Why Clinical Documentation Is Quietly Overwhelming Healthcare Teams

Clinical documentation burden spans EHR notes, orders, inbox work, review, and administration. Learn why the workload accumulates and how practices can assess team-based documentation and scribe support.

Clinical documentation can overwhelm healthcare teams because the work is much larger than writing a visit note. It includes EHR documentation, orders, chart review, inbox messages, billing and insurance tasks, and coordination across systems. The burden builds across a workflow, often spilling beyond clinic hours. Reducing it means measuring the whole process and deciding which tasks can be redesigned or shared—not assuming that faster typing or one new tool will solve the problem.

What counts as clinical documentation burden?

“Documentation burden” is often used as shorthand for a range of work done in and around an electronic health record (EHR). The Agency for Healthcare Research and Quality (AHRQ) groups relevant measures into categories that include overall EHR time, documentation activity, inbox management, clinical review, orders, after-hours work, billing and insurance administration, workflow fragmentation, efficiency, EHR activity rate, and usability. That breadth matters: counting only time spent composing notes can miss much of the work involved.

The burden is not an argument for dispensing with reliable clinical records. The practical question is whether work is structured and distributed so that records remain useful without unnecessary duplication or poorly coordinated steps. The AHRQ technical brief on measuring documentation burden provides a framework for looking beyond note-writing time.

How much time does the work take?

Two AMA-reported findings describe different aspects of physician workload. They should not be treated as interchangeable: one is a survey breakdown of a workweek, while the other summarizes time-and-motion research in ambulatory practice.

Measure Reported finding Context
Total physician work 57.8 hours per week American Medical Association (AMA), 2024 Organizational Biopsy survey data, reported October 13, 2025.
Direct patient care 27.2 hours per week Same AMA survey; this is the direct-care category, not total work.
Indirect patient care 13 hours per week Same AMA survey. The AMA includes documentation, order entry, test-result interpretation, and referrals in this category.
Administrative tasks 7.3 hours per week Same AMA survey. The AMA examples include prior authorization, insurance forms, and meeting attendance.
EHR and desk work around ambulatory care Nearly two hours for every hour of direct clinical face time, plus another one to two hours of clerical work at home on many nights AMA summary of ambulatory time-and-motion research. The page does not identify a publication year for the underlying study.

The survey figures are reported physician averages, not a workload estimate for every clinician or health-system worker. The ambulatory time-and-motion finding describes a different measure and setting; it should not be added to the survey categories. The AMA account of its 2024 Organizational Biopsy survey and its summary of physician time in ambulatory practice provide the respective contexts.

Why does the work accumulate?

Documentation is one part of a chain of tasks: a clinician may review existing information, enter an order, respond to a message, interpret a result, and complete administrative steps as well as record the encounter. Inbox volume and workflow interruptions compete for attention. When information does not move smoothly between systems, staff may face additional coordination work. These pressures interact; the burden is therefore better understood as a workflow and organizational issue than as an individual typing-speed problem. The AMA’s overview of EHR-use research identifies excessive EHR tasks and inbox volume, interruptions, and poor interoperability among relevant factors.

Some work also spills beyond scheduled clinic time. The AMA’s survey reports substantial indirect-care and administrative work in a physician workweek, while its ambulatory time-and-motion summary describes clerical work at home on many nights. Together, these findings illustrate why looking only at time spent with patients—or only at the note-writing portion of an encounter—can leave important work out of view.

The effects are not established equally for every profession. The figures above concern physicians; they should not be generalized to nurses, medical assistants, or other members of a care team. AMIA’s report on its 2024 TrendBurden Pulse Survey quotes Vicky Tiase, PhD, RN-BC, FAMIA, FAAN, FNAP, 25×5 Task Force Policy Workstream Lead: “The time and effort required by healthcare professionals for documentation is severely impacting their work-life integration.” The statement appears in the AMIA 25×5 initiative.

How can healthcare teams reduce EHR documentation burden?

There are two broad approaches in the cited guidance: redesign how work is shared across the care team, and use documentation assistance such as human or ambient/virtual scribes. Neither is a guaranteed fix. A useful local evaluation asks what work actually moves, what remains the clinician’s responsibility, and whether the change improves the overall workflow rather than shifting the burden to someone else.

Approach What it can support What to evaluate locally
Team-based documentation Training documentation assistants and sharing appropriate work with clinical staff, scribes, or patients, as described in AMA guidance. Which tasks are appropriate to share; training and supervision needs; effects on colleagues’ workloads; and changes in total EHR time, after-hours work, documentation completeness, and patient interaction.
Human, ambient, or virtual scribe assistance Documentation support. The AMA identifies ambient scribes as a possible option and summarizes studies associating virtual scribes with improvements in documentation time and quality. Fit with specialty, visit type, staffing, and EHR; integration and data handling; review controls; implementation effort; and measured results in the practice. The AMA overview does not provide a single effect size that applies to every service or setting.

The AMA describes a six-step team-documentation toolkit and guidance for training documentation assistants. Those resources offer a starting point for evaluating work allocation, not a promise that a particular staffing model or product will save time. Notes produced with assistance still require appropriate clinical review and sign-off.

How should a practice tell whether a change is working?

Before adopting a staffing change or documentation service, define what burden the practice is trying to reduce. Use measures that reflect the actual work system rather than relying on a single proxy, such as note completion time. AHRQ’s measurement categories suggest a broader view:

  • Time and spillover: overall EHR time, documentation activity, and after-hours work.
  • Work volume and fragmentation: inbox management, clinical review, orders, billing and insurance administration, and interruptions or fragmented workflow.
  • Usability and efficiency: whether the EHR supports the intended work and whether activity is becoming more efficient.
  • Quality and team impact: documentation completeness and quality, patient interaction, and whether work has shifted to another team member.

Compare results in the relevant setting—by specialty, visit type, staffing model, and EHR setup—and include the effort needed for training, supervision, and implementation. For technology-assisted documentation, also assess integration, privacy and data handling, and controls for clinician review. These checks help distinguish a genuine reduction in total work from a faster note that leaves other tasks or colleagues’ workloads unchanged.

What the available evidence does—and does not—show

The AMA encourages continued EHR research “to ensure that EHRs are assets to medical practice and not a burden,” as stated on its EHR use research page. The evidence summarized there includes findings associating virtual scribes with better documentation time and quality, but it does not establish one result that applies to all products and practices.

The cited material does not establish a universal causal effect of documentation volume on burnout, quantify the patient-safety tradeoffs of a specific burden-reduction intervention, or show that a named commercial tool will improve a particular health system’s results. Nor does it provide head-to-head comparisons of scribe vendors. That makes local measurement and clinical review essential when a practice tests a change.
