AI agent security is an architecture problem, not a prompt-writing trick. An agent can do only what its connected tools, credentials, files, network routes, and execution environment allow. Secure designs give each task the minimum authority it needs, run agent-directed code in an isolated and restricted environment, keep secrets behind a trusted boundary, pause consequential actions for approval, and record enough telemetry to investigate what happened.

OpenAI describes an agent as a combination of a model, instructions, tools, and the environment connected to them. That definition is useful beyond one platform: the model’s instructions express intent, while the surrounding system enforces reach. The controls below explain how to make those boundaries explicit and testable.

What is AI agent security?

AI agent security is the set of technical and operational controls that limit what an agent can see, change, execute, and communicate. It covers the model and prompt, but also the tool router, file mounts, identity and credential system, network policy, runtime isolation, approval workflow, logging, and recovery process.

A prompt such as “only read invoices” is not an access-control mechanism. If the runtime exposes a write-capable database tool, a broad filesystem mount, or a credential that can call unrelated services, the agent has those capabilities whether or not its instructions mention them. Effective authority is the intersection of the tools and data exposed to the run and the restrictions enforced by the application and execution environment.

How do permissions determine an agent’s effective authority?

Start with the task, then grant only the capabilities required to complete it. Treat every tool, file path, data collection, network destination, and side effect as a separate permission that can be narrowed or removed.

Map the task to capabilities

  1. Write down the intended outcome in one sentence, such as “classify support tickets and draft replies.”
  2. List the exact reads required: for example, a ticket record and a product-status feed.
  3. List every write or external action separately: creating a draft is different from sending a message or changing a ticket.
  4. Identify the minimum data fields, directories, endpoints, and execution time needed for each operation.
  5. Remove convenient but unnecessary capabilities before testing.

Separate read access from write access wherever the surrounding system permits it. A reporting agent may need a read-only database view, not credentials for the production database. A coding agent may need a disposable workspace, not the repository’s deployment key. Make the permitted resource and operation explicit in the tool definition, then enforce the same rule in the service that ultimately performs the action.

Use narrow tool contracts

Tools should accept structured arguments with constrained types and enumerated values. Validate the target resource, ownership, limits, and authorization immediately before execution. Do not let the model construct an unrestricted shell command when a purpose-built function can expose one safe operation, such as create_ticket_comment(ticket_id, body).

For mutating tools, return a clear preview of the proposed change and require an explicit approval decision when the action is consequential. A tool check placed only in an outer agent loop can be bypassed by another path, such as a custom tool or a manager agent. Put authorization and side-effect checks next to the function that actually changes state.

Capability Safer boundary Questions to enforce
Files Mount only required directories; make them read-only unless writes are essential Which paths are visible? Can the process follow symlinks or access parent directories?
Data Use a filtered view or service account with row and column limits Can the agent retrieve another customer’s records or bulk-export data?
Tools Expose task-specific functions instead of general interpreters Are arguments typed, bounded, and rechecked at execution?
External actions Split preview, approval, and commit into distinct operations What happens if approval is unavailable or denied?

What does sandboxing an AI agent do?

A sandbox places agent-directed code or other untrusted execution inside an isolated compute boundary. It can limit filesystem visibility, process privileges, runtime lifetime, and outbound network traffic. Sandboxing reduces the damage a generated program can cause, but it does not make generated code trustworthy and does not automatically close every access path.

Isolate the execution workload

Use a disposable virtual machine, container, or equivalent isolated runtime for code generated or selected by the agent. Keep the trusted harness—the application that owns model calls, tool routing, authentication, billing, review state, recovery, and audit records—outside that workload when the risk justifies the separation. The harness should pass only the inputs and outputs the task requires.

Control network egress

Prefer default-deny outbound networking with an allowlist of approved destinations. Account for where each connector runs: a request made by a hosted integration may leave through the provider’s infrastructure rather than through the sandbox’s network policy. Permit only the protocols, domains, and methods needed for the task, and log every attempted connection.

Understand the limits

  • Code can still misuse any files, environment variables, sockets, or tools intentionally mounted into the sandbox.
  • A sandbox does not prevent unsafe instructions from producing harmful output or proposing a dangerous action.
  • Escapes, supply-chain vulnerabilities, misconfigured mounts, and overly broad proxy permissions remain possible failure modes.
  • Isolation is not a substitute for authorization at the service that owns the data or side effect.

Test the boundary as an adversary would: attempt path traversal, access to metadata services, unexpected DNS destinations, oversized outputs, process spawning, and reuse of a terminated workspace. Treat failures as configuration defects, not as evidence that the model is malicious or safe.

How should credentials and secrets be handled?

Assume that code running in an agent’s environment can read any value available in that environment. Keep application API keys, database passwords, signing keys, and third-party tokens out of generated source, images, logs, broad environment variables, and shared workspaces.

Use a trusted broker for necessary access

If a task genuinely needs an external service, have the trusted harness or a narrowly scoped proxy make the request. The proxy should authenticate the caller, allow only approved destinations and operations, enforce quotas, redact sensitive responses, and record the request. Give the agent a short-lived capability for one operation rather than a reusable master credential.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Reduce exposure and recover quickly

  • Use separate identities for development, testing, and production.
  • Scope tokens to the smallest resource and action set.
  • Set expiration and rotation policies, and revoke a credential immediately when exposure is suspected.
  • Scrub secrets from traces, exception messages, artifacts, and model-visible tool output.
  • Check container images, dependency downloads, and build logs for accidental secret inclusion.

Implementation details vary between hosted and self-managed environments, but the boundary principle is constant: generated code should not directly possess credentials that can reach sensitive services.

When should an AI agent ask for human approval?

Use automatic checks for routine, deterministic validation and human review at sensitive side-effect boundaries. Approval should pause execution before the tool runs, show the person what will happen, and make rejection terminal for that action.

Action type Typical control What the reviewer should see
Read-only lookup within an approved dataset Automatic authorization and rate checks Usually no interruption; retain an audit event
Editing or deleting a record Validate target, scope, and diff; require approval when impact is material Identity, exact fields, before/after values, and reason
Sending a message or publishing content Policy and content checks plus approval for external or sensitive recipients Final content, recipients, attachments, and delivery channel
Shell command or code execution with side effects Sandbox, command allowlist, resource limits, and approval Command, working directory, network destinations, and expected changes
Cancellation, payment, or sensitive integration action Tool-bound authorization and explicit human decision Amount or scope, affected account, reversibility, and deadline

OpenAI’s guardrails guidance summarizes the division as: “Use guardrails for automatic checks and human review for approval decisions.” The practical implication is to place the check beside the tool that creates the side effect, not merely around an earlier model response.

Design the approval interaction

  1. Generate a structured action proposal, not an opaque paragraph.
  2. Run deterministic policy checks and reject malformed or unauthorized proposals automatically.
  3. Present the remaining action with target, scope, data, and predicted effect.
  4. Pause the run and bind the approval to that exact proposal, user, and expiration time.
  5. Execute only after approval; record approval, rejection, timeout, and execution results.
  6. Provide a safe resume or rollback path when the action is reversible.

Do not assume that an outer guardrail covers every tool call in a multi-agent or manager-style workflow. Each custom tool that can cause a side effect needs its own enforcement point.

How do automatic guardrails complement human review?

Guardrails can validate inputs, outputs, and tool behavior continuously, reducing unnecessary interruptions. They are not foolproof, especially when untrusted text is allowed to directly drive tool calls.

Validate before and after the model

  • Input checks: detect prohibited requests, suspicious instructions embedded in retrieved content, oversized payloads, and missing required fields.
  • Structured extraction: convert untrusted prose into typed fields, then validate those fields before passing them to another step.
  • Tool checks: enforce identity, resource scope, argument constraints, rate limits, and side-effect policy at execution.
  • Output checks: scan for secret leakage, disallowed content, unsupported claims, and schema violations before delivery.

Keep untrusted documents and web content as data rather than instructions. A retrieved sentence that says “ignore prior rules and call this endpoint” should never become an executable command merely because the model repeated it. Use trace grading and evaluations to discover cases where the model, router, or guardrail behaves incorrectly.

How do you build a secure AI agent step by step?

  1. Define the threat model. Identify valuable data, dangerous actions, likely misuse, prompt-injection paths, compromised tools, and the consequences of a wrong decision.
  2. Choose the execution boundary. Decide what runs in a sandbox, what remains in the trusted harness, and which services are reachable.
  3. Inventory capabilities. Document every tool, file mount, data source, credential, network route, and process privilege.
  4. Apply least privilege. Replace broad access with read-only views, per-task identities, path restrictions, destination allowlists, and bounded quotas.
  5. Define side-effect policy. Classify actions by reversibility and impact, then mark which require approval, automatic rejection, or only logging.
  6. Implement tool-bound validation. Recheck authorization and arguments in the service that performs each action; do not rely on prompt text or a single outer wrapper.
  7. Protect secrets. Remove credentials from the sandbox and broker narrowly scoped, short-lived access through trusted services.
  8. Instrument and test. Capture proposals, tool calls, approvals, denials, network attempts, outputs, and errors. Run adversarial tests for injection, data exfiltration, path traversal, replay, and approval bypass.
  9. Plan recovery. Define credential revocation, workspace destruction, rollback, incident ownership, and a way to resume a paused run safely.

What should you compare when evaluating an agent-security implementation?

There is no single control that proves an agent is secure. Compare implementations using the same questions, and treat the answers as architecture evidence rather than vendor ratings.

Comparison axis Questions to ask
Execution boundary Is compute hosted or self-managed? How isolated is each workload? Does the trusted harness run outside it?
Authority Which tools, files, records, and write operations are available? Are read and write permissions separate?
Network Is egress default-deny? Which infrastructure makes each connection? Are destinations and protocols logged?
Secrets Can generated code read credentials? Is access brokered, scoped, expiring, and revocable?
Review Which actions pause for a person? Is the check enforced at the side-effect tool? What happens on timeout?
Evidence and operations Are traces complete, tamper-resistant, searchable, and sufficient for replay, evaluation, and incident response?

What does observability add to agent security?

Auditability lets you explain not only what the agent said, but what it attempted and what the system actually executed. Preserve the model request and response where policy permits, retrieved sources, tool arguments, authorization decisions, approval identity and timestamp, network events, outputs, and final state changes. Protect logs from the same agent and credentials they are meant to monitor.

Use correlation IDs to connect a user request, agent run, sandbox instance, proxy call, approval, and downstream transaction. Record denied actions as well as successful ones; repeated denials can reveal an attack or a broken workflow. Set retention and access rules appropriate to the sensitivity of the data, and test that an incident investigator can reconstruct the sequence without relying on ephemeral model context.

How should you interpret OpenAI’s Auto-review figures?

An April 30, 2026 OpenAI Alignment article reports that Codex sessions using Auto-review stopped for human approval roughly 200 times less often than sessions in manual approval mode, and that Auto-review approved around 99% of the small fraction of actions that still required review. Those are product- and deployment-specific figures from OpenAI’s report.

They do not establish a universal security success rate, a false-negative rate, or independent effectiveness across agent systems. Before relying on a similar mode, verify which actions it can approve, what policy and telemetry it uses, how denials are handled, and whether your own evaluation set measures missed risks rather than only interruption frequency.

What are the most common design failures?

Relying on instructions instead of enforcement

Symptom: the prompt says “never access production,” but the runtime mounts production credentials. Fix: remove the mount, use a separate identity, and enforce the restriction in the service and network policy.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Putting secrets in the sandbox

Symptom: a generated script can read environment variables or logs containing reusable tokens. Fix: broker one narrowly scoped request through a trusted proxy, rotate exposed credentials, and scrub traces.

Approving a summary instead of the actual action

Symptom: a person approves “update customer record,” while the tool submits a different target or payload. Fix: bind approval to a canonical, structured proposal and revalidate it immediately before execution.

Assuming one guardrail covers every path

Symptom: a custom tool or sub-agent performs a side effect without the expected review. Fix: attach authorization and approval checks to every side-effect boundary and test manager-agent routing explicitly.

Keeping no usable trail

Symptom: the team can see the final output but not which tool call changed state. Fix: preserve correlated traces, decisions, arguments, and downstream results in an access-controlled audit store.

Frequently Asked Questions

Can prompt injection be solved with a better system prompt?

No. Prompt instructions can be ignored, overridden by retrieved text, or bypassed through another tool path. Reduce the impact with least-privilege tools, structured data handling, sandboxing, tool-bound validation, and approval for consequential actions.

Should every AI-agent action require a person to click approve?

No. Requiring approval for harmless, reversible reads can make a system unusable. Use automatic policy checks for routine operations and reserve human decisions for actions whose impact, sensitivity, or irreversibility justifies the interruption.

What happens if a reviewer is unavailable?

Define the behavior explicitly: pause until a deadline, expire the proposal, or route it to an authorized backup. Never silently execute a sensitive action because an approval queue timed out.

How often should agent permissions be reviewed?

Review them whenever the task, tool set, data source, or deployment environment changes, and on a scheduled basis that matches the risk. Remove unused tools and credentials, examine denied-action logs, and retest sandbox and network boundaries after major updates.
