AI hallucinations are plausible-sounding outputs that are false, unsupported, or inconsistent with the available evidence. They happen because a large language model generates likely continuations from learned statistical patterns; it does not automatically know whether a sentence is true. Human review prevents the most expensive failures by checking each material claim against authoritative evidence, recording where that evidence came from, and escalating uncertainty before anyone publishes the text or acts on it.

The practical rule is simple: treat generated text as a draft of claims, not as evidence. The more a sentence could affect a patient, customer, court case, payment, safety decision, or executive communication, the more rigorous and domain-specific the review must be.

What an AI hallucination is

A hallucination is a fluent statement that cannot be supported by reliable evidence, conflicts with the evidence available, or invents a detail that was never established. The wording may be grammatical, specific, and confidently presented. Those qualities describe the output’s style, not its truth.

Fluency is not verification

A model can produce a convincing citation, case name, statistic, quotation, product specification, or date even when that item does not exist. It may also combine pieces of true information into a false conclusion. Reading smoothly is therefore not a quality test. Every externally checkable assertion needs its own evidence check.

Why the word “hallucination” can mislead

The term sounds like an occasional software glitch. In practice, unsupported generation is a structural reliability problem. The model is optimized to produce a useful-looking continuation of the prompt, while the prompt may not contain enough information to determine a correct answer. A response can be internally consistent and still be wrong.

How language models end up making things up

They predict likely continuations, not proven facts

During generation, a language model selects tokens that fit patterns learned from its training. That process is powerful for drafting, summarizing, translating, and organizing information. It does not constitute a database lookup or a proof. When several continuations sound reasonable, the model can choose one that is statistically familiar rather than one supported by the facts of your case.

OpenAI’s September 5, 2025 explanation captures the incentive problem: “standard training and evaluation procedures reward guessing over acknowledging uncertainty.” If a system is often rewarded for producing an answer and rarely rewarded for abstaining, it has a reason to complete an uncertain prompt instead of saying that evidence is missing.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Knowledge gaps and stale or incomplete information

A model may not have seen a new regulation, product release, court decision, internal policy, or private document. Even when it has encountered related material, the relevant version, jurisdiction, date, or exception may be absent. Filling the gap with a familiar pattern creates a plausible but unsupported claim.

Noisy and contradictory source material

Training data contains errors, duplicated claims, outdated pages, satire, and disagreements among sources. The model learns correlations across that material rather than a guaranteed hierarchy of authority. If the prompt does not identify which source controls, the output can blend incompatible statements.

Prompt pressure and missing context

Instructions such as “give the exact answer,” “do not hedge,” or “answer immediately” increase pressure to produce a definitive continuation. A short prompt can omit the facts that would change the result: the governing country, the date of a policy, a patient’s history, a contract version, or the meaning of a technical term. The model may silently assume those details.

Evaluation incentives and proxy targets

Many evaluations measure whether an answer resembles a reference response or satisfies a format. A confident guess can score better than an explicit admission that the information is unavailable, even when the admission is the safer behavior. This is why a high benchmark score does not guarantee factual reliability on your particular task.

Why an answer can sound certain when it is wrong

Confidence in the prose is not a calibrated probability of truth. Models are trained to produce natural language, and natural language normally states conclusions directly. They do not automatically expose which words came from strong evidence, weak associations, or an inference made to fill a gap.

The Nature paper on semantic entropy describes “false outputs and unsubstantiated answers” and examines uncertainty across semantically equivalent answers. If a model gives materially different answers to prompts that mean the same thing, that disagreement is a useful warning signal. If it gives the same answer every time, that consistency still does not prove the answer is correct; it may simply be repeating a learned error.

Use uncertainty signals to prioritize checking, not to replace checking. A low-uncertainty answer can be confidently wrong, while a high-uncertainty answer may contain a correct but poorly worded point.

How frequent are hallucinations?

There is no single hallucination percentage for “AI.” Rates change with the model, prompt, subject, source access, scoring rules, and benchmark date. The following reported figures must be read in their original context.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Reported result What was measured Rate How to interpret it
Stanford HAI AI Index 2024 ChatGPT responses across language, climate, technology, and other topics in the cited evaluation Approximately 19.5% A result for that evaluation, not a universal rate for every ChatGPT use or every model.
Stanford HAI AI Index 2025 o1-mini on the cited HHEM document-summarization benchmark 1.4% Benchmark-specific document summarization result.
Stanford HAI AI Index 2025 GPT-4o on the same cited HHEM document-summarization benchmark 1.5% Comparable only within the stated benchmark and test conditions.

The difference between these figures is the point: a rate from one task cannot be transplanted to legal research, medical advice, customer support, or your internal documents. Measure the workflow you actually intend to run, and report the model, task, benchmark, date, and definition of an error.

How human review stops a hallucination from becoming a mistake

Human review is effective when it is a claim-level control rather than a quick read for tone. The reviewer should be able to show what was checked, which source controlled, and what happened to any unresolved uncertainty.

  1. Extract the claims. Split the draft into independent statements: names, numbers, dates, quotations, product capabilities, causal explanations, and recommendations. One paragraph may contain several claims with different evidence status.
  2. Classify the risk. Flag legal, medical, financial, safety, privacy, reputational, and customer-facing assertions. Increase the review standard when an error could change treatment, legal position, payment, physical safety, or public communication.
  3. Locate primary evidence. Prefer statutes and court opinions for legal claims; regulators and standards bodies for compliance or safety; original research and dated datasets for scientific figures; and current company or product documentation for specifications. A secondary summary can help you find a source, but it should not silently replace the controlling source.
  4. Test entailment. Ask whether the source actually supports the exact sentence. Check the jurisdiction, date, population, version, definitions, exceptions, and qualifiers. NIST describes this as faithfulness or anti-hallucination evaluation: “does the source actually support the claim?” A source that discusses a related topic is not enough.
  5. Record provenance. Preserve the source URL, publication or version date, access date, relevant page or section, and the reviewer’s decision. Keep the original generated text and the edited version so another person can audit the change.
  6. Handle uncertainty explicitly. If evidence is missing or conflicting, label the claim as unresolved, narrow it to what is supported, or remove it. Do not convert an absence of evidence into a confident sentence merely to make the copy sound complete.
  7. Escalate consequential decisions. A qualified human should approve content that changes a diagnosis or treatment, legal strategy, financial commitment, safety procedure, privacy decision, or public statement. The reviewer needs authority to block release, not just suggest stylistic edits.

A practical way to fact-check ChatGPT output

You can apply the same process to a one-paragraph answer or a long report. The following sequence keeps the work fast without treating speed as proof.

  1. Copy the output into a review document. Keep the prompt, model name if available, date, attached files, and any system instructions. This context helps explain later why an answer was produced.
  2. Mark every checkable assertion. Highlight exact figures, proper names, dates, quotations, “must” or “always” statements, comparisons, and explanations of cause and effect. Separate a recommendation from the factual premise on which it depends.
  3. Ask for a source map, not a second unaudited answer. You can request that the model list the claim, supporting passage, and uncertainty. Treat that list as a lead sheet: verify each item yourself because a model can hallucinate its citations too.
  4. Open the underlying documents. Confirm that the cited text exists and that it applies to your location, time period, population, product edition, or contract version. Read enough surrounding context to catch exceptions and definitions.
  5. Assign a disposition. Use labels such as verified, partly supported, contradicted, unsupported, or needs subject-matter review. A claim should not be published merely because nobody has yet disproved it.
  6. Rewrite for the evidence. Add a date or scope limitation, replace an absolute with a qualified statement, cite the controlling source, or delete the sentence. Keep the evidence and decision in the audit record.
  7. Obtain the required approval. Route high-risk items to the appropriate legal, clinical, security, finance, or safety reviewer before release or action.

Use a risk-tiered review policy

One review standard for every use either wastes time on low-consequence drafts or leaves dangerous gaps in high-consequence work. Set the minimum control by the harm an error could cause.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Risk tier Typical use Minimum control before use
Low Brainstorming, private outlines, style variations, or non-factual drafting A human checks that the output fits the request and contains no claims that will be reused as facts.
Moderate Internal summaries, technical explanations, product documentation, or operational guidance Claim extraction, source checks for material facts, recorded provenance, and review by someone who understands the subject.
High Legal analysis, medical information, financial decisions, safety instructions, privacy decisions, or public-facing claims Primary-source verification, explicit uncertainty handling, complete audit trail, and approval by a qualified domain expert.

Stanford Impact Labs’ summary of 2024 legal research cautions against rapid, unsupervised integration of popular language models into legal tasks. The concern is not limited to a wrong citation: fabricated precedents can alter a legal argument. The Nature paper similarly identifies untrue facts in news and potential harm in medical domains such as radiology. These cases justify stricter controls even when a model appears reliable on ordinary writing.

Automated checks, retrieval, and human review: what each can and cannot do

Teams often combine several controls. Compare them on the dimensions that matter to your consequence level rather than asking which tool is “most accurate” in the abstract.

Method Evidence quality Claim coverage Uncertainty handling Domain judgment Auditability Cost and latency
Automated semantic-uncertainty or consistency check Can flag disagreement or unusual uncertainty; it does not establish an authoritative source. Usually analyzes generated text or samples, so coverage depends on implementation. Useful for triage and abstention signals, but a confidence signal can still be wrong. Limited context and policy judgment. Strong if inputs, scores, and thresholds are retained. Fast and relatively inexpensive once deployed.
Retrieval-augmented generation Can ground an answer in a selected document collection. Checks only what was retrieved and what the model actually used. Can expose missing documents when designed to abstain; retrieval errors remain possible. Still needs a person to interpret law, medicine, policy, or business context. Good when document versions and passages are logged. Adds retrieval and indexing work and some response latency.
Human claim-level review Can choose authoritative sources and judge whether they entail the claim. Can cover every material statement when the checklist is followed. Can state uncertainty and request escalation. Best option for contextual and consequential judgment. Auditable when decisions, sources, and versions are recorded. Slowest and most expensive, so reserve expert time for higher-risk work.

NIST’s evaluation-probe work describes comparing outputs with a human-curated reference corpus and asking whether the evidence supports the conclusion. NIST’s 2025 diversion-decoding publication describes actively challenging generated responses to create an uncertainty heuristic. Both approaches can prioritize human attention; neither removes the need for a source check when the result matters.

Build a reviewable production workflow

Define the unit of review

Decide what counts as a claim in your organization. A number, date, named entity, quotation, legal obligation, medical instruction, causal statement, and “all” or “never” assertion should each be independently checkable. This prevents a reviewer from approving a polished paragraph while overlooking one dangerous clause.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Give every claim a status

A simple register can contain the claim text, risk tier, source and passage, source date or version, reviewer, decision, and escalation owner. “Verified” should mean the evidence entails the wording, not merely that the sentence sounds reasonable. “Unsupported” is a valid final status when the claim is removed or clearly labeled as uncertain.

Version the source collection

Policies, regulations, APIs, and product manuals change. Store the version or publication date used for approval. If a document is replaced, rerun checks on claims that depend on it rather than assuming the old approval still applies.

Separate drafting from authorization

Let a model generate alternatives, extract claims, or locate passages, but keep publication and consequential action behind a human approval gate. The person who can block release should see unresolved claims and the evidence record, not only the final prose.

Measure the workflow, not just the model

Track unsupported-claim findings, citation-entailment failures, reviewer overrides, escalation time, and errors discovered after release. Report the model, task, benchmark, date, and error definition with any rate. A lower model error rate on a benchmark is useful only if the benchmark resembles the work you actually perform.

Common review failures and how to correct them

Reading for tone instead of evidence

Failure: The reviewer edits grammar and accepts a confident paragraph. Correction: Extract atomic claims first and require a source or an uncertainty label for each material one.

Checking only the citation title

Failure: A source is related to the topic but does not support the exact number, population, date, or conclusion. Correction: Read the cited passage and its qualifiers, then test entailment claim by claim.

Accepting model-generated references

Failure: The model supplies a plausible-looking case, paper, URL, or quotation that cannot be found. Correction: Open the original record independently; if it does not exist, remove the reference and investigate every claim that relied on it.

Using one universal hallucination rate

Failure: A benchmark percentage is presented as the probability that any answer is wrong. Correction: State the model, task, benchmark, date, and definition, and avoid extrapolating beyond those conditions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Letting the model resolve conflicting evidence

Failure: The system chooses a neat answer when authoritative sources disagree. Correction: Show the conflict, identify which authority governs your case, and escalate when the decision is consequential.

Removing all uncertainty language

Failure: An editor deletes “may,” dates, scope limits, or evidence gaps to make copy sound decisive. Correction: Treat those qualifiers as part of the factual content. A narrower true statement is safer than a broad unsupported one.

Examples of the control in practice

Legal work

Extract every cited case, statute, deadline, and statement about what a court “will” do. Verify the authority and jurisdiction, then have a qualified legal professional approve the result. A model’s confident case summary cannot substitute for reading the opinion.

Medical and health information

Separate factual descriptions, contraindications, dosage or timing instructions, and individualized recommendations. Check current clinical or regulatory sources and require a clinician’s review before the content can guide a patient’s care. Do not treat a stable-sounding answer as evidence that the patient’s circumstances were understood.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Financial decisions

Check rates, dates, eligibility rules, tax treatment, and calculations against current authoritative records. Preserve the input assumptions and calculation steps so a reviewer can reproduce them. Escalate recommendations that could commit money or create a reporting obligation.

Customer and public communication

Verify product capabilities, prices, service status, policy wording, and named people before publication. Keep a dated evidence record because public corrections are costly and a customer may act immediately on a false statement.

A concise operating rule

Use AI to generate and organize candidate content; use evidence and accountable people to authorize it. Extract claims, check them against primary sources, preserve provenance, expose uncertainty, and escalate decisions where an error could cause material harm. That division of labor addresses the reason hallucinations occur: generation is optimized for plausible language, while human review supplies the missing test of whether the language is actually supported.
