When an Xbox achievement does not unlock or appears stuck, start with reversible checks: confirm the achievement’s exact requirements, use the intended Xbox/Microsoft account, verify that the console or PC is online, then fully quit and relaunch the game. If it remains missing, sign out of the Xbox account, reboot the device, sign back in, and repeat the requirement while connected. A missing toast does not necessarily mean the achievement was lost; Xbox services can take time to display a record, and offline unlock requests may queue for later synchronization. Is the achievement actually missing, or is only the notification missing?

- No toast, but the achievement is listed as unlocked: The notification layer or game display was late or suppressed. You do not need to replay the activity.
- The game shows progress, but the Xbox record does not: The title may have stored local progress without successfully submitting the unlock.
- Neither progress nor the record changed: Recheck the requirement, mode, save, account, and connection before assuming a service fault.
Checking another Xbox client is useful because it distinguishes a display problem from an account-level problem. Do not repeatedly attempt a difficult challenge solely because a toast was absent.
What should I check before changing anything?
Use this order. It avoids deleting data or resetting the console before you know whether the requirement, account, network, or game is responsible.
- Read the requirement again. Confirm every condition, including the required mode, difficulty, save, character, collectible order, event window, or multiplayer setting. If even one condition was missed, repeating the activity is the correct fix.
- Confirm the account. Make sure the Xbox/Microsoft profile that earned the achievement is the profile currently signed in. On Windows, check both the game’s account and the account shown in the Xbox app or Xbox Game Bar.
- Confirm connectivity. Check that the device can reach Xbox services. A game may continue running while achievement requests fail because the service connection is unavailable.
- Fully quit the game. Do not rely only on suspended play or Quick Resume. Close the title, then launch it again so it can submit or process its achievement state from a fresh session.
- Repeat the requirement while online. If the first attempt happened offline or during a service interruption, perform the trigger again after reconnecting.

How do I refresh an achievement on an Xbox console?
The safest console recovery sequence is to close the game, refresh the account session, and try again without deleting saves.
- Press the Xbox button, open My games & apps, choose See all, highlight the affected game, press the Menu button, and select Quit. If the title appears in Quick Resume, remove it from the suspended session before relaunching.
- From the console’s profile menu, choose the option to sign out of the Xbox account that earned the achievement.
- Restart the console from the power menu. A normal reboot is the intended refresh step; do not begin by deleting local saves or resetting the console.
- After the main screen returns, sign back into the same Xbox/Microsoft profile.
- Launch the game, wait for its online sign-in to complete, and repeat the exact achievement requirement.
- Check the achievement list again. If it is still absent, install any pending game update and repeat the attempt after the update has finished.
Bethesda Support recommends signing out, rebooting, and signing back in when an achievement list has not synchronized, and notes that rebooting the game can re-enable achievement processing. Microsoft’s certification requirements also expect titles to unlock achievements after reconnecting to Xbox services and after resuming from connected standby. If a title repeatedly fails those conditions, more destructive console resets are unlikely to be a sensible first response.
This sequence is low risk because it does not alter save files or account ownership. It refreshes the session that submits the achievement and gives the game a clean opportunity to process queued state. If several games show the same symptom, perform the account and service checks before spending time replaying a particular mission.

- Open the Xbox app and verify the profile shown in its account menu.
- Open Xbox Game Bar with Win + G and check that its account is the same one.
- Inside the game, open its account or online-services menu. If it offers a Microsoft account sign-out option, sign out and sign back in with the intended profile.
- Exit the game completely rather than leaving it suspended in the background. Relaunch it after the account sign-in has completed.
- Confirm that the PC is online, install the game’s pending updates, and repeat the requirement.
- Look for the achievement in another Xbox client or in the game’s achievement view. A missing toast alone does not establish that the record is missing.
Halo Support specifically advises signing out of the Microsoft account in-game and signing back in to trigger synchronization of previously earned achievements. Cross-platform titles can also have separate local profiles, so verify the account inside the title as well as in Windows.
If an achievement appears on PC but not on the console, do not immediately replay it on both devices. Confirm that both devices use the same Microsoft/Xbox identity, reconnect each device, and check the record from a third client when possible. The discrepancy can be a propagation delay rather than a second unlock requirement.
What happens if I earned the achievement while offline?
Offline behavior depends on how the title implements achievements. Microsoft’s Game Development Kit documentation states that when a title unlocks an achievement while the device is offline, the API automatically queues the offline unlock requests. Microsoft’s API reference further explains that offline updates on PC and Xbox One can be posted after the connection is restored, even if the title is no longer running.

That does not guarantee that every title will recover every offline event. The practical sequence is:
- Reconnect the console or PC to the internet.
- Leave the device connected long enough for Xbox services to accept queued requests.
- Relaunch the game and allow its online sign-in to finish.
- Check the achievement record in the game or another Xbox client.
- If it remains absent, repeat the requirement while online.
Achievements do not unlock while a user is offline in the normal service flow. A title that queues an event may synchronize it later, but implementation and game state still matter.
Do not interpret “queued” as a promise that every offline event is recoverable. A game must implement the relevant offline behavior correctly, and the request still has to reach Xbox services after reconnection. If the first online check does not show the record, relaunch once, verify the account, and make a fresh online attempt rather than repeatedly toggling network settings.
How can I tell whether Xbox services are having an outage?
Check the official Xbox service-status page before repeating a difficult challenge or changing local data. Xbox is made up of multiple services, so an achievement-related service can have trouble while other features, such as browsing the store or joining a party, continue to work.

If the status page reports an incident, wait until the affected service is restored, then relaunch the game and retry the requirement. If no incident is listed, a short-lived propagation delay is still possible; status pages do not prove that every account request has completed immediately.
Service status is a diagnostic clue, not a guarantee. An all-clear does not prove that a particular title submitted its unlock, and a partial incident may affect only achievement traffic. Record what the page showed when you tested so the publisher can distinguish a local reproduction from a wider service event.
Should I update the game before trying again?
Yes, especially when only one title is affected. Install the game’s current update, restart it, and perform a fresh attempt under the documented conditions. A patch can correct achievement logic, event flags, or synchronization calls that a player cannot repair locally.
Use a new attempt or another save when appropriate, but do not delete a valuable save merely to experiment. First preserve the evidence of what happened: achievement name, criteria, platform, game version, account used, online state, and the result after relaunching and signing in again.

When is the problem probably the game rather than Xbox?

How do I report a broken achievement to the publisher?
Once account, network, relaunch, sign-in, update, and retry steps have been exhausted, use the game publisher’s official support channel. Include enough detail for the developer to reproduce the failure:

- The exact achievement name and its full description.
- The platform: Xbox console, Windows PC, or another supported device.
- The game version or build shown by the title.
- The Xbox/Microsoft account and platform combination used, without sending your password or security codes.
- The save, mode, difficulty, event window, and sequence of actions that satisfied the requirement.
- Whether the device was online, whether an outage was listed, and whether the achievement was attempted offline.
- What happened after fully quitting, relaunching, signing out, rebooting, signing back in, and updating the game.
- A screenshot or clip showing the completed criteria, if the publisher’s support form accepts attachments.
Quote the achievement exactly and explain whether it is absent from the account record or merely missing its notification. The publisher may need to ship a title update. Do not assume a support agent can add the achievement manually: Bethesda Support states that its customer-support team cannot unlock achievements.
A clear report should also say whether another achievement in the same game unlocked normally. That comparison helps identify a single broken trigger rather than a general account problem. Use the publisher’s official form or support site, not an unverified service that asks for account credentials.

Account refresh, reconnecting, relaunching, and repeating the requirement are reversible and have a broad evidence base. Deleting local data, clearing a cache, changing an alternate MAC address, reinstalling the game, or resetting the console can be disruptive, and the official sources do not establish any of them as a universal achievement fix.
- Do not delete saves as a first step. A local reset can remove useful evidence or progress without repairing a server-side record.
- Do not promise a fixed waiting period. Synchronization is eventually consistent, but no universal maximum delay is documented.
- Do not treat a missing pop-up as proof of failure. Check the achievement record in another client.
- Do not use cheat codes, console commands, or paid shortcuts. Microsoft’s requirements prohibit alternative direct-unlock options that bypass the corresponding gameplay activity.
- Do not buy network hardware expecting it to unlock an achievement. A Cat 6 Ethernet cable can help diagnose an unreliable wireless connection, but Microsoft does not present a particular cable as an achievement remedy.
If your only concern is an unstable connection, a Cat 6 cable is a reasonable, low-cost diagnostic accessory for testing a wired path. It can show whether network availability is part of the problem, but it cannot repair a title’s achievement logic or guarantee synchronization.

- Read the achievement description and verify every condition.
- Confirm the correct Xbox/Microsoft profile in the game and, on PC, in the Xbox app and Game Bar.
- Verify that the device is online and check the official Xbox service-status page.
