Why RAG Chatbots Break in Production: 7 TypeScript Failure Modes and Fixes

A RAG chatbot that works in a demo usually has a short, clean path: a few documents are indexed, one question retrieves the expected paragraph, and the model has plenty of context. Production adds document updates, concurrent tenants, long conversations, malformed inputs, retries, cost limits and questions nobody anticipated. The result is a chain of contract failures rather than one mysterious model problem.

The reliable fix is to make every boundary explicit: version and hash the source data, inspect retrieval independently of generation, validate runtime values even in TypeScript, budget context and retries, enforce grounded structured output, evaluate representative questions and trace the entire request while protecting sensitive data. The seven failure modes below map that work to concrete implementation steps.

What changes between a convincing demo and a production RAG system?

Retrieval-augmented generation has several contracts in sequence. Ingestion must represent the current source faithfully; chunking must preserve useful boundaries; indexing must expose those chunks; retrieval must select evidence relevant to the question; the prompt must fit the model’s context budget; generation must obey a grounding and output contract; and operations must be able to explain quality, latency, cost and security events.

A fluent answer only proves that the final model call produced fluent text. It does not prove that the evidence was current, that the cited passage supported the claim, or that the same path will survive a large corpus and real traffic. Diagnose the chain in order instead of changing the prompt first.

1. Ingestion and chunking drift silently corrupt the index

Production content changes. A revised policy may never be ingested, a deleted page may remain searchable, a retry may create duplicates, and a parser may split a definition from its exception. Retrieval can then be perfectly consistent while returning obsolete or incomplete evidence.

Make ingestion idempotent

  1. Normalize each source before indexing: use stable Unicode handling, line-ending normalization and deterministic removal of transport-only fields.
  2. Hash the normalized content with a cryptographic digest. Use the source identifier, version and content hash as an idempotency key.
  3. Persist source, version, section, URL or document key, ingestion timestamp and parser version as metadata on every chunk.
  4. Compare the new manifest with the previous manifest. Re-index changed records, remove records for deleted versions and skip unchanged hashes.
  5. Keep an ingestion run identifier and a per-record status so a failed batch can resume without duplicating successful work.
type SourceRecord = {
  sourceId: string;
  version: string;
  normalizedText: string;
  section?: string;
};

type ChunkRecord = SourceRecord & {
  contentHash: string;
  chunkIndex: number;
};

OpenAI’s documentation describes automatic vector-store processing and configurable chunking. Its documented defaults are a maximum of 800 chunk tokens with 400 overlap tokens; the same documentation lists a 512 MB maximum file size and 5,000,000 tokens per file (OpenAI documentation, 2026). Treat those as provider limits and defaults, not universal best settings. Measure whether your documents need smaller sections, less overlap or structure-aware splitting.

Detect bad boundaries before they spread

  • Inspect chunks from headings, tables, code blocks, lists and footnotes, not only ordinary paragraphs.
  • Reject empty chunks and flag chunks that contain only navigation, cookie notices or repeated boilerplate.
  • Store the parent section so a retrieved chunk can be expanded when a definition depends on the next paragraph.
  • Run a duplicate-content check after each ingestion batch; duplicated chunks distort ranking and waste context.

A clean re-index cannot repair a missing source. Keep a source manifest and make ingestion metrics visible: discovered, changed, skipped, deleted, failed and indexed counts should reconcile for every run.

2. Retrieval returns plausible but wrong context

The most dangerous retrieval bug is not an empty result. It is a plausible passage that answers a nearby question, contains generic boilerplate or applies to the wrong tenant, product or policy version. The model then produces a confident answer from evidence that looks credible to a human skimming the response.

Inspect retrieval as its own product

For every query, log the normalized query, tenant and other filters, result identifiers, scores, rank, a short snippet and the index version. Store enough information to replay the retrieval without calling the language model. Never log unrestricted document text when it may contain secrets or personal data; redact or hash sensitive fields.

Create a labeled question set in which each question has relevant document or section identifiers. Measure retrieval recall and inspect false positives, not just whether the final prose sounds good. Citation correctness should be checked against the retrieved passage and the underlying source, because a citation can point to a real document while failing to support the sentence.

Use the right retrieval stages

  1. Apply mandatory metadata filters first, such as tenant, product, locale, permission scope and document status.
  2. Retrieve a candidate set with semantic search, lexical search or both.
  3. Use a reranker when the initial candidate list contains near-duplicates or passages with similar vocabulary but different meaning.
  4. Deduplicate by source section and retain the highest-quality passage for the context assembly step.
  5. Record the final evidence set and its scores so a bad answer can be traced to a selection decision.

LlamaIndex’s TypeScript retrieval API documents hybrid search, reranking and metadata filters. These capabilities are especially useful when a question combines exact identifiers with natural-language intent. A product code may need lexical matching while the surrounding explanation benefits from semantic matching.

Recognize scale-related degradation

LlamaIndex production guidance warns that standard top-k retrieval with basic text splitting degrades as collections grow. Increasing k indefinitely is not a fix: it adds noise, consumes tokens and can push the decisive passage out of the model’s effective attention. Improve chunk boundaries, filters, candidate ranking and evaluation together.

3. TypeScript types stop at runtime boundaries

An interface or type annotation disappears when JavaScript runs. It cannot verify a file parsed from a queue, metadata returned by a vector store, an HTTP payload, a tool result or JSON emitted by a model. Asynchronous query and streaming APIs also return runtime values and rejected promises that the compiler cannot make safe for you.

Validate every external value

  • Compile with strict TypeScript settings, including strict null checks and no implicit unsafe values.
  • Parse untrusted input with a runtime schema validator or explicit type guard before it enters retrieval.
  • Validate document metadata and provider responses before reading fields such as score, content or source ID.
  • Parse model output as unknown, validate the required shape and reject extra or contradictory fields when your contract requires it.
  • Wrap every awaited retrieval, reranking, generation and streaming operation in explicit error handling.
type GroundedAnswer = {
  answer: string;
  citations: Array<{ sourceId: string; quote: string }>;
  confidence: 'supported' | 'insufficient_evidence';
};

function isGroundedAnswer(value: unknown): value is GroundedAnswer {
  if (!value || typeof value !== 'object') return false;
  const item = value as Record<string, unknown>;
  return typeof item.answer === 'string'
    && Array.isArray(item.citations)
    && (item.confidence === 'supported' || item.confidence === 'insufficient_evidence');
}

Return typed error results or controlled exceptions at the boundary. A malformed provider payload should become an observable, retryable or user-safe failure, not an undefined property that silently changes the prompt.

Handle streaming as an incremental protocol

For streaming responses, validate event types, detect termination and timeout events, and preserve the trace ID across every chunk. Do not assume that a stream ends with valid JSON merely because a non-streaming request would have returned JSON. Buffer and validate the final object, or use a protocol that supports structured events.

4. Context and conversation state overflow the useful budget

Long history and generous retrieval settings create a paradox: more context can hide the evidence that matters. Input tokens increase latency and cost, the model may truncate older or newer content, and a provider can reject a request that exceeds its context limit. Retries amplify the expense.

Set a request budget before assembling the prompt

Reserve space for system instructions, the current user message, retrieved evidence, conversation history and the expected output. A practical budget is a policy, not a guess: record the token count of each component and reject or reduce a request before the provider rejects it.

  1. Count the current message and fixed system instructions.
  2. Reserve an explicit output allowance for the answer and citations.
  3. Allocate the remaining input budget to retrieved chunks and history.
  4. Drop duplicate or low-scoring chunks first; never drop mandatory tenant or safety instructions.
  5. Summarize or window old conversation turns, preserving decisions, unresolved questions and user constraints.
  6. Record input tokens, output tokens, elapsed time, retry count and provider error for the request.

Control retrieval expansion

Cap the number of chunks and the maximum characters or tokens per chunk. If a chunk depends on surrounding context, expand only that section and then apply the same cap. Keep source IDs attached during truncation so a citation cannot refer to text that was removed.

Use separate policies for interactive chat, batch jobs and streaming. A chat request may need a strict latency ceiling, while an offline answer can spend more time reranking. The policy should fail clearly when no safe budget remains rather than silently sending an incomplete prompt.

5. Loose prompts and output contracts allow unsupported answers

A prompt that says “answer helpfully” leaves unanswered questions: What counts as evidence? How should conflicts between versions be handled? What happens when retrieval is empty? Which citation belongs to each claim? Without explicit rules, the model may fill gaps from its pretraining, omit citations or emit JSON that cannot be parsed.

Define a grounded response contract

  • Give every retrieved chunk a stable source ID and include that ID beside the text.
  • Instruct the model to use only supplied evidence for factual claims and to state when evidence is insufficient.
  • Require each material claim to cite one or more source IDs, with a short supporting quote where practical.
  • Specify how to handle conflicting versions: prefer the permitted current version and expose uncertainty instead of merging incompatible rules.
  • Request a structured object with fixed fields rather than prose that downstream code must guess how to parse.
const outputContract = {
  fields: {
    answer: 'string',
    citations: 'array of { sourceId, quote }',
    confidence: 'supported | insufficient_evidence'
  },
  rule: 'Refuse factual claims when supplied evidence does not support them.'
};

Validate and fail safely

Parse the generated object as unknown, validate it, verify that every cited source ID exists in the retrieved set and reject quotes that do not occur in the supplied evidence when your application requires exact support. If validation fails, return a controlled message or retry with a repair instruction; never display malformed JSON as if it were authoritative. If retrieval is empty, use a dedicated insufficient-evidence response instead of asking the model to improvise.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

6. There is no representative evaluation loop

A prompt change can improve a handful of demo questions while harming long-tail wording, unanswerable requests, newly indexed documents or citation behavior. Production quality needs a versioned evaluation set and a repeatable comparison, not anecdotal approval.

Build an evaluation set that reflects users

Include real questions after removing sensitive data, paraphrases, misspellings, multi-step questions, questions whose answer is absent, conflicting-version cases and permission-boundary cases. For each item, record the expected evidence identifiers and the acceptable refusal behavior. Version the set alongside ingestion, prompt and model changes.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Measure the complete contract

  • Retrieval relevance: whether the expected evidence appears in the candidate and final sets.
  • Answer faithfulness: whether claims are supported by supplied evidence.
  • Citation correctness: whether each citation actually supports the associated claim.
  • Refusal behavior: whether unsupported questions receive an explicit insufficient-evidence response.
  • Operational quality: latency, timeout and retry rates, token usage and cost.

LlamaIndex’s evaluation guidance frames objective measurement as necessary for improvement, and its production guidance notes that basic top-k retrieval degrades as collections scale. Run the set on every material change to chunking, embedding or reranking, prompt, model, provider, filter logic or output schema. Establish regression thresholds appropriate to your product and block deployment when a critical tenant or safety case fails.

7. Missing observability and security controls hide the cause

Without a connected trace, an engineer sees only a bad final answer. They cannot tell whether ingestion was stale, a filter was omitted, reranking changed the order, a retry used a different model or output validation discarded the response. Security gaps are equally serious: a correct answer can still be an incident if it crosses a tenant boundary or exposes personal data.

Trace the full request

Assign a trace ID at the edge and carry it through query rewriting, filters, retrieval, reranking, prompt assembly, generation, tool calls, validation and response delivery. Record document and chunk IDs, index version, model and prompt versions, token counts, latency by stage, retries and normalized error classes. Keep enough metadata to reproduce a decision without storing raw secrets.

LlamaIndex’s observability documentation states: “A key requirement for principled development of LLM applications over your data (RAG systems, agents) is being able to observe, debug, and evaluate your system – both as a whole and for each component.” Use that principle to make component-level traces a release requirement rather than an optional dashboard.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Protect data and boundaries

  • Apply tenant and authorization filters before retrieval, and test that a user cannot remove them through query text or tool arguments.
  • Use least-privilege credentials for ingestion, search and model calls; separate write access from read access.
  • Redact secrets, credentials, payment data and unnecessary personal information from logs and evaluation sets.
  • Encrypt stored traces and define retention periods that match your obligations.
  • Classify provider errors and avoid logging full prompts when they contain confidential content.

Security controls belong in the same trace as quality controls: an authorization failure, redaction event or cross-tenant filter mismatch must be searchable by trace ID and treated as a release-blocking defect.

Choosing a TypeScript retrieval layer

The right stack depends on which contract you need to own directly. The available official documentation describes different strengths rather than a universal winner.

Capability OpenAI native retrieval LlamaIndex.TS What to verify before adoption
Core abstraction Direct vector-store features and automatic vector-store file processing are documented. Query-engine and retrieval abstractions are documented for TypeScript. Confirm current SDK methods, supported stores and migration path.
Chunking Configurable chunking; documented defaults are 800 maximum chunk tokens and 400 overlap tokens (OpenAI documentation, 2026). Chunking and retrieval are composed through the framework’s indexing abstractions. Test structure-aware splitting on your own corpus.
Filtering and ranking Use the provider’s documented vector-store capabilities. TypeScript retrieval documentation covers hybrid search, reranking and metadata filters. Measure recall, citation correctness and filter enforcement.
Observability and evaluation Application-level tracing and evaluation remain your responsibility. Observability integrations and evaluation guidance are documented. Ensure traces include your tenant, index and prompt versions.
Provider portability Not established by the cited material. Not established by the cited material. Check embedding, reranking, storage and model substitution costs.
Latency, cost and security benchmarks Not established by the cited material. Not established by the cited material. Benchmark with representative traffic and enforce your own budgets and controls.

Whichever layer you choose, keep ingestion manifests, retrieval logs, runtime schemas, evaluation data and traces under your control. Framework abstractions can accelerate implementation, but they do not remove the contracts that production must enforce.

A practical hardening sequence

  1. Freeze a versioned source manifest and make a clean, idempotent ingestion run reconcile additions, updates and deletions.
  2. Capture retrieval candidates and labels for real questions before tuning prompts.
  3. Add runtime validation and explicit error handling at every file, metadata, provider, stream and model boundary.
  4. Set token, latency, retry and cost budgets, then instrument each component so violations are visible.
  5. Enforce source-linked structured output and an insufficient-evidence path.
  6. Run the versioned evaluation set on every material change and block regressions in evidence, citations or refusals.
  7. Require trace continuity, tenant isolation, redaction and least-privilege credentials before exposing the system to production traffic.

This sequence turns “the bot is wrong” into a diagnosable contract failure: stale data, bad evidence, unsafe runtime input, an exceeded budget, an unsupported output, an unmeasured regression or an operational/security defect. That distinction is what makes a TypeScript RAG chatbot repairable at production scale.
