AI researchers test safety-rule compliance by turning a written policy into observable pass-and-fail claims, running ordinary evaluations and adversarial red-team exercises, checking whether the test itself is valid, and then using the evidence to improve safeguards or make a scoped deployment decision. The result supports a claim about a particular model, configuration, test set and scoring method; it does not prove that the model will obey every rule in every future conversation.
That distinction matters because safety behavior depends on more than a model’s abstract capabilities. Prompts, tools, context windows, resource limits, monitoring and even the wording of a benchmark can change what evaluators observe.

A safety rule is an intended behavior, such as refusing a prohibited request, protecting private information, following a higher-priority instruction when instructions conflict, or avoiding an unsafe action while using a tool. Researchers cannot score an intention directly. They must define what a compliant response looks like and what evidence counts as a violation.
A policy or model specification therefore becomes a set of operational claims. For example, “the model refuses disallowed instructions” can be tested with a defined collection of prohibited prompts, a rubric for acceptable refusals, and rules for ambiguous answers. “The model follows instruction hierarchy” can be tested with scenarios in which a lower-priority message attempts to override a system-level constraint.

Anthropic’s public Constitution illustrates the difference between an intended standard and observed behavior. It describes how Claude is meant to act while acknowledging that training does not guarantee that every response will reflect those ideals. A test measures the gap between the stated target and behavior under specified conditions.
How do researchers turn a safety rule into a testable target?
They write the target before collecting results and specify the boundary of the claim. A useful target identifies the behavior, the context, the allowed alternatives and the scoring rule.
Define the behavior
Researchers state whether they are measuring a capability, a safeguard or a comparison. A capability test might ask whether a system can complete a dangerous technical task. A safeguard test might ask whether it declines that task or limits the assistance. A comparison test might hold the prompts and scoring constant while comparing two systems.
Define acceptable and unacceptable outputs
Rules need observable criteria. A refusal may be considered acceptable only if it does not provide the prohibited instructions, while still offering a safe alternative. A response that begins with a refusal but then supplies actionable details may fail. For privacy tests, the rubric may distinguish between refusing to reveal personal data, accurately explaining the limitation and accidentally confirming a sensitive fact.
Define the conditions
The claim should name the model version, system instructions, available tools, conversation history, token or time budget, temperature or other generation settings, and any monitoring or filters. Without those details, another evaluator cannot tell whether a difference came from the model or from the surrounding setup.
Define the population of cases
A test set is a sample of possible interactions, not a complete list of them. Researchers decide whether cases represent common use, high-impact edge cases, known attacks, or a mixture. They may reserve held-out cases so that tuning does not simply optimize for prompts the model has already seen.

Structured benchmark items
Benchmarks use repeatable prompts and a fixed scoring procedure. They are efficient for measuring consistency across versions or systems. Multiple-choice tests, classification tasks and standardized safety scenarios can be run at scale, but they may miss open-ended failures and can become familiar to the model.
Human judgments
Annotators compare responses or label them against a rubric. Crowdworker A/B assessments can reveal which answer people judge safer or more helpful, while domain experts are better suited to medical, legal, cybersecurity or other specialized risks. Human evaluation requires clear instructions, quality checks and a way to handle disagreement; otherwise the score may reflect annotator interpretation more than model behavior.
Interactive scenarios
Some safety properties appear only across multiple turns. Evaluators may vary the user’s framing, introduce conflicting instructions, or ask the model to revise an earlier answer. These scenarios test whether a safeguard persists after pressure rather than only on the first prompt.
Tool-use and agentic tasks
A model that can browse, execute code, send messages or modify files has opportunities to cause harm through actions, not just text. Evaluators therefore test the full workflow: the model’s plan, tool calls, permissions, confirmation steps, error handling and final result. A text-only score cannot establish safety for a system with materially different powers.
Expert audits and dangerous-capability checks
Anthropic’s discussion of model evaluations lists domain-expert red teaming and audits for dangerous capabilities alongside benchmarks and crowd assessments. These approaches can uncover specialized failure modes, but they are expensive and depend on the expertise, access and assumptions of the auditors.
What is red teaming, and how is it different from an ordinary evaluation?
Ordinary evaluation asks whether a system meets defined criteria on a planned set of cases. Red teaming deliberately searches for ways to make the system fail under adversarial, abusive or unexpected inputs. They complement one another rather than compete.
OpenAI’s red-teaming guidance summarizes the distinction as using evaluations to measure whether an AI system behaves as intended and using red teaming to probe behavior under adversarial, abusive or unexpected inputs. A benchmark might test a known jailbreak pattern; a red team might invent a new framing, combine several weaknesses, or exploit the application around the model.

- [Dont Hesite to Order] 24 hours customer services and professional technology team are standing by
Typical red-team approaches
- Prompt variation: paraphrasing, role-play, translation, obfuscation or long-context distractions.
- Instruction conflicts: placing a prohibited request inside quoted text, retrieved content or a lower-priority message.
- Multi-turn pressure: gradually escalating a benign conversation until the model is asked for unsafe details.
- Tool and workflow attacks: attempting unauthorized actions, manipulating tool outputs or exploiting missing confirmation steps.
- Application probing: testing rate limits, logging, access controls and other safeguards outside the model itself.
Red-team findings are often qualitative at first. A newly discovered exploit can lead to a reproducible test case, a training example, a policy change or a product-level control. The absence of a discovered exploit is not evidence that none exists.
How does the testing setup change the result?
The tested system is more than a set of neural-network weights. OpenAI’s May 29, 2026 playbook for trustworthy third-party evaluations emphasizes documenting the harness and the claim being tested because tools, context, workflow, resource budget and safeguards can materially affect outcomes.
| Setup element | Why it matters | What a report should disclose |
|---|---|---|
| Model and version | Different checkpoints or updates can have different refusal and capability patterns. | Exact model identifier, release or evaluation date. |
| System and developer instructions | Hidden instructions can reinforce, weaken or conflict with the policy under test. | Relevant prompts or a reproducible description. |
| Tools and permissions | Read-only access is not equivalent to permission to send, purchase, delete or execute. | Available tools, scopes, confirmation gates and error behavior. |
| Context and history | Long conversations, retrieved documents and user-provided files can alter interpretation. | Context construction, truncation rules and injected content. |
| Budget and stopping rules | More turns, tokens or retries can give an agent additional chances to find a failure. | Time, token, turn and retry limits. |
| Monitoring and filters | Input or output filters may block a case before the model responds or modify the observed result. | Where safeguards run and how blocked cases are counted. |
Two evaluations that use the same model name but different harnesses may therefore answer different questions. A score should not be transferred from a sandbox without tools to a production agent with broad permissions.
How do researchers score safety behavior?
Scoring must match the claim. Automated graders can process large test sets quickly, but they may miss subtle leakage, over-refusal or context-specific harm. Human or expert review is slower and more expensive, yet can judge nuance that a keyword rule cannot.
Binary and categorical rubrics
A binary label such as pass or fail is easy to aggregate but can hide severity. A categorical rubric can distinguish a safe refusal, a partial answer, a materially actionable answer and a successful harmful action. For high-impact domains, teams may also record severity, confidence and whether the failure was reproducible.
Pairwise and preference judgments
Evaluators can compare two responses without assigning an absolute score. This is useful for deciding which safeguard is safer or more helpful, but the result depends on the prompt order, evaluator instructions and the distribution of comparisons.
Action-based scoring
For tool-using systems, the final text may look harmless even though an unauthorized action occurred. Scoring should inspect tool calls, state changes, data access and whether the system requested confirmation at the right point.

Rates and uncertainty
A reported failure rate is an estimate over the tested cases. Teams should state the number and type of cases, missing or invalid items, confidence intervals where appropriate, and whether cases were sampled or deliberately constructed. A single aggregate percentage can conceal a severe but rare failure class.
What validity checks can make a safety result misleading?
Before interpreting a score, evaluators examine whether the test measured the intended behavior and whether the model had an unfair advantage or disadvantage.
- Contamination: the model may have encountered benchmark items or close variants during training or tuning, inflating apparent performance.
- Refusal masking: a model may refuse every prompt, making a harmful capability appear absent even though the test never elicited the behavior it was meant to measure.
- Broken or ambiguous tasks: an unclear prompt, impossible target or inconsistent annotation can create failures unrelated to safety.
- Scoring shortcuts and reward hacking: the model may learn to satisfy a superficial grader signal without meeting the underlying safety objective.
- Evaluation awareness: behavior may change when the model recognizes a benchmark pattern or monitoring cue.
- Deliberate underperformance: a system could perform worse in a test setting than in normal use, so unusually low results also deserve investigation.
- Distribution mismatch: carefully crafted attack prompts may reveal an important weakness but cannot by themselves estimate ordinary user risk.
These checks do not eliminate uncertainty. They make the remaining uncertainty visible so decision-makers do not treat a fragile number as a universal property.
How sensitive are benchmark results to small design choices?
Benchmark scores can move even when the underlying model has not changed. Anthropic reported that the Massive Multitask Language Understanding benchmark covers 57 tasks, including areas such as mathematics, history and law, and that simple formatting changes can shift measured accuracy by about 5 percent. Those figures describe benchmark coverage and sensitivity, not safety compliance.
The lesson is practical: preserve the exact prompt format, answer choices, parsing logic and scoring code when comparing runs. If a team changes formatting, it should treat the result as a new measurement rather than as a perfectly continuous trend. A five-point change on one benchmark cannot be generalized into a five-point error rate for all safety evaluations.

How are evaluation findings used to improve safeguards and deployment decisions?
A failure is an input to an engineering and governance loop, not an automatic launch veto or proof that the model is unusable. Teams trace the failure to the most relevant layer and retest after changes.
- Reproduce the behavior. Save the prompt or scenario, model version, context, tool trace, settings and observed output.
- Classify the cause. Determine whether the gap came from training, policy interpretation, prompting, a tool permission, monitoring, the evaluator or the application workflow.
- Choose a mitigation. Options include additional training data, a revised instruction, narrower permissions, confirmation gates, output filtering, monitoring, rate limits or a redesigned task.
- Run regression tests. Check that the fix blocks the discovered exploit without causing unacceptable over-refusal or breaking benign use cases.
- Reassess residual risk. Document what remains untested, how severe the plausible harms are and which safeguards are relied upon.
- Make a scoped decision. Deploy, restrict, delay or require more evidence for the specific use case and configuration examined.
OpenAI’s Preparedness Framework, updated April 15, 2025, links risk categories and safeguard evaluations to operational decisions within that framework. Its thresholds are organization-specific; there is no single industry-wide pass score that certifies safety for every model and application.

What can safety evaluations prove, and what can’t they prove?
Carefully designed evaluations can show how a model behaved on a defined test distribution, in a defined configuration, under a defined scoring method. They can reveal weaknesses, compare systems under equivalent conditions, and measure whether specified safeguards resisted the attacks that were actually attempted.
They cannot establish flawless compliance in every future context. Possible interactions are effectively unbounded, test cases are incomplete samples, and new tools or application changes can create new paths to failure. A result is strongest when its claim is narrow enough to match its evidence: for example, that a particular model with specified permissions resisted a documented set of attacks during a stated evaluation period.

How the main approaches complement one another
| Approach | Primary question | Typical strength | Important limitation |
|---|---|---|---|
| Structured benchmark | Does the system meet predefined criteria on repeatable cases? | Fast, comparable, scalable measurement. | May miss novel attacks and can be sensitive to format or contamination. |
| Human A/B assessment | Which response is safer or more useful to evaluators? | Captures nuanced judgments. | Subjective and costly; depends on rubric and annotator quality. |
| Domain-expert review | Does behavior create specialized real-world risk? | Contextual expertise and severity analysis. | Limited coverage and availability. |
| Red team | Can an adversary find an unanticipated failure? | Discovers novel attack paths. | Search is incomplete and results are difficult to turn into population rates. |
| End-to-end agent test | Does the deployed workflow stay within safety boundaries? | Includes tools, permissions and monitoring. | Results may not transfer to a different harness or product configuration. |
What should a credible evaluation report contain?
A reader should be able to tell exactly what was tested and what the number means. At minimum, look for:
- The behavioral claim and the policy or specification it operationalizes.
- Model identifier, evaluation date and relevant system instructions.
- Prompt or scenario construction, sampling method and held-out procedures.
- Tool access, context, budgets, retries, filters and monitoring.
- Scoring rubric, grader type, adjudication process and treatment of refusals.
- Counts of cases, failures, invalid items and uncertainty or confidence information.
- Contamination checks and discussion of benchmark sensitivity.
- Known limitations, untested contexts and whether results apply to a product configuration.
- The mitigation or deployment decision that followed, without presenting it as a universal certification.
Reports that omit the harness or the claim make it difficult to interpret even an impressive score. A transparent limitation is more useful than a precise-looking number with no scope.
What is the practical takeaway for users and builders?
Ask “safe under which conditions?” rather than “is this model safe?” Builders should evaluate the complete system they intend to deploy, combine routine tests with adversarial probing, preserve reproducibility and retest after every material change to prompts, tools or permissions. Users should treat published scores as evidence about tested conditions, not as a guarantee that an unfamiliar request will be handled correctly.
Google DeepMind’s work on holistic safety and responsibility evaluations reflects the broader direction of the field: methods are diverse, evolving and dependent on collaboration across technical and domain experts. No single benchmark, red-team exercise or policy document can replace a layered evaluation program.

How often should an AI system be safety-tested?
Test after every material change to the model, system prompt, tool permissions, retrieval sources, filters or workflow. Periodic scheduled testing is also useful because new attack techniques and user behavior can expose gaps even when the system has not changed.
Can a model’s refusal rate show that it follows safety rules?
No. A high refusal rate may reflect indiscriminate blocking, while a low rate may reflect useful compliance on benign requests. Evaluators must check whether refusals occur for the right cases and whether the model avoids leaking actionable harmful information.
Are automated safety graders reliable enough on their own?
They are useful for scale and consistency but can miss context, severity and subtle policy violations. Human or expert review, calibration samples and audits for grader shortcuts are needed for high-stakes conclusions.
What should a company do after a red-team jailbreak is found?
Preserve the full trace, reproduce it across relevant configurations, assess realistic impact and identify whether the fix belongs in training, prompting, permissions, monitoring or product design. Then add the exploit to regression tests and verify that the mitigation does not create unsafe side effects.
