If a WhatsApp appointment reminder seems to be missing, first ask the patient to open WhatsApp and check the clinic chat. A message in the chat with no alert points to a notification problem; no message in the chat means the clinic should check its recipient details and sending workflow. The next steps depend on whether the clinic sends reminders through the WhatsApp Business App or the WhatsApp Business Platform or a provider integration.
First check whether the message arrived
Ask the patient to open WhatsApp and look for the clinic conversation. A message visible there reached the chat, even if the patient did not see a banner or hear a sound. WhatsApp distinguishes push notifications from in-app messages in its notification help.
| What the patient sees | What to check next |
|---|---|
| The reminder is in the clinic chat, but no alert appeared | Check WhatsApp notification permission, background data, and battery settings on the patient’s device. |
| The reminder is not in the clinic chat | The clinic should check the number, recipient settings, reminder schedule, consent and account or template setup, then sender-side status information if available. |
If the message is present but there was no alert
Review WhatsApp’s notification permissions and the device settings that affect background data and battery use. The patient can also check the conversation directly for any reminder they may have missed. Avoid resending a reminder automatically: first confirm what is already in the chat.

If the message is absent
The patient cannot tell from their phone alone whether the clinic never sent the message or whether it failed later in delivery. Clinic staff need to check the sending workflow and, when available, the sender’s records.
What clinic staff should verify
- Confirm the recipient. Check that the number on file is the patient’s current WhatsApp number and includes the appropriate country code. Review any recipient rules that may have excluded the patient. WhatsApp’s Business App greeting-message troubleshooting specifically recommends checking recipient settings.
- Check the appointment and reminder workflow. Confirm that the reminder feature or integration is enabled, the appointment details are correct, and the reminder was scheduled for the intended recipient and time. Verify connectivity and whether the feature or app needs attention. WhatsApp’s greeting-message troubleshooting also suggests toggling that feature, updating WhatsApp Business, and restarting the device; those suggestions apply to greeting messages and do not establish the cause of every missing appointment reminder.
- Verify consent and messaging setup. WhatsApp’s Business Policy requires a business to obtain opt-in permission before contacting a person. It also requires an approved Message Template when a business initiates a conversation. Ask the account administrator or messaging provider to check the consent record, template, and account setup. Meta explains template use for businesses on the WhatsApp Business Platform.
- Check sender-side status, if the clinic uses the Platform or a provider. Ask the administrator or provider to inspect any message-status notifications or logs for the recipient and sending time. Meta’s webhook documentation describes status update notifications. The exact dashboard, retry process, and meaning of every status depend on the clinic’s setup; the documentation does not establish those details for every account.
- Use the clinic’s fallback procedure. If the appointment remains unconfirmed, follow the clinic’s established alternative contact and confirmation process. There is no single fallback procedure established for every clinic or jurisdiction.
Why the sending method matters
The WhatsApp Business App and the WhatsApp Business Platform are different workflows. App-level troubleshooting instructions do not necessarily describe a clinic’s appointment system or a third-party integration. For a Platform or provider setup, sender-side status information may be available; the clinic administrator or provider is best placed to interpret it.

WhatsApp says its Business App marketing-messages feature can be used for appointment reminders, with processing typically taking a few seconds but potentially up to three hours. That timing is specific to that feature, not a delivery guarantee for every reminder setup. WhatsApp also says funds are credited back for messages it cannot deliver through that feature. See its marketing-messages help.

Before sending appointment information, clinics should assess applicable law, their privacy obligations, and whether the product and workflow are suitable for the information involved. WhatsApp’s Business App terms place responsibility for legal and regulatory compliance on the business and state that WhatsApp does not represent that the Business App meets the heightened confidentiality needs of healthcare entities. The Business Policy also makes the business responsible for required notices, permissions, and consents.
