To add safe approval and rollback to an AI-agent workflow, treat every side-effecting tool call as a durable transaction: classify its risk, pause before execution, save a server-side checkpoint, obtain an authenticated decision, atomically consume that decision, execute with an idempotency key, and record either a reversible before-image or a compensating action. Read-only calls can usually run automatically, while emails, record edits, SQL writes, cancellations, shell commands, payments, and sensitive MCP tools should pass through human review. The same design lets an agent wait for hours or days and resume from its original state without replaying earlier actions.
This guide shows the implementation pattern, concrete flows for the OpenAI Agents SDK and LangGraph, the limits of Amazon Bedrock session APIs, durable orchestration choices, and a rollback plan for failures, retries, and rejected requests.

What is an approval gate in an AI-agent workflow?
An approval gate is a control point between an agent proposing an action and the system executing it. The agent may generate a tool call, but a policy layer decides whether that call can run automatically, must be reviewed by a person, or must be blocked. A gate is therefore different from an output filter: it governs an intended side effect, not just the text the model produced.
Use three classes of controls together:
- Input guardrails: validate user requests, identity, scope, and allowed destinations before the agent plans work.
- Output guardrails: check generated content for policy violations, secrets, unsafe instructions, or malformed responses.
- Tool guardrails and human review: validate arguments automatically, then require a reviewer for actions whose consequences cannot be safely inferred by deterministic rules.
OpenAI’s 2026 guidance summarizes the division as: “Use guardrails for automatic checks and human review for approval decisions.” Human review is specifically recommended for side effects such as cancellations, edits, shell commands, and sensitive MCP actions.

Which actions should be gated?
| Action type | Typical default | Reason |
|---|---|---|
| Read-only search, retrieval, or status lookup | Automatic, with input and output checks | No external state changes, although data-access permissions still apply. |
| Drafting an email, ticket, or document without sending | Automatic or sampled review | The draft is reversible until publication. |
| Sending an external message or publishing content | Human approval | Creates an external commitment and may disclose information. |
| Editing a customer, financial, or operational record | Human approval unless tightly bounded | Requires an auditable before-state and clear authorization. |
| SQL insert, update, delete, migration, or stored procedure | Human approval | Can affect many rows and may be difficult to reverse. |
| Refund, payment, cancellation, or order change | Human approval; stronger threshold for high value | Financial or contractual consequences. |
| Shell command, infrastructure change, or credentialed operation | Human approval with sandboxing | Potentially destructive and capable of escaping the intended scope. |
| Sensitive MCP operation | Human approval | The connected service may expose broad or irreversible capabilities. |
Approval granularity should match the blast radius. A low-risk, single-record edit might need one reviewer per call; a batch operation may need one approval for a precisely enumerated set of calls. Never turn a broad, changing batch into a vague “approve all future actions” decision.
How does the pause-and-resume pattern work?
The safe pattern is to stop before the tool executes, persist everything needed to make a decision, and resume the same root run after a decision is recorded. The worker that handled the original request may disappear while a reviewer deliberates; the checkpoint, not process memory, is the source of truth.
- Classify the proposed tool call. Resolve the tool identity, tenant, actor, policy, and risk class. Reject unknown tools or arguments before presenting them to a reviewer.
- Validate arguments automatically. Apply schema validation, authorization checks, destination allowlists, data-loss rules, rate limits, and business constraints. Do not ask a person to approve an invalid call.
- Create a server-owned checkpoint. Persist the run ID, root workflow version, agent identity, tool name, canonical validated arguments, relevant prior state, policy result, and a unique idempotency key. Include a hash of the canonical payload so later tampering is detectable.
- Create a pending decision. Give the decision a unique ID, expiration policy, required reviewer role, and status such as
pending. Store it transactionally with the checkpoint. - Show a least-privilege review view. Display only the information needed to decide: the proposed effect, scope, affected records, risks, and links to authorized context. Redact secrets and unrelated customer data.
- Authenticate and authorize the reviewer. Check the reviewer’s identity, role, tenant, separation-of-duties rules, and any two-person requirement at decision time, not only when the page loads.
- Record approve, edit, reject, or respond. Save the reviewer identity, timestamp, rationale, decision ID, and—if edits are allowed—the exact changed arguments after revalidation.
- Atomically consume the decision. Use a compare-and-set update or equivalent transaction that changes the decision from
pendingtoconsumedexactly once. Concurrent browser submissions and retries must receive the already-recorded outcome rather than execute again. - Resume the original root run. Pass the approved or rejected interruption back to the workflow engine with the original run state. Do not create a new run from a browser payload.
- Execute idempotently and log the result. The tool receives the stored idempotency key. Record success, failure, timeout, and any compensation attempt, then close the checkpoint with a final state.
What belongs in a durable checkpoint?
- Root run ID and parent or child run relationships.
- Workflow-definition version and tool-schema version.
- Agent identity, tenant, initiating user, and authorization context.
- Tool name, connector or MCP server identity, and canonical validated arguments.
- State needed to resume, including the last safe step and pending interruption ID.
- Policy decisions, required approval level, expiration, and idempotency key.
- Integrity hash, creation time, reviewer decision, execution result, and compensation result.
Keep snapshots on a server you control. If a client stores a serialized run state for convenience, treat it as an untrusted transport object: authenticate it, authorize it, validate its version, and atomically consume any pending decision before resumption.
How do I implement approval with the OpenAI Agents SDK?
The OpenAI Agents SDK human-in-the-loop flow pauses a run when a tool’s needsApproval evaluation requires review. The runner returns interruptions; your application calls state.approve(interruption) or state.reject(interruption), then resumes the original top-level run from the same RunState. The SDK documentation states: “When a tool call requires approval, the SDK pauses the run, returns interruptions, and lets you resume later from the same RunState.”
Implement the tool policy
Define approval as a function of the tool, arguments, actor, tenant, and current policy version. A read-only lookup can return false for needsApproval; a cancellation, shell command, or sensitive MCP action should return true. Keep this decision deterministic and side-effect free. It should not send an email, mutate a database, or call the tool merely to decide whether approval is needed.
Persist before returning the interruption
When the runner reports an interruption, serialize the relevant RunState on the server and create your own pending-decision record. Save the exact tool arguments that passed validation. The reviewer endpoint must reference the decision ID, not accept replacement arguments in the request body. If an edit is supported, apply the edit server-side, run validation again, update the canonical checkpoint, and retain both original and approved values in the audit record.
Resume safely
On approval, perform an atomic status transition, load the server-side state, apply the SDK approval to the matching interruption, and resume the original root run. On rejection, apply the SDK rejection and let the agent follow a defined denial path, such as explaining the block or requesting a narrower action. Nested handoffs and agent.asTool() approvals surface on the outer run, so the outer run’s checkpoint must preserve the relationship between the reviewer decision and the nested action.

Handle long waits
Serialize RunState for waits that outlive a request or worker, but do not rely on an in-memory runner. Set an expiration and a policy for stale approvals. If the workflow or tool schema changes while a decision is pending, reject or migrate the checkpoint explicitly; never interpret old arguments under a new schema without a compatibility check.
How does LangGraph human-in-the-loop differ?
LangGraph middleware pauses tool execution with an interrupt and supports four decisions: approve, edit, reject, and respond. Use approve for the original call, edit for a constrained argument change, reject for denial, and respond when the human is effectively acting as the tool.
Configure persistence
Human-in-the-loop requires checkpointing. A persistent checkpointer is appropriate for production because an interrupt may outlive the process that created it. The checkpointer should be backed by durable storage, include workflow and schema versions, and expose a transaction or conditional update for one-time decision consumption.

An edit decision is not permission to rewrite arbitrary JSON. Permit only fields that the reviewer is authorized to change, such as a recipient address or a row limit, and re-run the same policy and schema checks against the edited call. Preserve the original proposal, the edited proposal, and the reviewer’s rationale.
Prevent duplicate tool execution
LangGraph’s interrupt protects the pause, but the external side effect still needs idempotency. Pass a stable key derived from the checkpoint and tool-call identity to the connector or transactional outbox. If the graph is retried after a timeout, the connector should return the prior result for that key rather than perform the action again.
What does Amazon Bedrock session management provide?
Amazon Bedrock documents session-management APIs that checkpoint workflow stages for iterative testing and human-in-the-loop work, resume conversations and tasks after interruption, and provide session logs for analysis and debugging. Session access is controlled with IAM, and session data can be encrypted with AWS-managed or customer-managed KMS keys.
The documented APIs are labeled preview. Before production adoption, review API-stability commitments, regional availability, retention behavior, quotas, and migration options. A managed session checkpoint does not remove the need for your own approval policy, idempotency key, reviewer authorization, or compensation logic. Treat the service session as one layer in the workflow state, and keep an application-level audit record that explains exactly what was approved and executed.

Which framework should I choose for approval and rollback?
No framework automatically makes an external side effect reversible. Choose based on interruption semantics, checkpoint durability, identity integration, and how much compensation logic you are prepared to own.
| Capability | OpenAI Agents SDK | LangGraph | Amazon Bedrock session APIs |
|---|---|---|---|
| Approval trigger | Per-tool policy through needsApproval |
Middleware interrupt around tool execution | Session interruption and resume APIs |
| Decision types | Approve or reject interruption; application logic can implement constrained edits | Approve, edit, reject, respond | Session-level resume; exact decision vocabulary depends on the preview API |
| Checkpoint model | Resume the same serialized RunState |
Persistent checkpointer required for durable human review | Managed session checkpoint and logs |
| Nested-agent behavior | Nested handoffs and agent.asTool() approvals surface on the outer run |
Graph and subgraph behavior follows configured interrupt/checkpointer boundaries | Workflow-specific; verify behavior for nested tasks |
| Reviewer authentication | Your application must authenticate and authorize reviewers | Your application must authenticate and authorize reviewers | IAM controls service access; application roles still govern who may approve |
| Replay protection | Application-level atomic decision consumption and idempotent tools required | Application-level atomic decision consumption and idempotent tools required | Use application-level idempotency and conditional state transitions |
| Rollback or compensation | Implement per-tool before-images or compensating calls | Implement per-tool before-images or compensating calls | Implement outside the session API |
| Cloud lock-in | SDK-centric; storage and connectors remain your choice | Graph-centric; storage and connectors remain your choice | AWS service and IAM/KMS integration |
For approvals that can wait beyond a web request and need timers, retries, worker recovery, or multi-step compensation, consider a durable orchestration layer. OpenAI’s running-agents documentation names Dapr, Temporal, and Restate as integrations for durable, long-running agents with human-in-the-loop tasks. Evaluate deployment, operational ownership, data residency, and portability before committing.
How do I make tool calls idempotent?
Idempotency means that repeating the same logical request produces one external effect and a repeatable result. It is essential because a worker can time out after the provider accepted a request, a reviewer can click twice, or a workflow engine can retry after a crash.
Use a stable key
Generate the key when the checkpoint is created, not when a retry occurs. Bind it to the root run, workflow version, tool identity, and logical action. Store it with the canonical arguments. A changed argument set must receive a new key and a new approval.

One free scan finds every outdated or missing driver and matches the right update for your exact hardware.Free scan · exact hardware matchUse a transactional outbox for asynchronous effects
For email, tickets, payments, and database writes, commit the intent and idempotency key in the same database transaction as the business state change. A dispatcher delivers the outbox record and marks the result. If delivery is retried, the downstream provider or your dispatcher consults the key before sending again.
Define duplicate behavior
Decide whether a duplicate returns the original result, reports that the effect is already complete, or enters manual reconciliation. Do not silently execute a second action because the first response was lost.
How should rollback work for different side effects?
Rollback is not one universal button. Select a recovery operation when each tool is designed, and capture the information required immediately before execution.

| Side effect | Rollback strategy | Evidence to retain |
|---|---|---|
| Reversible database or CRM edit | Restore a prior version or apply a precisely scoped inverse update | Before-image, version ID, changed fields, actor, and timestamp |

| Irreversible deletion or disclosure | No reliable rollback; escalate prevention and incident response | Proof of approval, scope, recipients, and containment actions |
Capture the checkpoint immediately before the effect
Read the current version or snapshot, then persist it before invoking the external tool. If the read and write cannot be made consistent, use optimistic concurrency: include the version ID in the write and abort if another actor changed it. Never restore a stale before-image over a newer legitimate change.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How do I handle rejection, expiration, and edited approvals?
Rejection
A rejection should be a first-class workflow result. Return a structured denial reason to the agent, stop the blocked tool, and let the agent ask for a narrower request or provide a safe alternative. Do not let the model reinterpret a rejection as approval.
Expiration
Set an expiry based on the risk and volatility of the action. A shipping cancellation or production command may become unsafe within minutes; a draft review may remain valid longer. On expiry, mark the decision closed and require a fresh proposal with current state and a new idempotency key.
Edited approval
Display a before-and-after diff, re-run authorization and deterministic checks, and require the reviewer to approve the final arguments. If the edit changes the target, amount, recipient, row scope, or command, treat it as a new risk evaluation even when the same reviewer remains on the page.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsTwo-person review
For irreversible or high-impact actions, require two independent approvals. Enforce independence in the authorization service so the initiator cannot satisfy both roles, and consume both decisions atomically before execution.
How do I secure the reviewer interface and audit trail?
- Least privilege: expose only the fields and records needed for the decision, with secrets masked.
- Strong identity: use your organization’s authenticated identity provider, short-lived sessions, and step-up authentication for sensitive actions.
- Tenant isolation: bind every decision to a tenant and verify that the reviewer belongs to that tenant.
- Server-owned arguments: load canonical arguments by decision ID; never trust hidden form fields or client-supplied run state.
- Integrity: hash the canonical proposal and record the hash shown to the reviewer.
- Audit completeness: log proposal, policy evaluation, checkpoint, reviewer, rationale, decision, execution result, compensation, and final state.
- Observability: emit metrics and traces for time waiting, approval latency, rejection, expiry, retry, duplicate suppression, and compensation failure.
Audit logs should answer who approved what, under which policy and workflow versions, with which exact arguments, and what happened afterward. Protect logs from alteration and define retention according to your regulatory and contractual obligations.
Best Value
What failure modes should I test before production?
- Kill the worker after the checkpoint is written but before the reviewer sees the request.
- Submit two approval requests simultaneously and verify only one consumes the decision.
- Retry after a provider timeout and confirm the idempotency key prevents a duplicate effect.
- Change the workflow or tool schema while an approval is pending and verify safe rejection or migration.
- Edit an argument to exceed policy limits and confirm revalidation blocks execution.
- Expire a decision while the reviewer’s browser is open and require a fresh proposal.
- Restart during compensation and verify the compensation workflow resumes without duplication.
- Attempt cross-tenant access to a decision ID and confirm authorization failure without data leakage.
- Corrupt or replay serialized run state and verify integrity and one-time-consumption checks.
- Exercise partial provider success, duplicate webhooks, and out-of-order callbacks.
Run these tests with realistic connector sandboxes and representative data. A green model evaluation does not prove that the approval transaction, connector, database, and compensation path are safe under failure.
What is a practical implementation checklist?
- Inventory every tool and classify its side-effect risk.
- Define automatic guardrails and explicit human-review policies.
- Version workflow definitions, tool schemas, and policy rules.
- Design a durable checkpoint and pending-decision data model.
- Persist exact validated arguments and a stable idempotency key.
- Build authenticated, least-privilege reviewer screens with approve, edit, reject, and—where appropriate—respond actions.
- Consume decisions atomically and resume the original root run.
- Implement idempotency at the connector or transactional-outbox layer.
- Capture before-images, version IDs, or immutable snapshots before effects.
- Define compensating actions and explicitly mark irreversible operations.
- Log the complete lifecycle and alert on stuck, expired, duplicate, or uncompensated actions.
- Test crashes, retries, concurrent approvals, schema changes, and partial failures.
A workflow is ready only when the approval path and the failure path are both deterministic. If an action cannot be reversed, say so in the policy and require stronger prevention rather than promising a rollback that does not exist.
Frequently Asked Questions
Can an AI agent keep working while approval is pending?
It can continue only on independent, pre-authorized branches. Keep the gated branch paused and prevent later steps from assuming that its side effect succeeded; release dependent work only after the decision and execution result are committed.
How should approval notifications avoid leaking sensitive data?
Send a minimal notification containing a decision ID, risk label, and expiry, then require the reviewer to authenticate in the protected console. Do not place full SQL, customer records, credentials, or message contents in email or push text.
What happens when a reviewer approves an action but the target changes first?
Use optimistic concurrency or a fresh state check immediately before execution. If the version, balance, recipient, or row set differs from the approved checkpoint, invalidate the decision and create a new proposal.
Should rollback be exposed as another agent tool?
Only with strict scope and authorization. Prefer a deterministic compensation workflow that accepts the original operation ID, verifies its recorded result, and refuses to run when the effect is unknown or already compensated.
