It is visible, tangible and usable without a device interface; it can also fill gaps when a digital workflow stops at a form instead of capturing, routing and preserving the information. That makes paper locally rational—not automatically faster, more accurate, safer or legally required.
Why do we still use paper when everything is digital?
“Everything is digital” is often true only of part of a process. An organization may have electronic records or online systems while people still print documents to review them, carry information between steps, or work around a system that does not present what they need at the right time.

A qualitative study of U.S. healthcare organizations with electronic medical record and computerized order-entry systems found paper remained widely used at the sites studied. The authors grouped the reasons into continued value in old uses, new uses for paper and regulatory factors. Their methods included observation, informal interviews, focus groups and oral histories; this was not a randomized comparison of paper and digital outcomes, nor a measure of present-day paper use across healthcare. The 2009 study, “Persistent Paper: The Myth of ‘Going Paperless’”, is best read as evidence of mechanisms, not a current prevalence estimate.
Paper can suit the work in front of a person
In the healthcare settings studied, reported factors included familiarity and comfort, the ability to hold and view a document, difficulty interpreting some information on screen, and clinicians’ concerns that using a computer during an encounter could distract from the patient relationship. A paper page can also be an improvised coordination aid when the official system does not expose information in a useful way.

Those are contextual explanations, not proof that paper is universally easier to understand or produces better clinical results. Whether it helps depends on the task, the information and how the record is managed.
Paper use can grow alongside electronic records
One 300-bed hospital in the study reportedly printed or copied 1.6 million pieces of paper per month, with 40% printed and destroyed. That is an illustrative observation at one institution, not an industry average. It shows how digitizing a core system can leave—and sometimes add—paper around the work.

Why do online forms still need to be printed and signed?
Because putting a form on a website does not make the whole process digital. A person may fill it in online, print it for a wet-ink signature and mail it. The receiving organization then has to capture the information again, using optical character recognition (OCR) or manual data entry. Each handoff can add delay and create opportunities for omitted, mismatched or altered information.

The U.S. National Archives and Records Administration (NARA) describes these hybrid form workflows and recommends capturing information fully in digital form and, where appropriate, replacing wet-ink signatures with electronic or digital signatures. Whether a particular signature or paper original is legally required depends on the jurisdiction and transaction; the examples are federal guidance, not a universal rule.

In a companion article, NARA said federal agencies had been directed to manage records electronically to the fullest extent possible by June 30, 2024, while noting that agencies still printed born-digital documents for signatures or accepted paper submissions. It warns that converting a digital record to paper and then back to digital is inefficient and can risk information loss or alteration. That guidance applies to U.S. federal agencies; it does not establish the rules for private organizations or other countries. NARA also says agencies should verify claimed legal requirements with their general counsel and seek an exception where applicable.
What can paper do better—and where does digital help?
The useful comparison is the entire workflow, including access, handoffs, controls and preservation—not the material alone. Paper may work well as a visible working copy or in a setting where devices or system access are unavailable. A managed digital system can support structured capture, search and access by multiple people. Neither advantage is automatic: a paper page can be misplaced, while a digital record can be hard to retrieve or trust if it is poorly managed.

- Handoffs and duplication: Does the process involve printing, signing, scanning, OCR or re-entry? Where might information be missed or changed?
- Trust and accountability: Is there a designated authoritative copy, a retention schedule, an audit trail and a controlled process for changes and disposal?
- Long-term preservation: Can the organization retrieve and preserve the record for the required period? Digital files need managed systems and attention to migration and accessibility; paper needs physical custody and preservation.
NARA’s 1994 imaging report makes a broader point that remains useful: record integrity depends on procedures and controls, not the storage medium by itself. It discusses written records-management procedures and periodic audits to demonstrate that an imaging system produces accurate results. The report concerns older optical-storage technology, so it should not be treated as current operational or legal advice. See NARA’s report on imaging records.
When does paper become a records-management problem?
Paper is especially costly when it is a required detour rather than a useful working format, or when people cannot tell which copy is authoritative. A hybrid workflow can leave the organization with a signed sheet, a scan, a retyped record and an electronic original without a clear rule for which one must be retained. Reliable recordkeeping requires decisions about the official copy, access, retention, integrity and disposal, whether the record is paper or digital.

Historical federal evidence illustrates that digital records are not self-managing either. NARA’s report, drawing on data collected in 2001, described employees treating paper as the official record while electronic copies were deleted or left on desktops and shared drives; it also found wide variation in paper and electronic practices. Those findings are historical, not a statement about current federal prevalence. NARA’s recordkeeping report shows how habits and rules can sustain paper even when electronic copies exist.
A 2002 Government Accountability Office report likewise discussed earlier findings that many federal records were unscheduled and historically valuable electronic records were not being identified and transferred to the National Archives. It is evidence that electronic recordkeeping can fail without governance, not proof of current agency performance. The GAO report on electronic records underscores why replacing paper with files alone does not solve preservation.
So is paper rational?
Sometimes. It is rational when it makes a real task more workable or fills a gap in the available system, and when the organization manages the resulting record responsibly. It is less defensible when an avoidable print-sign-scan loop creates extra work, uncertainty or information risk. The practical goal is not paperlessness for its own sake: it is a workflow that captures the right information, makes the authoritative record clear, and preserves it for as long as required.
