What Do Atypical B Cells Mean for Immunity to Malaria?

Atypical memory B cells are linked to malaria exposure, but they are neither a simple sign of immune failure nor a proven measure of protection.

Atypical memory B cells are associated with malaria exposure, but finding them does not by itself mean that a person is protected—or that their immune system has failed. Some show features linked to dampened B-cell signaling, while human studies also show that atypical cells can produce antibodies with parasite-directed activity. Their overall contribution to durable protection against malaria remains unresolved.

What are atypical memory B cells?

Atypical memory B cells (atMBCs, also called aMBCs) are antigen-experienced B cells identified by a pattern of surface markers. Many malaria studies describe them as having reduced or absent CD21 and CD27, markers commonly found on classical memory B cells. Some atypical populations also express T-bet, CD11c, CXCR3, or FcRL5.

The word “atypical” describes a measured phenotype; it does not define one uniform cell type with a single known job. Researchers do not use an identical marker panel in every study, and the populations being examined can differ in their activation state, antigen specificity, and other characteristics. A cell classified as atypical in one experiment may therefore not be equivalent in every respect to a cell given the same label in another.

Classical memory B cells are often identified as CD21-positive and CD27-positive. Both classical and atypical memory B cells have encountered antigen, but a marker pattern alone cannot tell whether an individual cell is protective, impaired, or inactive. That distinction requires functional measurements, and even a laboratory function does not automatically demonstrate protection from illness.

This variation in definitions is one reason results can appear to conflict. Studies may use different marker combinations, examine different populations or parasite antigens, and test different functions. A result about a subset selected with one panel should not automatically be applied to every cell that another study calls atypical.

Why do these cells increase with malaria exposure?

Studies associate repeated exposure to Plasmodium and infection-related immune activation with expansion of atypical memory B-cell populations. This is an association: it indicates that these cells are more prominent in some malaria-exposed settings, not that a particular cell count reveals an individual’s degree of immunity.

One proposed mechanism concerns T-bet, a transcription factor encoded by the gene TBX21. A human study in malaria-exposed children found T-bet-high B cells with markers associated with atypical memory cells. In experiments, malaria-induced interferon gamma (IFN-γ), together with stimulation of the B-cell receptor (BCR), was implicated in inducing T-bet. The findings support a possible pathway for shaping these cells, but they do not establish a complete account of how all atypical memory B cells develop.

Their cellular origins also remain unsettled. Proposed possibilities include development from activated naïve B cells, from existing memory cells, or through more than one route. The available findings do not justify treating any one of these as the established pathway. The proposed IFN-γ and BCR-related mechanism is therefore one part of the picture, not a complete lineage explanation.

Why are atypical B cells sometimes described as dysfunctional?

Some atypical B-cell populations have features consistent with reduced or altered responses. Studies have reported expression of inhibitory receptors, reduced BCR signaling, and lower proliferation or antibody secretion in particular in-vitro conditions. In the child study, T-bet-high B cells showed reduced phosphorylation of BCR signaling molecules after BCR cross-linking.

These observations are relevant because BCR signaling helps a B cell respond when its receptor encounters a matching target. Reduced signaling in an assay can indicate that a measured population responds differently under those experimental conditions. It does not establish that every cell in the population is incapable of responding, nor does it show how that cell will behave in the body during infection. Likewise, an in-vitro result showing lower proliferation or secretion is a result about the tested cells and conditions, not a universal verdict on all atypical B cells.

For that reason, calling all atypical memory B cells “exhausted” goes beyond what the evidence supports. The label can suggest a fixed, uniformly defective state, while the research describes heterogeneous populations and context-dependent results. A dampened signal in one test and useful antibody production in another are not necessarily contradictory: they may involve different cells, conditions, antigens, or measures of function.

Can atypical B cells make useful antibodies?

Yes. Human studies have reported parasite-directed antibody activity from atypical memory B cells, providing evidence that at least some cells in these populations can perform useful functions. The findings speak to biological capacity in specific study settings; they do not by themselves establish lasting protection in a person.

Evidence from Plasmodium falciparum

A 2013 human study of natural P. falciparum infection reported that both classical and atypical memory B cells generated broadly neutralizing antibodies against blood-stage parasites. Both populations contributed to anti-P. falciparum serum immunoglobulin G (IgG), and atypical cells showed signs of active antibody secretion.

This result demonstrates antibody-producing capacity and parasite-directed activity in the study’s context. It does not show that atypical cells alone provide lasting clinical protection, or that the presence of these cells prevents future malaria illness. The reported contribution from both classical and atypical populations is also a reminder not to treat atypical cells as the only relevant source of malaria-directed antibodies.

Evidence from Plasmodium vivax

A 2025 human study after natural P. vivax infection identified antigen-specific atypical memory B cells and reported antibodies that inhibited erythrocyte binding across the tested variants of PvDBPII, an antigen involved in that assay. This is functional evidence in a distinct parasite-species and antigen context.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The P. vivax findings do not establish that the same activity applies to every malaria parasite, antigen, or atypical B-cell population. Questions about how durable the response is and how repeated exposure affects it remain unresolved. The result is evidence of inhibition in the tested setting, rather than proof that a person with those cells will be protected from clinical malaria.

What does the evidence say about protection from malaria?

The most defensible interpretation is that atypical memory B cells are associated with malaria exposure and can be biologically active, but they are not a validated measure of protection. Some experiments find features associated with reduced signaling; other human studies find antibody secretion, neutralization, or inhibition of parasite-related processes. Those results can coexist because studies differ in the cells and markers examined, the species and antigens involved, and the functions measured.

Most importantly, a laboratory result is not the same endpoint as clinical protection. Showing that an antibody neutralizes a parasite or inhibits a binding step under test conditions does not, by itself, establish that a person will avoid infection, develop less severe illness, or remain protected over time. The available evidence does not establish that a higher proportion of atypical B cells reliably predicts lower malaria risk or longer-lasting protection for an individual.

Accordingly, a blood measurement of atypical B cells should not be read as a personal immunity score. Their presence does not prove protection, and it does not prove immune failure. The evidence describes a complex cell population, not a stand-alone clinical test. No treatment decision or individual protection assessment follows from this cell phenotype alone in the evidence discussed here.

How to interpret a study about atypical B cells

When comparing findings, first identify what was actually measured. A study that counts cells by surface markers answers a different question from one that tests BCR signaling or measures antibodies. Neither kind of laboratory finding is interchangeable with evidence that people experienced less malaria disease.

Question to check Why it matters
Which markers defined the cells? Marker panels vary. Reduced or absent CD21 and CD27 are common identifiers, while T-bet, CD11c, CXCR3, and FcRL5 may be included for particular subsets.
Which parasite and antigen were studied? Evidence involving blood-stage P. falciparum or PvDBPII in P. vivax applies to those study contexts; it should not be generalized automatically to all malaria.
What function was measured? BCR signaling, proliferation, antibody secretion, neutralization, and inhibition of erythrocyte binding are distinct readouts.
Was the outcome laboratory function or clinical disease? A laboratory antibody effect does not itself demonstrate reduced infection, illness, or durable protection in people.

This framework helps reconcile apparently conflicting reports. A study may classify a population as atypical by one marker combination and find a signaling difference, while another isolates antigen-specific cells and detects antibody activity. Without aligning the markers, parasite context, assay, and endpoint, the results should not be treated as direct tests of the same claim.

It is also useful to separate three levels of evidence: an association between malaria exposure and cell frequency; a laboratory demonstration of signaling or antibody function; and a clinical link between the cells and disease outcomes. The research supports the first two in particular settings, but does not establish the third as a reliable predictor for an individual. A strong result at one level cannot simply be substituted for evidence at another.

What these findings do—and do not—mean

  • They do mean that malaria exposure is associated with atypical memory B-cell populations and that these cells merit study as part of the immune response.
  • They do mean that some measured populations show inhibitory features or reduced signaling in particular experiments.
  • They also mean that atypical cells can generate antibodies with parasite-neutralizing or parasite-inhibiting activity in reported human studies.
  • They do not mean that every atypical cell is dysfunctional, or that its phenotype alone tells whether it is protective.
  • They do not mean that counting these cells can currently determine an individual’s malaria protection or predict how long it will last.

Thus, atypical B cells are best understood as a varied component of immunity that may reflect exposure and can have different functional properties. Their presence is scientifically informative, but its clinical meaning cannot be reduced to “immune” or “not immune.”

Frequently Asked Questions

Are atypical B cells protective or dysfunctional in malaria?

The evidence does not support a single answer for every atypical memory B cell. Some populations show inhibitory features or reduced B-cell receptor signaling in particular experiments, while human studies have found atypical cells producing antibodies that neutralize or inhibit parasite-related processes. Their net contribution to durable clinical protection remains unresolved.

Do atypical B cells mean someone is immune to malaria?

No. Their presence or frequency is not an established personal immunity test. Studies show associations with malaria exposure and some parasite-directed antibody functions, but do not establish that a higher proportion reliably predicts lower risk or lasting protection in an individual.

How are atypical memory B cells identified?

Many malaria studies identify them by reduced or absent CD21 and CD27. Some subsets are also characterized by markers such as T-bet, CD11c, CXCR3, or FcRL5. Marker panels differ among studies, so the label does not always refer to exactly equivalent populations.

Why might malaria exposure increase atypical B cells?

Repeated Plasmodium exposure and infection-related immune activation are associated with expansion of these populations. In a study of malaria-exposed children, findings implicated malaria-induced IFN-γ together with B-cell receptor stimulation in inducing T-bet. This is a supported proposed mechanism, not a complete explanation of how all such cells develop.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can atypical B cells make antibodies against malaria parasites?

Yes. A 2013 human study of natural P. falciparum infection reported broadly neutralizing blood-stage antibodies from both classical and atypical memory B cells. A 2025 human P. vivax study identified antigen-specific atypical cells and antibodies that inhibited erythrocyte binding across tested PvDBPII variants. These findings demonstrate activity in particular study contexts, not guaranteed clinical protection.

The Bottom Line

Atypical memory B cells are neither a simple sign of immune failure nor proof of protection. Their markers, parasite context, and measured function vary across studies; some show dampened signaling, while others produce parasite-directed antibodies. Their role in durable clinical protection remains uncertain, and their frequency should not be used as an individual malaria-immunity score.
