Assessing an AI system before deployment is a lifecycle security decision, not a one-time vulnerability scan. A defensible approval process documents the system’s purpose, users, data, models, prompts, tools, providers and owners; models AI-specific and conventional threats; runs reproducible adversarial and abuse-case tests; verifies technical and operational controls; and records residual risk with an accountable go-live decision. After release, monitoring and repeat assessments are required whenever the model, data, prompts, tools, dependencies, configuration or hosting changes.

The practical outcome is an evidence-backed choice to approve, defer or reject production use. The threshold should reflect impact, exposure, autonomy, data sensitivity, affected people, provider dependence, detectability and reversibility—not a universal “secure” score.

What does an AI security assessment cover?

An assessment covers the complete AI-enabled system and its lifecycle: design, development, deployment and operation. The model is only one component. Applications, retrieval stores, prompts, orchestration code, plugins, identity systems, cloud services, logs, datasets and human workflows can all create attack paths.

NIST’s AI Risk Management Framework (AI RMF) 1.0, published January 26, 2023, is voluntary and use-case agnostic. It is intended for organizations that design, develop, deploy or use AI systems. It does not create a universal certification or pass/fail number, so your organization must set thresholds that match the system’s consequences and legal obligations.

Security questions to answer

  • What decision or task is the system performing, and what uses are prohibited?
  • Who can use it, who can be affected by it, and who can approve changes?
  • What data enters, leaves or is retained by each component?
  • What actions can the system take directly or through tools?
  • Which model, dataset, package, provider and hosting versions are in use?
  • How quickly can an unsafe change be detected, contained and reversed?

How should you define context and accountability first?

Start with a short, controlled system record before testing. Name the business owner who accepts the use-case risk, the security owner who coordinates controls, the data custodian for sensitive information and the escalation path for incidents. Record the intended users, affected groups, operating geography, autonomy level, prohibited uses and risk tolerance.

Include applicable legal, regulatory and sector requirements. A customer-support assistant handling public product information has a different impact profile from an agent that can alter financial records, administer infrastructure or make decisions about people. The second case needs stronger evidence, narrower permissions and more frequent review.

Minimum context record

  • Purpose, success criteria and out-of-scope or prohibited uses.
  • User groups, affected people, accessibility needs and human-approval points.
  • Business, security and data owners, plus an incident escalation route.
  • Impact rating for confidentiality, integrity, availability, safety, financial loss and rights.
  • Applicable law, contractual duties, sector rules and retention requirements.
  • Risk acceptance authority and conditions under which deployment must stop.

What must be in the AI attack-surface inventory?

Create an inventory that another engineer can use to reproduce the deployment. The UK guidance for the responsible development and deployment of AI calls for documenting data, models and prompts and securing the supply chain. Capture exact names and versions, and record hashes or other provenance evidence where available.

Inventory these components

  • Base model, fine-tuned model, tokenizer, safety model and inference runtime.
  • Training, fine-tuning, evaluation and retrieval data, including collection source, licensing, sensitivity and transformations.
  • System prompts, templates, policy messages, guardrail rules and retrieval instructions.
  • Embeddings, vector stores, document parsers, search indexes and synchronization jobs.
  • Tools, plugins, function definitions, web connectors, code execution and outbound destinations.
  • Application APIs, orchestration code, containers, operating systems, libraries and build pipelines.
  • Hosting regions, model providers, subcontractors, identity systems, secrets stores, logs, backups and monitoring services.
  • Human review queues, approval interfaces, support processes and emergency shutdown mechanisms.

Draw data-flow and trust-boundary diagrams. Mark where untrusted text can influence instructions, where credentials cross a boundary, and where an output can trigger an external action. Provider and package provenance matters because a compromised dependency or malicious model update can bypass controls that appear sound inside your application.

How do you threat-model an AI system?

Combine conventional software threat modeling with AI-specific abuse cases. The UK National Cyber Security Centre guidance, published November 27, 2023, places threat modeling, supply-chain security and lifecycle documentation across secure design and development. Model threats at each boundary, then connect each threat to a preventive control, a detection signal and a recovery action.

AI-specific threats to include

  • Prompt injection and adversarial inputs: malicious instructions hidden in user text, retrieved documents, images or tool responses can redirect the model.
  • Data poisoning: altered training, fine-tuning or retrieval content can bias behavior, implant triggers or weaken safety rules.
  • Model compromise: tampered weights, unsafe adapters or a malicious update can introduce hidden behavior.
  • Model extraction and inversion: repeated queries may reveal proprietary behavior, training examples or sensitive attributes.
  • Sensitive-information disclosure: prompts, logs, outputs, retrieval stores or provider processing may expose secrets or personal data.
  • Unsafe tool behavior and excessive agency: an agent may send messages, change records, execute code or spend money without appropriate approval.
  • Dependency and provider compromise: a breached package, API, account or hosting layer can alter inputs, outputs or availability.
  • Denial of service and rate abuse: crafted requests can consume inference capacity or cause runaway tool calls.
  • Configuration and update risk: a model, prompt, data, tool, provider or hosting change can invalidate earlier assumptions.

Also model ordinary threats such as broken access control, injection into databases, insecure deserialization, exposed secrets, vulnerable containers, weak tenant isolation, unencrypted traffic and inadequate backup. AI does not replace secure software engineering; it adds new ways for untrusted content to affect decisions and actions.

Turn threats into testable scenarios

For every scenario, state the attacker capability, entry point, target asset, expected impact, existing control, detection method, response owner and residual risk. For example, “a user places an instruction in a retrieved document” should lead to tests of instruction separation, source labeling, tool authorization and alerting—not just a general question about whether the model is safe.

What tests should an AI system pass before going live?

Define objectives and pass criteria before execution. Use representative benign inputs, edge cases, adversarial prompts and realistic abuse cases. Testing should be reproducible and retain enough detail for an independent or appropriately skilled tester to repeat it.

Build a repeatable test plan

  1. Set scope and criteria. Identify model and application versions, environments, enabled tools, data classes, test accounts, rate limits and stop conditions. Define severity levels and what constitutes a release blocker.
  2. Create controlled test sets. Include normal tasks, malformed requests, ambiguous instructions, multilingual or multimodal inputs where relevant, sensitive-data probes, jailbreak attempts and tool-abuse cases.
  3. Run security and red-team tests. Test prompt injection, data leakage, unsafe outputs, privilege escalation, poisoning, malicious updates, extraction, rate abuse and failure recovery.
  4. Exercise the full path. Follow an input through retrieval, model inference, output filtering, tool authorization, downstream systems, logging and human approval. A safe model response is not enough if the application executes an unsafe function call.
  5. Record exact evidence. Store prompts, datasets, seeds where applicable, model and dependency versions, configuration, expected and observed results, timestamps, tester, severity, remediation and retest status.
  6. Retest after fixes. Verify that a control blocks the original case without creating unacceptable regressions in legitimate use.

Pre-deployment test matrix

Area Example test Evidence to retain
Confidentiality Ask for secrets, another tenant’s records and memorized training or retrieval content. Prompts, account roles, expected denial, observed output and log review.
Instruction integrity Place conflicting commands in user text, retrieved files, tool responses and images. Input corpus, prompt version, model version and whether policy held.
Authorization Attempt restricted functions with low-privilege and expired identities. Token scope, authorization decision, downstream audit record and alert.
Tool safety Induce unexpected messages, code execution, data changes or outbound requests. Tool call trace, approval step, destination and rollback result.
Availability Send oversized, recursive or high-rate requests and repeated tool loops. Rate-limit behavior, resource usage, alert timing and service recovery.
Supply chain Verify model, package, dataset and provider provenance; simulate a changed artifact. Hashes or provenance records, approval trail and update rejection evidence.
Recovery Revoke a credential, disable a tool, roll back a model and restore a backup. Runbook, elapsed time, responsible owner and restored service state.

The UK code expects released models, applications and systems to undergo security assessment. Joint deployment guidance emphasizes controls that protect, detect and respond to malicious activity. Treat a red-team exercise as evidence about particular scenarios, not proof that every future attack is impossible.

Which engineering and operational controls should be verified?

Use the threat model and test failures to inspect controls in the running architecture. NIST’s generative-AI profile for Secure Software Development Framework (SSDF) 1.1, published July 26, 2024, adds generative-AI-specific practices across the software lifecycle.

Access, isolation and data protection

  • Apply least privilege to users, services, tools, datasets and model-management functions.
  • Separate development, evaluation and production accounts, networks and data.
  • Place code execution, browsers and high-impact tools in isolated environments with explicit egress rules.
  • Validate and constrain inputs, retrieved content, structured outputs and tool arguments at every boundary.
  • Keep secrets in a managed store, rotate them, prevent them from entering prompts or logs, and use narrowly scoped credentials.
  • Encrypt data in transit and at rest, and define retention and deletion for prompts, outputs, traces and backups.

Supply chain and change control

  • Pin and review dependencies; scan packages, containers and infrastructure code.
  • Record model, dataset, prompt, policy and provider versions, with hashes or attestations when available.
  • Require review and staged evaluation for model, prompt, data, tool, configuration and hosting changes.
  • Maintain a supported patching process and a way to reject or quarantine an untrusted artifact.

Detection, response and continuity

  • Log authentication, prompts and outputs according to privacy requirements, tool calls, policy blocks, model changes and administrative actions.
  • Alert on prompt-injection patterns, unusual extraction attempts, privilege failures, data-loss indicators, rate spikes and unexpected destinations.
  • Maintain backups, rollback packages, feature flags and a tested emergency disable path.
  • Document incident triage, evidence preservation, user notification, provider escalation and post-incident review.

Human oversight is a control only when it is designed, staffed and maintained. A reviewer who cannot see the source data, model uncertainty, proposed action or relevant policy cannot reliably catch an unsafe decision.

How do you make and document the go-live decision?

Prepare a decision record that an accountable owner can approve, defer or reject. It should state what was tested, what was not tested, which findings remain open and why the residual risk is acceptable for this use.

Go-live decision record

  • System purpose, scope, users, affected people, environment and release version.
  • Threat-model coverage and test dates, methods, limitations and tester qualifications.
  • Findings by severity, remediation status, retest evidence and compensating controls.
  • Residual risks, expected impact, likelihood assumptions and explicit risk acceptance.
  • Monitoring metrics, alert thresholds, named on-call and incident-response owners.
  • Rollback, backup and emergency-disable procedures with their last exercise date.
  • Exceptions with an owner, approval date, expiration date and required follow-up.
  • Reassessment triggers, including material model, prompt, data, tool, dependency, provider, configuration or hosting changes.

Do not treat an exception as permanent acceptance. Set an expiry date and a concrete condition for closure. If evidence is incomplete for a high-impact or highly autonomous system, defer deployment or reduce scope, permissions and exposure until the gap is addressed.

How should security be monitored after deployment?

Deployment starts the operational phase of the assessment. Track attack signals, drift, abuse, data leakage, unsafe outputs, dependency and model changes, incidents and control effectiveness. Review trends rather than relying on a single alert count.

Reassess after material change

Repeat relevant evaluations after changing the model or adapter, system prompt, retrieval corpus, tools, provider, dependency, safety policy, configuration or hosting. A prompt edit that changes tool-selection behavior can be as security-significant as a code release. Recheck assumptions when user populations, data sensitivity or business impact changes.

How do you compare two AI deployment designs?

Compare designs by risk drivers, not by model size or marketing claims. A read-only internal assistant generally presents less immediate impact than an externally exposed agent with sensitive data and write access, but the lower-risk design still requires access control and leakage testing.

Risk axis Lower-risk pattern Higher-risk pattern requiring stronger evidence
Impact Drafting or search with human verification. Decisions affecting safety, rights, finances or critical operations.
Exposure Restricted network and authenticated users. Public, anonymous or broadly shared endpoint.
Autonomy Read-only responses. Agent that changes records, sends messages or executes code.
Data Public or low-sensitivity content. Secrets, personal, regulated or proprietary information.
Supply chain Few pinned, well-understood components. Multiple providers, plugins, packages and opaque updates.
Detectability Complete logs, alerts and human review. Limited telemetry or delayed discovery.
Reversibility Easy rollback and bounded effects. Irreversible external actions or difficult recovery.

When a design scores high on several axes, narrow permissions, add approval gates, isolate tools, increase test depth and require more frequent reassessment. This is an application of the risk-based principles in NIST AI RMF, UK guidance and NCSC secure-development guidance.

What evidence should be in the security assessment pack?

Keep one versioned pack linked to the deployed release. It should be understandable to engineers, security reviewers, auditors and the accountable owner.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • Purpose statement, intended-use and prohibited-use register, owners and escalation path.
  • Architecture, data-flow, trust-boundary, identity and tool-access diagrams.
  • Model, dataset, prompt, dependency, provider and version inventory, with hashes or provenance where available.
  • Threat model mapped to AI-specific and conventional attack scenarios.
  • Reproducible security and red-team plan, results, severity ratings, remediation and retest status.
  • Access-control, isolation, secrets, encryption, logging, monitoring, backup, rollback and incident-response evidence.
  • Residual-risk register, decision owner, approval date, exceptions, expiration dates and reassessment triggers.

What commonly causes an assessment to fail?

Testing only the model

A model can refuse a malicious prompt while the surrounding application still leaks retrieved documents or executes an unsafe function. Test the complete data and action path.

Using generic prompts instead of realistic abuse cases

Short benchmark questions rarely expose multi-step attacks. Include hostile documents, malformed tool responses, low-privilege accounts, repeated requests and recovery exercises.

Skipping provenance and change control

Untracked model, package or prompt changes invalidate prior results. Pin versions, review updates and trigger reassessment on material change.

Recording findings without owners or expiry

An unresolved issue becomes operational risk when nobody owns it. Assign a decision owner, compensating control, deadline and expiration for every exception.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Assuming human review fixes everything

Review is ineffective when volume is too high, evidence is hidden or the reviewer lacks authority to stop an action. Design the interface, staffing, training and escalation path as security controls.

Frequently Asked Questions

Is there a certification that proves an AI system is secure?

No universal AI-security certification or pass/fail score is established by the cited frameworks. NIST AI RMF is voluntary, so organizations must document their own risk criteria and applicable legal or contractual requirements.

Who should perform an AI security assessment?

The system team can perform routine checks, but independent or appropriately skilled testers should review higher-impact, externally exposed or highly autonomous systems where feasible. Independence helps challenge assumptions about prompts, permissions, supply-chain trust and recovery.

How often should an AI system be retested?

Retest after every material model, prompt, data, tool, dependency, provider, configuration or hosting change, and after incidents or newly observed attack patterns. The monitoring plan should define recurring reviews for drift, abuse and control effectiveness rather than relying only on release dates.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

What should happen if a critical vulnerability is found after launch?

Use the incident runbook to contain the affected capability, revoke credentials or disable tools, preserve evidence, notify the responsible owners and roll back or restrict the release. Resume normal operation only after remediation is verified and the residual-risk decision is updated.
