An NFL team can use its designated emergency third quarterback only when both quarterbacks on its game-day active list are unable to play because of injury or disqualification. A player being called the “third-string quarterback” is not enough: he must have been designated before the game and meet the league’s roster requirements.
When can the emergency quarterback enter?
The NFL’s September 4, 2023 rule FAQ says the emergency third quarterback may enter only when both active quarterbacks are unable to play because of injury or disqualification. The league gives the example of QB1 being unable to play while QB2 is being evaluated for an injury.

That means an ordinary coaching decision does not open the door. A head coach cannot activate the emergency quarterback simply because QB1 or QB2 is playing poorly or has been removed for conduct reasons, as the NFL clarified in its 2023 explanation of the rule.

Who qualifies as the third quarterback?
The emergency player must be a bona fide quarterback on the team’s 53-player roster and must be designated as the emergency third quarterback before the regular-season or postseason game. QB1 and QB2 must also be on the 53-player roster. The club must have exactly two bona fide quarterbacks on its game-day active list; if three or more quarterbacks are active, the special emergency procedure is unavailable.

So “third-string” is informal shorthand, not a game-day eligibility status. A quarterback listed third on a depth chart can enter under this exception only if the team completed the formal designation and the injury-or-disqualification trigger is met.
Can a quarterback who left the game return?
Yes. If QB1 or QB2 is evaluated for an injury and cleared to return, that player may re-enter, according to the NFL’s FAQ. The emergency quarterback does not permanently replace an earlier quarterback merely by entering the game.

Does a practice-squad quarterback count?
An elevated practice-squad player cannot fill the emergency third-quarterback role under the rule documented by the NFL. The emergency player must come from the 53-player roster. Practice-squad elevation is a separate mechanism that places a player on the game-day active/inactive list.

Why does this exception exist?
The emergency option returned in 2023 after San Francisco lost usable quarterbacks Brock Purdy and Josh Johnson to injuries during the NFC Championship Game against Philadelphia. The NFL reported that 49ers coach Kyle Shanahan and general manager John Lynch supported an emergency option; the league’s account of their support provides that context. The current exception is tightly limited: it is for a team that has lost both active quarterbacks to injury or disqualification, not a general roster workaround.
