AI is already changing insurance claims, but it has not removed human judgment. Insurers now use software to read forms, emails, photographs and scans; machine-learning models to rank severity and fraud risk; and generative AI to summarize files and suggest next actions. Straightforward claims may move through near-automatic processing, while bodily-injury, disputed-liability, vulnerable-customer and unusual cases are routed to specialists who remain accountable for the decision.
The practical result is earlier handling of routine work and better prioritization of complex work. The strongest published examples show large reductions in cycle time, complaints and manual review, but they are case studies rather than universal benchmarks. Whether faster processing is acceptable depends on controls for accuracy, fairness, privacy, cybersecurity, explainability and appeals.

AI is applied as a series of connected layers rather than as one all-powerful claims robot. Each layer handles a different bottleneck, and the output of one becomes structured evidence for the next.
1. First notice of loss and document intake
When a claimant submits a form, email, photograph, PDF, scan or handwritten note, document-AI systems identify fields, read text and images, and place the information in a structured claim record. They can check whether required sections are missing, compare the loss description with policy details, and flag contradictory dates, names or amounts before an adjuster opens the file.

Direct Pojišťovna used Azure AI Foundry and Document Intelligence for windshield claims. Its system reads forms, photos, reports, scans, emails and legal documents, then validates the extracted information. The narrow scope matters: windshield claims have variable documents but a relatively bounded decision space, making them safer for early automation than disputed liability.
2. Triage and routing
Models classify probable severity, coverage complexity and the type of expertise required. A low-value claim with complete evidence may enter straight-through processing; a claim involving bodily injury, contested fault, a vulnerable customer or unusual circumstances should be routed to a specialist. Triage does not decide who is right. It decides what attention the file needs and how quickly it should receive it.

3. Fraud and anomaly detection
Machine-learning models look for patterns that are difficult to see across thousands of files: repeated participants, linked addresses, unusual timing, inconsistent damage descriptions, suspicious repair relationships or claim values that diverge from comparable cases. Network analysis can connect claims, people, vehicles, providers and policies, while investigators decide whether a pattern warrants contact or further evidence.
4. Assessment and decision support
Generative AI can summarize a long claim file, extract the policy clauses that matter, identify missing evidence and propose next actions. A reliable design retrieves information from approved policy and claims sources, shows the underlying documents and records the model version used. The recommendation supports an accountable adjuster; it should not silently determine disputed liability or a denial.
5. Settlement, recovery and communication
Once a decision is approved, automation can prepare payment instructions, draft status updates, route a vehicle or property repair, and identify possible subrogation or recovery leads. Swiss Re says its ClaimsGenAI tool screens corporate claims, extracts and validates information, produces concise summaries and highlights irregularities or recovery opportunities while claims experts remain in control.
Can AI approve an insurance claim?
It can approve some tightly defined, low-risk claims under rules set by the insurer, but most consequential decisions still require a human sign-off. The distinction is between straight-through processing for predictable cases and autonomous judgment about disputed or high-impact cases.

Where automatic approval can fit
- Complete documentation and a clearly active policy.
- A bounded product such as a simple windshield repair with established payment rules.
- Damage, amount and coverage that fall within pre-approved thresholds.
- No fraud, identity, vulnerability or liability indicators requiring investigation.
Direct Pojišťovna reported that per-claim handling for windshield losses fell from 15 minutes to about two minutes and that 60% of those claims were fully automated. The company also described a human-in-the-loop as essential, so the figure should not be read as proof that all claims can be approved without review.
Cases that should remain human-led
- Bodily-injury and personal-injury claims.
- Disputed liability, coverage exclusions or conflicting evidence.
- Claims involving children, elderly people, disability, financial distress or other vulnerability.
- Large commercial losses, catastrophic events and unusual policy wording.
- Denials, suspected fraud referrals and any decision likely to cause serious financial harm.
The UK Financial Conduct Authority and Bank of England reported in 2024 that 75% of surveyed financial-services firms used AI, 55% of use cases involved some automated decision-making, and only 2% were fully autonomous. That distribution is consistent with assistive and supervised automation being much more common than independent approval.
What evidence shows that AI speeds up claims?
Published results show meaningful gains, but each number belongs to a particular insurer, product, geography and operating model. They are not guarantees for every implementation.
| Organization and source period | Reported result | How to interpret it |
|---|---|---|
| Direct Pojišťovna, Microsoft customer story, 2026 | Windshield-claim handling fell from 15 minutes to about 2 minutes; 60% of those claims were fully automated. | A narrow line of business with human oversight, not an all-claims benchmark. |
| Aviva case study reported by McKinsey | More than 80 models; 23 fewer days to assess liability on complex cases; 30% better routing accuracy; 65% fewer complaints; staff trained for more than 40,000 hours. | An operating-model redesign combining digital and human paths, not model deployment alone. |
| Swiss Re, live since mid-2024 | ClaimsGenAI screens more than 40,000 corporate claims annually. | An assistive tool for unstructured commercial claims; experts retain responsibility. |
| EIOPA, 2024 | 50% of non-life insurers and 24% of life insurers reported using AI in the insurance value chain, including claims and fraud work. | Adoption is substantial but varies by line of business and use case. |
| Shift Technology customer story published by Microsoft, 2025 | More than 2.6 billion policies, claims and supporting documents analyzed. | A vendor-reported scale figure, not an independently audited savings or accuracy rate. |
Aviva’s case is especially instructive because the reported gains came with a “double helix” operating model: digital and human paths could switch as evidence changed, and personal-injury claims defaulted to human interaction. The lesson is that process redesign, training and escalation rules matter as much as model quality.

Fraud analytics combines individual-claim scoring with network analysis and investigator tools. A model may flag an implausible repair estimate, but the more powerful signal can be a relationship among many otherwise ordinary claims.
From anomaly score to investigation
- Collect signals. The system reads claim documents, policy history, payment data, images, provider details and relevant external information permitted by law.
- Score and connect. Models estimate the likelihood of an anomaly and link people, addresses, vehicles, businesses, witnesses and prior claims into networks.
- Prioritize. Investigators receive ranked cases, explanations of the signals and the documents that triggered them.
- Verify. A human contacts parties, requests evidence and determines whether the claim is legitimate, exaggerated or fraudulent.
- Record the outcome. Confirmed, cleared and inconclusive cases feed monitoring and help detect false-positive patterns.
The economic incentive is large. The Association of British Insurers said £1.16 billion in fraudulent UK general-insurance claims were identified in 2024, covering more than 98,400 fraud-related claims. Aviva reported stopping more than 12,700 suspect claims worth £127 million in 2024. Those are detected or prevented amounts, not an estimate of all fraud.

The National Insurance Crime Bureau’s 2025 annual report describes an early workers’ compensation model, a questionable-claims network model and a large-language model for investigative workflows. These tools surface leads; they do not establish criminal intent. Treating a risk score as proof can unfairly delay or deny a legitimate claim.
Why might an AI-assisted claim still take a long time?
Automation removes data-entry work, but a claim can remain slow when evidence is incomplete, systems cannot exchange data or a case requires specialist judgment.
Recommended Free Tools
- Missing or poor-quality evidence: blurry photographs, unreadable scans, conflicting dates or absent invoices force requests for clarification.
- Legacy integration: policy, claims, payment, repair and customer-service systems may use incompatible formats or lack reliable interfaces.
- Complex coverage: multiple policies, exclusions, third-party liability and bodily injury require legal and medical review.
- Fraud escalation: a network alert can add investigation steps even when most claims are legitimate.
- Human capacity: severe weather or catastrophe events can overwhelm adjusters after automated intake has finished.
- Quality assurance: insurers may hold an automatically prepared payment for a mandated review or a confidence check.
The Lloyd’s Market Association reported data-quality or availability concerns among 49% of respondents and integration problems among 46%. Better models cannot compensate for missing data contracts, inconsistent document standards or disconnected core systems.
Will AI replace claims adjusters?
Current evidence points to role redesign, not elimination. Adjusters spend less time transcribing and searching, and more time interpreting evidence, communicating with claimants, handling exceptions and taking responsibility for outcomes.
In Aviva’s case study, personal-injury claims defaulted to human interaction while digital tools handled routing and information work around them. Swiss Re likewise keeps claims experts responsible for corporate-claims decisions. Human-in-the-loop is not merely a safety slogan: it requires authority to override a model, clear escalation routes and enough staffing to review the cases that automation deliberately excludes.
New specialist work also grows around the models: data-quality management, model-risk review, fraud investigation, privacy, cybersecurity, vendor oversight and audit. An insurer that reduces adjuster headcount without funding these controls can increase operational and regulatory risk.

Is AI fair and safe in claims decisions?
Fairness cannot be inferred from speed or average accuracy. An insurer must check whether error rates, referral rates, settlement times and appeal outcomes differ across claim types and protected characteristics where lawful to measure.
| Risk | Control that a responsible insurer should require |
|---|---|
| False positives and missed fraud | Track precision, recall, calibration, cleared referrals and appeal outcomes by product and segment. |
| Unaccountable automation | Set mandatory human review for injury, vulnerability, disputed liability, denials and unusual claims; preserve override rights. |
| Unclear explanations | Log source documents, model version, evidence or features used, recommendation, override and final rationale. |
| Privacy and discrimination | Limit data access, test disparate outcomes, define retention periods and provide a usable explanation to the claimant. |
| Cybersecurity and vendor dependence | Perform third-party due diligence, enforce least-privilege access, maintain incident plans and monitor model changes. |
| Generative-AI errors | Use retrieval from approved sources, confidence thresholds, deterministic payment rules and human sign-off for consequential actions. |
The U.S. Treasury’s Federal Insurance Office said in September 2024 that insurers use AI in claims processing, underwriting, marketing, fraud detection and rating. Treasury roundtables have also focused on discrimination and privacy protections. OSFI-FCAC identifies cybersecurity and third-party dependencies as growing risks. These concerns make governance part of claims performance, not a separate compliance exercise.

A staged program limits harm and makes it possible to prove value before expanding scope.
- Choose a constrained use case. Start with a line where documents and decisions are varied enough to matter but bounded enough to review, such as a defined windshield workflow.
- Map the human process. Document every intake field, policy check, handoff, exception and approval. Decide which outcomes are assistive, recommendatory or eligible for straight-through processing.
- Set evidence and data standards. Define accepted formats, source systems, retention, identity checks and the minimum evidence required for each decision.
- Build evaluation before launch. Measure precision, recall, calibration, cycle time, cost, leakage, complaints, overrides and appeals against a representative historical set.
- Design mandatory review. Encode automatic escalation for injury, vulnerability, disputed liability, denial, low confidence and high-value losses.
- Run a controlled rollout. Compare assisted and existing workflows, monitor drift and review errors by product, geography and relevant protected characteristics.
- Expand reusable components. Document extraction, policy retrieval, audit logging and escalation services can move to property, liability or pet claims only after their limits are understood.
- Govern vendors continuously. Contract for data residency, access controls, incident notification, model-change notices, audit rights and exit plans.
How should buyers compare claims-AI systems?
Price or a large model count is a poor shortcut. Compare the operating fit and evidence behind each capability.

| Question | What to examine |
|---|---|
| Which claims? | Line of business, geography, severity range and complexity supported. |
| How much automation? | Assistive summaries, recommendations, conditional straight-through processing or fully autonomous actions. |
| What evidence can it read? | Forms, handwriting, PDFs, emails, photographs, scanned reports, audio or video, with confidence scores. |
| How does it find fraud? | Individual anomaly scoring, entity resolution, network analytics, investigator workflow and feedback loops. |
| Does it integrate? | Policy administration, claims, payments, CRM, repair networks, document storage and identity services. |
| Can decisions be explained? | Source citations, model versioning, audit trails, override controls and claimant-facing reasons. |
| What outcomes are measured? | Cycle time, cost, accuracy, complaint rate, leakage, referral quality and appeal results, with the source and date. |
| Where can it operate? | Regulatory obligations, data-residency limits, service availability and rules for cross-border processing. |
Ask whether a claimed improvement is vendor-reported, independently audited or a named case study. A figure such as “23 fewer days” is meaningful only when its baseline, claim population and process changes are disclosed.
What should policyholders expect from an AI-assisted claim?
A claimant may receive faster acknowledgements, fewer requests to retype information and more accurate status updates. They should still be able to reach a person, correct a misread document, submit additional evidence and challenge a decision.

Keep original photographs, receipts, correspondence and timestamps. If an insurer cites an automated flag or requests clarification, ask what information is missing, how to provide it securely and how to request human review. Do not assume that a delay proves fraud suspicion; incomplete evidence, specialist review or system integration can cause the same symptom.
Frequently Asked Questions
What information does an insurer’s claims AI usually read?
Depending on the product and consent rules, systems may process policy records, claim forms, emails, photographs, scanned reports, invoices, repair estimates and prior-claim relationships. The insurer should tell you which categories it uses, why they are needed and how long they are retained.
How can I challenge an automated claims decision?
Ask for the decision reason, the evidence considered and the route to human review or appeal. Provide corrections with the original document and a clear explanation of the discrepancy; keep a dated record of every submission and response.
Does a fraud score mean the insurer has accused me of fraud?
No. A score is a triage signal indicating that a claim deserves additional verification. A trained investigator must examine the underlying facts, and legitimate claims can be cleared after that review.
What happens to claims data sent to an external AI provider?
The insurer remains responsible for contractual controls over access, storage, security, retention and model training. Ask whether your data stays in the required region, whether it is reused for provider training and how a breach or supplier failure would be handled.
