AI safety gatekeepers are the people and governance groups who examine a model’s capabilities, behavior, and risks before and after deployment, then recommend or decide what safeguards and release conditions are needed. There is no single industry-wide committee or test: companies assign different roles, and the people who evaluate a model may not have final authority to release it.
In published accounts from OpenAI and Google DeepMind, review can combine capability testing, behavior evaluations, adversarial red teaming, risk analysis, mitigations, and continued monitoring. These activities provide evidence for a decision; they do not prove that a model is risk-free.

So who decides whether an AI model is safe to release, and what does a review actually establish? The answer depends on the developer’s own governance framework. The useful question is not simply whether a model “passed,” but who assessed it, what they tested, what risks they considered, and what happened in response to the findings.
Who decides whether an AI model is safe to release?
There is no universal AI safety gatekeeper. The term describes a set of functions inside an organization: specialists assess model behavior and capabilities, cross-functional groups review evidence, and executives or other governing bodies may decide whether and how deployment proceeds.

These responsibilities can be distributed across technical teams and governance bodies. Evaluators produce evidence about a model; advisers interpret that evidence and may recommend actions; decision-makers determine whether a release can proceed, under what conditions, or not yet. The separation matters because a report identifying a risk is not itself the same thing as a launch decision. A review process is also not necessarily a single meeting or final checklist: published descriptions include evaluation during training and deployment, mitigation planning, and later monitoring.
OpenAI: advisory review, leadership decision, board oversight
OpenAI says its cross-functional Safety Advisory Group reviews capability reports and makes recommendations ahead of deployment. Company leadership makes the final decisions, while the Board of Directors exercises oversight. The distinction matters: an advisory group can flag risks or recommend conditions without itself holding the final release authority. OpenAI also says it used more than 70 external experts to help assess risks associated with GPT-4o through external red teaming in 2024; that is a company-reported count, not an independent measure of the quality or completeness of review. OpenAI’s safety practices
“Cross-functional” is significant here: safety evidence can involve more than one specialty, while an advisory group’s recommendations still feed into a separate leadership decision. The public description does not enumerate every participant or internal debate for every launch, so it should not be read as a complete account of each individual review.
Google DeepMind: responsibility and safety review
Google DeepMind describes its Responsibility and Safety Council as a longstanding internal group that evaluates research, projects, and collaborations against the company’s AI Principles. It advises or partners with research and product teams on high-impact work. A separate AGI Safety Council focuses on safeguarding processes, systems, and research against extreme risks that could arise from powerful AGI systems. Those descriptions establish a company-specific governance arrangement, not an industry standard. Google DeepMind’s responsibility and safety overview
The two council descriptions point to different scopes: one considers research, projects, and collaborations against company principles, while the other concentrates on extreme risks associated with powerful AGI systems. That is useful context when comparing organizations, but the public descriptions do not provide a common score by which to rank the groups.
External scrutiny is input, not automatically the decision
Independent experts, academics, civil society, government bodies, and other labs can provide evidence or scrutiny. OpenAI, for example, describes external participation in red teaming and collaboration with public institutions. Such contributors can uncover weaknesses or challenge assumptions, but their involvement does not by itself make them the organization’s internal deployment decision-maker.
External reviewers can bring different expertise or perspectives to a test plan, but a headcount alone cannot show how comprehensive or effective the review was. OpenAI’s reported participation of more than 70 external experts for GPT-4o is a concrete example of outside red teaming, not a universal requirement or an independent certification.

OpenAI’s account of earlier frontier-risk arrangements describes a joint Deployment Safety Board with Microsoft that approves certain deployments above a capability threshold. The page also says roles may evolve, so this should be read as the process described there rather than a complete statement of current governance. OpenAI’s Preparedness Framework update
That caveat is important for readers: governance pages describe a process at a point in time, and organizations may revise frameworks or responsibilities. When comparing published arrangements, check which framework or update is being described instead of assuming that an older page captures every current role.

How do AI companies test a model before launch?
Reviewers gather different kinds of evidence because no single test answers every question. A capability evaluation asks what a model can do; a behavior evaluation measures whether it responds as intended; a red team actively tries to provoke failures; and risk analysis connects findings to possible harms and safeguards.
A practical way to understand the review is as a chain of questions: What capabilities does the system demonstrate? How does it behave on defined tasks? Can adversarial or unexpected inputs expose weaknesses? What harms could those weaknesses enable, and how serious might they be? What mitigations are available, and what evidence would support proceeding? The answers can lead to more testing or a different deployment plan, not just a pass-or-fail label.

Capability evaluations estimate whether a model can perform tasks that could contribute to significant harm. OpenAI’s frontier-risk account describes evaluations and monitoring across areas including cybersecurity, persuasion, chemical and biological threats, and autonomy. These tests concern capability and potential risk, not merely whether a model produces polite or policy-compliant answers. OpenAI’s Preparedness Framework update
For example, a model’s capacity in a risk domain is a different question from whether it refuses a particular harmful request in a test. A capability evaluation is intended to characterize what the system may be able to do; the risk review then considers why that capability could matter and whether mitigation is needed. The named domains are examples from OpenAI’s published account, not a claim that every company tests the same categories or defines them identically.
Safety and behavior evaluations: how does it respond?
Behavior evaluations measure defined outputs or tendencies, such as harmful responses, adherence to policies, factuality, alignment, and other desired or undesired behavior. OpenAI says it uses evaluations throughout training and deployment to inform training, launch, and mitigation plans. Results are evidence about tested behaviors under tested conditions; they are not a guarantee about every possible prompt, user, or context. OpenAI’s safety and alignment overview
An evaluation is most informative when its question and criteria are clear: what response or capability is being measured, under what conditions, and what outcome counts as a failure? Defined tests make it possible to examine a particular behavior or compare evidence against a criterion. But a result only speaks to the behavior and conditions that were examined. It cannot, by itself, establish that the model will behave the same way with a different prompt, an unfamiliar tool, or a context outside the test.

Outbyte PC Repair FREERepair Windows errors before they cause bigger problemsFix Now →Outbyte Driver Updater FREEFix the driver behind crashes, sound loss and screen glitchesFind Drivers →Red teaming: can reviewers make it fail?
Red teaming is adversarial probing. Reviewers try abusive, unexpected, or hostile scenarios to find misuse cases and failure modes that routine testing may not surface. It differs from an evaluation designed to measure specified behaviors: red teaming searches for weaknesses, while evaluations measure against defined criteria. A mature review process can use both.
OpenAI’s developer guidance recommends testing representative inputs as well as inputs that reflect someone trying to break an application, including prompt-injection attempts. The guide also describes red teaming as a way to probe systems rather than as a substitute for other evaluations. OpenAI’s safety best practices OpenAI’s red-teaming guide
For a developer, this means testing ordinary, representative use and deliberately difficult cases. A prompt-injection attempt, for instance, is not just another routine quality check: it tests whether hostile instructions can cause the application or model to behave in an unintended way. The point is to discover and document weaknesses, not to treat a successful attack as a complete map of every possible failure. Red-team findings can inform additional evaluations and mitigations, while the evaluation results help measure whether defined behaviors meet the intended criteria.

Risk analysis and safety cases: are risks manageable?
Risk analysis links a model’s capabilities to plausible harms, assesses severity and likelihood, and identifies mitigations. Google DeepMind’s Frontier Safety Framework describes systematic risk identification, capability analysis, decisions about whether risks are acceptable, and detailed safety-case review when relevant Critical Capability Levels are reached. The framework update says tracked capability levels can also help identify less extreme risks earlier. Google DeepMind’s Frontier Safety Framework update

A safety case is not simply a benchmark score. It is an assessment of whether the available evidence and safeguards support managing identified risks at the relevant level. That analysis can inform a governance decision, but it is distinct from the decision itself.
Keeping these terms separate helps make review descriptions precise. An evaluation measures defined capabilities or behavior; a red team probes for weaknesses using adversarial scenarios; a safety case considers whether identified risks have been reduced to manageable levels; and a governance body or executive may make the deployment decision. One kind of evidence does not automatically replace the others.
Mitigation and monitoring: what happens after testing?
Findings can lead to additional training, safeguards, access restrictions, monitoring, more evaluation, or altered deployment conditions. OpenAI’s frontier-risk account connects evaluations to mitigations and ongoing monitoring. Google DeepMind says it continues to review safeguards and safety cases after deployment. In other words, launch is not necessarily the last review point: real-world use can reveal risks that pre-release tests did not capture.
Mitigation is the bridge between identifying a risk and deciding what to do about it. A team may need to address a concern, gather more evidence, or change how a system is deployed. Published sources describe these broad connections, but do not establish that every organization uses every possible safeguard or that any specific mitigation eliminates the underlying risk. Continued monitoring matters because the evidence available after release can differ from what was available in pre-release testing.
What can stop or change a launch?
Thresholds and consequences differ by organization. They are company-specific rules, not universal definitions of a safe model. A threshold is a decision point in a particular framework; it should not be interpreted as a universal boundary between safe and unsafe AI.
OpenAI’s published Medium-risk rule
OpenAI says it will not release a new model if it crosses a “Medium” risk threshold in its Preparedness Framework until sufficient interventions bring the post-mitigation score back to “Medium.” The published account also separates advisory review by the Safety Advisory Group from leadership’s final decision and board oversight. This is OpenAI’s stated process, not a rule that all AI developers follow. OpenAI’s safety practices
The phrase “post-mitigation” is important: the stated rule links release to the risk level after interventions, not just an initial finding. If the threshold is crossed, the account says the model is not released until sufficient interventions bring the score back to the stated level. That describes a published OpenAI rule; it does not tell readers that every developer applies the same label, threshold, scoring method, or consequence.
Google DeepMind’s Critical Capability Levels
Google DeepMind’s framework uses Critical Capability Levels: thresholds at which a frontier model or system may pose heightened risk of severe harm without mitigation. Reaching a relevant level prompts detailed safety-case analysis and consideration of mitigations. The framework’s threshold-based approach is a way to connect capability evidence with review, rather than a claim that every system above a level will cause harm. The framework also describes tracking capability levels to identify less extreme risks earlier. This is a different emphasis from treating a threshold as a simple launch switch: a capability level can help trigger analysis and mitigation work, while the broader safety case examines evidence and safeguards. The public description supports explaining that relationship, not assuming that reaching a threshold automatically produces one identical outcome in every case.
Possible review outcomes
- Proceed with safeguards: release under conditions or protections intended to reduce identified risks.
- Gather more evidence: run further evaluations, red-team exercises, or risk analysis where uncertainty remains.
- Change the deployment: limit access, features, or use cases, or strengthen monitoring and other safeguards.
- Delay or withhold release: if a threshold or risk assessment is not satisfied, the organization may postpone deployment while addressing the findings.
These are general types of outcomes implied by the published processes, not a promise that every company uses each option or publishes every decision. The important point is that a review can affect the mitigation plan or deployment conditions; it is not merely a score recorded after testing.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. Can a model pass safety tests and still behave badly?
Yes. Passing a test means the model met a particular criterion under particular conditions; it does not establish that it will behave safely in every interaction. Tests can miss rare combinations of inputs, new misuse techniques, or contexts unlike those represented in the evaluation. Red teaming can find failures that a defined benchmark overlooks, while monitoring can surface issues that appear only after deployment.
This is why “passed safety tests” should be understood narrowly. It reports evidence against the tests that were run, not proof that every relevant risk was anticipated. Evaluation, adversarial probing, risk analysis, mitigation, and post-release monitoring answer different questions and work together to make a review more informative. Even then, the published accounts describe efforts to assess and manage risk, not a guarantee of risk-free behavior.
Public policy descriptions also have limits: they explain declared processes, but do not reveal every internal test, disagreement, or real-world outcome. A company’s published framework is evidence of its stated approach, not an independent audit proving that every risk has been eliminated. For readers, that means a public document is useful for understanding the declared roles, methods, and thresholds, but cannot establish the full quality of every individual review.
What should users and developers take from a safety review?
For people choosing whether to rely on an AI output
A model’s review status should not replace judgment about the task. OpenAI’s safety guidance recommends human review wherever possible for high-stakes uses and emphasizes giving reviewers access to source information so they can verify outputs. If an answer could affect health, finances, legal rights, safety, or another consequential outcome, a person should be able to check the underlying evidence rather than treating fluent output as proof.
Human review is most useful when the reviewer has enough context to check the output, including access to the source information it relies on. A person who cannot inspect the evidence may be unable to catch a confident but unsupported response. This does not make human review a guarantee either; it is a practical safeguard that keeps consequential decisions from resting solely on an AI output.
For teams building AI applications
Test both normal use and plausible misuse. Include representative cases, adversarial prompts, and prompt-injection attempts; define what a failure looks like before testing; and connect findings to concrete mitigations. Re-test after meaningful changes to prompts, models, tools, or deployment settings, because prior results do not automatically apply to a changed system. OpenAI’s developer guidance provides further detail on application safety and red teaming. Safety best practices Red-teaming guide
For practitioners looking for a tool reference, OpenAI’s red-teaming guide names Promptfoo, an open-source framework for evaluating prompts, agents, and applications, and also describes a managed red-teaming offering for enterprise customers. These are examples mentioned in that guide, not endorsements or a substitute for a team’s own risk analysis. Any commercial referral or partner relationship should be verified before linking to it.

How to compare published safety review systems
A meaningful comparison asks the same questions of each organization rather than ranking frameworks by a single label or score. Start with the people and authority involved, then look at which risks and capability levels the framework covers. Consider what evidence the process uses, how findings translate into mitigations or deployment decisions, and whether review continues after release.
| Comparison question | What to look for |
|---|---|
| Who evaluates, and who decides? | Identify technical evaluators, advisory groups, executives, boards, and any external contributors. Separate recommendations from final authority. |
| Which risks and thresholds are covered? | Check the stated risk domains, capability thresholds, and what happens when a threshold is reached. |
| What evidence is used? | Look for defined evaluations, adversarial red teaming, capability analysis, safety cases, and external input. |
| How do findings affect deployment? | Find whether findings map to mitigations, access conditions, further testing, delay, or a decision not to release. |
| Does review continue after release? | Check for post-deployment monitoring and reassessment of safeguards as new evidence emerges. |
The public descriptions from OpenAI and Google DeepMind provide evidence for comparing these features, but not for a simple ranking of which system is most effective. Their frameworks, scopes, and governance arrangements are organization-specific and can change over time. A fair comparison uses the same axes for each and distinguishes a published commitment from independently verified results.
Frequently Asked Questions
Who decides whether an AI model is safe to release?
It depends on the developer’s governance process. OpenAI says its Safety Advisory Group reviews capability reports and recommends actions, while company leadership makes final decisions and the Board of Directors provides oversight. Google DeepMind describes internal safety councils that evaluate work and advise or partner with teams. These are company-specific arrangements, not a universal industry model.

What do AI red teams actually do?
Red teams deliberately try adversarial, abusive, or unexpected inputs to uncover weaknesses and failure modes that routine testing may miss. OpenAI’s developer guidance recommends testing representative cases as well as attempts to break an application, including prompt-injection attempts. Red teaming probes for weaknesses; it is distinct from an evaluation that measures behavior against defined criteria.
Can a model pass safety tests and still behave badly?
Yes. A passing result applies to the criteria and conditions tested; it does not establish safe behavior in every interaction or context. Red teaming may find failures outside a defined benchmark, and monitoring can reveal issues after deployment. The published review processes describe risk assessment and mitigation, not a guarantee that all risks are eliminated.
What is a safety case, and is it the same as a safety test?
No. A test or evaluation measures a defined capability or behavior, while a safety case assesses whether evidence and safeguards support managing identified risks at a relevant level. Google DeepMind’s framework connects relevant Critical Capability Levels with detailed safety-case analysis. A safety case can inform a governance decision, but it is not itself the decision.
Can outside experts make the final release decision?
External experts can contribute evidence and scrutiny, including by participating in red teaming, but that does not automatically make them the organization’s internal decision-makers. OpenAI reports that more than 70 external experts helped assess GPT-4o risks through red teaming in 2024; this is a company-reported count, not an independent measure of review quality.
Recommended Free Tools
Does a public safety framework prove a model is safe?
No. Public frameworks explain an organization’s stated roles, tests, thresholds, and mitigation approach. They do not reveal every internal test or disagreement, and they are not an independent audit proving that every risk has been eliminated. Frameworks and governance arrangements can also change over time.
The Bottom Line
AI safety gatekeepers do more than run a final test: they gather capability and behavior evidence, probe for weaknesses, assess risks, and connect findings to mitigations and deployment conditions. The evaluator may advise while leadership or another governing body decides. Because frameworks vary and tests cannot cover every situation, a review is evidence for managing risk—not proof that a model is safe in every use.
