What to Know About Brain-Computer Interface Surgery, Risks, and Recovery

Brain-computer interface surgery varies by device and may involve cranial implantation or catheter delivery through a vein. Here’s what the evidence says about risks, recovery, trial commitments, and care after a study ends.

Brain-computer interface (BCI) surgery is not one standardized operation. An implant may be placed through cranial surgery or delivered by catheter through a blood vessel, and the procedure, risks, follow-up, and training depend on the device and clinical-trial protocol. These devices are investigational; the limited human evidence described here cannot predict an individual’s outcome or establish a general complication rate or recovery timeline.

What does brain-computer interface surgery involve?

Implanted BCIs record or use neural signals to help a person operate a computer or another device. The surgical route depends on the system. For example, the Neuralink PRIME study brochure describes a robot placing the investigational N1 implant in a brain region involved in movement intention. A published early study of Synchron’s endovascular system describes a different route: a catheter-delivered stent-electrode array placed in the superior sagittal sinus, a vein next to motor cortex, with a telemetry unit in a pocket under the skin of the chest. These are distinct procedures, not two names for the same operation. PRIME study brochure; Synchron study.

The FDA’s 2021 guidance addresses nonclinical testing and study design for implanted BCIs for paralysis or amputation. It is guidance for industry and FDA staff, not a universal surgical protocol or a set of personal post-operative instructions. FDA guidance on implanted BCI devices.

Approach described in the sources Route and placement Human evidence and known follow-up details
PRIME study’s R1/N1 system The sponsor brochure describes robot-assisted placement of the N1 implant in a brain region involved in movement intention. The brochure does not state a comparable participant-level surgical-risk rate or recovery timeline. PRIME study brochure. The cited brochure describes a study protocol and schedule; it is not a comparative clinical safety study. PRIME study brochure.
Synchron endovascular system The 2021 report describes catheter delivery of a stent-electrode array into a vein adjacent to motor cortex and a telemetry unit in a subcutaneous chest pocket. 2021 study. The first-in-human early feasibility report included two participants with ALS, with unequal follow-up: 12 months for one and three months for the other. It cannot establish a general risk rate. 2021 study.

The sources do not support ranking these approaches as safer or better. The studies differ in device, procedure, protocol, and amount of human evidence.

What are the risks of a brain implant?

There is no single risk list or reliable population-level complication rate that applies to all implanted BCIs. The study team should explain the risks for the specific device and procedure, including which are known and which remain uncertain. In covered clinical investigations, FDA consent requirements include “A description of any reasonably foreseeable risks or discomforts to the subject” under 21 CFR 50.25(a)(2). FDA informed-consent requirements.

What the small Synchron report can—and cannot—show

In the 2021 Synchron report, the authors observed no serious adverse events and no device-related adverse events, including headache or infection, during the reported follow-up of two participants with ALS. One participant had brief fainting after the procedure, associated with sinus pauses on the first day; the authors attributed it to post-procedural vagal tone and reported that no intervention was needed. The authors said larger studies were needed to characterize safety. Results in two people over different follow-up periods cannot establish that the system, or implanted BCIs generally, are risk-free. 2021 Synchron study.

Questions to resolve before consenting

  • What risks are known for this device and procedure, and what uncertainties remain?
  • What monitoring and treatment are available if a complication occurs? Who should be contacted, and what care is covered if the research causes an injury?
  • Could participation involve costs beyond routine care, and who pays for them?
  • What happens if the study team ends your participation, you choose to withdraw, or the sponsor stops supporting the device?
  • What alternatives are appropriate for you, including not joining the study?

FDA consent requirements for covered research address the study and its experimental procedures, possible benefits and alternatives, confidentiality, injury-related contact information, and voluntary participation. Depending on the study, consent should also explain uncertain risks, circumstances in which participation may be ended, additional costs, withdrawal consequences, new findings, and approximate enrollment. Ask for the current consent form, a plain-language explanation, and time to consider it. The regulation says participation is voluntary and that a participant may discontinue without penalty or loss of otherwise entitled benefits. FDA informed-consent requirements.

How long does it take to recover from BCI surgery?

There is no established recovery duration that applies across BCI procedures. Hospital discharge, wound healing, device setup, decoder training, and independent use are separate milestones; reaching one does not mean the others are complete.

In the 2021 report of two Synchron participants, one was discharged home two days after the neurointervention and the other after four days. Training began between days 42 and 92 after the procedure, following time for wound recovery and optimization of communication between telemetry units. Unsupervised home use began on days 71 and 86. Those dates describe two people in one protocol, not a forecast for another patient or device. 2021 Synchron study.

The PRIME brochure instead describes continuing research sessions and follow-up, rather than a single return-to-normal date. Its stated schedule is specific to that sponsor’s protocol and may change. PRIME study brochure.

What happens after the BCI clinical trial ends?

Trial obligations can continue well beyond implantation. The Neuralink PRIME brochure describes an approximately six-year total study, comprising an approximately 18-month primary study and five years of long-term follow-up. It calls for at least two one-hour BCI research sessions each week during the primary study and 20 visits during long-term follow-up. This is one sponsor’s protocol schedule, not a field-wide requirement; schedules can change. The sponsor’s trial listing, accessed October 4, 2026, labels computer/robotic-arm control and speech-decoding studies active and vision-restoration studies upcoming. Confirm current status, location, eligibility, and recruitment directly with the study team and an authoritative trial registry. PRIME study brochure; sponsor trial listing.

Planning for what happens after a trial is particularly important for an implanted device. A March 25, 2026 NIH notice described draft, voluntary resources that highlight possible ongoing needs such as battery replacement, software updates, adjustments, repairs, infection monitoring, and explantation. The notice also raises questions about commercial hardware or software compatibility, future availability of replacement parts, and whether insurance will cover continuing interventions. These resources are drafts, not binding requirements. NIH notice NOT-OD-26-061.

Ask who is responsible for long-term care

  • Does the device remain implanted when the study ends, or is removal planned? Who makes that decision, and who performs and pays for removal if needed?
  • Who funds and provides maintenance, repairs, software updates, monitoring, and follow-up after sponsor support ends?
  • Who can service the device, and what happens if compatible parts or software are no longer available?
  • Which expenses may fall to you or your insurer, including care after withdrawal or study completion?

Can the implant be removed?

The materials cited here do not establish one removal policy for implanted BCIs. Whether an implant stays in place, can be removed, or should be removed depends on the device and study protocol, and removal may involve another procedure. Do not assume either that the implant will be removed automatically at the end of a trial or that it can remain indefinitely without maintenance. Ask the team to explain the device-specific plan, possible consequences of removal, and who is responsible for follow-up and costs before enrolling.
