What to Expect During and After Radiopharmaceutical Therapy

Radiopharmaceutical therapy uses a radioactive medicine designed to reach a biological target associated with disease. Your visit may involve an infusion, the treatment dose and sometimes imaging; afterward, temporary radioactivity, treatment-specific precautions, side effects and scheduled monitoring are possible. The exact plan comes from your nuclear-medicine, oncology and radiation-safety teams, not from a universal schedule.

What radiopharmaceutical therapy is and how it works

Radiopharmaceutical therapy (also called molecular radiotherapy, or radioligand therapy for selected treatments) combines two parts: a radionuclide that releases therapeutic radiation and a molecule chosen to recognize a biological feature of the disease. The molecule carries the radionuclide to cells or tissues with that target, so radiation is delivered at or near the disease site. Different cancers and disease markers require different medicines, doses, schedules and eligibility checks.

This is not one standardized treatment. Peptide receptor radionuclide therapy (PRRT), prostate-specific membrane antigen (PSMA)-targeted therapy and other approaches use different targeting molecules and radionuclides. The target must be present in a way that makes the treatment appropriate, and the team also considers factors such as kidney function, blood counts, prior treatment and the ability to follow radiation-safety instructions.

Because the medicine is radioactive for a limited period, a small amount can remain in the body after administration. It may leave through urine or feces. That temporary radioactivity is why the facility gives individualized instructions about contact with other people, bathroom use, hygiene, travel, work and follow-up.

Before the appointment: questions and practical preparation

Preparation depends on the specific radiopharmaceutical and your facility’s protocol. General patient materials do not establish one preparation routine for every therapy, so ask the treating team for written instructions before the day of treatment.

Confirm what you need to do

  • Ask whether you must fast, change your usual diet or drink extra fluids before arriving.
  • Ask how long you should expect to be at the center, including registration, laboratory checks, infusion time, observation and any imaging.
  • Give the team a complete list of prescription medicines, over-the-counter products and supplements. Ask which ones should be continued, paused or discussed with the prescribing clinician.
  • Ask whether you need a ride home or whether someone should remain available afterward. The answer can depend on the treatment, your symptoms and local radiation-safety rules.
  • Ask whether a caregiver may stay nearby during the visit and whether there are restrictions on visitors after you leave.

Plan around your household and work

Before treatment, tell the radiation-safety team who lives with you, whether anyone is pregnant or an infant, and whether your work involves close contact with people. Ask for specific guidance on sleeping arrangements, childcare, public transportation, air travel, workplace attendance and contact with pets. Do not assume that an instruction supplied for one therapy applies to another.

What happens during a treatment visit

A clinical team selects and administers the therapy. The exact sequence differs by medicine and center. You may have identification and safety checks, clinical assessment, an intravenous line, laboratory review, supportive medication, the radiopharmaceutical itself, observation and imaging. Ask the center which of these steps applies to you and in what order.

A six-hour Lutathera example

Memorial Sloan Kettering Cancer Center’s patient guide describes a Lutathera visit that includes an amino-acid infusion, administration of the radiopharmaceutical and a nuclear-medicine scan to check where the medicine traveled. That guide says the infusion visit and scan take about six hours. This is a Lutathera-specific example at MSK, not a typical duration for all radiopharmaceutical therapies.

The amino-acid infusion in that example is part of the treatment protocol and can be associated with nausea. Your center may use a different protective infusion, different medicines or no comparable step. Ask whether anti-nausea treatment will be offered and what to do if you feel unwell during the infusion.

Monitoring and communication during the visit

Tell staff promptly about nausea, vomiting, pain, dizziness, shortness of breath, a new rash or any other sudden change. Ask who will explain the scan, when you can leave and whether you will receive written radiation-safety instructions before discharge. If the plan changes because of laboratory results, symptoms or another clinical concern, ask what changed and whom to contact afterward.

Immediately after treatment: radioactivity and discharge instructions

After some treatments, small amounts of radioactivity remain temporarily in the body and are excreted in urine or feces. The facility’s radiation-safety team determines how long precautions are needed. In the MSK Lutathera example, patients receive written instructions and the length of precautions depends on the individual treatment.

Instructions may include keeping distance from other people for several days, limiting prolonged close contact, and using careful bathroom hygiene. The details are treatment- and patient-specific. Follow the written sheet you receive rather than a generic internet checklist, and keep it available for household members or caregivers who need to understand the restrictions.

Questions to ask before leaving

  • How many days do the contact and hygiene precautions last for this dose?
  • What distance and time limits apply to a partner, children, pregnant people and infants?
  • Can I work, attend school, travel or use public transportation, and from what date?
  • What should I do about toileting, handwashing, laundry, dishes, bedding and accidental spills?
  • Are there instructions for storing or disposing of bodily fluids, dressings or other contaminated items?
  • When may normal physical contact resume?
  • Whom should I call if I vomit, have diarrhea, cannot urinate, lose the written instructions or cannot follow a restriction at home?

Hydration, urination and close-contact precautions

Some protocols emphasize fluids and frequent urination to help clear radioactivity and reduce exposure to the bladder. The 2023 joint guideline from the European Association of Nuclear Medicine and the Society of Nuclear Medicine and Molecular Imaging discusses hydration and frequent voiding for lutetium-177 PSMA therapy. It also says these measures must be considered in light of cardiovascular condition and clinical judgment. Do not increase fluid intake on your own if you have heart failure, kidney disease, fluid restrictions or another condition requiring a prescribed limit.

The same guideline discusses minimizing prolonged close contact with pregnant people or infants after outpatient Lu-177 PSMA treatment. The duration and exact wording of the restriction depend on the administered activity, the facility’s protocol and your circumstances. A different radiopharmaceutical may require different precautions.

Situation What may apply What you should do
Any radiopharmaceutical therapy Temporary radioactivity and excretion in urine or feces may lead to short-term contact or hygiene precautions. Use the written instructions from your facility; ask about household, work, travel and toileting arrangements.
Lutathera example at Memorial Sloan Kettering Written safety instructions are provided; the precaution period depends on the individual treatment. Follow the MSK radiation-safety team’s directions for contact and hygiene rather than assuming a fixed number of days.
Lu-177 PSMA therapy Guidance discusses hydration, frequent voiding and limiting prolonged close contact with pregnant people or infants after outpatient treatment. Confirm the plan with your treating team, especially if you have cardiovascular disease or a fluid restriction.

Side effects: what can happen and when to call

Side effects vary with the radionuclide, targeting molecule, dose, other medicines, disease and your underlying health. A symptom listed for one treatment is not a prediction for every patient.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Examples reported for particular therapies

  • SNMMI patient information on ligand-targeted prostate radiotherapy lists fatigue, dry mouth and decreased blood counts among common effects.
  • SNMMI’s PRRT guidance discusses nausea associated with the amino-acid infusion used in some protocols.
  • The same PRRT material discusses possible longer-term effects involving blood counts or kidneys.

Ask your team which effects are expected, when they usually begin for your treatment, how long they may last and which supportive medicines are appropriate. Do not start a supplement or stop a prescribed medicine to manage a symptom without checking with the treatment team.

Urgent and non-urgent contact plans

Before discharge, obtain a daytime number and an after-hours number. Ask which symptoms require an immediate call, an emergency evaluation or routine discussion at the next appointment. Examples that deserve prompt clarification include persistent vomiting or diarrhea, inability to keep fluids down, markedly reduced urination, fever, bleeding, severe weakness, new confusion, chest pain, breathing difficulty or a rapidly worsening condition. The correct response depends on your clinical situation; use the emergency service for a life-threatening problem.

Radiation effects can also be delayed. The National Cancer Institute’s general radiation-therapy guidance notes that some late effects may appear months or years after treatment. That information covers radiation therapy broadly, not radiopharmaceutical therapy specifically, so ask your clinician which delayed effects are relevant to your medicine and how they will be monitored.

Follow-up tests and assessing response

Follow-up is part of the treatment plan, not an optional add-on. Your team may schedule blood tests to check counts and kidney function, clinical visits, imaging or additional treatment cycles. The timing depends on the medicine, dose, disease, prior therapy and your recovery.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Ask these questions before the first dose:

  • When will my first blood test be, and which results could delay another treatment?
  • How will kidney function and blood counts be tracked over time?
  • Will I have imaging to assess where the medicine went or whether the disease is responding?
  • What result would lead to another cycle, a dose change, a pause or a different treatment?
  • Who reviews results, and how will I receive them?

Keep a record of treatment dates, symptoms, medicines, fluid restrictions, laboratory results and questions. Bring it to each visit, particularly if more than one team is involved.

When clinicians compare treatment options

There is no universal ranking of radiopharmaceutical therapies. If your care team is discussing alternatives, compare the options using the same questions rather than focusing only on the name of the drug.

Decision area Questions for the care team
Target and eligibility Which disease marker does each treatment target? What test confirms that I am eligible, and how certain is that evidence?
Schedule and visit burden How many administrations are planned? How long is each visit, including monitoring and imaging?
Short-term effects Which symptoms are most likely with this medicine, and what prevention or treatment is available?
Delayed effects What longer-term blood-count, kidney or other risks are relevant, and for how long will monitoring continue?
Radiation precautions How long will I need limits on close contact, work, travel or household activities?
Response assessment Which blood tests, scans or clinical measures determine whether treatment is working?

A practical discharge checklist

  1. Collect the written radiation-safety instructions and confirm the date and time when each restriction ends.
  2. Write down the treatment name, dose if provided, emergency contact and next appointment.
  3. Confirm fluid and urination instructions, including any limits caused by heart or kidney conditions.
  4. Tell household members about the relevant contact and hygiene rules without adding restrictions that the team did not give you.
  5. Plan transportation, work, childcare and sleeping arrangements for the precaution period.
  6. Record which symptoms should trigger a same-day call and which require emergency care.
  7. Attend the scheduled blood tests, imaging and clinical reviews even if you feel well.

Are radiopharmaceutical therapies safe?

Safety depends on choosing an appropriate target and dose, checking organ function and blood counts, monitoring you during and after treatment, and following the facility’s radiation-safety instructions. The existence of temporary radioactivity does not by itself mean treatment is unsafe, but it does mean the instructions matter. A rule that is suitable for Lutathera or Lu-177 PSMA may be unsuitable for another medicine.

Ask your own team to explain the expected benefit, alternatives, short- and long-term risks, precautions and monitoring schedule in terms of your diagnosis. If instructions conflict, contact the treating radiation-safety or nuclear-medicine team and follow the most current written plan they confirm.
