The “Windows was unable to complete the format” message does not point to one specific fault. First protect any files you need, confirm Windows has the right drive selected, and check whether the disk is read-only. Only try formatting or repartitioning if you are prepared to erase the drive; repeated failures or I/O errors are a reason to stop and investigate the device rather than keep issuing destructive commands.
Before troubleshooting, decide whether the files matter
Formatting creates a new file system and root directory, disrupting normal access to existing files. DiskPart’s clean command removes partition or volume formatting; clean all goes further and sets every sector to zero. Neither is a data-recovery step. If the drive contains files you need, do not format it or run either command. Preserve the device and seek appropriate recovery help before making changes.

If you are willing to erase the drive, work through the checks below in order. The same error can occur alongside a read-only state, a damaged or unrecognized volume, a connection problem, or possible device failure; the message itself does not tell you which one applies.

Reconnect and isolate removable drives
For a USB flash drive or external disk, reconnect it securely. If practical, try another USB port or another computer. If the device fails in the same way on a second computer, that is a warning sign, not proof that the drive itself is faulty.

Verify the disk in Disk Management
Open Disk Management and inspect the disk’s capacity and status. Make sure the drive you intend to erase is the one Windows is reporting. This matters especially before using DiskPart: its commands operate on the object currently in focus, so an incorrect disk selection can affect a different drive.
Check whether DiskPart reports the disk as read-only
If you have confirmed the target disk and are prepared to change its settings, use an elevated Terminal or Command Prompt. DiskPart changes apply to the selected disk, so check its number and capacity carefully before issuing the attribute command.

- Open Windows Terminal or Command Prompt as an administrator.
- Enter
diskpart, thenlist disk. - Identify the intended disk by its capacity. Enter
select disk N, replacingNwith that disk’s number. Do not copy an example number without checking your own list. - Enter
attributes diskto view the selected disk’s attributes. - If it is marked read-only and you have confirmed the selection, enter
attributes disk clear readonly. Then exit DiskPart and, if the drive is accessible and its contents can be erased, try formatting it again.
Microsoft documents these commands for inspecting and clearing the selected disk’s read-only attribute. Clearing that attribute is not a guaranteed fix for every form of write protection, including a persistent device-level condition.
Retry formatting only if erasing the drive is acceptable
Use Windows’ normal format controls after confirming the target and accepting the loss of normal access to its existing files. Choose a file system that suits the device’s intended use and the devices that need to read it. Microsoft’s Format command documentation lists NTFS, FAT, FAT32, exFAT, ReFS, and UDF as options, but the right choice depends on your particular use case.

A quick format deletes the file table and root directory of an already formatted volume; it does not scan each sector for bad areas. A quick-format failure therefore does not diagnose the underlying problem. A full format is not guaranteed to repair a drive that is failing.
Reserve DiskPart clean for deliberate repartitioning
Use clean only if your goal is to remove partition or volume formatting from the selected disk and you have verified that it is the correct one. It is destructive, not a routine first troubleshooting step. Do not use clean all as a stronger format attempt: it sets every sector to zero.

DiskPart’s commands act on the object in focus. Microsoft’s example sequence uses list disk and select disk before changing a disk; the example’s disk number is not a number to copy blindly. If clean returns an I/O device error, repeating destructive commands is unlikely to establish whether the cause is software or hardware.

Stop repeated format or clean attempts if the drive continues to fail across computers, remains read-only after you clear the attribute, disappears, or returns I/O errors. These symptoms can indicate a connection or device problem, but they do not establish a definitive diagnosis. Microsoft Q&A community reports describe drives that failed across computers and a RAW disk that returned an I/O error during DiskPart clean; those reports are anecdotal, not official diagnoses.

If files matter, prioritize recovery rather than further changes. If they do not, check the device’s warranty or support options, or consider replacing a removable drive that remains unreliable. Replacement may be reasonable in that situation, but it is not a guaranteed fix for every formatting error.
