It does not mean the player is definitely out, and the NFL does not define it as a set percentage chance of playing.
What does questionable mean in football?
The NFL’s 2017 Injury Report Policy defines “Questionable” as “Uncertain if player will play.” The designation describes the club’s expectation about the player’s availability for its next game—not a prediction that the player will or will not take the field.

The NFL removed the former “Probable” category. These labels are not official percentage bands, so “Questionable” should not be translated into a specific probability unless a separate source gives an estimate and is clearly identified as its source.

Does questionable mean a player will play?
No. It means the outcome is uncertain. A player listed as Questionable may play or may not play; the designation alone does not settle the decision.
Does questionable mean the player missed practice?
Not necessarily. The practice report and the game-status report cover different things. Practice reporting describes the player’s injury status and participation during the practice week, while the Game Status Report describes anticipated availability for the next game.

The NFL policy says an injured player may need to appear on a practice report even when the club is certain he will play. In the policy’s example, that player would not be listed on the Game Status Report. As a result, “Questionable” alone does not tell you whether a player missed practice, was limited, or followed a particular practice pattern.
When is the designation reported, and can it change?
Reporting deadlines depend on the season and game schedule. For the 2026 regular season, the NFL’s important-dates calendar lists practice reports on Wednesday, Thursday, and Friday for a Sunday game, with the Game Status Report due Friday. The NFL also says a change in a player’s condition after the initial Game Status Report must be reported.

For a specific game, check its latest report: the initial status may no longer reflect the player’s condition later in the week or on game day.
