First preserve the rejection notice, determine whether you attempted a trustee-to-trustee transfer or a 60-day rollover, and establish whether the money was ever distributed to you. Then ask the receiving institution whether it accepts the exact account type, asset and network. If the 60-day deadline is approaching, withholding occurred, or anyone suggests moving IRA-owned cryptocurrency to your personal wallet, stop and obtain advice from a tax professional or fiduciary adviser experienced with self-directed IRAs.
Start with the transaction’s legal category
The word “rollover” is often used for two different transactions. The distinction controls whether a 60-day deadline, withholding and late-rollover relief are relevant.

| Transaction | How it works | Typical tax treatment | What a rejection means |
|---|---|---|---|
| IRA-to-IRA trustee-to-trustee transfer | The sending trustee sends funds directly to another trustee; you do not receive the money. | Generally not a rollover and generally tax-free because the distribution is not paid to you. IRS Publication 590-A (2025) describes a direct transfer from one traditional IRA trustee to another as not being a rollover. | Usually an operational problem between institutions. Ask the custodians to correct and resend it through their approved process. |
| Rollover | The distribution is paid to you, and you contribute it to another eligible IRA or plan. | Generally must be completed within 60 days after you receive the distribution. Tax withholding and eligibility rules can apply. | The clock continues while you hold the money. A rejected contribution can become a late rollover unless the receiving institution accepts a corrected deposit or you qualify for relief. |
Do not rely on the label in an email or portal. Ask both institutions, in writing, whether the transaction was coded as a trustee transfer or a distribution to you. Keep the payment date, check or wire details, tax forms and every rejection message together.

Immediate troubleshooting sequence
1. Obtain the exact reason in writing
Request a written explanation from the sending and receiving institutions. Ask whether the failure involved an account-title mismatch, an unsupported asset or blockchain network, incomplete paperwork, a missing signature or medallion guarantee, plan eligibility, a contribution limit, or an internal risk rule. There is no universal IRS crypto-custodian rejection-code list, so the institution’s notice and account agreement are important evidence.

Ask for the specific correction, not merely a status such as “failed” or “unable to process.” For example, the custodian may require a new form with the legal registration exactly as shown on the statement, a different delivery address, a particular network, or an original signature.
2. Confirm where the money is now
If the assets or cash remain with the trustees, do not withdraw them to make the process easier. Ask the custodians to void or return the failed instruction and resend it using their documented transfer procedure. Obtain confirmation of who has custody while the correction is pending.
If a check or wire was paid to you, record the date you received it and the amount before tax withholding. Keep it separate from ordinary spending and do not commingle it with personal crypto. Ask the receiving institution whether it can accept the contribution before the 60-day deadline and exactly how it must be delivered.
3. Verify the receiving institution’s acceptance rule
An IRA or retirement plan is not required to accept rollovers. If it does accept them, the funds must come from an eligible plan or IRA, be a permissible type of funds under the plan document, and satisfy timing and documentation requirements. A crypto platform may accept a traditional IRA but not a Roth, SEP, SIMPLE or inherited IRA, or it may accept cash but not an in-kind digital-asset transfer.
Ask for a written yes or no on all of these points:

- the exact registration: traditional, Roth, SEP, SIMPLE or inherited IRA;
- whether the institution accepts a trustee transfer, a rollover, or both;
- whether the asset is accepted in kind or must be liquidated to cash;
- the exact token, chain and network used for delivery;
- the legal owner of the receiving wallet and who controls signing keys;
- required forms, signatures, medallion guarantees, tax forms and delivery addresses; and
- any processing, liquidation, transfer or wire fees and how they will be reported.
4. Reconcile the account title and registration
The name on the sending account, receiving account and transfer form must describe the same legal ownership. A personal name, trust name, employer-plan name and IRA registration are not interchangeable. Have the institutions compare the full title, account number, tax identification details and successor or beneficiary designation before a second attempt.
5. Escalate before moving the assets again
Contact a certified public accountant, enrolled agent, tax attorney or fiduciary adviser familiar with self-directed IRAs when the 60-day date is close, the distribution was paid to you, taxes were withheld, the amount is substantial, or a custodian proposes sending crypto to a personal wallet. Ask the adviser to review the transaction documents rather than relying on marketing language about an “IRS-approved crypto IRA.”

Account-title or registration mismatch
A transfer can fail when one institution uses “John Doe Traditional IRA” and the other expects the trustee’s legal name followed by a specific IRA registration. Correct the title with the custodians before sending new instructions. Do not alter a statement or invent a shortened title.
Unsupported coin, chain or network
Digital assets with similar symbols can exist on different blockchains. A custodian may support a token but not the network selected in the instruction. Sending on an unsupported chain can make assets unrecoverable. Obtain the receiving institution’s written asset-and-network specification and use only its approved deposit route.
Incomplete forms or authentication
Missing pages, stale forms, an absent medallion guarantee, an electronic signature where an original is required, or an unverified bank account can stop a transfer. Ask for a checklist and resubmit one complete package. Keep a PDF of every signed page and proof of delivery.

Plan or account ineligibility
The receiving plan may not accept the source account, the particular type of rollover, or an in-kind contribution. A plan document can impose rules that are narrower than the general IRS rules. Request the relevant acceptance provision or written explanation and identify another eligible destination only after a professional confirms the tax consequences.

Internal risk or compliance review
Custodians may pause or refuse transactions under their security, anti-fraud or digital-asset risk policies. This does not establish that the IRS prohibited your transaction. Ask whether the institution will accept a corrected instruction, cash instead of an in-kind asset, or a transfer through a different approved network. Never bypass a compliance hold by routing the asset through a personal wallet.
What to do when the 60-day deadline is approaching
For a rollover paid to you, count 60 days from the date you received the distribution. The general rule in IRS Publication 590-A (2025) is that the contribution must reach an eligible plan or IRA within that period. A rejection does not, by itself, extend the deadline.
- Write down the distribution date, gross amount, tax withheld and current location of the funds.
- Ask the receiving institution for its fastest compliant deposit method and a written confirmation that it will accept the contribution.
- Send only through the institution’s approved channel, retaining tracking, wire confirmation and the completed forms.
- If acceptance is uncertain or the deadline is near, have a tax professional assess whether a different eligible IRA or plan can receive the funds and how any replacement of withholding must be handled.
- Do not spend, pledge, lend, stake or convert the distributed assets while the tax treatment is unresolved.
If the money was transferred directly between trustees and never paid to you, the 60-day rollover clock generally does not apply. The institutions should correct the operational failure rather than treating it as a personal distribution.
Late-rollover relief: when it may help
IRS Publication 590-A (2025) describes automatic self-certification under Revenue Procedure 2020-46 for certain listed reasons, including some errors by a financial institution. Self-certification is a statement that you qualify; it is not an IRS ruling and does not prevent the IRS from later deciding that the conditions were not met. Follow the procedure’s model language and keep evidence of the event that caused the delay.

A private letter ruling may be available when the facts justify a waiver, but it is discretionary, can take time and does not automatically make an unsuitable crypto transaction valid. A tax adviser should determine whether requesting one is proportionate to the amount and facts.
Late-relief analysis depends on why the rollover failed, how long the delay lasted, whether the funds were held separately, whether you acted reasonably and whether the receiving plan is willing to accept the contribution. Do not describe a self-certification as “IRS approval” to a custodian or future adviser.

The Commodity Futures Trading Commission warns that the IRS does not approve or review IRA investments. Its customer advisory says: “Never transfer or rollover your IRA or 401(k) directly to a virtual currency or investment promoter.” The advisory also recommends verifying promoters and custodians with state or federal regulators and warns that retirement-plan status does not remove crypto volatility or loss risk.
- Confirm the custodian’s exact legal name and regulatory registration using government or state records.
- Confirm whether the account is a traditional, Roth, SEP, SIMPLE or inherited IRA and who legally owns the receiving wallet.
- Confirm the exact asset, contract or token address where relevant, blockchain and network before any in-kind movement.
- Use deposit instructions supplied through the custodian’s authenticated channel, not an address sent only by an unverified promoter.
- Keep screenshots, statements, transaction hashes, forms and correspondence in a dated file.
Marketing that calls an arrangement “IRS-approved” does not mean the IRS endorsed the promoter, custodian, token or investment strategy. The IRS sets tax rules; it does not certify a particular crypto investment.
Tax and prohibited-transaction dangers
Do not borrow against the IRA, pledge it as loan security, sell personal property to it, or use IRA-owned cryptocurrency personally. IRS Publication 590-B states that a prohibited transaction is generally any improper use of an IRA by you, your beneficiary or a disqualified person.
Best Value
If the account stops being an IRA because of a prohibited transaction, Publication 590-B explains that the account is treated as distributing all assets at fair market value on the first day of the year. The publication describes a 15% tax on the amount of a prohibited transaction and a 100% additional tax when the transaction is not corrected in the circumstances covered by the rule. The exact result depends on who engaged in the transaction and the facts.
Keeping crypto in a wallet you personally control can create ownership and prohibited-transaction questions even if the intent was temporary. Leave IRA assets with the custodian until a tax professional confirms the legal structure and the institution’s written procedures.
What the 2026 IRS rollover guidance does—and does not—change
Treasury and IRS Notice 2026-49, together with related IR-2026-91 sample procedures, addresses rollovers between retirement plans and IRAs. The IRS states that this guidance does not apply to IRA-to-IRA transfers. It therefore does not automatically require a crypto IRA custodian to accept an IRA-to-IRA transfer, waive its asset restrictions or overlook missing documentation.

Choose the next move using the facts
| Situation | Most sensible next step | Main risk to control |
|---|---|---|
| Trustee-to-trustee transfer rejected; funds never reached you | Have the custodians correct the title, forms or delivery instructions and resend through the approved process. | Accidental distribution caused by withdrawing the funds yourself. |
| Rollover check or wire is with you and more than several days remain | Obtain written acceptance instructions and deposit promptly into an eligible destination. | Missing the 60-day deadline or failing to replace withheld tax where required. |
| Deadline is close or has passed | Stop new transfers and consult a CPA, enrolled agent or tax attorney about self-certification, another eligible recipient or a private letter ruling. | Assuming a custodian’s refusal automatically qualifies for relief. |
| Custodian proposes a personal wallet or unverified promoter | Decline, preserve the assets in custodial channels and obtain independent fiduciary or tax advice. | Loss of custody, fraud, network error or a prohibited transaction. |
| Receiving institution will not accept the account type or asset | Ask whether cash liquidation or another eligible institution is permitted, then obtain tax advice before changing the investment. | Creating a taxable distribution or violating the IRA’s investment and custody rules. |
Document packet to assemble
- Original transfer or rollover request and every signed page.
- Statements showing the source account, registration and asset quantities.
- Rejection notices, ticket numbers, call dates and names or departments contacted.
- Check images, wire confirmations, transaction hashes and blockchain network details.
- Distribution date, gross amount, withholding amount and the date any contribution was attempted.
- The receiving institution’s written acceptance policy and fee schedule.
- Any self-certification, professional advice or private-letter-ruling correspondence.
This record lets a tax professional distinguish an uncompleted trustee transfer from a distribution that is subject to the 60-day rule, and it gives the custodians the information needed to correct an operational error without improvising a new transaction.
