope How many records, users, systems or tenants can one call affect? Wildcard database access, organization-wide messaging or unrestricted network egress
Dependency How many external providers, tools or other agents must remain trustworthy? Long chains of plugins, vendors or agents passing instructions without authentication

Start pilots with clearly defined, low-risk, non-sensitive tasks. Limit autonomy around sensitive data and critical systems, and require a person to approve actions whose impact is high, externally visible or difficult to reverse.

How do you threat-model an AI agent?

Use NIST AI RMF’s Govern, Map, Measure and Manage functions as the program structure, and add the Generative AI Profile for risks specific to generative systems. Use OWASP’s agent guidance to enumerate failure modes and MITRE ATLAS to describe adversary techniques and mitigations.

Prompt and context injection

Model both direct attacks from a user and indirect attacks embedded in documents, email, web pages, images, retrieved passages or tool responses. A malicious instruction can attempt to override the agent’s goal, reveal hidden prompts, change approval logic or persuade the agent to call a privileged tool. Treat all external content as untrusted data, not as policy.

Tool abuse and excessive agency

Ask whether an attacker can make the agent call a tool outside its intended purpose, change parameters, chain harmless operations into a harmful result or exploit a confused-deputy relationship. A calendar-reading agent that can also send mail, for example, may be induced to exfiltrate private event details through a message.

Privilege escalation and identity confusion

Check whether the agent inherits a user’s broad session, uses a shared administrator account or can exchange one token for a more powerful credential. Verify authorization at the tool and resource layer; a model instruction such as “do not access payroll” is not an access control.

Data exposure

Trace sensitive data through prompts, retrieval indexes, context windows, citations, tool parameters, outputs, caches and logs. Include cross-tenant and cross-user contamination, accidental disclosure to external recipients and retention beyond the approved period.

Memory and retrieval poisoning

Test whether a user can plant a persistent instruction, alter a shared memory item or poison a retrieval source so that later sessions follow the attacker’s guidance. Memory must have an owner, access policy, provenance, expiration and a way to remove contaminated entries.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Supply-chain compromise

Include models, model updates, plugins, tool packages, APIs, connectors, vector stores, datasets and build dependencies. A trusted component can change behavior after an update, or a compromised connector can return instructions alongside data.

Multi-agent trust failures

When agents communicate, define which messages are commands, which are data and which require independent authorization. Attackers may exploit an intermediary agent to relay a request that the receiving agent would reject from the original sender, or trigger cascading actions across agents with different privileges.

Denial-of-wallet and resource exhaustion

Bound loops, retries, token use, tool calls, recursion depth, execution time and network transfers. Otherwise a prompt or tool failure can generate runaway model calls, cloud charges or an outage even without data theft.

Tampering and monitor evasion

Protect prompts, policies, routing code, model settings, logs and detectors from unauthorized changes. Test whether an agent can hide tool calls, alter audit records, degrade monitoring or behave differently when it detects an evaluation environment.

MITRE ATLAS is a living adversary knowledge base. At access in 2026 it listed 16 tactics, 208 techniques, 40 mitigations and 73 case studies. Use those entries to turn broad concerns into concrete attack paths and test cases.

What should you test before production?

Test the full agent system, not only the underlying model. Include normal tasks, edge cases and adversarial scenarios while varying permissions, data context, timing, other-agent presence and environmental conditions.

  1. Define expected behavior. For each task, specify permitted data, tools, recipients, transaction limits, approval points, refusal conditions and an observable success criterion.
  2. Build an abuse-case suite. Include direct and indirect prompt override, unauthorized tool use, privilege escalation, memory poisoning, retrieval poisoning, data exfiltration, recursive tool abuse and denial-of-wallet scenarios.
  3. Vary trust boundaries. Run the same test with hostile documents, malformed tool responses, stale permissions, a compromised connector, a second agent and a user who lacks the required role.
  4. Verify enforcement. Confirm that policy is enforced by identity, network, data and tool layers rather than merely requested in a prompt. Capture both allowed and denied calls.
  5. Exercise human gates. Test approval, rejection, timeout, delegation, emergency stop and separation-of-duties paths. An approval screen should show the actual operation, parameters, target and likely impact.
  6. Measure containment. Determine the maximum records, systems, spend and external communication an agent can reach before detection and shutdown.
  7. Retest after changes. Repeat the suite before production and after material changes to prompts, tools, memory, retrieval, policies or model providers.

Continuous capability evaluation should include attempts to bypass guardrails, monitors, communication barriers and human-in-the-loop controls. Keep test inputs, expected outcomes, model and tool versions, permissions and evidence of enforcement so results are comparable over time.

Which controls should an AI agent have before production?

Controls should match the impact classification. The following are baseline expectations for any agent with company data or tools; high-impact agents need stronger isolation and approval.

Identity and least privilege

  • Give every agent a unique, attributable identity; never share a generic administrator credential.
  • Grant only the operations and resources required for the declared task, with separate read and write permissions.
  • Use short-lived credentials, rotation, audience restrictions and explicit authorization at every tool boundary.
  • Separate development, test and production identities and data.

Scoped tools and independent authorization

  • Expose narrow, purpose-built operations instead of raw database, shell or network access.
  • Validate parameters, destination, record count and transaction value before execution.
  • Separate planning from execution. An agent may draft a change, while an independent service or person authorizes the actual write.
  • Require independent approval for irreversible, financial, administrative, safety-related or externally visible actions.

Isolation and input handling

  • Sandbox code, browsers and file operations; restrict network egress and access to host credentials.
  • Authenticate and authorize tool responses as well as requests, and sanitize content before it enters the decision context.
  • Treat retrieved documents, emails and web pages as untrusted; keep instructions and data structurally separate where possible.
  • Isolate memory and context between users, tenants and sessions, with retention and deletion controls.

Bounded execution and cost controls

  • Set limits for rate, tokens, spend, retries, recursion, chain depth, execution time and data volume.
  • Fail closed when a limit, authorization check or dependency is unavailable.
  • Provide a kill switch that does not depend on the agent cooperating.

Logging and monitoring

  • Record structured metadata for identity, model and policy versions, prompt or task identifiers, retrieved sources, tool calls, parameters, approvals, denials, outputs, errors, cost and latency.
  • Redact secrets and unnecessary personal data while preserving enough evidence to reconstruct a decision.
  • Alert on unusual tool sequences, privilege changes, data movement, repeated refusals, high-cost loops and degraded performance.
  • Protect logs from alteration and restrict who can view sensitive traces.

Recovery and learning

  • Maintain tested procedures to detect, contain, eradicate, recover from and learn from agent compromise.
  • Revoke credentials, quarantine memory and retrieval sources, disable tools, preserve evidence and notify affected owners in a defined order.
  • Keep clean backups and rollback paths for data or configuration the agent can change.

How do deployment options compare?

Deployment choice should be based on autonomy, data sensitivity, external connectivity, approval strength and recovery capability—not on whether an agent is labeled a pilot or a production feature.

Deployment pattern Typical capabilities Main risks Minimum posture
Read-only retrieval agent Searches approved internal sources and summarizes results without changing systems Prompt or retrieval injection, data leakage, cross-user contamination and inaccurate citations Per-user authorization on retrieval, tenant-isolated indexes, source provenance, output filtering and protected logs
Internal workflow agent Reads and updates business records or opens internal tickets Confused deputy, excessive data access, incorrect updates, memory poisoning and runaway tool chains Unique service identity, operation-level scopes, parameter validation, transaction limits, approval for material changes and rollback
System-changing or externally communicating agent Deploys code, changes infrastructure, sends external messages, executes transactions or controls critical systems Maximum blast radius, irreversible effects, credential theft, supply-chain compromise and cascading multi-agent actions Strong isolation, separate planning and execution, independent authorization, dual control where appropriate, bounded egress, continuous monitoring and rehearsed emergency shutdown

How should the program be governed continuously?

Assign legal accountability and risk ownership; document prohibited uses, approved data, decision rights and escalation paths. Review third-party providers, privileged architectures and material model or tool changes before they reach users.

The UK Code of Practice for the Cyber Security of AI expects documented requirements and risks, defenses against adversarial attacks and unexpected inputs, audit trails, regular threat-model review, human responsibility and secure infrastructure. These expectations fit an agent program even when the agent is built internally.

The Canadian Centre for Cyber Security states that "Strong governance, explicit accountability, rigorous monitoring and human oversight are not optional safeguards but essential prerequisites." Make those responsibilities explicit in contracts, operating procedures and service-level objectives rather than leaving them to an informal team agreement.

NIST describes AI RMF as a voluntary framework intended to improve incorporation of trustworthiness considerations into the design, development, use and evaluation of AI systems. Its functions provide a repeatable cadence: govern the program, map context and risks, measure performance and controls, then manage residual risk and corrective action.

There is no authoritative cross-industry figure for an AI-agent incident rate, breach rate or average loss. Use your own exposure measures instead: number of privileged tools, maximum affected records, time to revoke access, approval bypass rate, unbounded-call events, detected policy violations and recovery time.

What should an assessment report contain?

A useful report lets an approver understand both the remaining risk and the evidence behind the decision. Include:

  • System diagram showing models, orchestrators, tools, identities, data stores, network paths and human gates.
  • Action inventory with impact, likelihood, reversibility, scope and owner for each operation.
  • Threat model linking attack paths to assets, preconditions, controls, test cases and residual risk.
  • Test results with model and configuration versions, permissions, prompts, inputs, expected behavior, observed behavior and severity.
  • Control exceptions, expiration dates, compensating measures and the person authorized to accept each risk.
  • Monitoring, incident-response, rollback and shutdown procedures, with evidence that they were exercised.
  • Change triggers that require reassessment, including provider updates, new tools, altered prompts, new retrieval sources, memory changes, policy changes and expanded autonomy.

Approve production use only when the owner can explain the agent’s maximum credible blast radius, how a human or independent control stops high-impact actions, and how the organization will recover if the agent or one of its dependencies is compromised.

Frequently Asked Questions

Should an AI agent use the same account as the employee who launched it?

No. Use a distinct service identity and preserve the initiating user as a separate attribution field. This allows permissions, rate limits, revocation and forensic review to distinguish the person’s request from the agent’s subsequent actions.

What evidence will an auditor expect for an AI-agent risk assessment?

Provide the current architecture and action inventory, approved threat model, repeatable adversarial test cases, versioned configurations, access reviews, approval records, monitoring alerts and recovery-exercise results. Evidence should show enforcement at technical boundaries, not only policy text or screenshots of a chat.

How can teams protect confidential data while still using agent logs for debugging?

Define a purpose-based retention schedule, redact secrets and unnecessary personal data before storage, and separate restricted raw traces from broader operational metrics. Limit trace access, log every access to sensitive traces and test that redaction does not remove the fields needed to investigate tool misuse.

What should happen when a model provider announces a new model version?

Treat the change as a reassessment trigger. Freeze the previous configuration for rollback, rerun high-impact and abuse-case tests, compare tool-call and refusal behavior, review data-handling terms, and obtain the same approval required for any other material change before enabling the new version.
