Lenders should treat every AI-extracted mortgage value as unverified until it is checked against the original document and the rest of the loan file. If a discrepancy appears, the lender should isolate the affected field, determine which decisions and submissions used it, correct the data or obtain a corrected source document, rerun the required underwriting assessment, and preserve the reasoning and evidence. Reviews should be risk-based and documented, with prefunding checks designed to catch defects before closing and post-closing quality control to find anything that escaped. If the error contributed to a denial or other adverse action, the notice still must give specific, accurate reasons. Fannie Mae’s exact AI-governance requirements also need a current check: its April 2026 letter says it was superseded by an Information Security and Business Resiliency Supplement published October 21, 2026.

What should a lender do first when AI misreads a mortgage document?

Stop treating the extracted value as established fact. Preserve the original page, compare the AI output with the source and corroborating records, and place the loan in a controlled exception workflow until the impact is known.

The lender, not the extraction model or its vendor, remains responsible for the accuracy and integrity of the loan-file data used to make a credit decision. A practical first response is to freeze downstream use of the suspect field, identify every system that received it, and determine whether it affected income, assets, liabilities, identity, occupancy, loan terms, or eligibility.

Materiality matters. A punctuation error in an address may have no underwriting effect; a misplaced decimal on a pay stub can change qualifying income and the approval result. Do not assume an error is harmless merely because an automated underwriting system returned an approval.

How do lenders catch AI errors before funding?

Prefunding quality control is the best opportunity to correct a bad extraction before money is advanced. Fannie Mae describes written prefunding procedures, risk-based selection, verification of documents and data, and review independent of production where practical. The purpose is to identify and address defects before closing or acquisition, not to rely on a later cure.

1. Detect discrepancies against source records

Compare extracted fields with the underlying pay stubs, tax forms, bank statements, employment records, credit reports, purchase contracts, and other evidence in the file. Reconcile the value with other documents rather than allowing one apparently clean extraction to override contradictory evidence.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use full-file review for higher-risk loans and targeted component review for lower-risk samples, based on the lender’s documented risk assessment. Prioritize fields that drive eligibility, capacity, collateral, fraud controls, or regulatory reporting, as well as values that are anomalous or conflict with another source.

2. Triage and contain the issue

Record the document and page, the extracted value, the source value, when the discrepancy was found, and the model or workflow version if the lender can identify it. Note which downstream systems, disclosures, underwriting submissions, conditions, or closing documents used the value. Assign a severity that distinguishes an informational defect from one that could change the credit decision or loan eligibility.

Route the loan to a person with authority to stop or condition closing. Access to the original image or PDF should be retained, because screenshots or reprocessed copies may not establish what the model actually saw.

3. Correct the data or obtain a corrected source document

Correct the lender’s internal record and every downstream field populated from it. If the source document itself is wrong, incomplete, or altered, request a corrected document from the borrower, employer, financial institution, or other relevant provider. Fannie Mae’s post-closing guidance says errors identified in review should be assessed for significance and that data and documents should be promptly corrected, including by obtaining corrected documents from a vendor or service provider when appropriate.

Do not silently overwrite the old value. Retain the original extraction, the verified value, who changed it, the date, and the evidence supporting the change. That history lets quality-control staff and auditors distinguish a corrected model error from a later manual override.

4. Reassess underwriting on verified facts

Determine whether the corrected value requires a new automated underwriting submission. Apply the relevant investor tolerance rules; do not assume a small numerical difference is automatically permitted. If resubmission is required, use the corrected data and retain the new findings. If resubmission cannot be performed, complete the applicable comprehensive manual risk assessment and document why that path was used.

The reviewer must decide whether the loan still meets the program’s eligibility and approval conditions. An approval generated from incorrect income or assets is not evidence that the corrected file remains approvable.

5. Document the disposition before closing

Record the correction, the underwriting reassessment, any changed conditions or disclosures, and the final eligibility decision. If the loan was not eligible as delivered, follow the applicable self-reporting and investor-notification process. The exact reporting route depends on the investor and program; Fannie Mae requirements should not be presented as a universal procedure for every lender or government-backed loan.

What does post-closing quality control require?

Post-closing review is a second line of defense for errors that were not found before funding. In the Fannie Mae seller/servicer context, selected loans are reviewed for more than the extracted fields themselves.

Review area What reviewers should verify
Application Completeness, consistency, and whether the information used in the decision is supported.
Credit, income, and assets Underlying documents, calculations, and the relationship between source evidence and submitted data.
Underwriting support Conditions, compensating factors, calculations, and the rationale for the decision.
Third-party outputs Reports or analyses that fed the decision, including whether an automated result was based on accurate inputs.
Automated underwriting data Data submitted to Desktop Underwriter when applicable, and whether it matches verified file information.
Transaction and closing documents Purchase, title, settlement, and final loan terms, including changes made after the initial decision.

These checks should be selected and documented through written procedures. A post-closing finding should trigger significance assessment, correction, underwriting review, and any required reporting rather than being filed as a harmless model variance.

How should conflicting data be resolved and underwriting rerun?

Start with the hierarchy of reliable evidence required by the applicable program. A machine-readable value is not inherently more reliable than an original statement, and a source document is not automatically correct if it conflicts with independent evidence.

  1. Identify the conflict. State exactly which field differs, such as gross pay, year-to-date earnings, account balance, debt payment, or employment date.
  2. Check program tolerances. Determine whether the difference falls within a published tolerance and whether that tolerance applies to the specific field, document type, and submission.
  3. Correct the authoritative record. Update the verified value and obtain a replacement source document when the original is defective or incomplete.
  4. Resubmit when required. Send corrected data through the applicable automated underwriting system and retain both the prior and current findings.
  5. Use a manual assessment when resubmission is not possible. Follow the program’s prescribed comprehensive review rather than inventing an informal workaround.
  6. Decide eligibility. Base the decision on verified facts, not on the first automated recommendation, and escalate any ineligible-as-delivered loan through the required channel.

Error codes from an automated underwriting system are signals to investigate data quality or validity. They do not prove that the source document is accurate, nor do they identify whether the extraction step or the underwriting logic caused the problem.

What records should an AI-error case contain?

A defensible case file should allow a different reviewer to reconstruct what the model produced, what the lender verified, and why the final decision changed or stayed the same. A practical record includes:

  • Original document image or file and its receipt date.
  • Extracted field, confidence or exception flag if available, and the workflow or model version.
  • Verified source value and the evidence used to establish it.
  • Systems and documents populated from the erroneous value.
  • Assessment of severity, materiality, and potential effect on eligibility, pricing, disclosures, or adverse action.
  • Correction history, including employee, date, reason, and any replacement document.
  • Automated underwriting resubmission or manual assessment, with findings and conditions.
  • Final eligibility, closing, repurchase, or self-reporting decision and the approving authority.

This schema is an operational control design, not a verbatim regulator-prescribed form. Lenders should map it to their record-retention, privacy, and investor requirements.

How are extraction errors different from automated-underwriting errors?

Document processing and underwriting are separate control points. Optical character recognition or an AI parser may read a pay stub incorrectly; an automated underwriting system may then correctly evaluate the incorrect number it receives.

The Government Accountability Office describes enterprise automated underwriting systems as providing error codes and notes that AI may assist document review, while the lender remains responsible for underwriting decisions. Controls therefore need traceability at both stages: prove what was read from the document, and prove what data and rules were used to reach the credit result.

Failure point Typical symptom Required investigation
Document extraction Wrong digit, omitted page, misclassified form, or field mapped to the wrong borrower. Inspect the original image, compare neighboring fields and independent records, then correct the data or source document.
Data transformation Correct extracted value changes format, sign, unit, or period when sent to another system. Trace interfaces, calculations, and field mappings between the document platform and loan-origination or underwriting systems.
Automated underwriting Error code, changed recommendation, or unmet condition after submission. Review submitted data, applicable rules, tolerance treatment, and the resulting findings; do not infer source-document accuracy from the code alone.
Human decision Underwriter accepts unsupported data or fails to act on a contradiction. Review procedures, training, approval authority, and whether the file contained enough evidence for the conclusion.

What must adverse-action notices say when AI contributed?

If an error contributes to a denial, withdrawal, counteroffer, or other adverse action, the creditor must provide specific and accurate principal reasons under the Equal Credit Opportunity Act and Regulation B. The reasons should describe the actual factors considered, not a generic label such as “internal policy” or a checklist item that did not drive the decision.

The Consumer Financial Protection Bureau’s Circular 2022-03 states that a creditor cannot excuse noncompliance because its technology is too complicated or opaque to understand. In a September 19, 2023 release, CFPB Director Rohit Chopra put the point plainly: “Creditors must be able to specifically explain their reasons for denial. There is no special exemption for artificial intelligence.” See the CFPB Circular 2022-03 and the CFPB’s 2023 guidance release.

Before sending notice, reconcile the adverse-action reasons with the corrected file and the final underwriting record. If the lender discovers that an erroneous extracted value caused the action, counsel and applicable program rules should guide whether the application must be reevaluated, reopened, or otherwise remedied; the notice requirement alone does not answer every remediation question.

How should lenders build a risk-based control program?

A sustainable program combines preventive review, independent testing, vendor oversight, and feedback from actual defects. It should not depend on a single accuracy percentage: the reviewed guidance establishes duties to verify, correct, and govern data but does not set a universal AI-extraction accuracy bar.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Set sampling and escalation rules

Use loan, document, and field risk to determine review frequency. Increase sampling for new document types, unusual layouts, self-employed income, foreign-language records, poor image quality, high loan amounts, and workflows with recurring exceptions. Escalate any pattern that could affect multiple loans, not just the file where it was first found.

Keep review independent where practical

People who produced or approved the loan should not be the only people testing the extraction. Fannie Mae’s prefunding guidance calls for review independent of production where practical; lenders should define that separation in procedures and document unavoidable exceptions.

Monitor vendors and workflow changes

Require notice of model, OCR engine, parsing-rule, interface, and training-data changes that could alter outputs. Track error types by document and field, turnaround time for corrections, repeat defects, and loans affected. Feed those trends into sampling, staff guidance, and vendor remediation. This monitoring design is a prudent control inference, not a claim that a particular regulator mandates one dashboard or threshold.

Protect data and auditability

Limit access to borrower documents, retain immutable versions where possible, and log manual overrides. Test that a reviewer can retrieve the original page, the extracted value, the corrected value, and the underwriting result without relying on a vendor’s unavailable interface.

What Fannie Mae guidance is current in October 2026?

Fannie Mae’s Selling Guide quality-control provisions remain relevant to lenders operating in its seller/servicer framework. The prefunding QC guidance addresses written procedures, selection, verification, and timely defect detection. The post-closing QC guidance addresses file review, conflicting data, correction, resubmission or manual assessment, eligibility, and reporting.

Do not quote the April 8, 2026 AI letter as a current standalone requirement. Its PDF says: “This Lender Letter has been superseded by the Fannie Mae Information Security and Business Resiliency Supplement, published on October 21, 2026.” The replacement supplement’s detailed provisions should be checked directly before a lender states specific AI or machine-learning governance obligations. These Fannie Mae materials do not automatically govern every investor, FHA or VA loan, jurisdiction, or mortgage creditor.

What happens if AI misreads a pay stub or bank statement?

Suppose an AI parser reads year-to-date pay as $68,400 instead of $58,400 and the inflated figure is sent to underwriting. The lender should preserve the pay-stub image, verify the number against the stub and employment evidence, identify every submission and calculation that used $68,400, and correct the loan record.

If program rules require it, the lender resubmits the corrected $58,400 figure and reviews the new findings. If the lower income changes debt-to-income ratios, reserves, or eligibility, the lender updates conditions and the approval decision before closing. If the loan already closed, the discrepancy enters post-closing QC, where significance, correction, eligibility, and any investor reporting are assessed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The same logic applies to a bank statement balance, but the reviewer must also check statement period, account ownership, deposits, transfers, and whether the value was counted as an asset or reserve. A single corrected number is not enough if it changes the context of the underwriting analysis.

Which mistakes create the greatest compliance risk?

  • Allowing an AI result to flow into underwriting without source-document verification.
  • Correcting a field but failing to update a downstream submission, disclosure, or closing document.
  • Relying on an automated approval after material income, asset, liability, or identity data changes.
  • Deleting the original extraction or overwriting it without an audit trail.
  • Using a broad adverse-action reason that does not match the actual factors considered.
  • Treating an investor-specific QC rule as universal law, or relying on superseded governance guidance.
  • Assuming a vendor’s stated model accuracy replaces lender sampling, oversight, and accountability.

Frequently Asked Questions

Do all AI discrepancies require a lender to stop closing?

No. The lender should assess significance and potential effect on eligibility, disclosures, and transaction terms. A documented risk decision may allow closing to proceed for a nonmaterial issue, while a material discrepancy should remain on hold until corrected and reassessed.

Is there a required accuracy percentage for mortgage-document AI?

The cited Fannie Mae, CFPB, and GAO materials do not establish one universal extraction-accuracy percentage. Lenders must set risk-based testing and acceptance criteria appropriate to their documents, fields, programs, and loss or compliance exposure.

What if a document-processing vendor will not issue a corrected document?

Use an authoritative alternative source when program rules permit, document the vendor’s response, and escalate the defect through vendor-management and quality-control channels. Never treat a vendor refusal as proof that the original document or extracted value is correct.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do these procedures apply identically to FHA, VA, and nonagency loans?

No. The cited quality-control instructions are specifically framed for the Fannie Mae seller/servicer context, while adverse-action duties arise under applicable consumer-credit law. Each investor, government program, and jurisdiction may add different review, reporting, and remediation requirements.
