Oncologists should treat every AI-generated clinical summary as a draft—not as a verified account of the patient’s cancer or a ready-to-use treatment recommendation. Before it informs care, check each material claim against the relevant original chart, test result, medication record, guideline, or study; reconcile the patient’s disease and treatment context; and document corrections and unresolved questions. The clinician remains responsible for diagnosis, treatment decisions, and communication with the patient.
What should an oncologist check first?
Start by establishing what the summary represents and what information it used. A summary without clear provenance or a defined time boundary can conceal missing records, stale information, or a mismatch between the intended task and the output. Do not assume that a fluent narrative reflects a complete chart review.

- Identify the tool and model version, the date the summary was generated, and the records or other sources supplied to it.
- Check the retrieval window or chart cut-off date. A summary made before a new scan, pathology addendum, treatment change, or laboratory result may no longer describe the current situation.
- Establish the intended use: for example, summarizing a visit, preparing a handoff, or supporting a literature review. A summary prepared for one purpose should not silently become the basis for another.
- Look for links or references that let you return to the source. If attribution is absent or the date boundary is unclear, do not treat the output as verified.
Verify that it is the right patient and episode of care
Check patient identifiers and the relevant encounter before reviewing clinical content. Then make sure the narrative distinguishes the current cancer and treatment episode from prior cancers, historical therapies, copied-forward notes, or information about another episode of care. A correct fact can still mislead if attached to the wrong patient, date, or disease context.
How can an oncologist tell whether an AI summary is accurate?
Accuracy requires source-level checking, not a general impression that the narrative sounds plausible. For each statement that could affect care, find the supporting source and confirm that the summary preserves its meaning, timing, and uncertainty.

Reconcile the oncology context
Review the variables that determine how a result or recommendation applies to this patient. At minimum, reconcile the cancer type and histology, stage, relevant molecular or biomarker results, treatment line, prior therapies, response assessments, adverse effects, comorbidities, allergies, and the patient’s goals of care. A study result or treatment option can mean something different for a different biomarker subgroup, disease stage, or line of therapy. ESMO’s consensus discussion of AI use in oncology highlights the risk of misrepresentation and missing methodological detail when interpreting findings.
Use the chart to resolve discrepancies rather than accepting the version that appears most often in generated text. For example, check a biomarker claim against the pathology or molecular report, and verify a treatment history against dated orders and administration records. If the original source itself is ambiguous or conflicting, record that uncertainty instead of turning it into a definite statement.
Check dates, results, and treatment details against original records
Follow material facts back to the most direct available evidence. For a test result, that may be the laboratory report; for pathology, the report and any addendum; for imaging, the radiology report; for treatment exposure, the medication record or administration history. Confirm dates and units where they matter, and distinguish a planned intervention from one that was actually given.
Check whether a summary has collapsed a sequence of events into a misleading snapshot. A response assessment needs its date and context; a toxicity description may have changed after dose modification or supportive care; and a pending test should not be represented as a completed result. Where the chart contains a correction or later update, make sure the summary reflects the current authoritative record.
Look for omissions as well as false statements
A summary can be factually correct sentence by sentence yet unsafe because it leaves out information that changes the interpretation. Compare it with the source note and ask what is missing, including pending tests, contraindications, dose modifications, progression dates, toxicity grades, performance status, trial eligibility, and patient preferences. These details can alter whether an option is appropriate or whether a plan is consistent with the patient’s circumstances.

Do not use a clean-looking summary as evidence that the chart has no unresolved issues. If a clinically important item is absent, retrieve it from the source record or ask the responsible team to clarify it. If it cannot be resolved in time for the decision at hand, make that limitation explicit and escalate according to the clinical workflow.

Separate what a source says from what the AI infers. Open each cited guideline or paper and verify that its population, intervention, comparator, endpoint, biomarker subgroup, and publication date support the wording in the summary.
Read the source, not just the citation
A reference may be real but still fail to support the specific claim attached to it. Check whether the cited result applies to the patient’s cancer type, stage, biomarker status, and treatment line; whether the described outcome matches the endpoint reported; and whether the article is reporting evidence, a recommendation, or an interpretation. Look for omitted qualifiers, such as eligibility criteria or a subgroup restriction, that could make a broad-sounding statement inapplicable.
ESMO’s ELCAP consensus advises clinicians to verify AI-suggested information using guidelines, peer-reviewed studies, or expert consensus. That means checking the underlying source and its scope—not treating a citation list as proof that an answer is correct. If a citation cannot be opened, is unrelated, or does not substantiate the claim, mark the claim as unverified and seek an authoritative source.
Check whether the information was current at the decision date
Confirm the versions and dates of guidelines, approvals, trial results, and medication information relevant to the decision. Record the date checked and whether a later update supersedes the source the summary used. A previously accurate statement can become outdated as evidence or guidance changes; a summary’s generation date alone does not establish that its references were current.

Identify inference and uncertainty
Watch for confident wording that goes beyond the chart or cited evidence, unsupported causal claims, omitted alternatives, and conclusions that assume facts not documented. Ask whether uncertainty, missing information, or limitations in the source are visible. If the tool cannot show its basis or distinguish source facts from inference, do not fill that gap with trust in the wording; verify independently or leave the claim out of the clinical decision.
What should happen when the summary and chart disagree?
Use the source record and appropriate clinical review to resolve the discrepancy; do not silently choose the AI version. The resolution should be traceable, especially when it affects treatment, diagnosis, or patient communication.
- Pinpoint the claim. Identify the exact statement, date, and decision it could affect, rather than describing the entire output as simply right or wrong.
- Locate the most relevant source. Review the original report, result, order, administration record, guideline, or study that should support the claim.
- Determine whether the issue is an error, omission, stale information, or unresolved conflict. Those distinctions help the team correct the record and decide what further review is needed.
- Correct the working summary and preserve the change. Record what was changed and the source used. Do not overwrite the only copy of the generated output if doing so would erase the audit trail.
- Escalate unresolved material discrepancies. Follow the institution’s clinical process and involve the responsible clinician or relevant specialist before relying on a disputed claim.
Keep the generated summary, its source references, reviewer identity, corrections, clinical decision, and any escalation in the appropriate record or approved system. This creates an audit trail that supports accountability and later quality review. The precise documentation location should follow local policy.
Who is responsible for the decision and patient communication?
The oncologist retains responsibility for clinical judgment. ESMO’s ELCAP consensus calls for human oversight, especially for critical decisions, and says final responsibility for diagnoses, treatment plans, and patient communication should not be delegated to a large language model. ASCO likewise cautions that AI outputs may not be correct or appropriate for a particular use case and are not a substitute for professional judgment.

How should a clinic evaluate an AI summarization tool?
Evaluate the tool for the intended oncology workflow rather than relying on a broad accuracy claim. The relevant question is whether it reliably handles the records, patient population, and tasks for which the institution plans to use it—and whether staff can detect and correct failures.
Compare tools on safety-relevant capabilities
| Evaluation area | What to examine |
|---|---|
| Provenance and citations | Can reviewers see which records or sources informed a statement and open the cited material? |
| Oncology context and completeness | Does the tool preserve stage, histology, biomarkers, treatment line, dates, response, toxicity, and relevant patient factors in the intended use case? |
| Population and workflow validation | Has performance been assessed in the patient population, record types, and workflow where it will be used? |
| Uncertainty and limitations | Can the tool expose missing information, uncertainty, and limits instead of presenting unsupported certainty? |
| Privacy and institutional controls | Is the tool approved for the data and use, with appropriate security and institutional authorization? |
| Auditability and workflow fit | Can staff review, override, correct, and document output in a way that fits clinical practice? |
| Updates and monitoring | Are changes to models, data sources, or workflows tracked, and is there a process for detecting errors or drift? |
FDA clinical decision support guidance describes a clinician’s ability to independently review the basis for a recommendation as an important criterion for software that supports—not replaces—clinical decision-making. That is a useful design principle for summarization systems too: a reviewer needs to inspect the basis of material claims, not just receive a polished conclusion.
Validate before use and keep oversight active
Before deployment, validate extraction and summarization in the intended oncology workflow and in new contexts where the system may be used. ESMO recommends validation, continuous monitoring for errors, bias, and model drift, staff training to question suspicious findings, institutional governance, and re-checking when workflows or data sources change. FUTURE-AI similarly recommends human-in-the-loop quality checks, the ability to overrule outputs, use-case-specific oversight, periodic audits and updates, and accessible user training.

These controls continue after launch. A change to a model, source system, patient population, or workflow can change how the output behaves. Set institutional processes for review and escalation, train staff to check rather than defer to generated text, and periodically audit whether the tool is omitting or distorting clinically important information. Do not assume that an evaluation in one use case establishes safe performance in another.
How should patient privacy be protected?
Do not paste identifiable or confidential patient information into an external AI tool unless the institution has approved it for that purpose with appropriate security, authorization, and patient permissions. A tool’s convenience or general availability is not evidence that it is suitable for sensitive clinical data.
Best Value
Check institutional policy before entering chart material, including whether the service retains prompts or outputs and what safeguards apply. ASCO’s AI Tools Notice specifically says its AI tools are not secure environments for individually identifiable or sensitive information. That warning is specific to those tools; it also reinforces why clinicians should verify the approved data-handling rules for any system they use rather than assuming all AI tools have the same protections.
Is there a universal accuracy percentage for AI-generated oncology summaries?
No general accuracy percentage is established for AI-generated oncology clinical summaries across tools and workflows. A single number would not tell an oncologist whether a particular system preserves the details that matter for a given cancer, source record, and clinical task.
Use tool- and workflow-specific validation instead. The evaluation should match the intended setting and examine clinically meaningful errors, omissions, and the ability of reviewers to find and correct them. A reported result from one model, institution, or task should not be generalized to a different use case without supporting validation.
Frequently Asked Questions
Can a patient correct an AI-generated summary of their cancer history?
Patients can point out errors or missing history and ask the care team to review them against the medical record. Whether a patient can amend a formal record directly, and how a correction is recorded, depends on the provider’s records process and applicable local rules.
Should an AI-generated summary be kept in the medical record?
That depends on the institution’s documentation and data-governance policies. If the output informs care, the recordkeeping process should make the reviewer’s corrections and the basis for the clinical decision traceable rather than presenting unreviewed generated text as an established fact.
Can AI-generated summaries be used for a second opinion?
A summary may help organize records for a second opinion, but it should not replace the original reports and records needed by the reviewing clinician. The receiving clinician should be able to inspect source material, particularly for pathology, imaging, biomarkers, and treatment history.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
What if an AI system gives no citations for a clinical claim?
Treat the claim as unsupported until it can be checked against an appropriate primary record or authoritative source. If the claim is material and its basis cannot be established, do not rely on it to guide care.
