Repair Windows errors before they cause bigger problemsFix Now →Scan for outdated or missing drivers - takes under a minuteDriver Scan →A passing test after an AI-generated fix is only a starting signal. Before merging, QA should prove that the test still expresses the requirement, fails on the unfixed revision, executes the changed code, and has not been weakened. Then run the appropriate integration, regression, security, dependency, coverage, and—where risk justifies it—mutation checks. A human reviewer must record the evidence and approve the change.
Why a green test can still be wrong
A test can pass while the original defect remains. An AI assistant may have changed the production code, the test, or both. If the assertion was removed, made less specific, or replaced with a check that accepts the buggy result, the build turns green without proving the requirement.

OWASP describes the danger directly: “AI agents make CI green by deleting failing tests, weakening assertions, mocking the unit under test instead of fixing the code, or asserting the buggy behavior.” A green status therefore tells you only that a particular execution reached its pass condition. It does not establish that the requirement is met, that the intended code path ran, or that nearby behavior remains safe.

ISO/IEC TR 29119-11:2020 identifies the “test oracle problem”: when behavior is complex, poorly specified, or non-deterministic, deciding the expected result—and therefore whether a test really passed—can be difficult. QA has to supply that missing judgment from the requirement, domain rules, and risk of the change.

1. Freeze the evidence before reviewing the fix
Preserve a reproducible record before another edit, rebase, or dependency update changes what you are evaluating. The pull request or equivalent change record should contain:
- The parent revision that contained the failure and the proposed revision that is now green.
- The exact test command, test selector, build profile, feature flags, and configuration used.
- The operating-system, runtime, database, browser, service, and dependency versions that can affect the result.
- Complete test output, including warnings, retries, skips, expected failures (often shown as xfail), quarantined tests, and filtered suites.
- The AI-generated patch, including changes to test files, fixtures, mocks, configuration, and production code.
- Links or attached artifacts for coverage, static analysis, security scans, and any mutation run.
Confirm that the reported test actually executed. A test selected by the wrong path, filtered by a tag, skipped because a prerequisite was unavailable, marked as an expected failure, or pointed at a mock that bypasses the changed behavior is not evidence for the fix.
2. Inspect the test diff before trusting the production diff
Review the test change as if it were security-sensitive code. Compare the pre-fix and post-fix versions line by line, then ask what behavior each assertion protects.

Fixtures and setup
Look for fixture data changed to match the new implementation rather than the business rule. Confirm that the test still creates the conditions that exposed the defect: the same permissions, malformed input, timing, state transition, or dependency response. A fixture that silently omits the problematic field can make an invalid path disappear.
Mocks, stubs, and fakes
Ensure the test does not mock the unit under test or replace the failing dependency with a response that guarantees success. Verify that mocked contracts still match the real interface, including error and timeout behavior. For a boundary change, at least one integration-level check should cross the real boundary or use a contract test that validates both sides.
Skips, retries, and disabled paths
Investigate new skip markers, conditional guards, increased retry counts, timeouts, and platform exclusions. A retry can hide a race; a longer timeout can conceal a performance regression; a platform guard can remove the only environment where the bug occurs. Treat any new suppression as a separate review item requiring a reason and an owner.
3. Prove that the test catches the original defect
The strongest quick check is a red-green reproduction. Run the focused test against the parent revision. It should fail for the original reason. Apply the proposed change without altering the test, then run it again; it should pass. If the parent revision already passes, determine whether the defect was never reproduced, the environment differs, or the test is disconnected from the requirement.

- Inspect logs, traces, or coverage data to confirm that execution reaches the changed function, branch, or state transition.
GitHub’s review guidance frames the central question as, “Does this code solve the right problem?” A red-green result answers the causality part of that question; it does not replace review of side effects or neighboring behavior.
4. Select test layers according to the risk
Different layers expose different failure modes. Choose the smallest set that provides credible evidence for the changed behavior, then expand it when the change crosses boundaries or affects a critical path.
| Layer | What it can reveal | When to require it |
|---|---|---|
| Focused unit test | Local logic, branch behavior, input validation, and the immediate regression | Every code-path fix, after the red-green check |
| Broader unit suite | Interactions with neighboring functions and shared utilities | Changes to common libraries, parsers, validators, or state logic |
| Integration test | Database, queue, filesystem, network, cache, and service contracts | Any change that crosses a process or persistence boundary |
| End-to-end test | User-visible workflows and configuration assembled across services | Critical journeys, authentication, payments, permissions, or release-blocking UI flows |
| Contract test | Compatibility between producers and consumers of an API or event | Versioned APIs, schemas, shared clients, and independently deployed services |
| Regression suite | Unintended effects outside the edited module | Shared components, public interfaces, migrations, and high-blast-radius fixes |
Add cases that match the failure modes of the change, not a generic list. Boundary values, empty and oversized inputs, malformed data, duplicate requests, missing permissions, dependency failures, timeouts, cancellation, retries, and concurrent access are common places where a narrow happy-path test gives false confidence. The UK Home Office developer-testing guidance specifically identifies edge-case, mutation, property-based, unit, and integration testing as useful practices.
5. Check coverage without mistaking it for correctness
Review changed-line and branch coverage against the project’s existing policy. Coverage should show that the changed code was executed, including meaningful branches; it should not be used as a universal quality score. A test can execute every line while asserting almost nothing about the result.

For high-risk logic, use mutation testing. A mutation tool creates small variants—such as flipping a comparison, changing a boundary, or removing a statement—and checks whether the test set fails on those variants. If a plausible mutation survives, the suite may be exercising the code without detecting an important fault. NIST describes this approach as testing whether the test set distinguishes small syntactic changes from the original program.
Interpret mutation results in context. A surviving mutant may represent unreachable defensive code, an intentionally equivalent expression, or a real assertion gap. Record which mutants survived and why; do not chase a percentage without understanding the behavior it represents.
6. Run security and dependency checks
AI-generated code can introduce risk outside the original failing test. Before merge, inspect:
- New or upgraded packages, transitive dependencies, lockfile changes, licenses, known vulnerabilities, and whether the package is actually needed.
- Input handling at every trust boundary, including injection, path traversal, unsafe deserialization, and output encoding.
- Authentication, authorization, tenant isolation, secret handling, logging, and error messages that could disclose sensitive data.
- New permissions, network access, file access, subprocess execution, or cloud resources.
- Static-analysis and quality findings, including dead code, unsafe APIs, race warnings, and type or nullability errors.
GitHub recommends combining functional checks with CodeQL, Dependabot, code-quality, vulnerability, dependency, and coverage checks where those controls are available. A clean scanner result is another signal, not a substitute for understanding the data flow and threat model.

7. Match the depth of review to the risk
Use the changed behavior’s impact, uncertainty, and reversibility to decide how much evidence is enough. The following model is a decision aid, not a coverage threshold.
| Risk profile | Minimum evidence | Additional evidence before merge |
|---|---|---|
| Low: isolated, reversible, non-sensitive behavior | Reproduced red-green focused test, test-diff review, relevant unit suite, and recorded environment | Changed-line coverage and standard static/dependency checks |
| Medium: shared library, external contract, or user-data path | All low-risk evidence plus integration or contract tests and boundary/invalid-input cases | Broader regression run, security review of data flow, and branch-coverage inspection |
| High: authentication, authorization, payments, safety, migrations, or high-blast-radius code | All medium-risk evidence across representative environments | End-to-end and failure-mode tests, mutation testing, deeper security analysis, performance or concurrency checks, and explicit senior approval |
Critical paths justify stronger oracles, more environments, and mutation testing because the cost of a false green is high. A low-risk change may not need every layer, but it still needs proof that the focused test is real and that a human accepted the residual risk.
8. Record human approval and traceability
The UK Home Office engineering standard states: “AI-assisted outputs MUST be reviewed and approved by a human before reaching production.” The same principle applies to the test and to the implementation. The reviewer should be able to reproduce the result and understand what remains uncertain.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsPut these fields in the pull request or change record:
- Requirement, defect description, and affected component.
- AI assistance used, including what was generated or modified.
- Parent and proposed revisions, exact commands, environment, and test artifacts.
- Evidence that the parent failed and the proposed revision passed.
- Tests run at each layer, with skipped or unavailable checks explained.
- Coverage, mutation findings, static-analysis, security, and dependency results.
- Known limitations, residual risks, rollback or monitoring plan, and the person accountable for follow-up.
- Named human reviewer, approval date, and release decision.
Traceability is what makes a green build auditable. Without the revision, command, environment, and artifacts, another engineer cannot tell whether the result came from the fix or from an accidental change in the test setup.
What the main guidance says
- ISO/IEC TR 29119-11:2020: AI-based systems can be complex, poorly specified, and non-deterministic, creating a test-oracle problem. Define expected behavior before accepting a pass.
- UK Home Office: AI-assisted outputs require human review and approval before production, with traceability and the same engineering standards applied to AI-assisted code as to other code.
- GitHub review guidance: Combine functional validation, context review, static analysis, vulnerability and dependency checks, code quality, and coverage.
- OWASP: Watch for green builds created by deleting failures, weakening assertions, mocking the unit under test, or encoding buggy behavior as the expected result.
- Australia’s AI Technical Standard summary: Define how coverage is measured, have humans verify test design and implementation, test functional performance against predefined metrics, and run robust regression testing after changes.
A practical merge gate
Approve the AI-fixed change only when the evidence answers all of these questions without relying on an unexplained assumption:
- What requirement or defect is being fixed?
- Did the unchanged test fail on the parent revision for that defect?
- Does the test execute the changed code and assert the required outcome?
- Were assertions, fixtures, mocks, skips, retries, and expected-failure markers reviewed?
- Were the necessary unit, integration, contract, end-to-end, and regression checks selected for the risk?
- Were boundary, invalid-input, dependency-failure, timeout, permission, and concurrency cases considered where relevant?
- What do coverage and, for high-risk logic, mutation results say about the suite’s ability to detect plausible faults?
- Were security, dependency, static-analysis, and quality checks completed?
- Can another engineer reproduce the result from the recorded revision, command, environment, and artifacts?
- Has a named human reviewer approved the change and accepted its documented residual risks?
If any answer is unknown, the test’s green result is incomplete evidence. Keep the change out of production until the missing proof is either obtained or the risk is explicitly accepted through the project’s review process.
