AI agents become risky in production when an organization treats them like ordinary chatbots. An agent can plan a multi-step task, call software tools, read enterprise data and change records, so a plausible final answer does not prove that the workflow was safe or correct. The reliable path is to define a bounded business job first, grant only the access that job needs, assign an accountable owner, treat all ingested content as untrusted, preserve human control over consequential actions, test the complete workflow, and operate the system continuously after launch. The controls should scale with impact: an internal, read-only assistant needs less oversight than an agent that edits customer records or initiates payments.
What can go wrong when deploying AI agents?
The seven most damaging mistakes are governance and design failures rather than model-selection errors. Leaders often start with a platform demonstration, connect it to broad corporate permissions, and discover too late that no one can explain what the agent is allowed to do, which data it used, or who can stop it. NIST reported in its May 18, 2026 summary of comments on AI-agent security that respondents broadly see novel threats requiring adaptations to baseline cybersecurity practices. The practical response is a production control system covering purpose, identity, data, tools, oversight, evaluation and lifecycle operations.

How should we define an agent’s job before choosing technology?
Start with a narrowly bounded business task and a written contract for the agent. Selecting a model or orchestration platform before defining that contract is the first mistake because an impressive demo can hide an undefined decision authority.
Write a task contract
Document the business outcome in one sentence, the inputs the agent may use, the tools it may call, the records it may change, and the conditions that end the run. Specify measurable success criteria such as classification accuracy, response time, escalation rate or reconciliation accuracy. Also list actions that are never permitted, including examples of ambiguous requests that must be routed to a person.

For example, “triage incoming support tickets into approved queues and draft a response” is bounded. “Handle customer support autonomously” is not. The first definition can limit the agent to ticket metadata, a knowledge base and a draft endpoint; the second leaves data access, communication authority and exception handling unspecified.
Make prohibitions deterministic
Do not rely on a model remembering a policy prompt. Put hard blocks in the application and tool layer for prohibited operations, destinations, data classes and transaction amounts. Microsoft’s agent-risk guidance treats clear purpose, boundaries and deterministic controls as foundational task-adherence measures. A policy engine should reject an unauthorized tool call even when the model requests it repeatedly or presents a convincing rationale.

How do I secure an AI agent’s access to company data?
Give the agent a dedicated identity with the minimum tools, data and operations required for its defined task, and deny everything else by default. Broad or inherited permissions turn a small prompt or software defect into a potential enterprise-wide incident.
Separate identity, tools and data
Create a service identity for each production agent or tightly governed agent class; do not reuse a human administrator’s token. Scope permissions to specific APIs, tables, repositories, folders and environments. A read-only reporting agent should not inherit write access because another workflow needs it. Separate development, test and production credentials, and make secrets short-lived where the platform supports that pattern.
Apply existing organizational identity and data-governance practices rather than inventing an exception for AI. Enforce tenant, department, geography and record-level restrictions at the data and API layers. Mask or omit sensitive fields before retrieval when the task does not need them. Log each authorization decision so an investigator can determine whether a response exposed data because of a policy error, a retrieval error or an approved exception.
Use a deny-by-default tool broker
Place tool calls behind a broker or policy gateway that validates the requested operation, parameters, target and user context. Allow-list functions such as read_ticket or create_draft instead of exposing a general database connection or shell. Validate formats, ranges and destinations server-side; never assume that a model-generated argument is safe because it matches a schema. Microsoft’s governance baseline groups identity, security and data governance as continuing control-plane responsibilities, not one-time setup tasks.

How can we keep every agent accountable?
Maintain an inventory with an accountable owner before an agent reaches production. An unregistered or ownerless deployment becomes “shadow AI”: it may retain credentials, process data or continue making decisions after its original team has moved on.
Minimum registry fields
Record the agent’s business purpose, owner and backup owner, platform and model, environments, data classifications, tools and permission scopes, human-approval points, deployment date, version, dependencies, contact path and retirement criteria. Link the record to architecture and threat assessments, evaluation results, incident history and change approvals. The entry should identify whether the agent is internal-only, customer-facing or able to affect regulated or financial records.
Manage the full lifecycle
Use a defined path from proposal to design review, testing, approval, release, periodic recertification, suspension and retirement. Recheck ownership and permissions when a team reorganizes, a model changes, a data source is added or a vendor integration is removed. Discovery scans, gateway logs and cloud inventories can find unregistered endpoints, but technical discovery does not replace an accountable human decision about whether an agent should exist.
Microsoft’s enterprise governance guidance emphasizes control-plane ownership, lifecycle management and observability. Treat the registry as an operational system with stale-record alerts, not as a spreadsheet created for an audit.
Why can retrieved content hijack an agent?
Retrieved documents, web pages, email messages and tool results are data, not trusted instructions. NIST’s CAISI described agent hijacking as indirect prompt injection: an attacker places malicious instructions in content an agent later ingests, causing unintended actions.
Keep instructions and data separate
Use typed message channels and explicit delimiters so the model can distinguish system policy, developer rules, user requests and retrieved text. Tell the agent that documents may contain instructions but cannot change its operating policy. Do not paste untrusted content into a privileged instruction field or let a retrieved page redefine tool permissions.

Constrain the response path
Filter and classify sources before retrieval, preserve provenance, and mark content that came from outside the organization. Require the agent to quote or reference the supporting passage for sensitive decisions, while still treating that passage as untrusted. Validate every tool parameter independently of the content that suggested it. High-risk actions should require a fresh authorization check after retrieval, because the data may have changed the proposed target or amount.
Test with adversarial documents containing hidden text, conflicting instructions, encoded commands and poisoned tool outputs. Observe whether the agent attempts a prohibited call, leaks context, follows the injected instruction or correctly ignores it. NIST’s January 17, 2025 technical guidance frames this as an evaluation problem, not merely a prompt-writing problem.
When must a person approve an agent’s action?
Human control is required when an action is high-impact, difficult to reverse, ambiguous or exposed to untrusted content. Removing approval to make a demo feel autonomous can create an unreviewable path to financial loss, privacy harm, safety incidents or regulatory breaches.
Design approval gates by consequence
Let the agent gather evidence, prepare a change and explain its reasoning, then pause before execution when the action changes a customer record, sends an external communication, grants access, places an order, moves money or deletes data. Show the reviewer the proposed parameters, affected objects, evidence sources, policy checks and confidence or uncertainty indicators. The reviewer must be able to edit, reject, defer and request a new plan.
Make interruption real
Provide a kill switch that revokes tokens or blocks the broker, not merely a prompt telling the model to stop. Set timeouts, transaction limits and rate limits so a runaway loop cannot continue while a person investigates. For lower-impact, reversible tasks, sampled review or post-action rollback may be sufficient; for irreversible or externally visible actions, require explicit approval each time or within a tightly bounded batch.
Microsoft’s guidance treats oversight as risk-dependent and distinguishes intelligibility, disclosure and intervention from blind trust in model behavior. Tell users when an agent is acting, what it can do and when a human must decide.

How should we test an AI agent before production?
Evaluate the entire workflow, not only the final answer. Agents plan, retrieve evidence, select tools and update state across multiple steps; a fluent response can conceal a wrong query, missing evidence, unsafe parameter or unauthorized side effect.
Test the path, not just the text
Define tests for task adherence, authorization, retrieval quality, tool selection, parameter validation, error handling, escalation and termination. Include normal cases, incomplete requests, conflicting records, service outages, expired credentials, malformed tool results and repeated retries. Verify that the agent stops when a required condition is missing instead of inventing a value or continuing with a guessed target.
Use adversarial, in-workflow probes
NIST’s agentic-AI evaluation work describes probes that act as adversarial verifiers during a workflow and produce a machine-readable audit trail. Such probes can check whether the agent followed policy at each step, used the expected evidence, called only permitted tools and left the system in the intended state. Store the trace: prompts and retrieved identifiers, tool calls and arguments, policy decisions, approvals, outputs, latency, retries and errors. A final-answer score without this trace cannot distinguish a lucky result from a safe process.
Set release thresholds
Agree in advance on pass criteria and owners for exceptions. A release may require zero unauthorized tool calls, complete approval capture for high-impact actions, a maximum escalation rate, bounded latency and successful recovery from injected instructions. Re-run the suite after model, prompt, tool, retrieval-index or policy changes; these changes can alter behavior even when the business interface looks unchanged.

What must happen after an AI agent goes live?
Launch is the beginning of operations, not the end of governance. Keep monitoring, auditability, lifecycle ownership and intervention mechanisms active for as long as the agent has access.
Monitor behavior and context
Track tool-call volume, denied calls, unusual destinations, data-access patterns, approval overrides, latency, retries, fallback frequency, user corrections and outcome quality. Alert on privilege changes, new tools, prompt or retrieval-index changes, sudden shifts in task distribution and behavior outside the agent’s normal envelope. Retain enough context to reconstruct an incident while applying retention and privacy rules to logs.

Prepare an incident procedure
Define who can suspend the agent, revoke credentials, quarantine a data source, restore records and notify affected parties. Practice a scenario in which a poisoned document triggers an attempted external action, and another in which a model update increases erroneous writes. After containment, preserve the trace, identify the failed control, correct policy or code, and require a documented reapproval before reactivation.
Review models and dependencies
Record model versions, vendor components, connector versions and retrieval indexes in the registry. Reassess supply-chain exposure when a dependency changes, and retire agents that no longer have an owner or business justification. The OWASP GenAI Security Project’s “State of Agentic AI Security and Governance,” version 2.01 dated June 1, 2026, reflects the field’s focus on security and governance as continuing disciplines rather than a one-time launch checklist.

Use business impact, autonomy, data sensitivity, permission scope and approval needs to set the control level. The following pattern avoids applying an expensive approval process to every low-risk assistant while preventing high-consequence agents from receiving casual treatment.
| Deployment pattern | Typical capability | Minimum controls | Additional controls as impact rises |
|---|---|---|---|
| Low-impact internal assistant | Read-only answers from approved internal material | Registered owner, scoped identity, source labeling, basic logging and quality tests | Stronger retrieval filtering, periodic access review and sampled human review |
| Operational copilot | Drafts changes or communications for a user | Tool allow-list, parameter validation, workflow traces and explicit user confirmation | Approval evidence, rollback, rate limits and adversarial testing |
| Autonomous record-changing agent | Writes systems of record or triggers external actions | Dedicated identity, deny-by-default broker, deterministic blocks, complete audit trail and release gates | Per-action approval for irreversible steps, transaction limits, continuous alerts and rehearsed shutdown |
| High-consequence or regulated workflow | Decisions affecting money, access, safety, rights or regulated data | Named accountable owner, documented rationale, segregation of duties and strict data controls | Independent review, formal change management, retention requirements and tested recovery procedures |
What is a practical production gate for an AI agent?
Before approval, require evidence that the agent has a bounded purpose, an owner, least-privilege access, untrusted-content defenses, appropriate human intervention, workflow-level evaluation and an operating plan. A concise gate review can ask:
- Can a reviewer name every tool, data source and side effect?
- Does the system reject prohibited actions in code when the model requests them?
- Are credentials, records and logs separated across development, test and production?
- Can an operator stop the agent and revoke access without waiting for model cooperation?
- Do tests cover injected instructions, tool failures, ambiguous requests and model or dependency changes?
- Is there a current owner, escalation route, review date and retirement condition?
If any answer is no, narrow the task or reduce autonomy before expanding the deployment. Reliable agent programs are built by making authority explicit, observable and reversible, then increasing autonomy only when evidence supports it.
Frequently Asked Questions
Should every AI agent use a separate model or platform?
No. A shared platform can be appropriate when identities, tool policies, data boundaries and audit trails remain isolated per agent. Choose the platform that can enforce those boundaries and support your required review and shutdown procedures, rather than selecting a separate stack merely to create the appearance of isolation.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteHow often should an AI agent’s permissions be recertified?
Set the interval by risk and change rate, with an immediate review after ownership, model, tool, data-source or business-process changes. High-impact agents generally need scheduled reviews at least quarterly or more frequently under internal policy; low-impact read-only agents can use a longer interval if automated alerts detect unexpected access.
What evidence should an incident investigator request first?
Preserve the complete execution trace before rotating or deleting anything: identity and token scope, user request, system and developer instructions, retrieved item identifiers, tool arguments and results, policy decisions, approvals, model version, timestamps and resulting system changes. Correlate that trace with API, identity and data-access logs to determine whether the failure was unauthorized access, malicious content, policy misconfiguration or an incorrect but authorized action.
Can retrieval-augmented generation prevent agent hijacking by itself?
No. Retrieval can improve grounding but also imports content containing indirect prompt injections. Treat retrieved material as untrusted, enforce tool and authorization controls outside the model, and test whether hostile documents can alter the workflow or trigger a prohibited action.
