If a breach notice lists only your name and address, save the notice, verify it with the organization, and check whether it identifies any other exposed information. A name-and-address exposure alone is not proof of identity theft, and the cited U.S. guidance does not make a credit freeze or paid monitoring an automatic requirement. Your next steps depend on whether passwords, financial details, or a Social Security number were also exposed.
Start by checking exactly what was exposed
- Keep the notice. Save a copy and note the incident date, the data fields involved, and any steps the organization says it has taken.
- Verify an unexpected notice. Contact the organization using its known website or contact details, rather than relying on links or phone numbers in a message you were not expecting.
- Look for other exposed data. Check whether the notice also names a password, payment-card or bank-account details, a driver’s-license number, or your Social Security number. The Federal Trade Commission’s breach checklist recommends different actions for different types of information; it is dated September 28, 2015.
If the notice confirms only a name and address, the cited FTC guidance does not prescribe a special account-recovery step, paid service, or credit freeze for that exposure by itself. Stay alert for signs of misuse, and use the steps below if the notice lists additional data.

Take action based on the other information exposed
If a password or login was exposed
Change the password on the affected service and on any other account where you reused it. Check the affected account for unfamiliar activity, especially if it stores payment information.

Contact the card issuer using the number on your card or its official website. The FTC checklist advises asking the issuer to cancel the affected card and provide a replacement. Review transactions and update automatic payments tied to the old card.

If bank-account details were exposed
Contact your bank promptly. The FTC checklist advises asking the bank to close the affected account and open a new one. Review transactions and update automatic payments after the change.
If your Social Security number was exposed
The FTC checklist recommends checking your credit reports, considering a credit freeze, and taking steps to guard against tax identity theft. A freeze restricts access to your credit report, which can make it harder to open new credit in your name; it can also mean extra steps when you apply for credit or another service that checks your report. The FTC’s specific recommendation to consider credit protection and monitoring appears in its Social Security number guidance, not as a blanket response to a name-and-address-only exposure.

Know the difference between a fraud alert and a credit freeze

The FTC explains how to place a fraud alert or credit freeze and provides current credit-bureau contact links. A fraud alert is the lighter-touch option; a freeze places a restriction on access to your report. Consider either when the exposed data or signs of misuse make new-credit fraud a concern, rather than treating either as mandatory for every name-and-address notice.

Respond promptly if you see signs of identity theft
A breach notice alone does not establish that anyone has used your information. Warning signs include unfamiliar accounts or charges, unexplained withdrawals, or debt-collector calls about debts you do not recognize. If you see one:
- Contact the affected company’s fraud department and ask it to close or freeze any compromised account.
- Change the passwords for relevant accounts and document the dates, people, and actions involved in your contacts.
- Use the FTC’s IdentityTheft.gov reporting and recovery process. The FTC says its form can create an Identity Theft Report and a personalized recovery plan.
- Check your credit reports for accounts you do not recognize. The FTC directs consumers to AnnualCreditReport.com for free reports and says consumers can check weekly for free.
