AI can help detect coronary plaque, estimate how much an artery is narrowed, and predict whether a narrowing is restricting blood flow—but it does not scan your heart by itself or make a diagnosis without a clinician. CT, MRI, ultrasound, or nuclear cameras first acquire the images. Algorithms then trace vessels, measure heart structures, quantify plaque, analyze motion or blood flow, and flag patterns associated with disease.
For suspected blocked coronary arteries, coronary CT angiography (CCTA) provides an anatomic map. AI can add plaque measurements and, with CT-derived fractional-flow-reserve (CT-FFR) software, a physiologic estimate of whether a lesion is likely to reduce myocardial blood supply. Stress PET, stress echocardiography, and stress cardiac MRI answer a different question: whether the heart muscle becomes under-perfused or dysfunctional when stressed.

AI is an analysis layer applied after a conventional cardiac scan. The scanner still produces the underlying CT, MRI, ultrasound, or nuclear-medicine images. Software may then segment chambers, calculate ejection fraction, evaluate wall motion, measure plaque, estimate blood flow, or detect patterns associated with disease. The American Heart Association’s 2024 scientific statement describes these uses across echocardiography, cardiac CT, cardiac magnetic resonance (CMR), and nuclear imaging.
This distinction matters because an algorithm can only be as reliable as the images and clinical information supplied to it. A qualified clinician must review the images and AI measurements alongside symptoms, medical history, examination findings, and other tests.

Can AI see blocked arteries?
It can help show and measure blockages, especially on CCTA, but “a blockage” and “reduced blood flow” are not identical findings. CCTA shows the coronary lumen and the plaque in the artery wall. A narrowing is an anatomic observation. Ischemia means that the narrowing—or another process—is actually limiting blood flow to heart muscle.
What CCTA reveals
During CCTA, X-ray images acquired with intravenous contrast create a three-dimensional view of the coronary arteries. AI can trace vessels through the scan, separate calcified from non-calcified plaque, calculate plaque volume, estimate the percentage of narrowing (stenosis), and produce risk-related measurements. It may also help clinicians identify disease that is not severe enough to obstruct the lumen but still represents atherosclerosis.

CT-derived fractional-flow-reserve software uses the CCTA data to build a three-dimensional model and simulate pressure and blood flow through the coronary tree. The resulting estimate helps identify lesions that are more likely to reduce blood supply, including cases in which an anatomic narrowing is below 50 percent. The U.S. Food and Drug Administration describes HeartFlow FFRCT as software that analyzes previously acquired coronary CT DICOM data and reports computed blood-flow, pressure, and velocity information to support assessment of coronary artery disease. It is intended for use by qualified clinicians, not as an at-home scan.

Why the distinction changes treatment
A visible narrowing may not be flow-limiting, while diffuse plaque or a lesion that appears moderate can still affect perfusion. Conversely, stress testing can show an abnormal blood-flow response even when a single severe-looking narrowing is not obvious. Clinicians therefore combine anatomic and physiologic evidence rather than treating an AI percentage as an automatic instruction to place a stent.
How does an AI heart scan work from appointment to report?
- Choose the clinical question. A clinician decides whether the priority is coronary anatomy, inducible ischemia, ventricular function, scar, plaque burden, or another concern.
- Acquire the images. The patient undergoes CCTA, CMR, echocardiography, PET, SPECT, or another indicated study. The scanner—not the AI—captures the data.
- Check image quality and patient factors. Heart rate and rhythm, body size, motion, heavy calcification, kidney function, implanted devices, and claustrophobia can affect which modality is suitable and how interpretable the images are.
- Run validated software. The algorithm processes the image files, often producing vessel centerlines, chamber measurements, plaque categories, perfusion metrics, or a CT-FFR model.
- Review the output. A qualified reader checks segmentation, measurements, artifacts, and clinically unusual findings. The 2023 real-world CCTA study explicitly concluded that implementation is feasible but does not replace expert review and clinical correlation.
- Integrate the evidence. The final assessment combines the AI results with symptoms, risk factors, prior studies, examination, and treatment goals. Further testing or invasive angiography may be needed when uncertainty remains or an intervention is being considered.
Which cardiac imaging test should answer the question?
No single modality is best for every patient. The practical choice depends on whether the clinician needs anatomy, physiology, muscle function, scar characterization, or a combination, as well as radiation, contrast, and patient-specific constraints.
| Modality and AI role | Main question answered | Useful information | Important constraints |
|---|---|---|---|
| CCTA with AI | What do the coronary arteries and plaque look like? | Lumen, calcified and non-calcified plaque, plaque volume, estimated stenosis; CT-FFR can add simulated pressure and flow. | Image quality can fall with morbid obesity, high or irregular heart rate, and severe calcification; CT uses radiation and commonly requires iodinated contrast. |
| Stress PET | Does stress reduce myocardial perfusion? | Perfusion, myocardial blood flow, and flow reserve; can support ischemia and risk assessment. | Uses ionizing radiation and requires a suitable stress protocol. In the FDA’s 2024 Flyrcado documentation, sensitivity for CAD defined as at least 50% narrowing ranged from 74% to 89% and specificity from 53% to 70%. |
| Stress SPECT | Does the heart muscle become under-perfused during stress? | Relative perfusion patterns and risk information. | Uses radiation; attenuation and image quality can affect interpretation. |
| Stress echocardiography | Does stress cause abnormal contraction or wall motion? | Dynamic ventricular function and stress-induced wall-motion abnormalities. | Results depend on acoustic windows and image quality, which can be challenging in some body types. |
| CMR, including stress CMR | Is there ischemia, abnormal motion, or prior infarction? | Wall motion, perfusion, ventricular function, tissue characterization, and infarct (scar) assessment. | Device-safety screening and claustrophobia matter; availability and scan time vary by center. |
ACC/AHA criteria recognize CCTA for nonobstructive and obstructive plaque, CT-FFR for ischemia assessment, CMR for wall motion, ischemia and infarction, and PET for myocardial blood flow and reserve. For patients with persistent symptoms who need ischemia testing, the chronic-coronary-disease guideline lists stress PET/SPECT, CMR, or stress echocardiography as appropriate options.

How accurate and fast is AI-assisted CCTA?
A prospective randomized European Radiology study published in 2024 compared fully automated and semi-automated CCTA processing in 1,801 adults. The automated workflow was substantially faster while meeting the study’s prespecified non-inferiority criterion for diagnosing obstructive coronary artery disease.
| Measure | Fully automated processing | Semi-automated processing | What the result means |
|---|---|---|---|
| Mean image-processing time | 121.0 ± 18.6 seconds | 433.5 ± 68.4 seconds | Automation reduced processing time in this prospective study. |
| Diagnostic accuracy for obstructive CAD | 94.7% | 94.3% | The difference met the study’s non-inferiority criterion; it does not mean every patient will receive a 94-percent-accurate result. |
A separate 2023 real-world study of 532 patients found that AI-assisted CCTA could quantify calcification, plaque types, and plaque volume. Combining CCTA with FFRCT identified physiologically significant disease even when anatomic stenosis was below 50 percent. The investigators still required expert review and clinical correlation.
These figures describe specific study populations, software, readers, and image-quality conditions. They are not a universal guarantee for every scanner, hospital, patient, or disease pattern. Poor-quality or unusual images can reduce reliability, and algorithms can inherit biases from the data used to train them.

What can make an AI cardiac result unreliable?
- Motion and rhythm: A high or irregular heart rate can blur coronary images and make vessel measurements less dependable.
- Heavy calcification: Calcium can create blooming artifacts that obscure the true lumen and complicate stenosis estimates.
- Body habitus: Morbid obesity can lower CCTA image quality and affect other modalities differently.
- Incomplete or atypical anatomy: Algorithms trained on common patterns may perform less predictably with unusual anatomy, prior procedures, or rare disease.
- Technical artifacts: Poor contrast timing, motion, low signal, or missing image data can produce incorrect segmentation or unusable CT-FFR calculations.
- Population bias: Performance may vary when a patient differs from the populations represented in software development and validation.
When a result conflicts with symptoms or another test, the answer is not to accept the algorithm blindly. The interpreting clinician may inspect the source images, compare prior studies, repeat imaging with a more suitable technique, or order a physiologic or invasive test.
Is AI cardiac imaging safe?
The safety issues come mainly from the underlying modality and contrast agent, not from the algorithm itself. CT and nuclear imaging involve ionizing radiation; CCTA commonly involves iodinated contrast. MRI requires screening for device compatibility and can be difficult for people with claustrophobia. Stress studies also require a supervised stress protocol.
Before scheduling, tell the care team about kidney disease, previous contrast reactions, pregnancy, implanted devices, irregular heartbeat, inability to lie flat, or severe claustrophobia. The team can then weigh alternatives and determine whether the expected diagnostic benefit justifies the scan’s risks and preparation.
Best Value
When is invasive coronary angiography still needed?
Invasive coronary angiography remains important when the coronary anatomy must be confirmed directly, when noninvasive tests disagree, or when a procedure is being planned. A CCTA or CT-FFR report can guide that decision, but it does not itself deliver an intervention. In urgent or high-risk situations, clinicians may proceed directly to an invasive pathway rather than waiting for an AI-enhanced analysis.
What should patients ask before an AI-assisted heart scan?
- Which question is this test intended to answer: plaque and narrowing, inducible ischemia, heart-muscle function, or scar?
- Will the study use radiation or contrast, and are there kidney, pregnancy, allergy, device, or claustrophobia concerns?
- What happens if the images are nondiagnostic or the AI measurement conflicts with my symptoms?
- Who will review the original images and sign the final report?
- Could the result change medication, prompt another noninvasive test, or lead to invasive angiography?
The American Heart Association noted in 2024 that few AI tools had yet demonstrated enough improvement in cardiovascular and stroke care to become widely adopted. That caution supports using AI as decision support within a clinical workflow—not as a stand-alone consumer diagnosis.
Frequently Asked Questions
Can AI cardiac imaging detect a previous heart attack?
Yes, cardiac MRI can assess infarction and scar as well as wall motion and ischemia. Whether MRI is appropriate depends on the clinical question, device compatibility, availability, and the patient’s ability to complete the examination.
Is AI cardiac imaging available at every hospital?
No. Availability depends on local scanners, software clearances, trained readers, and integration with the hospital’s imaging system. A regulated tool may be authorized for clinical use without being installed at every imaging center.
Can a patient order an AI artery scan directly online?
The regulated workflow described for CT-FFR requires previously acquired clinical CT data and interpretation by qualified clinicians. It is not an at-home product or a substitute for a medical evaluation.
What if my AI result says a blockage is less than 50 percent?
That percentage describes an anatomic narrowing and does not by itself prove that blood flow is normal. Clinicians may use symptoms, stress imaging, CT-FFR, and risk factors to decide whether the lesion is clinically significant.
